Objective: To explore the path selection of national obligations in realizing the social security rights. Methods:Exemplification is applied to explore the problems in the realization of social security rights through the analysis of the specific circumstances of the operation of the New Rural Cooperative Medical System(NRCM). Results: Through analyzing the problems in the realization of social security rights under NRCM in China, the realization progress of social security rights not only have imperfections in legal system, but also have problems in insufficient fund and incomplete system, which lead to the low social security level of residents. Conclusion: To improve the realization of residents’ social security rights, the specific pathway was analyzed according to the pathway selection of fulfilling the China National Obligations. Pathway 1: enhance the legislation, implement the legal system of social security; pathway 2: based on the function display of administrative organ, enhance the supervision of administrative management; pathway 3: based on judicial remedy, secure the realization of social security right.

Plastic and cosmetic surgery is an emerging disci-pline which can create beauty with medical intervention.It can be say that plastic and cosmetic surgery can combine medicine with aesthetics and art.Plastic surgeon is the angel of this perfect combination.In this paper, the author discusses how to be a qualified plastic surgeon to real-ize the combination of medical, aesthetics and art through more than 30 years clinical experience and teaching practice in plastic and cosmetic surgery.

The scar tissue, resulting from aberrant process of wound healing , can bring about cosmetic deformeties and func-tional limitation, even the displacement of tissues and organs .The widely use of autologous fat graft in regenerative medicine offers a relatively new option for scar treatment .Lipofilling has improved symptoms and appearance of scars in several studies .The application , therapeutic effect and mechanism research of autologous fat graft for scar treatment are summarized in this paper .

Inflammatory bowel disease (IBD) is a group of diseases characterized by recurrent episodes of chronic intestinal inflammation including ulcerative colitis (UC) and Crohn's disease (CD).The prevalence of IBD has shown a rapid growth in China.Malnutrition is often seen in IBD patients,especially in those with CD.Appropriate diets and supplementation of trace elements,calcium,vitamin D,and probiotics are beneficial for disease remission and control.Nutrition support therapy can improve the nutritional status and meanwhile may facilitate the induction and maintenance of remission in CD patients.

Objective Uterine epithelial cells were isolated from pregnant mice and cultured in vitro , and exam-ined the effect of CD82 on the expression of integrin αV,β3, E-cadherin and β-catenin in the cells.Methods The uter-ine epithelial cells were primarily isolated from pregnant mouse uterus .The recombinant adenovirus containing mouse CD 82 gene which had been constructed in our lab infected the uterine epithelial cells .Immunocytochemistry was used to detect the protein expressions of integrin αV,β3, E-cadherin and β-catenin in the uterine epithelial cells , which were infected with CD82 adenovirus or not .Results 1.The purity of primary cultured cells was (93.2 ±0.6)%.2.The transfection efficiency of CD82 recombinant adenovirus was ( 92 ±4.5 )%.The adenoviral particles carrying CD 82 gene indeed ex-pressed CD82 gene and protein in the primary uterine epithelial cells after 24 hours or 48 hours.3.The uterine epithelial cells of pregnant mice on d4 expressed integrin αV, β3, E-cadherin and β-catenin.4.In contrast to the control group, when CD82 adenovirus infected cells , the uterine epithelial cells of pregnant mice on d 4 increased the expression of integrinαV,β3 and β-catenin protein, had no significant changes of E-cadherin.Conclusions CD82 may have effect on the ex-pression of integrin αV,β3 and β-catenin in mouse uterine epithelial cells before implantation .

Lumbar fusion is currently the major treatment of lumbar degenerative disease, spinal instability and discogenic disease etc. Transforaminal lumbar interbody fusion (TLIF) is a novel lumbar fusion technique in recent years, and with the progress of minimally invasive spinal surgery, minimally invasive TLIF (mini-TLIF) technique has also got access to rapid development, which has more advantages compared with traditional open TLIF. The authors review the indications and contraindications, surgical method, development and advantages and minimally invasive surgical assistant instruments of mini -TLIF.

