1.Clinical application of ERCP combined with biliary microlithiasis exploration in acute cholangitis

Objective To study the clinical application of endoscopic retrograde cholangiopancre-atography(ERCP)combined with biliary microlithiasis exploration in acute cholangitis.Methods The clinical data of consecutive 54 cases of acute cholangitis were analyzed.Before ERCP,all cases were examined by routine examinations,including abdominal uhrasonography,CT or magnetic resonance cholangiopancre atography(MRCP).The bile Was collected through ERCP or endoscopic mtsobiliary drainage(ENBD),then all the samples were choked by polarization microscope.Results Thirty-six cases(66.67%)out of 54 patients with common bile duct(CBD)stones or dilation of common bile duct were diagnosed by routine examination.ERCP Was performed in all patients,only 3 cases failed,and the rest were successful.Through ERCP,39 cases with CBD stones(76.47%),4 cases with CBD inflamnmtory smcmre were diagnosed,and 12 cases were normal.Among the 12 cases,9 cages were found with biliary microlithiasis by polarization microscope.The diagnostic effectiveness of ERCP combined with biliary microlithiasis exploration was highest(94.12%),and there Was significant difference between them(P<0.05).Conclusions It may improve the diagnostic accuracy to combine ERCP with biliary microlithiasis exploration in CBD stones.Biliary microlithiasis may play an important role in the pathogenesis in unknown aetiology acute unexplained cholangitis.
2.Human papillomavirus vaccination behaviors among gynecological outpatients based on extended unified theory of acceptance and use of technology

Objective:
To investigate the factors affecting human papillomavirus (HPV) vaccination behaviors among gynecological outpatients based on extended unified theory of acceptance and use of technology (UTAUT2), so as to provide insights into the development of HPV vaccination behavioral interventions.
Methods:
Patients at ages of 45 years and younger that were admitted to the outpatient department of gynecological of Shanxi Provincial People's Hospital from October 2021 to August 2022 were recruited, and the factors affecting HPV vaccination behaviors were identified using UTAUT2.
Results:
A total of 431 female outpatients were enrolled, including 163 patients at ages of 36 to 45 years (37.82%), 272 cases with an educational level of college degree and above (63.11%) and 253 patients with per capita monthly household income of more than 3 000 Yuan (58.70%). The coverage of HPV vaccination was 24.36%, and the main cause of non-vaccination was difficulty in high-valent HPV vaccine appointment. Price value, social impact and efficacy expectation posed a positive impact on HPV vaccination behaviors via intention of vaccination (β=0.11, 0.08, 0.07, all P<0.05) and intention of vaccination and effort expectancy (β=0.10, 0.07, 0.06, all P<0.05), and effort expectancy played a mediating effect between intention of vaccination and vaccination behaviors (β=0.28, P<0.05).
Conclusion
Efficacy expectation, social impact, price value, intention of vaccination and effort expectancy may positively affect HPV vaccination behaviors.
3.Analysis of T lymphocyte subsets in patients with primary biliary cirrhosis

Objective To investigate the significance and characteristics of T lymphocyte subsets and co-stimulatory CD28 in peripheral blood of patients with primary biliary cirrhosis. Methods Tri-colour flow-cytometry was used to detect the levels of T lymphocyte subsets in peripheral blood in 98 patients with primary biliary cirrhosis and 30 age and gender matched healthy controls. Results Compared to control group the percentage of CD4+ T increased and CD8+ T lymphocyte decreased in the PBC group. The CD4+/CD8+ ratio in the PBC group was higher than that in the control group (P<0.05). And the percentage of CD4+CD28- T cells and CD8+CD28- T cells increased, too (P<0.05). Conclusion There are immunological abnormalities in PBC and the expression of co-stimulator CD28 is significantly decreased. CD8+CD28-T lymphocytes may have immune regulatory effect in PBC.
4.Clinicopathological features and multivariate analysis of prognostic factors for patients with gallbladder cancer

