Tissue engineering provides urologists a new way to fix or reconstruct the impaired organs.Reconstitution of corporal bodies of penis with engineered tissue substitutes has been applied in animal models.In hypospadias reconstruction,the use of engineered tissue substitutes has been applied clinically.The clinical application of bladder tissue substitutes has been ongoing phase II clinical trial.Great progress has been made in renal replacement therapy with clinical application of human progenitor cells in hemofiltration units,and the engineered intracorporeal renal replacement unit will come true by additional studies.The current status of tissue engineering in clinical practice of urology is reviewed in this paper.

Objective To investigate the efficacy of interferon alpha (IFNα) plus adefovir dipivoxil (ADV) and tebivudine (LdT) monotherapy for patients with HBeAg positive chronic hepatitis B (CHB) who have poor response to IFNα treatment.Methods A total of 86 HBeAg positive CHB patients admitted to the Sixth People' s Hospital of Shaoxing during February 2010 and April 2013 were enrolled in the study.All the patients received IFNα monotherapy for 24 weeks and had poor responses.The patients were voluntarily divided into three groups:IFNα monotherapy group (n =21),IFNα plus ADV group (n =30) and LdT monotherapy group (n =35).Chi-square test was used to compare ALT normalization rates,rate of HBV DNA load ＜ 500 copies/mL,HBeAg seroconversion and HBsAg seroconversion rates among three groups.Results After 48 weeks of treatment,the ALT normalization rate in IFNα monotherapy group was 52.6％ (10/19),which was lower than those in IFNα plus ADV group (86.7％,26/30) and LdT monotherapy group (84.8％,28/33) (x2 =6.913 and 6.361,P ＜ 0.05).The rate of HBV DNA load ＜500 copies/mL in IFNα monotherapy group was 26.3％ (5/19),which was lower than those in IFNα plus ADV group (60.0％,18/30) and LdT monotherapy group (54.5％,18/33) (x2 =11.33 and 3.895,P ＜0.05).No HBeAg negative conversion or seroconversion was observed in IFNα monotherapy group,but it was observed in 6 (20.0％,6/30) patients in IFNαt plus ADV group and 7 (21.2％,7/33) patients in LdT monotherapy group (x2 =4.330 and 4.657,P ＜ 0.05).No HBsAg seroconversion was observed in three groups.There were no statistical significant differences in ALT normalization rates,rate of HBV DNA load ＜ 500 copies/mL,HBeAg seroconversion and HBsAg seroconversion rates between IFNαt plus ADV group and LdT monotherapy (x2 =0.042,0019 and 0.064,P ＞ 0.05).Conclusion For patients with HBeAg positive CHB who had poor response to IFNα treatment,IFNα plus ADV therapy and LdT monotherapy have the same efficacy in improvement of both liver function and virological response.

Objective To detect Snail and Raf kinase inhibitor protein (RKIP) expressions in NSCLC (Non-Small Cell Lung Cancer) and their relationship with the pathological characteristics of tumor. Methods Immunohistochemistry was used to detect Snail and RKIP expression in 124 NSCLC samples and 67 paraneoplastic normal lung tissue samples. Snail and RKIP positive expression rates were compared upon clinicopathologic characteristics. Correlation of Snail expres-sion with RKIP expression was also analyzed. Results Positive expression rate of Snail was 79.03%in lung cancer, which is higher than that in normal lung tissue 19.40%(χ2=63.538, P＜0.01);positive expression rate of RKIP was 36.29%in lung cancer which is lower than that in normal lung tissue 77.61%(χ2=29.716,P＜0.01), Snail and RKIP expressions are corre-lated with tumor differentiation, TNM stage, lymph node metastasis and postoperative survival time ( P＜0.05, respectively) . Snail protein expression is negatively correlated with RKIP expression in NSCLC (P＜0.05). Conclusion High Snail ex-pression and the low RKIP expression might be important biological markers for invasion and metastasis of NSCLC, which might be used as important prognostic indicators in NSCLC.

BACKGROUND:Bone marrow mesenchymal stem cels have the ability to differentiate into a variety of non-hematopoietic tissue cels. Effects of magnetic fields on the differentiation of bone marrow mesenchymal stem cels have attracted a lot of attention in recent years. OBJECTIVE:To summarize the effects of magnetic fields on the differentiation of bone marrow mesenchymal stem cels towards osteoblasts, chondrocytes, adipocytes, nerve cels and cardiomyocytes, which provide references for the research and application of tissue engineering seed cels as wel as the clinical applications of magnetic fields. METHODS:The first author performed a data retrieval of PubMed and CNKI databases from 2000 to 2015 to search the articles addressing the effects of magnetic fields on the differentiation of bone marrow mesenchymal stem cels, and reviewed the literatures systematicaly. Finaly, 40 articles were chosen for further analysis. RESULTS AND CONCLUSION:Magnetic fields can promote the differentiation of bone marrow mesenchymal stem cels towards osteoblasts, chondrocytes, nerve cels and cardiomyocytes, and inhibit the differentiation of bone marrow mesenchymal stem cels towards adipocytes. There are optimal frequency and intensity in the induction of magnetic fields on the differentiation of bone marrow mesenchymal stem cels. In general, low-intensity and low-frequency magnetic fields have more obvious effects on bone marrow mesenchymal stem cels. The facilitation of magnetic fields on the differentiation of bone marrow mesenchymal stem cels is also a time-dependent behavior.

