1.Long-term result of urethroplasty using circular fasciocutaneous prepuce flap for the treatment of extensive urethral stricture (report of 7 cases)

Objective To evaluate urethroplasty using circular fasciocutaneous prepuce flap for the treatment of extensive urethral stricture. Methods From 1993 to 1998,one stage urethroplasty using distal prepuce circular fasciocutaneous flap with Buck's fascia was conducted for 7 cases of extensive anterior urethral stricture of 6~10 cm long. Results All the patients have been followed up for 14~58 months with good voiding.No postoperative urethral stricture has been observed. Conclusions One stage urethroplasty using circular fasciocutaneous prepuce flap is an ideal operative procedure for the treatment of extensive anterior urethral stricture.The technique is simple and convenient to carry out.
2.Silencing long non-coding RNA HIF1A-AS2 inhibits proliferation, invasion and migration of cervical cancer cells

OBJECTIVE:
To explore the inhibitory effects of silencing long non-coding RNA (LncRNA) HIF1A-AS2 on epithelialmesenchymal transition (EMT) and tumor stem cell-like phenotype in cervical cancer cells.
METHODS:
We designed 3 shRNA constructs for silencing HIF1A-AS2 in CaSki cells, and the shRNA with the strongest interference effect was selected for subsequent experiment. CaSki cells were transfected with shRNA-NC or Sh-HIF1A-AS2, and the changes in cell viability, invasion ability, EMT, expressions of EMT-related proteins, formation of cell spheres and expressions of stem cell markers were detected.
RESULTS:
Transfection with shRNA-NC and Sh-HIF1A-AS2 did not significantly affected the viability of CaSki cells (
CONCLUSIONS

3.Determination of 5 Hazardous Elements in Animal Traditional Chinese Medicines by ICP-MS

Objective: To establish a method to determine 5 hazardous elements in animal traditional Chinese medicines (TCM) by microwave digestion-inductively coupled plasma mass spectrometry (ICP-MS).Methods: HNO 3-H 2 O 2 was used to decompose animal TCM, the working parameters of ICP-MS were optimized, and the matrix effect and the mass spectrum interferences were corrected by the in-line addition of Ge, In and Bi internal standard solution and the collided reaction cell technology (KED mode), respectively.Results: The detection limits of 5 hazardous elements were within the range of 0.04-0.52 μg·L-1 , the relative standard deviations (RSD) were within the range of 1.1%-4.8% , and the recoveries were within the range of 92.4%-110.0%.The method was applied to determine the 5 hazardous elements in 40 batches of animal TCM.The results indicated that some kinds of hazardous elements in animal TCM were high, and more attention should be paid to the problems in the production and use of animal TCM.Conclusion: The analysis method is simple, rapid and accurate, and suitable for the determination of hazardous elements in animal TCM.

To compare the clinical effect of needling the Ashi point (4cm lateral to the lower border of the spinous process of cervical vertebra) plus Yaotongdian on the hand and point injection on the Ashi point with Triamoninde. The curative effect of acupuncture was better than that of point injection.
5.The influence of alprostadil on the serum adiponectin and leptin in patients with early diabetic nephropathy

Objective To study the influence of alprostadil on the serum adiponectin and leptin in patients with early diabetic nephropathy.Methods 98 patients with early diabetic nephropathy were randomly divided into two groups,the control group(n =48 cases) and the treatment group(n =50 cases).The patients of the control group were treated through the conventional treatment.However,the patients of the treatment group were treated plus alprostadil.They were treated for 21 days.The urinary albumin excretion rate (UAER),adiponectin,leptin and insulin resistance were detected before and after treatment.Results The UAER,adiponectin leptin and insulin resistance were improved after treatment,while those of the treatment group were more significantly improved than those of the control group(P < 0.05 or P < 0.01).Conclusion Alprostadil can improve the conditions of early diabetic nephropathy which is through improving the adiponectin and insulin resistance.
6.Changes and correlation of serum interleukin-10 and interleukin-8 levels in the patients with cerebral infarction

Objective To explore the clinical significance and the dynamic changes of serum anti-inflam-mation factor interleukin-10(IL-10)and inflammatory factor interleukin-8(IL-8)in cerebral infarction.Methods The serum levels of IL-10 and IL-8 in 30 patients were measured by double-antibody sandwich enzyme-linked inunu-nosorbent assay(ELISA)on days 1.3,7 and 14 respectively after infarction.Results The serum levels of IL-10 and IL-8 were higher in patients than those in control group on days 1,3 and 7(P<0.05).Conclusion IL-10 and IL-8 are involved in the pathophysiology of acute cerebral infarction.
7.Study on clinical application of serum C-type natriuretic peptide ,IGF-Ⅱ ,ET ,NSE and S100B levels in craniocerebral trauma