Objective To explore the clinical pathologic characteristics of gallbladder cancer and prognostic factors for patients with gallbladder cancer.Methods We retrospectively reviewed 108 patients diagnosed pathologically with gallbladder cancer and treated in HuBei Three Gorges Polytechnic Medical College,Yichang Central People's Hospital and Yichang First People's Hospital from January 2003 to December 2012.Kaplan-Meier method was used to estimate survival.The association between the clinicopathologic characteristic and the prognosis in gallbladder cancer patients was assessed using Log-rank test.Cox proportional hazards model was used to determine independent risk factors.Results All patients were followed up from 3 to 102 months,the median follow-up time was 46 months.There were 77 deaths (71.3%),31 patients are alive.Survival time after operation was from 4-98 months.The mean and the median survival time were 28.4 months and 17 months respectively.The 1,2,3 and 5-year overall survival rates were 54.2%,37.5%,31.3% and 21.6%.Multivariate analysis indicated independent risk factors were such as incidental gallbladder cancer,lymph node metastasis,poor differentiation.Conclusions Gallbladder cancer is a malignancy with poor prognosis.Incidental gallbladder cancer,lymph node metastasis,tumor poor differentiation are independent risk factors.
5.Ethacrynic acid promotes apoptosis in lung cancer A549 cells when combined with cisplatin chemotherapy

Objective To investigate the killing effect of ethacrynic acid (EA) on lung cancer A549 cells derived spheres and explore the underlying mechanism.Methods A549 spheres were cultured in serum-free medium,and the protein expression of CD133,SOX2,EpCAM and ABCG2 was detected by Western blotting.MTT assay was used to evaluate the cell viability of A549 spheres and A549 cells after treated by 1,2,5,10 and 20 mg/mL cisplatin (DDP) for 48 h.The activity of glutathione S-transferase (GST) was measured by colorimetric method after A549 spheres were treated with 10,50,100 and 200 μmol/L EA,respectively.Flow cytometry,Western blotting,real-time PCR and luciferase assay were used to analyze the levels of cellular reactive oxygen species (ROS),formation of A549 spheres,mRNA and protein expression levels of β-catenin,Sox2 and ABCG2,and promoter activity of β-catenin upon 200 μmol/L EA treated cells for 48 h.A549 sphere was infected with β-catenin adenovirus for 24 h,followed by 200 μmol/L EA treatment (in presence or absence of 5 mg/mL DDP) for 24 h.The expression of β-catenin,Sox2 and ABCG2 at mRNA and protein levels was detected by real-time PCR and Western blotting,and cell growth of A549 spheres was evaluated by MTT assay.Results The A549 spheres,with high expression of tumor stem cells markers CD133,SOX2,EpCAM and drug resistance related molecule ABCG2,and resistance to DDP at different doses,were successfully derived.After 200 μmol/L EA had treated A549 sphere for 48 h,the levels of ROS were significantly increased (P < 0.05),and the mRNA and protein levels of β-catenin,Sox2 and ABCG2,and promoter activity of β-catenin were notably decreased (P < 0.05).The treatment of 200 μmol/L EA enhanced the inhibitory effect on proliferation and the promoting effect on apoptosis in A549 spheres induced by 5 mg/mL DDP (P < 0.05).Up-regulation of β-catenin by adenoviral infection partly reversed the effects of 200 μmol/L EA on suppressing the expression levels of β-catenin,Sox2 and ABCG2,compared to the spheres infected with blank adenovirus.Additionally,β-catenin over-expression significantly remitted the inhibitory effect of 200 μmol/L EA and 5 mg/mL DDP on the proliferation in A549 spheres.Conclusion EA exerts inhibitory effect on the proliferation and stemness of A549 spheres through suppressing GST activity and β-catenin expression,and then promotes cell apoptosis.EA might be a novel drug in treatment of lung cancer and cancer stem cells.
6.Disease burden of prostate cancer among residents at ages of 60 years and older in Zhangjiagang City from 2006 to 2022

