1. Diagnosis and treatment of primary plasma cell leukemia

Primary plasma cell leukemia (PPCL) is a rare, highly aggressive plasma cell disease characterized with a poor prognosis and a low response rate to conventional therapy. It occurs de novo in the absence of a prior history of plasma cell myeloma. Recently, the use of novel drugs such as bortezomib and the approaches such as stem cell transplantation may improve the outcomes of PPCL. This review summarizes the diagnosis and treatment of PPCL in order to provide reference information for clinical practice. Copyright© 2012 by TUMOR.
2.Effects of acupuncture and moxibustion of Yangchi point on carpal tunnel syndrome

Objective To explore the difference of treating carpal tunnel syndrome (CTS) with needling Yangchi point and moxibustion on Yangchi point.Methods 38 cases of CTS were randomly recruited into a treatment group (19 cases) and a control group (19 cases).The treatment group was treated with electric acupuncture on Yangchi point and the control group was treated with moxibustion on Yangchi point.VAS (visual analogue scale) and GSS (overall symptom score table) scores were measured before and after the treatment.Results After the treatment VAS,GSS scores were (3.95± 1.87),(13.42±7.81) in the treatment group and (4.26±2.38),(14.63±9.97) in the control group,both decreased than those in the same group before the treatment [treatment group were (6.89± 1.56),(25.58±8.09) ; the control group were (5.79± 1.69),(20.37±8.67)],and the difference between the two groups was significant (t=-3.06,-3.39,P<0.05).3 months follow-up after the treatment,the value of VAS and GSS scores in treatment group were (3.29±2.31)% and (8.06±7.77); in the control group were (3.28±2.95) and (10.44±9.82),both decreased than those in the same group before the treatment [treatment group were (6.76± 1.56),(24.76±8.17); the control group were (5.78± 1.73),(20.33± 8.92) (P<0.05)].The difference of VAS score was not significant between the two group (t=-1.49,P>0.05); while the difference of GSS score showed statistical difference between the two groups (t=-2.64,P<0.05).Conclusion Acupunctureon Yangchi point can effectively improve pain,numbness and motor activity in patients with the carpal tunnel syndrome.

5.Content Mensuration of Dehydroandrographolide in Ganmaoqing Capsule by HPLC

Objective To establish the determination of Dehydroandrographolide in Ganmaoqing Capsule. MethodsThe determination was carried out by HPLC with a KromasilC-18 column(250 mm ×4.6 mm,5μm),methanol-water(65:35)severed as the mobile phase,the speed was 1mL/min and the detection wavelength was at 254 nm. ResultsDehydroandrographolide showed a good linear relationship at the range of 0.04872 μg ~0.38976 μg(r2 =0.9995,n= 8);The average recovery of Dehydroandrographolide was 100.1%(RSD= 2.92%). ConclusionHPLC method was sensitive,accurate,reproducible,specific and could be used for quality control of Ganmaoqing Capsule efficiently.
6.Preliminary Study on Transduction of Tumor-infiltrating Lymphocyte with Tumor Necrosis Factor Gene and Its Clinical Trial

We selected defective retrovirus LXSN approved by the recombinant DNA Advisory Comittee, NIH, USA as a vector to study the transduction of human TIL with TNF gene, on the basis of which, we engaged in the clinical application. The results showed that the logarithmic growth phase was the optimal transducing time when the TIL were isolated and cultured from 9 to 25 days. It also showed that the transduction efficiency was correlated to the MOI value, the more MOI value, the more TIL being transduced. Successful gene inserted and expressing were confirmed by the polymerase chain reaction and the L929 cell test. The clinical trial of treatment for metastastic hepatic cancer suggested that the better therapeutic effectiveness and less side effect depend on the specificity of the TIL, the proper infusion pathway and the dose of the transduced TIL.

Objective To investigate the role of WNK4 gene Exon8 Ala589Ser polymorphism in the pathogenesis of essential hypertension. Methods This study included 259 hypertensive patients and 235 normotensive patients as control subjects. Exon8 Ala589Ser polymorphism in WNK4 was investigated by PCR-RFLP and confirmed by direct sequencing. Genotype, genotype frequencies and allele frequencies of the polymorphism were compared among patients with essential hypertension and control subjects. BACKGROUND:Recent studies have found that femoral neck angleαin patient with femoral neck hernial fossa increased significantly. However, whether the femoral neck hernia fossa can be used as imaging diagnostic indicator for femoroacetabular impingement syndrome remains controversial. OBJECTIVE:To evaluate the association between alpha angle and herniation pit in asymptomatic hip joints, influencing factors of the above indexes, and to provide evidence for the diagnosis of hip impingement syndrome. METHODS:Adults with asymptomatic hip joints from September 2013 to December 2015 were retrospectively studied. Alpha angles were measured on oblique axial MRI images. The incidence and size of herniation pit were determined by MRI. Size measures, prevalence, and statistical analyses were conducted regarding its association with age, gender, laterality (right or left hip). Intra-and inter-observer consistency was determined by intra-class correlation coefficient. RESULTS AND CONCLUSION:(1) 105 patients with asymptomatic hips were included, and 185 sides of hip MRI images were obtained. The range of age was 18-80 years old. The prevalence of herniation pit in asymptomatic hips was 21.6%. (2) The range of alpha angle was 27.6°–65.0°. Alpha angles of hip joints of 17 cases were ≥ 55°. (3) There was no association between alpha angle≥55° and the size of herniation pit (P>0.05). The prevalence of herniation pit was not correlated with age, gender and affected side (P=0.160, 0.157, 0.110). Inter-observer consistency of alpha angle was 0.523 between first measurements of first vs. second observer, respectively. Intra-observer consistency of alpha angle was 0.654, respectively. (4) There is no association between alpha angle ≥ 55 degrees and presence of herniation pit or demographic variables (age, gender and affected side). The presence of herniation pit and the increased alpha angle need to be interpreted with caution. Clinical symptoms and other examinations should be used in the diagnosis of hip impingement syndrome.
9.Clinical analysis of ischemic ophthalmopathy after cataract phacoemulsification

Objective To investigate the cause and treatment for ischemic ophthalmopathy after cataract phacoemulsification.Methods A total of 3000 patients with cataract underwent phacoemulsification were retrospectively analyzed,and 13 patients with ischemic ophthalmopathy.The ischemic optic neuropathy (ION) were in 8 cases,7 cases occurred in age related cataract,1 cases occurred in the dislocation of lens;The central retinal artery occlusion (CRAO) were occurred in 5 cases,1 cases of age-related cataract,2 cases of congenital cataract,2 cases of traumatic cataract.The patients were diagnosed with fundus,visual field and fundus fluorescein angiography respectively,and then were treated with dilation of blood vessels,nutrition nerves and symptomatic treatment.Results The visual acuity in 6 cases with ION after the treatment were improved in different degrees;The visual acuity in 2 cases with CRAO after treatment were improved,3 cases was not recovered.Conclusion Ischemic ophthalmopathy caused by phacoemulsification cataract surgery may be related to the main vascular disease or optic nerve ischemia and hypoxia during operation,and with early detection and treatment,visual acuity can be improved to some extent.

Objective To investigate the expression and the effect of nuclear factor-?B(NF-?B) in neonatal sepsis.Methods We separated 77 newborn infants into 3 groups, which were septic group (26 cases),non-septic group (31 cases) and control group (20 cases). NF-?B existed in PBMC was detected in 3 different periods, including at admission, after the 24th hour and 48th hour of admission, of the septic group and the non-septic group by flow cytometry.
