3.Right hemihepatectomy through the right retrohepatic tunnel

Objective To study the clinical value of anatomizing and ligating the short hepatic veins (SHVs) and right hepatic veins through the right Retrohepatic Tunnel (RT) in right hemibepatectomy of giant hepatic carcinoma.Methods We performed the right hemibepatectomy on 23 patients with huge tumors which diameters were lager than 8 cm.There were four main procedures during the operation.Firstly,sperated the interspace between right hepatic veins and middle hepatic veins through secundum porta hepatis.Secondly,seperated and ligated the right hepatic portal vein and hepatic artery in the Glisson's system.Thirdly,anatomized and ligated SHVs and right hepatic veins through the right RT conversly.Finally,set a blocking-belt at the left of the central liver and then cut the right hemi-hepar.Results The anatomy of right hepatic artery,Portal Vein,retrohepatic inferior vena cava (RHIVC),right hepatic veins was well done.The intraoperative blood loss of 7 patients was less than 400 mL,and 11 was 500-700 mL,2 was 800-1 000 mL,3 was 1 100-1 400 mL,and average was 640 mL.The hepatic function was changed in the third day afteroperation.The numerus of TBIL of 16 patients was 20-40 μmol/L,and 6 was 45-50 μmol/L,and 1 was 60 μmol/L.The numerus of ALT of 14 patients was 150-200 U/L,and 9 was 250-400 U/L.The numerus of AST of of 13 patients was 160-200 U/L,and 9 was 230-400 U/L,and 1 was 430 U/L.The numerus of GGT of 14 patients was 160-200 U/L,and 8 was 220-310 U/L,and 1 was 420 U/L.The hepaticfailure happened to 1 patient because of deep venous infection.Conclusions The right hemibepatectomy through the right retrohepatic tunnel is feasible in the tumor without invasion the RHIVC and diaphragm,or the volume was not too huge.The operation could reduce the blood loss and make for the recovery of hepatic funtions.
5.Correlation between 6-minute walking test and oxygen uptake

BACKGROUND: 6-minute walking test (6MWT) is a submaximal test, characterizing by easily processing and low cost, thus it is widely used in the clinic. However, how to change walking distance into maximal motor capacity needs to be further studied.OBJECTIVE: To study the correlation of 6-minute walking distance (D), work (W), peak oxygen uptake (VO_2 peak) and maximal oxygen uptake (VO_2 max) detected by Bruce.DESIGN, TIME AND SETTING: This study was performed at tshe Department of Rehabilitation Medicine, Zhongda Hospital affiliated to Southeast University from March to May 2009.PARTICIPANTS: A total of 25 healthy volunteers were enrolled, including 14 males and 11 females, with a mean age of (22.0±2.3) years.METHODS: All 25 subjects were tested with cardiopulmonary exercise testing (CPET) according to Bruce, VO_2 max and anaerobic threshold (AT) was detected respectively, then 6MWT" was adopted to detect 6-minute walking distance (D), work (W) and maximal oxygen uptake (VO_2 max) of subjects. Data of pulmonary gas exchange in breathing was detected by wireless remote sensor K4b~2 to obtain VO_2 max and VO_2 peak.MAIN OUTCOME MEASURES: ① Regularity of cardiac rate, oxygen uptake and respiratory frequency varied with time passing; ② Walking distance, work, oxygen uptake, cardiac rate and respiratory frequency were compared before and after testing; ③ Maximal oxygen uptake and anaerobic threshold with CPET were compared to peal oxygen uptake with 6MWT; ④ Correlation of distance, work between maximal oxygen uptake and peal oxygen uptake.RESULTS: There was no significant difference between AT from CPET and VO2 peak from 6MWT (P > 0.05). 6-minute walking distance was not correlated with either VO_2 peak or VO_2 max. However, 6-minute walking work was linearly correlated with both VO2 peak (r=0.779 7, P < 0.001) and VO_2 max (r=0.894 1, P < 0.001).CONCLUSION: 6-minute walking test is an exercise test at level of anaerobic threshold. 6-minute walking work can reflect both submaximal exercise capacity and maximal exercise capacity.
6.Role of calcium activated potassium channel in cardioprotection induced by anoxic postconditioning in isolated rat heart