Objective:
To investigate the trends in incidence, mortality and disease burden of prostate cancer among residents at ages of 60 years and older in Zhangjiagang City, Jiangsu Province from 2006 to 2022, so as to provide insights into improvements in the prostate cancer control strategy.
Methods:
The incidence and mortality of prostate cancer among residents at ages of 60 years and older in Zhangjiagang City from 2006 to 2022 were collected from the Chronic Diseases Monitoring and Management System in the National Health Information Platform of Zhangjiagang City, and the crude incidence and mortality of prostate cancer were calculated and standardized to data from the sixth national population census in China in 2010. Based on the disease burden of prostate cancer captured from the 2019 Global Burden of Disease Study datasets, the years of life lost due to premature death (YLL) and years of life lived with disability (YLD) and disability-adjusted life years (DALY) due to prostate cancer were measured, and trends in incidence, mortality and disease burden of prostate cancer were analyzed using average annual percent change (AAPC).
Results:
The crude incidence, standardized incidence, crude mortality, standardized mortality, crude DALY rate and standardized DALY rate of prostate cancer were 89.85/105, 83.87/105, 32.31/105, 25.45/105, 546.39/105 and 483.50/105 among residents at ages of 60 years and older in Zhangjiagang City from 2006 to 2022, which all appeared a tendency towards a rise (AAPC=5.346%, 4.219%, 6.648%, 3.697%, 4.198% and 2.200%, all P<0.05). The crude incidence, mortality and DALY rate of prostate cancer all appeared a tendency towards a rise with age (all P<0.05), with a tendency towards a rise seen for the crude incidence of prostate cancer among residents at ages of 60 to 64 years, 65 to 69 years and 70 to 74 years (AAPC=4.888%, 8.086% and 3.005%, all P<0.05), and a tendency towards a rise for the crude mortality and DALY rate among residents at ages of 80 years and older (AAPC=10.243% and 9.693%, both P<0.05).
Conclusion
The incidence and mortality of prostate cancer showed a tendency towards a rise among residents at ages of 60 years and older in Zhangjiagang City from 2006 to 2022, and the disease burden due to prostate cancer continued to increase, with a more remarkable increasing tendency seen for the incidence of prostate cancer among residents at ages of 60 to 74 years and for the mortality among residents at ages of 80 years and older.
7.Correlation analysis of positive rate of HPV genotyping test and HPV nucleic acid loads

Objective To investigate the correlation between the positive rate of PCR-reverse dot blot genotyping test and the loads of the viral nucleic acid. Methods The fluorescent PCR assay was used to detect the high-risk HPV(HR-HPV)DNA loads in the cervical mucus samples from 1162 female pa-tients. Patients with positive HR-HPV DNA were further analyzed by PCR-reverse dot blot hybridization as-say for HPV genotyping. Results The overall positive rate of genotyping test was 68. 8% . The positive rate of genotyping test had a significant positive correlation with the Log Koc of HR-HPV DNA loads(r=0. 944, P﹤0. 01). The quadratic curve fitting formula was Y= -1. 806+0. 558X-0. 031X2(Y for genotyping positive rate,X for Log Koc of HR-HPV DNA loads). There were significant differences with the positive rate of gen-otyping test among patients with different viral loads(P﹤0. 01). When HR-HPV DNA loads were 104-105 copies/ ml,105-106 copies/ ml,106-107 copies/ ml and more than 107 copies/ ml,the positive rate of HPV genotyping test were 27. 8% ,48. 5% ,74. 0% ,97. 5% and 33. 3% ,51. 5% ,78. 0% ,97. 5% respective-ly by using different genotyping detection reagents. Conclusion The positive rate of PCR-reverse dot blot genotyping test was correlated with the loads of HPV nucleic acid.
8.Investigation and analysis on current situation of blood lipid abnormality among urban and rural residents in Ganzhou City

