1.Experimental Study of Dermal Toxicity of Shangyangyu Ointment

OBJECTIVE:To study the toxic action of Shangyangyu ointment on animal skin.METHODS:Acute dermal toxicity test and dermal irritation test were conducted in healthy rabbits,and dermal sensitivity test was performed in guinea pigs.RESULTS:Shangyangyu ointment had no acute toxicity or irritation to the intact or damaged skin of rabbits and was not allergy-inducing to the skin of guinea pigs.CONCLUSION:Shangyangyu ointment is safe and nontoxic and can be used in clinic practice.
2.Impact of saiga horn capsule on blood pressure variability,pulse pressure and left ventricular mass in elderly patients with isolated systolic hypertension

Objective:To observe the efficacy of saiga horn capsule combining with benazapril on blood pressure variability,pulse pressure and left ventricular mass in elderly patients with isolated systolic hypertension (ISH).Methods:78 elderly patients with ISH were treated with saiga horn capsule combining with benazapril for 4 months.We compared the change of 24hours systolic blood pressure (SBP),24hours diastolic blood pressure (DBP),pulse pressure (PP) and left ventricular mass.Results:After four months treatment,reduction of 24 hour SBP variability,day SBP variability,DBP variability,night SBP variability day,24 hour and day average BP variability,pulse pressure and left ventricular mass had statistical signifi cance (P0.05).Conclusion:The elderly patients with ISH treated with saiga horn capsule and benazapril can lower blood pressure,pulse pressure more effective,therefore protect target organ better than with benazapril only.
3.Clinical efficacy of short term intensive insulin pump therapy and biosynthetic human insulin therapy and its effect on maternal and neonatal outcomes in gestational diabetes mellitus

Objective To investigate the clinical effect of insulin on gestational diabetes mellitus(GDM).Methods GDM patients were randomly divided into group A and group B,group B were treated with short term intensive insulin pump therapy,and group A were treated with insulin pump in short term combined with biosynthetic human insulin therapy.Results The body mass index and diabetes family history rates pre-pregnancy in group B were significantly higher than group A(P<0.05).Logistic analysis showed that family history of BMI pre-pregnancy and diabetes family history were independent risk factors for GDM drug treatment(P<0.05).Compared with the group B,the improvement of blood glucose levels and the incidence of complications in patients with group A were better than the former(P<0.05).Conclusion Rational application of insulin can improve the efficacy of clinical treatment of GDM, reduce the incidence of complications, with high safety.
4.Determination of Ursolic Acid in Zhike Chuanbei Pipa Syrups by HPLC

Objective:To establish a method to determine the content of ursolic acid in Zhike Chuanbei Pipa syrups. Methods:An HPLC method was used with a Cosmosil-C18 column(250 mm × 4. 6 mm, 5 μm). The mobile phase was methanol-water-glacial acetic acid-triethylamine(90∶10∶0. 03∶0. 06) with a flow rate of 1. 0 ml·min-1 , the column temperature was 25℃, and the detec-tion wavelength was set at 215nm. Results:The linear range of ursolic acid was 16-80μg·ml-1(r=0. 999 5, n=5), and the aver-age recovery was 98. 67℅ (RSD=1. 22, n=6). Conclusion:The method is simple, accurate and reliable with good specificity and reproducibility, and it can be used in the quantitative determination of ursolic acid in Zhike Chuanbei Pipa syrups.
5.Effects of acupuncture and moxibustion of Yangchi point on carpal tunnel syndrome

Objective To explore the difference of treating carpal tunnel syndrome (CTS) with needling Yangchi point and moxibustion on Yangchi point.Methods 38 cases of CTS were randomly recruited into a treatment group (19 cases) and a control group (19 cases).The treatment group was treated with electric acupuncture on Yangchi point and the control group was treated with moxibustion on Yangchi point.VAS (visual analogue scale) and GSS (overall symptom score table) scores were measured before and after the treatment.Results After the treatment VAS,GSS scores were (3.95± 1.87),(13.42±7.81) in the treatment group and (4.26±2.38),(14.63±9.97) in the control group,both decreased than those in the same group before the treatment [treatment group were (6.89± 1.56),(25.58±8.09) ; the control group were (5.79± 1.69),(20.37±8.67)],and the difference between the two groups was significant (t=-3.06,-3.39,P<0.05).3 months follow-up after the treatment,the value of VAS and GSS scores in treatment group were (3.29±2.31)% and (8.06±7.77); in the control group were (3.28±2.95) and (10.44±9.82),both decreased than those in the same group before the treatment [treatment group were (6.76± 1.56),(24.76±8.17); the control group were (5.78± 1.73),(20.33± 8.92) (P<0.05)].The difference of VAS score was not significant between the two group (t=-1.49,P>0.05); while the difference of GSS score showed statistical difference between the two groups (t=-2.64,P<0.05).Conclusion Acupunctureon Yangchi point can effectively improve pain,numbness and motor activity in patients with the carpal tunnel syndrome.

