1.Effect Mechanism and Law of Sterilization by 60Co-γ Ray Irradiation on Chemical Composition of Chinese Materia Medica: A Review

60Co-γ ray irradiation has the unique advantages of high efficiency, strong penetration, operation at room temperature and no residue, which has been widely used in the sterilization of Chinese medicinal materials, decoction pieces, Chinese patent medicine. However, the irradiation effect may cause changes in the content of chemical components in Chinese materia medica or the emergence of new radiolysis products, leading to reduced efficacy and uncontrollable safety risks. This paper reviewed the relevant literature at home and abroad, summarized the effect of irradiation sterilization on various types of chemical compositions of Chinese medicinal materials and their preparations, and analyzed and explored the rule of change. The results showed that the content changes of various chemical components in Chinese materia medica after 60Co-γ ray irradiation sterilization varied. The contents of most flavonoids, terpenoids, phenylpropanoids and quinones decreased after irradiation, and the degree of decrease increased with the elevated irradiation dose. The contents of lignans, alkaloids, isoflavones and some terpenoids did not change significantly before and after irradiation, while the content changes of triterpenoid saponins, dihydroflavonols, chalcones, sugars and glycosides after irradiation were not yet uniform. Therefore, it is recommended to pay attention to the compositional changes of irradiated Chinese medicines, strengthen the research on the standards of irradiated Chinese medicines, and standardize the irradiation and sterilization of Chinese medicines in order to promote the healthy and rational application of irradiated Chinese medicines.
2.Association relation of C0-C2 Cobb angle and cervical disc herniation.

OBJECTIVE:
Objective To investigate the relationship between cervical disc herniation and C0-C2 Cobb angle.
METHODS:
The clinical data of 301 patients with cervical disc herniation from 2020 to 2024 were retrospectively analyzed. The median value of C0-C2 Cobb angle measurements from 301 patients was used as the boundary, cervical disc herniation patients were divided into two groups, C0-C2 Cobb angle <28.50 group and 151 patients with C0-C2 Cobb angle≥28.50 group. Among them, 150 patients in C0-C2 Cobb angle <28.50 group included 53 males and 97 females, aged 23 to 76 (57.32±12.55) years, with a disease duration of 7 to 19 (13.81±5.32) months;the othor 151 patients with C0-C2 Cobb angle≥28.50 group including 61 males and 90 females, aged 25 to 74 (56.86±12.51) years, with a disease duration of 8 to 18 (14.13±5.56) months. The cervical lordosis angle (C0-C2 Cobb angle and C2-C7 Cobb angle), T1 inclination slope (T1S) and cervical sagittal axial distance (C2-C7 SVA) were measured on the lateral cervical radiographs. The correlation between C0-C2 Cobb angle and cervical disc herniation range, protrusion position, average protrusion size and other parameters was analyzed.
RESULTS:
When the C0-C2 Cobb angle<28.50°, the average protrusion size was (2.21±0.56) mm, the C2-C7 Cobb angle was (19.92±12.06)° and the C2-C7 SVA was (1.10±1.20) mm. When the C0-C2 Cobb angle≥28.50°, the average protrusion size was (2.38±0.60) mm, the C2-C7 Cobb angle was (12.01±13.09 )°, the C2-C7 SVA was (1.53±1.36) mm, and the difference was statistically significant (P<0.05). Between the two groups of patients with C0-C2 Cobb angle < 28.50° and C0-C2 Cobb angle≥28.50°, there were significant differences in the size of C3,4, C4,5, C5,6, C6,7, C7, T1 disc herniation in single segment (P<0.05 ). C0-C2 Cobb angle was correlated with age(r=-0.135, P<0.05 ), C2-C7 Cobb angle (r=-0.382, P<0.01 ), C2-C7 SVA (r=0.293, P<0.01), average protrusion size (r=0.139, P<0.05), and the size of C3,4 (r=0.215, P<0.01 ), C4,5 (r=0.176, P<0.01 ), C5,6 (r=0.144, P<0.05 ), C6,7 (r=0.158, P<0.05 ), C7T1 (r=0.535, P<0.05) disc herniation.
CONCLUSION
There is a positive correlation between C0-C2 Cobb angle and the size of cervical disc herniation. C0-C2 Cobb angle can reflect the degree of cervical disc herniation. Previous studies have shown that the biomechanical changes between C0-C2 Cobb angle, C2-C7 Cobb angle, C2-C7 SVA and cervical extensor muscle group may be risk factors for accelerating cervical disc herniation and this may be one of the mechanisms that C0-C2 Cobb angle is positively correlated with the size of cervical disc herniation.