Objective To understand the status quo of dyslipidemia among urban and rural residents in Ganzhou City.Methods The multistage stratified cluster random sampling method was adopted to select the urban and rural residents aged more than 35 years old as the respondents,among them 4 829 respondents had the intact data,the investigation items included age,sex,height,body mass,waist circumference(WC),FBG,Scr,blood pressure,TC,TG,HDL and LDL.The body mass index was calculated.Results The prevalence rates of hypercholesterolemia,high TG and low HDL among the urban and rural residents in Ganzhou City were 8.76 %,16.38% and 24.04% respectively.Compared with the persons of normal blood lipid,FBG,Ser,WC and BMI in the persons of high blood lipid were much higher (P<0.05).Conclsion The blood lipid abnormality in the urban and rural residents in Ganzhou City is dominated by low HDL and high TG.
9.Intravitreal ranibizumab injection combined with triamcinolone acetonide for macular edema secondary to type Ⅱ optic disc vasculitis

Objective To evaluate the efficacy and safety of intravitreal ranibizumab injection combined with triamcinolone acetonide (TA) for macular edema secondary to type Ⅱ optic disc vasculitis.Methods Totally 19 cases(19 eyes) with macular edema secondary to type Ⅱ optic disc vasculitis from January 2013 to December 2015 were injected ranibizumab intravitreally and triamcinolone acetonide 2 weeks later.The visual acuity,intraocular pressure,ocular fundus,OCT,FFA and related complications were observed during the 6 months follow-up period.If the macuiar edema returns during the follow-up period,the eyes would be given ranibizumab and TA again until the macular edema subsided.Results The visual acuity was 0.16 ± 0.09 and the central retinal thickness was (694.88 ± 79.06) μm before treatment.Compared with pretreatment,the visual acuity of all cases were improved in different degree except for one at 1 month,3 months and 6 months (all P < 0.05).Also central retinal thickness were decreased and macular edema were subsided,there were statistical differences (all P <0.05).Compared with results at 1 month,the visual acuity and central retinal thickness was no significant different at 3 months and 6 months (all P > 0.05).The intraocular pressure of all cases were normal throughout the follow-up period but one was increased transitorily in the first day after intravitreal rauibizumab injection.FFA examination at 1 month showed retinal hemorrhage and effusion absorbed obviously,besides,the optic disc and macular fluorescein leakage reduced significantly.The macular edema of 1 eye returned at 3 months and subsided after giving ranibizumab and TA again.The average number of injected eyes was 2.11 times.No complications associated with intravitreal injection and drugs were observed during the follow-up period.Conclusion It is effective and safe that intravitreal ranibizumab junction combined with TA for macular edema secondary to type Ⅱ optic disc vasculitis,which not only shorten the period of treatment but also reduce the times of drug injection,surgical trauma and complications.
10.Effect of Electroencephalograph Bionic Electrical Stimulation on Persistent Vegetative State: Cerebral Blood Flow and Metabolism

Objective To study the effect of electroencephalograph (EEG) bionic electrical stimulation at Wangu (GB12), Tianzhu (BL10), Neiguan (PC6) on cerebral blood flow and metabolism in patients with persistent vegetative state (PVS). Methods 60 patients with PVS were divided into observation group (n=30) and control group (n=30) according to the random number table. The control group received routine treatment, including basic management, hyperbaric oxygen therapy, awaking medicine, sensory stimulation, and so on; while the observation group received EEG bionic electrical stimulation at bilateral Tianzhu, Wangu (cheif electrodes) and Neiguan (auxiliary electrodes) in addition. They were treated for 30 days. They were assessed with the PVS score, and observed with transcranial Doppler ultrasound (TCD) and magnetic resonance spectroscopy (MRS) one day before and one day after treatment. Results The incidence of improvement was 86.67% in the observation group, more than 60.00% in the control group (P<0.05). The difference of blood flow velocity before and after treatment (ΔVm) of anterior-middle cerebral artery was more in the observation group than in the control group respectively (P< 0.001), as well as those of posterior cerebral-vertebral-basilar artery (P<0.01). The N-acetyl aspartic acid/creatine (NAA/Cr) increased more in the observation group than in the control group respectively (P<0.01), and the choline/creatine (Cho/Cr) decreased more (P<0.01), after treatment. Conclusion EEG bionic electrical stimulation can improve the cerebral circulation and metabolism in patients with PVS, which may associate with the wake promoting.
