1.Clinical application of circulating microRNA in laboratory medicine

microRNA(miR) in body fluids shows good stability and the change in its expression profile and level is associated with cancer and other diseases.Thus,circulating miR has been proposed to be an useful biomarker in diagnosis of the onset,prognosis and risk of diseases.Although great discoveries have been obtained on all aspects of miR,there are some shortcomings existing in the present studies,such as inconsistent specimens,diverse detection methods,lack of standardization on data analysis and other defects,resulting in poor reproducibility of data and inconsistent clinical conclusion,that make circulating miR difficult to be used in clinical laboratory testing.So lots of work is needed for us to implement,such as standardizing sample selection,improving detection reagents and methods,and normalizing test results before we can ultimately establish a fast,easy,low-cost detection of circulating miR in the clinical laboratory.
2.Correlation study between resistin and the insulin resistance and the urinary albumin excretion rate

ObjectiveTo investigate the correlation between the level of resistin and the insulin resistance(IR) and the incorporation of microamount albuminuria(MA) of type 2 diabetes mellitus (T2DM).MethodsTwo hundred and twenty T2DM patients and 40 normal subjects (control group) were enrolled in this study.Two hundred and twenty T2DM patients were divided into 4 groups,IR group (group A,79 cases),non-IR group (group B,48 cases),IR complicating MA group (group C,51 cases),non-IR complicating MA group(group D,42 cases).The fasting serum resistin was measured by enzyme-linked immunosorbant assay (ELISA),and the fasting plasma glucose (FPG),glycosylated hemoglobin (HbA1c),fasting insulin (FINS) was also determined.ResultsThe level of fasting serum resistin in group A,B,C and D was higher than that in control group [(33.45 ± 1.37),(23.36 ± 1.47),(44.45 ± 1.39),(37.45 ±1.57) μ g/L vs.( 17.44 ± 1.26 ) μ g/L],and there was significant difference among 5 groups (P < 0.01 ).The level of fasting serum resistin in group A was higher than that in group B (P < 0.01 ),and the level of fasting serum resistin in group C was higher than that in group D (P < 0.01 ).The level of fasting serum resistin was both higher in group C and D than that in goup A and B (P< 0.01 ).Correlation analysis showed the fasting serum resistin was positively correlated with body mass index(BMI),FPG,HbA1c and FINS in group A(r =0.35,0.46,0.37,0.49,P <0.05),and the fasting serum resistin was positively correlated with HbA1c,BMI,systolic blood pressure and diastolic blood pressure in group C(r =0.45,0.32,0.37,0.29,P < 0.05 ).ConclusionsSerum resistin may participate in the process of IR and the formation of MA.It may become one of the diagnostic standard of the IR and one of the important index to estimate the MA.
4.The clinical significance of color digital fundus photography in the physical examination

Objective To study the color digital fundus photography of the fundus in the physical examination in screening and clinical importance to guide treatment.Methods 50 245 cases (100 490 eyes) healthy subjects with Topcon TRC NW100 digital color fundus camera to take pictures after the screening analysis.Results The number of the Inspector General,50 245 cases,45 373 cases of normal,accounting for 90.3%,a general description of those 359 cases,accounting for 0.7%,the positive findings in 4513 cases,accounting for 9.0%.Positive comparison between men and women of all age groups showed no significant difference ( P > 0.05 ).Positive findings of retinal arteriosclerosis in the detection rate of 7.4%,0.1% of the diabetic retinopathy,macular degeneration,0.1%,0.6% of retinopathy,optic atrophy 0.1%,the optic nerve C/D greater than 0.7%.Screening of the positive findings of the subjects to give timely and accurate diagnosisandtreatmentrecommendations.Conclusion Digitalcolorfundusphotography objectivity,reproducibility andnon-invasive characteristics of the image data to facilitate thestorage and exchange,andretinalmicrovascular disease couldbetter predictthe effects on cardiovascular and cerebrovascularevents,and therefore have important applications in the field ofhealthy value.
5.Study on preparation of ?-cyclodextrin inclusion compound of volatile oil from Fructus Amomi Rotundus