This study aimed to comprehensively examine the association of gallstones, cholecystectomy, and cancer risk. Multivariable logistic regressions were performed to estimate the observational associations of gallstones and cholecystectomy with cancer risk, using data from a nationwide cohort involving 239 799 participants. General and gender-specific two-sample Mendelian randomization (MR) analysis was further conducted to assess the causalities of the observed associations. Observationally, a history of gallstones without cholecystectomy was associated with a high risk of stomach cancer (adjusted odds ratio (aOR)=2.54, 95% confidence interval (CI) 1.50-4.28), liver and bile duct cancer (aOR=2.46, 95% CI 1.17-5.16), kidney cancer (aOR=2.04, 95% CI 1.05-3.94), and bladder cancer (aOR=2.23, 95% CI 1.01-5.13) in the general population, as well as cervical cancer (aOR=1.69, 95% CI 1.12-2.56) in women. Moreover, cholecystectomy was associated with high odds of stomach cancer (aOR=2.41, 95% CI 1.29-4.49), colorectal cancer (aOR=1.83, 95% CI 1.18-2.85), and cancer of liver and bile duct (aOR=2.58, 95% CI 1.11-6.02). MR analysis only supported the causal effect of gallstones on stomach, liver and bile duct, kidney, and bladder cancer. This study added evidence to the causal effect of gallstones on stomach, liver and bile duct, kidney, and bladder cancer, highlighting the importance of cancer screening in individuals with gallstones.

6.Effects of extreme temperature exposure on other infectious diarrhea morbidity risk in Taizhou, Zhejiang

Background Other infectious diarrhea (OID) refers to infectious diarrhea other than cholera, dysentery, and typhoid/paratyphoid fever. In the global climate change context, frequent extreme temperature events pose a significant threat to population health, yet the association between extreme temperatures and OID remains unclear. Objective To understand the impact of extreme temperature events on the incidence of OID in Taizhou region, and provide a scientific basis for formulating targeted public health intervention strategies. Methods Case data of OID from 2016 to 2024 and meteorological and pollutant data of Taizhou, Zhejiang during the same period were collected. The case data were derived from National Notifiable Infectious Diseases Reporting System, while the meteorological data and pollutant data were obtained from National Meteorological Science Data Center and Taizhou Ecology and Environment Bureau, respectively. An extreme heat event is defined as a period of two or more consecutive days in the warm season during which the daily mean temperature exceeds the 90th percentile value for that season. Similarly, an extreme cold event is defined as a period of two or more consecutive days in the cold season during which the daily mean temperature falls below the 10th percentile value. A distributed lag nonlinear model based on Poisson regression was used to assess the impact of extreme temperatures on OID incidence. Furthermore, subgroup analyses were conducted by gender and age. Results From 2016 to 2024, a total of 108 670 OID cases were reported in Taizhou, with higher incidence in males (61 253 cases) than females (47 417 cases). In terms of single-day lag effect, exposure to extreme cold presented an inverted U-shaped risk pattern: a significant risk effect began to emerge on the 4th day after exposure, continued to rise, and reached a peak on the 7th day (RR=1.044, 95%CI: 1.029, 1.058), whereas extreme heat had no significant impact. In terms of cumulative lag effect, exposure to extreme cold presented a J-shaped cumulative effect curve: short-term exposure (0-2 days) showed no risk effect, but as time extended (0-9 days), a risk effect emerged and reached the maximum risk value during the 0-12 d cumulative period (RR=1.243, 95%CI: 1.057, 1.461). In contrast, extreme heat had no cumulative lag effect. The subgroup analysis revealed that under extreme cold exposure, the single-day lag effects for both males and females exhibited an inverted U-shaped curve, with the risk peaking on day 7 for males (RR=1.045, 95%CI: 1.026, 1.065) and day 8 for females (RR=1.041, 95%CI: 1.019, 1.064). The risk pattern in the 1-5 years age group was similar to that of the overall population but with a higher effect size (RR=1.063, 95%CI: 1.041, 1.085). In terms of the cumulative lag effects, the cumulative risk curves for both males and females under extreme cold exposure showed a J-shaped pattern with varied risk profiles. Significant variations in risk patterns were observed across different age groups. No significant cumulative lag effects were observed for extreme heat exposure in any subgroup. Conclusion Extreme cold can increase the incidence of OID in Taizhou, and the risk effects are heterogeneous among different groups. No risk effect is observed for extreme heat.
7.Study on The Promotion of Tenocyte Proliferation and Differentiation by Oriented Fiber Membrane Loaded With Nano-zinc Oxide