[Objective]To evaluate the clinical value in treatment of subtrochanteric fracture of the femur by dynamic condylar screw(DCS).[Method]This study retrospectively reviewed 29 patients who suffered from subtrochanteric fracture of the femur.There were 18 males,11 females,with an average age of 52.2 years.Seven cases were caused by traffic accident,8 cases caused by falling from hight,14 cases caused by pedestrian injury.According to Seinsheimer classification,there were 5 cases of typeⅡ(2 of typeⅡB,3 of typeⅡC),12 cases of typeⅢ(7 of type ⅢA,5 of type ⅢB),5 cases of type Ⅳ,7 cases of type Ⅴ.All patients accepted the treatment with traction reduction and dynamic condylar screw fixation and were fellowed up for 9～24 months(average 16 months).Outcoms were assessed by Sanders traumatic hip rating scale and postoperative complications were analyzed.[Result]All patients obtained bone union in average 3.5 months(3～5 months),Only one case who had obtained bone union suffered subtrochanteric refracture 16 months after operation because of plate breakage.All patients were encouraged to early exercise and walk by nonweight-bearing.There was no postoperative infection,deep venous thrombosis,internal fixation loosening,limb shortening and varus deformity.According to sanders traumatic hip rating scale,18 cases were excellent,10 cases were good,1case was fair.The excellent and good rate was 96.5%.[Conclusion]In treatment of subtrochanteric fracture of the femur,DCS not only maintain satisfying reduction with reliable fixation,but also prevent femoral shortening,rotation and varus deformity,thus get good curative effect,fast recovery,satisfying function and so on.So it is one of the best choices to heal subtrochanteric fracture.

Objective To construct the recombinant retroviruses vector PCLNRX-ICN and to explore over-activation Notch1 on the growth of human colon carcinoma cells HT-29.MethodsICN(intracellular domain of Noctch1) genes were inserted into retrovirus expression vector pCLNRX.The expression vector pCLNRX-ICN and packaging vector pCL-10A1 were co-transfected into 293 package cells.The recombinant retroviruses were used to infect HT-29 cells.After being infected,proliferation of HT-29 cells was observed by MTT assay.The expression of c-Myc and ?-catenin detected by RT-PCR and Western Blot.ResultsRecombinant retrovirus vector pCLNRX-ICN was successfully constructed.Overactivation of Notch1 by over-expressing exogenous ICN significantly inhibited the growth of HT-29 cells,and down-regulated ?-catenin,a key regulator of Wnt signaling,in protein level but not in mRNA levels.However,the mRNA or protein levels of c-Myc were not affected by overactivation of Notch1.ConclusionOver-activated Notch1 signaling could inhibit the growth of HT-29 cells partly through down-regulation of Wnt signaling independent of c-Myc inhibition.

Objective To evaluate the role of real-time three-dimensional transesophageal echocardiography(RT3D-TEE) in non-valvular atrial fibrillation with left atrial appendage(LAA) stunning after catheter ablation.Methods Clinical and echocardiographic variables were analyzed by univariate regression in order to investigate the relationship between the group of LAA stunning and others in 28 patients after catheter ablation.Results Univariate analysis revealed difference in persisting time of atrial fibrillation[(16.4 ± 11.6)weeks vs (21.3 ± 18.6) weeks,P ＜0.05],left atrial diameter[(43.4 ± 8.3) mm vs (47.6 ± 5.9) mm,P ＜0.05 ],left atrial emptying fraction (0.38 ± 0.04 vs 0.30 ± 0.09,P ＜0.05).LAA emptying fraction based on three-dimensional volume measurements had significant difference (0.20 ± 0.03 vs 0.12 ± 0.02,P ＜ 0.001) between the group of LAA stunning and other cases.Conclusions LAA ejection fraction calculation by RT3D-TEE is feasible and more accurate than by clinical and other echocardiographic in LAA stunning after ablaton of atrial fibrillation.
