1.Role of plasma B type natriuretic peptide concentration in the diagnosis and predicting prognosis of patients with heart failure

Objective To study the role of plasma B type natriuretic peptide(BNP)concentration in the diagnosis and predicting prognosis of patients with heart failure(HF).Methods Plasma BNP concentration was measured with immunofluorescenee assay in 90 patients with dyspnea.Left ventricular ejection fraction(LVEF)and pulmonary capillary wedge pressure(PCWP)were determined by eehoeardiography and Swan-Ganz catheter respectively in these patients at the same time.The gradation of HF was based on the criteria of cardiac function classification setted by New York Heart Association(NYHA).The HF patients were classified into 2 groups according to BNP concentration measured just before the discharge:patients with BNP<400 ng/L and BNP≥400 ng/L.The re-admisslon rate within 30 days because of the HF between the two groups was compared.Results Plasma BNP levels were higher in the patients with left heart failure than those in the non-heart failure patients[(569?222)ng/L vs.(52?10)ng/L,P<0.01],and BNP levels were increased in accordance with the severity of HF.The sensitivity,specificity,and negative predictive values of plasma BNP levels≥100 ng/L for predicting HF were 94.7%,91.7% and 96.7%,respectively.Plasma BNP levels were positively related to PCWP(r=0.55,P<0.01),and negatively related to LVEF(r=-0.62,P<0.01).There was no significant difference of plasma BNP levels between patients with different causes of HF.The re-admission rate within 30 days after discharge was notably high if the plasma BNP concentrations were over 400 ng/L.Conclusion Plasma BNP concentration can be regarded as a valuable marker for diagnosis and predicting prognosis of patients with HF.
2.Clinical study on pathogenic flux and drug resistance of neonatal sepsis

Objective To study the characteristic of etiology and drug resistance of neonatal sepsis for rational use antibiotics in clinic.Methods The etiology and drug resistance of 167 positive hemoculture examples of 365 neonatal sepsis examples were retrospectively analyzed,and the characteristic of etiology diversify and drug resist-ance characteristic of neonatal sepsis were also analyzed.Results In 365 blood culture examples of neonatal sepsis, there were 167 positive cases,positive ratio was 45.75%.Among pathogenic bacterium,the first etiology was staphylo-coccus epidermidis,there were fifty -eight examples and account of 34.73%.The second was staphylococcus haemo-lyticus and accounts twenty -nine examples and 17.37%.The third was staphylococcus aureus which had twenty -two examples and account 13.17%.The annual infection rate of staphylococcus epidermidis and staphylococcus hae-molyticus in the second five years were increased obviously.Gram -positive coccus drug resistance to penicillin,cep-hazolin,oxazocilline and cefoxitin were very high and the rate of the second five years were higher than that of the first five years.Gram -positive coccus drug resistance to third -generation cephaloporins and imipenem and meropenem were higher than vancocin and the rate of the second five years were higher than that of the first five years.Gram -positive coccus was 100.00% sensitive to vancomycin.Gram negative bacilli drug resistance rate to penbritin and cep-hazolin and third -generation cephaloporins was the highest.Gram negative bacilli to meropenem and imipenem was hypersensitive in all antibiotics.Conclusion The chief pathogenic bacteria of neonatal septicemia is staphylococci. Among pathogenic bacterium,the main pathogenic bacteria is CoNS,and staphylococcus epidermidis and staphylococ-cus haemolyticus goes up significantly.Among gram -negative bacilli,the escherichia coli goes up.The drug resist-ance to bacteria goes up in neonatal sepsis,it is very important to monitor the drug resistance.
3.Effects of GATA6 silencing mediated by lentivirus on apoptosis of human liver cancer Huh-7 cells

AIM:To explore the effect of GATA6 gene silencing on apoptosis of hepatocellular carcinoma Huh-7 cells.METHODS:RNA interference vectors of the target gene GATA6 mediated by lentivirus were constructed in vitro to transfect the hepatocellular carcinoma cell line Huh-7.The apoptotic rate of transfected cells was measured by flow cytome-try.The protein expression of GATA6, NF-κB and Bcl-2 in transfected cells was determined by Western blotting.RE-SULTS:The transfection efficiency was 57.4%.The mRNA and protein expression of GATA6 reduced significantly after the carcinoma cell line Huh-7 being transfected by RNA interference vectors mediated by lentivirus.The apoptotic rate of the carcinoma cells with silent GATA6 gene was significantly increased (P<0.05).The protein expression levels of NF-κB and Bcl-2 were also significantly decreased.CONCLUSION: Lentiviral vector-mediated RNA interference of GATA6 has an inhibitory effect on the expression of the gene itself, and promotes the apoptosis of hepatocellular carcinoma cells.Regu-lation of the apoptosis-related protein expression by the NF-κB signaling to influence the proliferation and apoptosis of hepa-tocellular carcinoma cells might be one of the possible mechanisms.
5.The effects of statins on the cerebral haemodynamics measured by transcranial Doppler in ischemic stroke patients