AIM: To investigate whether calcium activated potassium channel(KCa) and mitochondrial permeability transition pore(mPTP) contribute to cardioprotective effect elicited by anoxic postconditioning.METHODS: The isolated perfused hearts of male Sprague-Dawley rats were subjected to 30 min global anoxic followed by 120 min reoxygenation.Formazan content of myocardium was measured at 490 nm spectrophotometrically,and the level of lactate dehydrogenase(LDH) in the coronary effluent was detected.The absorbance of isolated heart mitochondria at 520 nm was determined.RESULTS: Anoxic postconditioning increased formazan content,reduced LDH release,improved the hemodynamic parameters of the left ventricular developed pressure,maximal rise/fall rate of left ventricular pressure,left ventricular end-diastolic pressure and rate pressure product and attenuated the decrease of coronary flow during reperfusion.Pretreatment with paxilline(1 ?mol/L) inhibited the effect of anoxic postconditioning.The opening of mPTP was suppressed in the mitochondria isolated from A/R hearts treated with anoxic postconditioning.CONCLUSION: The findings indicate that in the isolated rat heart,anoxic postconditioning protects myocardium against anoxic/reoxygenation injury via inhibiting KCa and the mPTP opening.
7.Clinical characteristics of mild and severe types of influenza A/H1N1

Clinical data of 150 patients with mild and 38 patients with severe confirmed influenza A/H1N1 were collected and retrospectively analyzed in this paper with descriptive epidemiology. Mild patients mainly presented with symptoms similar to seasonal influenza with few complications, however,persistent high fever, cough with bloody sputum expectoration, chest distress and short of breath manifested in severe patients with respiratory failure and acute respiratory distress syndrome in some cases. Proportion of obesity and underlying diseases in severe patients, who were at high risk for severe influenza A/H1N1, was higher than that in mild ones (P<0. 05). Average course of the illness spanned five to 11 days in patient with mild influenza A/H1N1 with all cured, and 34 of severe cases discharged with better recovery and other four died with a case-fatality of 10. 5 percent.
8.Clicinal analysis of 21cases with thyroid nodule associated with thyroid cancer

Objective To analyze the clinical manifestation of thyroid carcinoma in patient with nodular goiter,and explore the effective diagnosis and treatment method.Methods Retrospective analysis was made on patients operated upon for nodular goiter with coexisting diffetentiated thyroid carcinoma.Results Tiny papillary car-cinoma 9 cases,papillary carcinoma 1 1 cases,and follicular carcinoma 1 case.Four Chad cervical lymph node involve-ment,2 cases in the cervical compartment and 2 cases in the central area.We followed up for 10 months to 10 years, all of the patients were survival.Conclusion Early diagnosis can provide scientific basis for surgical treatment.Oper-ation should master the indication and scope,to ensure the satety,impove survival rate.
9.The pathophysiological mechanisms of HIV-related neuropathic pain

HIV-related sensory neuropathy ( HIV-SN) mainly contains the HIV infection-related distal sensory polyneuropa-thy (DSP) and antiretroviral toxic neuropathies (ATN).HIV-DSP is associated with proinflammatory cytokines , chemokines, ros and which is induced by gp 120;and HIV-ATN may be related to mitochondrial toxicity which is induced by the application of anti retroviral drugs, such as ddC, and similar as the molecular mechanism of HIV-DSP, this means that the current conventional method for the treatment of neuropathic pain in AIDS may further aggravate the neuropathic pain of the patient .Therefore, developing the study on the neurochemical and pharmacological mechanisms of HIV-related neuropathic pain will provide novel targets for the new effective drugs .
10.Current status of Middle East respiratory syndrome in children
