1.Comparison of effect of total hip replacement and artificial head of femur replacement in patients with failed femoral neck fracture

Objective To study effect of total hip replacement and artificial head of femur replacement in patients with failed femoral neck fracture.Methods 62 cases with failure of the conservative treatment or fixed steel femoral neck fracture were enrolled as research subjects.They were randomly divided into the two groups,the control group (n =31) and the observation group (n =31).They were operated through hemiarthroplasty in the control group,while they were operated through total hip replacement in the observation group.The operative time,blood loss,drainage,hospital stay,hip function,VAS score and quality of life changes,early complications were observed.Results The operative time,blood loss,drainage,length of hospital stay were (87.4 ± 21.3) min,(209.6 ± 34.5) mL,(174.2 ±29.4)mL and (17.8 ±5.6)d in the control group and (104.2 ±33.7)min,(229.1 ±35.1)mL,(182.2 ± 30.5) mL and (18.2 ± 6.2)d in the observation group.The operative time in the observation group was significantly longer than that of the control group (t =4.927,P < 0.05).The blood loss,drainage,length of hospital stay were no significant difference between the two groups (t =0.386,0.413,0.775,all P > 0.05).The excellent rate of hip function was 83.9% (26/31) in the control group and 96.8% (30/31) in the observation group,the observation group was significantly higher (x2 =6.92,P < 0.05).Score changes of VAS and quality of life were (23.4 ± 9.2) and (36.1 ± 11.2) in the control group and (35.1 ± 10.6) and (42.8 ± 12.6) in the observation group.Score changes of VAS and quality of life in the observation group were higher (t =6.032,3.924,all P < 0.05).The early complications occurred in the control group was 25.8% (8/31),which in the observation group was 9.8% (3/31),the early complications in the observation group was significantly lower than that in the control group (x2 =13.64,P <0.01).Conclusion For patients in the conservative treatment or femoral neck fracture fixation failure or steel,the use of total hip replacement surgery was complicated,but it can significantly improve hip function,reduce pain,improve quality of life and reduce early complications.
2.The effects of combination anti-inflammatory and anti-oxidation drugs in acute stroke

Objective Research the effect of anti-inflammatory and anti-oxidation drugs on acute stroke patients. Methods 128 patients with acute ischemic stroke are divided into two groups- combined treatment group (71) and control (57). Test serum biomarkers and evaluate neurological function and living ability before and after therapy, compare between groups and intra-group. To observe the effects of combined therapy on serum biomarkers and prognosis in acute stage. Results ( 1 ) MMP-9 of combined treatment group (3.23 ±0. 99) ng/L is higher than control (2. 82 ± 4. 21 )ng/L before therapy (P <0. 05). (2) The level of pre-treatment ox-LDL-Ab in combined treatment group ( 0. 08 ± 0. 01 ) U/ml higher than posttreatment (0. 07 ±0. 01 ) U/ml (P < 0. 05 ). The level of pre-treatment NIHSS in control group (5.76 ±6.61) score higher than post-treatment (4.22 ±5.45) score,P<0.05. Conclusion Combined treatment can degrade serum MMP-9 and ox-LDL in acute ischemic stroke patients, but it can't obviously improve the earlier prognosis.

Overexpression of the epidermal growth factor receptor (EGFR) is a common characteristic of head and neck squamous cell carcinomas (HNSCC) , and initiates important signal transduction pathways in carcinogenesis. Now the EGFR is a validated target for cancer therapies in HNSCC. However, the effect of EGFR-targeted therapies is only modest because of primary and/or acquired resistance. Therefore, an improved understanding of the molecular mechanisms of resistance to EGFR inhibitors may establish new treatment options to overcome resistance. In this review, the molecular mechanisms of resistance and the strategies to overcome it were summarized.

8.The Mechanisms of Cerebral Vasospasm Caused by Subarachnoid Hemorrhage and Its Clinical Treatment Progress

Cerebral vasospasm is one of the most severe complications of subarachnoid hemorrhage. Its incidence is as high as 30-90%. It often causes severe regional cerebral ischemia or delayed ischemic brain damage, even resulting in cerebral infarction, and it thus becomes the primary cause of mortality and disability. In recent years, with the development of studies, people have realized that the oxyhemoglobin, inflammatory reaction, accumulation of vasoconstrictive substances, apoptosis, blood hypercoagulability and blood vessel cell proliferation play important roles in the development of cerebral vasospasm. Although its mechanism remains unclear, better effects have been achieved by using related treatment methods according to the available pathogenesis.
9.The feasibility of laparoscopical left hepatectomy with the method of Glisson pedicle transsection

Objective To explore the feasibility of laparoscopical left hepatectomy with the method of Glis-son pedicle transsection.Methods Clinical data of anatomical left hepatectomy patients with the method of Glisson pedicle transsection from February 2012 to April 2012 in our hospital were retrospectively analyzed.Results All patients were completely suffered laparoscopical left hepatectomy with the method of Glisson pedicle transsection. Operation time form 2-4 hours,postoperative hospitalization for 6-7 days,all these patients were cured and discharged, without any complication of bile leakage, hemorrhage, peritonitis, intestinal obstruction, postoperative liver function recovered rapidly.There was no death case.Conclusion It is safe and feasible of laparoscopical left hepatectomy with the method of Glisson pedicle transsection and should be worth to popularize.
10.Principles and strategies for the prevention and treatment of parathyroid and larynge-al nerve injuries during thyroid surgery

Surgery is the major treatment option for malignant tumors and some benign neoplasms of the thyroid, most of which are differentiated thyroid carcinoma. Despite the progresses that have been made in surgical techniques, iatrogenic injuries of the parathy-roid and laryngeal nerves, including superior and recurrent laryngeal nerves, could not be completely avoided in the surgical manage-ment of thyroid tumors. In this review, the causes of intraoperative injuries of parathyroid and laryngeal nerves are systematically ana-lyzed with respect to types and extents of surgical operations, changes in topical anatomy, and secondary thyroid surgeries. The princi-ples and strategies for protecting and restoring injuries of the parathyroid and laryngeal nerves are also elucidated for the effective prevention and adequate treatment of these major complications in the thyroid surgery.