Objective: To study optimum inclusion process conditions for volatile oil from Fructus Amomi Rotunds. Methods: The study was carried out with orthogonal design. The process conditions were studied by determining the utilazision ratio of volatile oil from Fructus Amomi Rotundus. Results: The optimum preparation conditions for inclusion were established as: volatile oil:? CD was 1∶8, The inclusion temperature was at 50 ?C and inclusion time for 2h, The ultilizasion ratio of volatile oil was 79.7%. Conclusion: The method can be used for mass production.
6.Preparation and Quality Control of Weishu Emulsion

OBJECTIVE:To prepare weishu emulsion and to establish its quality control method.METHODS:Weishu emulsion was prepared with sodium carboxymethylcellulose,etc.as the adjuvants and the content of dyclonine hydrochoride was determined by UV-spectrophotometry.RESULTS:The linear range of dyclonine hydrochloride was2.0~12.0?g/ml(r=0.9999)and the average recovery rate was99.38%,RSD=0.47%,(n=6).CONCLUSION:The emulsion is feasible in preparation method and controllable in quality.

Objective To observe the influences of Yangxinshi pill on ET, NO, CRP in patients with angina pectoris belonging to manifestation of Qi-deficiency and blood stagnation. Methods 78 patients with angina pectoris were randomly divided into two groups. 40 patients in treatment group were treated with Yangxinshi pill, 4 pills once, three times a day 8 weeks. 38 patients in control group were treated with isosorbide mononitrate 8 weeks. The symptoms and the ET, NO, CRP level before and after treatment were recorded. Results Obvious drop of ET, NO, CRP were observed after treatment in the treatment group. The level of ET was reduced from (67.47?15.72)pg/mL to (37.97?7.82) pg/mL. The level of NO was increased from (78.24?13.81)?mol/mL to (96.25?12.72)?mol/mL. The level of CRP was reduced from (5.31? 2.02)mg/L to (3.42?2.17) mg/L. There was a significant difference between the two drugs (P 0.05). The total efficacy rate in improving symptom was 95%. Objectives A rat model sciatic nerve crush injury was reproduced in rat,and behavioral changes,pain threshold,thermal withdrawal latency(TWL)and the changes in c-fos expression in cornu dorsale medullae spinalis were examined,and the relationship between peripheral nerve regeneration and hyperpathia was explored.Methods 40 male Wistar rats weighing 180-200g were randomly divided into sciatic nerve injury group(group A,n=20)and sham-operation group(group B,n=20).The right sciatic nerve of rats in group A was crushed with a hemostat in the middle of the nerve for 30 seconds,until the nerve appeared translucent.The right sciatic nerve of rats in group B was only exposed without crush injury under the same anesthesia.Behavioral response,pain threshold and TWL were assessed on 18,21,24,27,30,33,36 and 39d after operation.c-fos expression was examined immunohistochemically on 21,27,33 and 39d after operation after rats were euthanized.Results The cumulative pain scores of group A were higher than those of group B at every time point(P
10.A comparison between endoscopic retrograde and magnetic resonance cholangiopancreatography for the diagnosis of pancreaticobiliary diseases

Objective To determine how endoscopic retrograde cholangiopancreatography (ERCP) compared with magnetic resonance cholangiopancreatography (MRCP) in the diagnosis of pancreaticobiliary diseases. Methods A total of 40 patients with suspected pancreaticobiliary diseases underwent both MRCP and ERCP. Images obtained from ERCP and MRCP were compared. Results Pictures of both the examinations in the 40 patients had come out satisfactorily revealing the pancreatic duct and the biliary tree. The sensitivity, specificity and accuracy were 87% (34/39), 100% (1/1) and 88% (35/40) in MRCP and 100% (39/39), 100% (1/1) and 100% (40/40) in ERCP, respectively, without significant differences between the two examinations. Conclusions Although MRCP offers a diagnostic means equivalent to ERCP, it cannot take the place of the latter as regards identification of biliary obstruction.
