Objective To study the role of chromoendoscopy with toluidine blue to diagnose gastric cancer and precancerous change.Methods572 cases were randomized into two groups.288 cases in testing group took gastroscopy with 0.1% toluidine blue sprinkles,while ordinary gastroscopy was applied in 284 cases of control group.We took biopsy in all of those cases for pathology examination.The biopsy was taken from heterochrome area if heterochrome area appeared in testing group,otherwise from suspected affection.ResultsDetection rate of gastric cancer of test groups(9.72%)was higher than that of control groups(5.28%).Both of the methods were significantly different(P

The addition of long-term noninvasive positive pressure ventilation (NPPV) to a pulmonary rehabilitation (PR) program in stable patients with advanced chronic obstructive pulmonary disease (COPD) can enhance the effects of PR, continuely improve exercise tolerance and dyspnea, stabilize pulmonary function, enhance health-related quality of life. Moreover, it maybe improve the prognosis of the patients with COPD. This article introduced the implement of the combination of PR and NPPV in stable patients with advanced COPD.
5. Modeling method for allocation optimization of military medical equipments

Objective: To optimize the allocation scheme of military medical equipments based on the characteristics of modern war and the progression of the military medical equipment of People's Liberation Army (PLA). Methods: Aiming to optimize the allocation of military medical equipment, we designed the integrated framework of the allocation system and analyzed the need of optimized allocation. Using the linear programming and queuing theory, we established the optimized allocation models for both medical treatment equipments and evacuation equipments. Results: Based on the health service characteristics of military medical equipments, the optimized allocation models for medical treatment equipments and evacuation equipments were successfully established. The application of the established models was studied in the medical service station of the division level. Conclusion: The optimized allocation scheme of military medical equipment has been proposed in the present study, which provides evidence for decision-making on the future development of military medical equipments of PLA.
6.A clinical study on the efficacy of oral minirin in transsphenoidal surgery patients with central diabetes insipidus

Objective To observe the effect of oral minirin in postoperative transsphenoidal surgery patients with central diabetes insipidus. Methods The changes in the urine volume and osomlality after two weeks of minirin medication (0. lmg, 3 times each day) were observed in 34 patients with central diabetes insipidus underwent transsphenoidal surgery. Results After two weeks of minirin therapy,the average daily urine volume decreased from 7985.40 ±410. 36 ml to 2277. 87 ± 328. 94 ml,and the average plasmas osmolarity increased from 301. 68 ± 3. 59 ml/d to 313. 26 ±4. 87 mOsm/ kg. No adverse reaction was observed during the therapy. Conclusions Minirin is effective and safe in the therapy of postoperative transsphenoidal surgery patients with central diabetes insipidus.

8.Analysis of the 5th and 8th exon mutation in presenilin-1 gene in sporadic Alzheimer disease

BACKGROUND: Most cases of early-onset familial Alzheimer disease (AD) are associated with presenilin-1 mutation, which mainly occurs in the 5th and 8th exons. Except for the onset age and family history, sporadic AD is basically the same as early-onset familial AD regarding clinical manifestations and pathological characteristics. Is its pathogenesis also related to the mutation of presenilin-1 gene?OBJECTIVE: To probe into the relationship between sporadic AD and presenilin-1 mutation.DESIGN: Case control analysis based on clinical diagnosis.SETTING: Neurology Department of the First Hospital Affiliated to Sun Yat-sen University.PARTICIPANTS: Outpatients who came to the Memory Obstacle Clinic of the First Hospital Affiliated to Sun Yat-sen University between April 1998 and June 2000 but without familial history of AD were selected and divided into 3 groups: [1] AD group: There were 68 cases confirmed as probable having AD according to the clinical diagnostic standard of NINCDS-ADRDA. [2] Vascular dimentia (VaD) group: There were 25 cases confirmed as probable having vascular dimentia according to the clinical diagnostic standard of NINCDS-ADRDA. [3] Normal control group: There were 20 healthy elders (spouses of some patients in the above two groups)without dimentia.METHODS: The conventional phenol extraction method was used to withdraw DNA from peripheral blood, and polymerase chain reaction-single strand conformation polymorphism analysis (PCR-SCCP) was applied to detect the mutation of the 5th and 8th exons in presenilin-1 gene.RESULTS: All the 5th and 8th exons in presenilin-1 gene were proved to be composed of two single strands and a double strand by PCR-SCCP analysis.No abnormal electrophoresis was found, suggesting no that mutation occurred.CONCLUSION: Sporadic AD differs from familial AD in pathogenicity;mutation does not occur or seldom occurs in the 5th and 8th exons in presenilin-1 gene, thereby cannot be considered as an important causive factor for sporadic AD.
9.Protective effect of hongjingtian on hippocampal area and dentate gyrus of complete cerebral ischemia-reperfusion in rats