7.Burn Wound Infection:Causes and Countermeasures

OBJECTIVE To explore the risk factors and countermeasures of burn wound infection.METHODS The data of burn patients in hospital from 2005 to 2007 were retrospectively analyzed.RESULTS From 341 burn patients 257 got infected and total infection rate was 75.4%.Investigation shows that the probability e of infection in 3 days was 12.9%,but the rate became 72.7% after 4 to 10 days.The bigger the wound area,the higher the possibility to got infected.The infection rate was 43.8% when BSA was smaller than 20.0%.When BSA was bigger than 41.0%,the rate was up to 88.0%.When BSA was bigger than 61.0%,the rate was 100.0%.Except that,antibiotics and standard medical performance were the important factors related to burn infection.CONCLUSIONS To strengthen the aseptic concept among medical staff,pay attention to the environment of hospital,and the sterilization of the materials,debridement and drainage in time,and rational usage of antibacterial drugs,are the effective countermeasures to reduce burn wound infection.
8.The Effective Way for Doctor-patient Relations From Tense to Harmony

Setting up harmonious relations between doctors and patients is an important part of setting up the socialistic harmonious sociaty.But the tense relation between doctors and patients is still a very outstanding topic.This paper analyzes the reasons of the tense relation between doctors and patients from a new visual angle of the government.the social public and human right guarantee,and investigates the path of setting up harmonious relation between doctors and patients.It thinks the reasons of tense relation between doctors and patients lie in the clash of hospital public welfare and market survival way,the patients are easy to get harm but difficult to guarantee their rights.Therefore,setting up harmonious relations between doctors and patients not only depends on the medical service organization but also depends on the government undertaking the responsibility of the medical treatment guarantees,depends on the reform in the medical service system to make real medical service for common people,depends on the doctors' warm hearts and depends on establishing an organization of the patients who are in bad situation in order to contend with the medical service organization which is in good situation.
9.Correlation analysis between FXR gene polymorphisms and susceptibility to T2DM

Objective The pathogenesis of T2DM has not been elucidated.This study aimed to explore the association of FXR gene polymorphisms with the risk of T2DM in Eastern China population. Methods We collected 467 cases in the Health Examination Center of our hospital from January 2011 to December 2012.Ligase detection reaction was performed to test the genotypes in 240 T2DM patients and 227 controls.Unconditioned logistic regression analysis was used to explore the association of FXR gene polymorphisms with the risk of T2DM in different genetic models, after adjusting for age, sex, smoking and drinking status. Results Compared with AA genotype, GG significantly increased the risk of T2DM (P=0.017), while GA+GG approached marginal significance in association with T2DM (P=0.049) in rs1030454.For rs10860598, GA reduced the incidence of T2DM (P=0.025) compared with AA genotype, while GA+GG had marginal significance with T2DM (P=0.049).For rs11110411, CT or CT+CC could reduce the risk of T2DM compared with TT with P value of 0.016 and 0.024 respectively.Compared with AA, GA or GA+GG in rs17030270 was negative with the risk of T2DM with P value of 0.018 and 0.032 respectively. Conclusion FXR gene polymorphisms are associated with the risk of T2DM in northern Chinese Han population and the variation of FXR gene may play a role in the pathogenesis of T2DM.
10.The relationship between vascular endothelial growth factor, endostatin and type 2 diabetic nephropathy

Objective To investigate the possible association between vascular endothelial growth factor (VEGF), endostatin (ES) and type 2 diabetic nephropathy. Methods The level of serum VEGF and ES were measured by ELISA in eighty type 2 diabetic patients including thirty subjects without diabetic nephropathy , twenty with microalbuminuria and thirty with macroalbuminuria, and the correlation was analyzed. Results The level of serum VEGF and ES in patients with macroalbuminuria were both significantly higher than that of patients with and without microalbuminuria (P < 0.05). The level of serum VEGF and ES in patients with microalbuminuria were significantly higher than that of patients without diabetic nephropathy (P<0.05). Both VEGF and ES level were positively correlated with the level of microalbuminuria (mAlb) (r=0.226, P<0.01; r=0.491, P<0.01), and negatively correlated with the level of estimated glomerular filtration rate (eGFR) (r=-0.237, P<0.05;r=-0.620, P<0.01). There was a significantly positive correlation between VEGF and ES in patients with microalbuminuria and macroalbuminuria (r1=0.633, P1<0.01; r2=0.532, P2<0.05). However, there was no correlation between VEGF and ES in patients without diabetic nephropathy (r3=0.175, P3>0.05). Conclusion The levels of VEGF and ES in type 2 diabetic nephropathy had varying degree of increasing in different diabetic nephropathy stage , and closely related to mAlb and eGFR. Disequilibrium of VEGF and ES may speed up the progression of diabetic nephropathy.