ObjectiveTo simulate the microstructure and mechanical properties of tendon tissue and promote its regeneration and repair, electrospinning technology was used to prepare L-polylactic acid (PLLA) fiber membranes loaded with different nano zinc oxide contents and with oriented structures. Physical and chemical characterization and biological performance evaluation were carried out to explore their effects on tendon cell proliferation and differentiation. MethodsPreparation of PLLA fiber scaffolds and PLLA/ZnO fiber scaffolds containing different mass fractions of nano ZnO was performed using electrospinning technology. The physicochemical properties of the scaffold were characterized by scanning electron microscopy, mechanical stretching, and EDS spectroscopy. The scaffold was co-cultured with mouse tendon cells to detect its biocompatibility and regulatory effects on cell differentiation behavior. ResultsThe fiber scaffolds were arranged in an oriented manner, and zinc elements were uniformly distributed in the fibers. The tensile strength and Young’s modulus of PLLA/0.1%ZnO fiber scaffolds were significantly higher than PLLA fiber scaffolds. The number of cells on the surface of PLLA/0.1%ZnO fiber scaffold was significantly higher than that of the PLLA group, and the activity was better; mouse tendon cells exhibit directional adhesion and growth along the fiber arrangement direction. ConclusionThe oriented PLLA/0.1%ZnO fiber scaffold had excellent physicochemical properties, which can significantly promote the oriented growth, proliferation and differentiation of tendon cells. It is expected to be used for tendon tissue regeneration and repair in the future.
8.Distribution characteristics and sources of heavy metals and polycyclic aromatic hydrocarbons in PM2.5 in an urban area of Taizhou City from 2017 to 2022

Background Fine particulate matter (PM2.5) in ambient air is an important carrier of heavy metals and polycyclic aromatic hydrocarbons (PAHs), and long-term inhalation of heavy metals and PAHs poses a potential threat to human health. Objective To analyze the ambient PM2.5 concentrations and the distribution characteristics and sources of heavy metals and PAHs in PM2.5 in an urban area of Taizhou City, aiming to provide basic data and a scientific basis for local air pollution management and population health prevention. Methods From 9am to 8pm on the 10th to 16th of each month from 2017 to 2022 (23 h·d−1), a monitoring point was set up on the top floor of a building in the main urban area of Taizhou to collect PM2.5 samples (sampling flow rate: 2.3 m3·h−1) and measure its concentration. The characteristics of 12 heavy metals and 16 priority PAHs in PM2.5 were analyzed. The pollution sources of heavy metals and PAHs were determined by enrichment factor method and isomer characteristic ratio method, respectively. Results From 2017 to 2022, the average daily concentration of PM2.5 in this urban area ranged from 4 to 141 μg·m−3, and the median annual concentrations were lower than the national secondary standard limit. The seasonal concentration of PM2.5 was as follows: winter>spring>summer and autumn, and the difference was statistically significant (H=134.077, P<0.01). The concentration of PM2.5 in 2017.1 to 2020.1 was higher than that in 2020.2 to 2022.12 (Z=−3.989, P<0.01). The main heavy metal pollutants in this area were manganese (Mn), lead (Pb), arsenic (As), nickel (Ni), and chromium (Cr). Except for beryllium (Be) and mercury (Hg), the concentrations of other heavy metals had seasonal differences (P<0.01), higher in winter and spring than in summer and autumn. Except for manganese (Mn) and arsenic (As), the concentrations of other heavy metals were higher from 2017.1 to 2020.1 than from 2020.2 to 2022.12, and the difference was statistically significant (P<0.05). The results of enrichment factor indicated anthropogenic pollution sources of antimony (Sb), cadmium (Cd), and selenium (Se). The average daily concentration of PAHs ranged from 0.88 to 24.56 ng·m−3, with a seasonal trend of winter>spring>autumn>summer, and the difference was statistically significant (H=231.387, P<0.01). Among the 16 PAHs, the top pollutants were benzo [b] fluoranthene (BbF) (25.36%), indene [1,2,3-c,d] pyrene (IcdP) (11.22%), and benzo [g,h,i] perylene (BghiP) (9.02%). The results of characteristic ratio method showed that the 16 PAHs pollutants were produced by a combined action of gasoline buring, coal burning, and biomass burning. Conclusion In selected urban area of Taizhou City, the concentrations of PM2.5, heavy metals, and PAHs all show a downward trend year by year at a relatively low level, and obvious seasonal distributions which are higher in winter and spring. Man-made pollution is the main source of heavy metals, and automobile exhaust emissions, industrial coal burning, and waste incineration may be the main sources of PAHs.
9.Recent advance in neuroprotectants combined with reperfusion in acute ischemic stroke