Objective To study the effects of serum C-type natriuretic peptide (CNP) ,insulin-like growth factor II (IGF-Ⅱ ) , endothelin (ET) ,neuron-specific enolase (NSE) and S100B protein(S100B) on the prognosis of the patients with traumatic brain injury .Methods A total of 110 patients with craniocerebral trauma admitted in our hospital from January 2016 to January 2017 se-lected as the craniocerebral trauma group and further divided into the mild ,moderate and severe craniocerebral trauma groups ac-cording to the Glasgow Coma Scale (GCS) .Then the levels of serum CNP ,IGF-Ⅱ ,ET ,NSE and S100B in all cases were analyzed by enzyme-linked immunosorbent assay (ELISA) .Their influence on the prognosis of the patients with craniocerebral trauma and the correlation among various indicators were analyzed .Results The levels of CNP and IGF-Ⅱat admission in the craniocerebral trauma group were significantly decreased ,while the levels of ET ,NSE and S100B were significantly increased ,the difference com-pared with the control group was statistically significant (P<0 .05) .Serum CNP and IGF-Ⅱlevels in the death group ,plant survival group and disabled group were significantly decreased .The difference was statistically significant (P<0 .01) .Serum CNP and IGF-Ⅱlevels in the moderate and severe craniocerebral trauma groups were gradually increased with the disease course progress ,while serum ET ,NSE and S100B levels were gradually decreased with the disease course progress ,the difference was statistically signifi-cant(P<0 .05) .In the patients with craniocerebral trauma ,the positive correlation existed between CNP and IGF-Ⅱ ,between ET and S100B ,between ET and NSE ,and between NSE and S100B(P<0 .01) ,while the negative correlation existed between IGF-Ⅱand ET ,between IGF-Ⅱ and S100B ,between CNP and ET ,and between IGF-Ⅱand NSE (P<0 .01) .Conclusion Serum CNP , IGF-Ⅱ ,ET ,NSE and S100B are correlated to the severity of craniocerebral trauma ,which has a higher clinical application value for judging the disease condition ,evaluating the prognosis in cradiocerebral trauma .
8.Mechanism of Yiyuan Qiwei Pills in Treatment of Diabetes Mellitus-induced Erectile Dysfunction

ObjectiveTo observe the therapeutic effect of Yiyuan Qiwei pills (YYQW) on diabetes mellitus-induced induced erectile dysfunction (DMED) in rats and explore its regulation on the nitric oxide (NO)-cyclic guanosine monophosphate (cGMP) signaling pathway. MethodFifty-five healthy SD male rats of clean grade aged 2-3 months underwent intraperitoneal injection of streptozotocin (STZ) to induce the DMED model, and another 10 healthy SD male rats of clean grade aged 2-3 months were assigned to the control group. The model rats were randomly divided into a model group, a sildenafil group (5 mg·kg-1, ig), and low-, medium-, and high-dose YYQW groups (1.5, 3.0, 6.0 g·kg-1, ig). The rats in the model group and the control group were given normal saline by gavage at 10 mL·kg-1, once a day for two months. After intervention, the penile erectile function of rats in each group was measured by a pressure detection system. The pathological changes and ultrastructure of penile corpus cavernosum were observed by hematoxylin-eosin (HE) staining and transmission electron microscopy, respectively. The level of NO in the corpus cavernosum was detected by nitrate reductase. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of cGMP and advanced glycation end products (AGEs). Real-time quantitative polymerase chain reaction (Real-time PCR) was used to detect the mRNA expression of endothelial nitric oxide synthase (eNOS), neurogenic nitric oxide synthase (nNOS), total nitric oxide synthase (NOS), and phosphodiesterase type5 (PDE5) in rat penile tissues. The expression of above proteins was detected by Western blot. ResultCompared with the control group, the model group showed decreased intracavernous pressure (ICP), NO, and cGMP levels, reduced mRNA and protein expression of nNOS and NOS, and increased PDE5 mRNA and protein expression (P<0.05). Compared with the model group, the sildenafil group and the YYQW groups displayed increased ICP, NO, and cGMP levels, elevated mRNA and protein expression levels of nNOS and NOS, and reduced PDE5 mRNA and protein expression levels (P<0.05). There were no pathological changes in the tissues and cell ultrastructure of the corpus cavernosum in the control group, while serious pathological changes were observed in the model group. Additionally, the sildenafil group and the YYQW groups were superior to the model group, the optimal effect was observed in the high-dose YYQW group. ConclusionYYQW can improve the penile erectile function of DMED rats and reduce the pathological damage of corpus cavernosum. The mechanism may be related to the promotion of nNOS and NOS expression, the inhibition of PDE5 expression, and the activation of the NO/cGMP signaling pathway.
9.Value of transvaginal three-dimensional ultrasound in evaluating the curative effect of Yangmo decoction in the treatment of uterine adhesion.