BACKGROUND: Hippocampal formation of brain, a cerebral area related with learning and memory, is closely related to spatial cognitive activity.Peroxidative stress following the onset of cerebral ischemia can induce DNA injury in hippocampal 'area and dentate gyrus, the fall of the ability of DNA plerosis and correspondingly a decline in the function of learning and memory.OBJECTIVE: To investigate the effect of hongjingtian on the expression of nucleic acid of hippocampal area and dentate gyrus, the learning and memory area of rats with complete cerebral ischemia-reperfusion injury.DESIGN: Randomized controlled study.SETTING: Central Laboratory of Armed Police Medical College.PARTICIPANTS: The experiment was completed at the Central Laboratory of Armed Police Medical College from April 2002 to April 2004. Totally 60 Wistar male rats were selected and divided randomly into 5 groups Model group: Rats were perfused daily with distilled water of a volume the perfused daily with distilled water of a volume the same as that in medication group.METHODS: Rats in each group were perfused incessantly for 7 days.Modified Pulsinelli-4 vessel blocking method was used to reproduce the rat model of acute complete cerebral ischemia-reperfusion. Rats in sham-operation group were not treated with the operation of burning vertebral artery and clipping common carotid artery. The changes of DNA and RNA expressions in cerebral hippocampal area and dentate gyrus were observed with acridine orange staining method after model establishment.MAIN OUTCOME MEASURES: Expression of DNA and RNA in cerebral hippocampal area and dentate gyrus of rats in each group.RESULTS: All 60 rats entered the final analysis. DNA and RNA in shamoperation group distributed evenly, border of fluorescent reflex was legible and response intensity was strong. Border of DNA and RNA fluorescent reflex was illegible and response intensity was obviously weak in model ly, border of fluorescent reflex was legible and response intensity was group was not of obvious difference as compared with that in model group.CONCLUSION: The illegibility of DNA and RNA fluorescent response in operation group is related with the breakage of DNA chain induced by oxidative stress during cerebral ischemia-reperfusion injury. Border of DNA and RNA fluorescent reflex in hippocampal area and dentate gyrus is legihibit the breakage of DNA chain induced by oxidative stress and protect learning and memory function in hippocampal area and dentate gyrus of rats with complete cerebral ischemia-reperfusion.
10.Efficacy of jaw thrust device in the upper airway obstruction

Objective To evaluate the efficacy of jaw thrust device in the upper airway obstruction. Methods Thirty-five ASA Ⅰ or Ⅱ patients aged 18-64 yr with body mass index < 30 kg/m~2 undergoing elective surgery under general anesthesia were included in this study. The anesthesia face mask was placed and held tightly by 4 straps of JTD and connected to the anesthesia machine. Anesthesia was induced with midazolam, propefol, fentanyl and atracurium. When muscle relaxation was achieved, tracheal intubation was not performed. The airway obstruction was assessed by airway peak pressure and stridor score (breathing sounds detected by auscultation over the trachea). And then tracheal intubation was performed, and the patients were mechanically ventilated. Results There was no significant difference in airway pressure and stridor scores between JTD and jaw thrust maneuver. Conclusion JTD can effectively lift the jaw and improve the upper airway obstruction.