Objective To assess the efficacy and safety of emergency transcatheter arterial embolization in the management of intractable postpartum hemorrhage. Methods Twenty-five patients with intractable postpartum hemorrhage were undertaken superselective catheterization into the bilateral internal iliac arteries or uterial arteries to find the causes and sites of bleeding through DSA and then followed by arterial embolization with gelfoam particles. Result All of the 25 patients with obstetrical bleeding were successfully controlled by TAE, the procedure lasted for 25 - 60 min, (mean 42.5 ? 4.6 min); with both catheterization and bleeding halt successful rates of 100%. Objective To investigate the effects of painless ward standardized pain management on pain control and postoperative recovery of patients with inguinal hernia after surgery.Methods From January 2015 to January 2016,44 cases of inguinal hernia in Department of General Surgery,First People's Hospital of Huainan,control group (22 cases) and observation group (22 cases) were retrospectively studied.The patients in observation group underwent painless ward standardized pain management and those in control group underwent traditional pain mnanagement model.Pain score after surgery,time for get out of bed after surgery,postoperative intestinal exhaust time,duration of night sleep,and satisfaction degree for pain-control were comparrd between two groups of patients.Results For pain score after surgery,the observation group 3 h after operation,postoperative 1 d,2 d pain score respectively (3.46 ± O.56),(2.72 ± O.21),(2.20 ± O.43) points,the control group 3 h after operation,postoperative 1 d,2 d pain score respectively (4.48 ± 1.52),(3.55 ± 0.40),(2.80 ± 0.40) points,there were significant difference between two groups (P ＜ 0.05).For sleep time,the observation group on the day of surgery,postoperative 1 d,2 d sleep time respectively (5.23 ± 0.98) h,(6.57 ± 0.54) h,(7.50 ± 0.54) h,the control of sleep time of 2 d group on the day of surgery,postoperative 1 d,respectively (4.35 e 1.28) h,(5.17 ± 0.45) h,(6.72 ± 0.61) h,there were significant difference between two groups (P ＜ 0.05).The satisfaction rate of pain control in the observation group was 86.3％,which was higher than that of the control group 50.0％ (P ＜ 0.05).The observation group first ambulation time,anal ventilation time,hospital stay were (10.5 ± 2.0) h,(16.8±2.1) h,(6.6±1.4) d,the control group for the first time,ambulation time,anal ventilation time,hospital stay were (15.9 ± 3.O) h,(22.6 ± 3.3) h,(7.5 ± 1.2) d,there were significant difference between two groups (P ＜ 0.05).However,there was no difference between the two groups in the total cost of hospitalization.Conclusions Application of painless ward standardized pain management for patients with inguinal hernia after surgery can shorten time for get out of bed after surgery,postoperative intestinal exhaust time,prolong duration of night sleep,improve satisfaction degree for pain-control during hospitalization.

Objective To study the risk factors of MRI for the prediction of collapse in patients with avascular necrosis of the femoral head.Methods Twenty-two patients (39 hips) diagnosed avascular necrosis of femoral head by MR were enrolled in our study.The following MR appearances were evaluated:bone marrow edema,joint fluids,signal intensity and location of the lesion.The volume and surface area of the necrosis zone were calculated.The time of follow-up was 18-84 months (median,25 months).Logistic regression analysis was used to predict the risk factors by SPSS 13.0.The maximum value of Youden index was selected as the critical point to predict the collapse of femoral head and to define the sensitivity,specificity and accuracy.Results In the 39 hips with femoral head necrosis,21 hips had collapse.Bilateral collapse occurred in 5 cases.In 25 hips with the necrosis surface larger than 25％,collapse occurred in 21 (84％); In 8 hips with the volume of femoral head necrosis larger than 30％,collapse occurred in all cases; 1n 33 hips with the necrosis locating at the superolateral quadrant,collapse occurred in 21 (63.6％); In 22 hips with necrotic areas showing heterogeneous signal intensity,collapse occurred in 18(81.8％) ;In 25 hips with large amount of joint effusion,collapse occurred in 16 (64％) ;in 18 hips with bone marrow edema,collapse occurred in 13 (65％).Joint fluid,heterogeneous signal intensity and lesions in the superolateral quadrant,volume ratio,and area ratio were the high risk factors,while bone marrow edema was a relatively low risk factor.The area under ROC curves for area ratio of NASA was greater than that for volume ratio (0.987 vs 0.902).When the critical value for area ratio was 26.7％,the true positive rate was 95.2％,true negative rate was 94.4％,and Youden's index was 0.896.Conclusions The collapse of necrosis of femoral head may result from many factors.The femoral head was easy to collapse when it had large enough area of necrosis and mixed signal intensity,a large amount of joint effusion,bone marrow edema,and superolateral quadrant location.The critical value for area ratio to predict the collapse of femoral head was about 26.7％.The area ratio is more accurate than volume ratio in predicting the collapse of necrosis of femoral head.