OBJECTIVES:
Intrauterine adhesions (IUA) is the damage of the basal layer of the endometrium caused by various reasons, resulting in adhesion of the uterine muscle walls to each other, which is manifested as clinical symptoms such as spanomenorrhea, amenorrhea, and infertility. Hysteroscopic adhesiolysis (HA) is the main treatment, for patients with moderate or severe adhesion or angular adhesion, the incidence of postoperative adhesion is high. Traditional Chinese medicine "Yangmo decoction" can promote endometrial growth. Three-dimensional transvaginal ultrasonography (3D-TVUS) can judge IUA and evaluate uterine receptivity through three-dimensional imaging. This study aims to investigate the value of 3D-TVUS in judging the efficacy of Yangmo decoction in the treatment of intrauterine adhesions.
METHODS:
The clinical data of patients who underwent HA at two different centers in department of Gynecology of Third Xiangya Hospital of Central South University and Changsha Jiangwan Hospital from January 2021 to August 2021 were retrospectively collected. A total of 275 eligible patients were included. According to the postoperative management measures, the selected patients were divided into two groups. Yangmo decoction group (n=138): the use of Yangmo decoction and uterine-shaped silicon stent post HA; Hormone group (n=137): the use of estrogen, progesterone and uterine-shaped silicon stent post HA. The preoperative general data, preoperative and postoperative 3D-TVUS parameters of the two groups were analyzed.
RESULTS:
The endometrial thickness of Yangmo decoction group was thicker than that of hormone group (P<0.001), the intercornual distance was wider (P=0.016), the endometrial echo was more homogeneous (P=0.018), the percentage of bilaterally visible tubal opening was higher (P<0.001), the endometrial morphology was better (P=0.012), and endometrial blood flow, endometrial motility and uterine motion were better in Yangmo decoction group than that in the hormone group (all P<0.001).
CONCLUSIONS
The endometrial thickness, echo, blood flow, and peristalsis, the number of visible tubal opening, uterine motion, and the intercornual distance obtained by 3D-TVUS examination are important factors to evaluate the prognosis of postoperative drug treatment for IUA. 3D-TVUS is of great significance in evaluating the efficacy of Yangmo decoction in the treatment of IUA.

10.Separation and Identification of Chemical Components in Ethyl Acetate Fraction and Water Fraction from Tripterygium wilfordii

OBJECTIVE: To separate and identify chemical components in ethyl acetate fraction and water fraction from Tripterygium wilfordii, and to provider basis for further pharmacological study. METHODS: The ethyl acetate fraction and water fraction from T. wilfordii were separated and purified by MCI GEL-CHP 20P column chromatography, C18 RP silica gel column chromatography, Sephadex LH-20 gel column chromatography and HPLC. The structures of compounds were analyzed and identified by 1H-NMR, 13C-NMR and physicochemical properties. RESULTS: Two compounds were isolated from ethyl acetate fraction of T. wilfordii, namely orthosphenic acid (compound 1), dibutylphthalate (compound 2). Eight glucosides were isolated from water extract of T. wilfordii, namely 2,6-dimethoxy-4-hydroxymethyl-phenyl-1-O-beta-D-glucopyranoside (compound 3), 2,6-dimethoxy-4-hydroxyphenol-1-O-β-D-glucoside(compound 4), 4-hydroxy-1-(2-hydroxyethyl)-phenyl-3-O-β-D-glucopyranoside (compound 5), 3,4-dimethoxy-phenyl-1-O-β-D-glucopyranoside (compound 6),β-adenosine (compound 7), ligustrin (compound 8), epicatechin-8-C-β-D-galactoside (compound 9) and 2-hydroxynaringenin-7-O-β-glucoside (compound 10). CONCLUSIONS: Chemical components of ethyl acetate fraction and water fraction are separated and identified from T. wilfordii.