Objective To evaluate the effects of statins on the cerebral haemodynamics of ischemic stroke patients with transcranial Doppler(TCD). Methods 70 inpatients with acute ischemic stroke were divided into two groups according whether they had taken statins after stroke.The change of the cerebral haemodynamics was studied in the two groups with TCD three months after stroke. Results Three months later,the systolic flow velocities and mean flow velocities of bilateral middle cerebral arteries in the statins groups significantly increased (P<0.05). The pulse index of bilateral middle cerebral arteries in statins group significantly decreased (P<0.05).The flow velocities and pulse index of bilateral middle cerebral arteries in control group don't change significantly(P>0.05). Conclusions Statins may have the effects of improving the situation of cerebral haemodynamics of patients with ischemic stroke.
6.Testing of full-field digital mammography

Objective To test the key indexes of full-field digital mammography to keep it in good condition.Methods The testing was carried out based on clinical experiences and GBZ 186—2007 Mammography Quality Control Testing Specification.Results The testing items and indexes were proposed to facilitate the staff to check and operate the machine.Conclusion Clinical safety of full-field digital mammography and image quality are both enhanced greatly.

8.Treatment of open tibial fractures of type ⅢB with unreamed locking intramedullary nails and skin flap grafting

Objective To investigate the methods for emergency treatment of open tibial fractures of type ⅢB. Methods From February 1999 to February 2002, 16 cases of open tibial fractures of type ⅢB (age range: 19 47 years; average age: 27 years; the skin defect area: 4.5cm ?3cm-14cm ?6cm) were treated with administration of antibiotics, debridement, stabilization by unreamed locking intramedullary nails, and finally transfer or transplantation of cutaneous or musculo cutaneous flaps. Autografting from the iliac crest was done for 4 cases with bone defects. Results The follow ups averaged 9 months (ranging from 6 months to 14 months). The survival rate of flaps was 100%and all were free of deep infection. All fractures healed within 6 months in 14 patients and within 10 months in 2 patients. According to the Johner Wruchs method, the functions of ankle and knee were excellent in 14 cases, and good in 2 cases. Conclusion Emergency management of open tibial fractures of type ⅢB with unreamed locking intramedullary nails and skin flap grafting results in good outcome.
9.Stereotactic excision of benign breast lesions using the Mammotome system

Objective To discuss the clinical value of u ltrasound-guided stereotactic biopsy and excision using the Mammotome system in the treatment of benign breast lesions. Methods A retrospecti ve analysis was made on the efficacy of ultrasound-guided Mammotome biopsy and e xcision in the management of 61 patients clinically diagnosed as having benign b reast lesions between October 2001 and October 2002. Results T he lesions were histologically confirmed as fibroadenoma in 51 patients and mast opathia in 10 patients. The operative time was 20.6?4.1 min. All procedures w ere performed smoothly. Of the 61 patients, no infection or breast deformity occ urred on short-term follow-up examinations (1 month) except for mild subcutaneou s hemorrhage in 3 patients, and no obvious scarring was seen on long-term follow -up (2 years). Recurrence was found in only 2 patients. Conclusions Treatment of benign breast diseases by the Mammotome system can offer comp lete excision of the lesion, minimal invasion, a low recurrence rate, and good c osmetic outcomes.
10.Influence of inhibiting the expression of integerin-linked kinase on the expression of connexin 43 in rat mesangial cells

Objective To investigate the effects of the expression of integerin-linked kinase (ILK) on connexin 43 (Cx 43) in rat mesangial cells (RMCs). Methods RMCs were divided into three different groups (6 for each group): RMC group, ILK-con siRNA group and ILK-siRNA group. ILK siRNA was synthesized, and then transfected into RMCs by LipofectAMIN 2000. RMCs were transiently transfected with ILK-con siRNA, ILK-siRNA and lysed 24h later, and mRNA was then extracted and detected by reverse transcription-polymerase chain reaction (RT-PCR) analysis using ILK and Cx43-specific primers, and an aliquot of protein from each sample was subjected to western blot analysis using ILK and Cx43 antibodies. Cells were seeded into 96-well plates (2?103 cells/well) and then transfected with ILK-con siRNA and ILK-siRNA. After incubation for 24, 48 and 72 hours, respectively, 20?l of 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) (5mg/ml) was added into each well and incubated for 4 hours. Subsequently, 150?l of dimethyl sulfoxide (DMSO) was added to each well to dissolve the formazan crystals, and the absorption at 492nm was measured. Results ILK mRNA and protein levels decreased by 30%-50% after being transiently transfected with ILK-siRNA, while Cx 43 mRNA and protein levels increased by 30%-40% and 60%-70%, respectively. The viabilities in ILK-siRNA infected RMCs at 24, 48 and 72 hours were significantly higher than that in ILK-con siRNA infected RMCs. Conclusion Inhibition of ILK pathway will up-regulate the expression of Cx 43 and the viability of RMCs, implying that the regulation of connexin 43 might possibly be achieved via ILK pathway.