Acute ischemic stroke (AIS) is a kind of central nervous system disease that seriously threatens human health and life. Current treatment for AIS is mainly reperfusion. However, the time-sensitive of reperfusion limits its clinical application, and a considerable part of patients within the time window cannot achieve the expected effect after reperfusion; related complications of reperfusion have not been completely solved. So far, some clinical trials have confirmed that neuroprotectants are useful supplements and adjuncts to reperfusion. This paper reviews the recent advance in neuroprotectants combined with reperfusion in AIS to provide references for AIS treatment.
10.Analysis of the efficacy of bronchoalveolar lavage for severe lobar pneumonia and the risk factors affecting the duration of the disease

Objective To study the clinical efficacy of using bronchoalveolar lavage in severe lobar pneumonia and to analyze the high-risk factors affecting the duration of the disease.Methods The clinical data of children with severe lobar pneumonia diagnosed in the Department of Pediatrics,the First Affiliated Hospital of Shihezi University from January 2018 to January 2022 were retrospectively collected,and the children in the lavage group and the control group were matched 1:1 according to whether bronchoalveolar lavage was performed using the propensityscore matching(PSM)method to compare the therapeutic effects of the two groups.At the same time,the children in the lavage group were divided into the long-duration group and the short-duration group according to whether the duration of the disease was more than 2 weeks,and the reasons for the difference in the duration of the disease between the two groups were analyzed.Results ① After treatment,the children in the lavage group had cough relief time,fever reduction time,lung rales disappearance time,lung imaging manifestations reduction time,white blood cell(WBC),neutrophil to lymphocyte ratio neutrophil-to-lymphocyte ratio(NLR),C-reactive protein(CRP),erythrocytes sedimentation rate(ESR),and procalcitonin(PCT)were all lower than those in the control group,and the differences were statistically significant(P<0.05).② In the lavage group,the CRP,lactate dehydrogenase(LDH),and fever peak value of children in the long-course group were significantly higher;The proportions of multi-pulmonary lobe infection,combined pleural effusion,and multiple pathogen infections were significantly higher than those in the short-course group.The difference was statistically significant among disease course groups(P<0.05).③ Two-category Logistic and receiver operating characteristic curve(ROC curve)analysis showed that LDH and CRP were independent risk factors for the disease duration in children in the lavage group>2 weeks,and the optimal critical values of LDH and CRP for predicting a disease duration of>2 weeks in children with severe lobar pneumonia were 333U/L and 32.6mg/L,respectively.Conclusion ① Bronchoalveolar lavage can shorten the treatment time of children with lobar pneumonia,speed up the recovery of inflammation,and significantly improve the efficacy.② Multiple pathogenic mixed infections,concurrent pleural effusion,LDH≥333 U/L,and CRP≥32.6 mg/L are independent risk factors for the disease duration>2 weeks after alveolar lavage in children with lobar pneumonia.We need to be alert to the possibility of prolonged disease course.
