1.Comparison of laparoscopic and open radical resection for rectal cancer

ObjectiveTo compare the results of laparoscopic and open radical operation for rectal cancer.MethodsThree hundred and twelve patients with laparoscopic operation and 226 cases with open operation during the period of June 2004 to August 2009 were included.The long-term survival,operative data,postoperative recovery and complications were compared between the two grougs.ResultsThere were no significant differences in age,sex,tumor stage and histologic types between the two groups.The 3 and 5- year-survival rate was 84.5% and 66.7% in laparoscopic group,83.3% and 64.8% in traditional operation group with no significant difference by Life-table method.The intraoperative blood loss in laparoscopic group was obviously less than that in open group (61 ± 13 nl vs 174 ±84 ml,t =23.24,P <0.05).The time of p assage of gas by anus and hospital stay in laparoscopic group were significantly shorter than those in openg roup (2.7 ±1.3 d vs 3.6 ±1.8 d,t =6.61,P <0.05;9.1 ±2.4 d vs 12.0 ±3.4 d,t =11.8,P <0.05).No significant difference was observed between the two groups in the lymph nodes clearance ( 11.0 ± 2.7 vs 12±3.6,t=1.72,P >0.05),specimen length (16.0 ±3.4 cm vs 16.0 ±4.3 cm,t =0,P>0.05) and distal margin (3.2 ± 1.3 cm vs 3.2 ± 1.7 cm,t =0,P >0.05).Surgical site infection of incision developedin 28casesinopensurgerygroupandin8casesinlaparoscopicgroup(P< 0.05 ).ConclusionsLaparoscopic surgery for rectal cancer can achieve similar long-term survival as conventional laparotomy with minimal invasion,quicker recovery and less complications.
2.Effect of rapid rehabilitation surgery nursing on patients' recovery and reduce health care costs after colon cancer operation

Objective To investigate the effects of rapid rehabilitation surgery nursing on patients' recovery and reducing health care costs after colon cancer operation.Methods A total of 122 patients with colon cancer were selected and randomly divided into two groups on average.The patients of observation group were treated with rapid rehabilitation surgery nursing while the patients of control group were treated with routine care.We used statistical software SPSS 17.0 1 d before discharge to assess patient hospitalization time,hospital costs,complications during hospitalization and gastrointestinal function for all patients in beth groups.Results After the intervention,the time of hospitalization and costs of hospital in the experimental group were significantly lower than those in the control group [(14.91 ±2.09) vs (18.63±2.52)d,(2.07 ±0.49) vs (2.62 ±0.92)yuan],and these differences were statistically significant between the two groups (t =10.309,4.787;P <0.05).The pain score,bed recovery time and recovery time of bowel movements in experimental group were better than these of the control group [(4.01 ±0.59) vs (6.87 ±0.76) score,(4.61 ±0.30) vs (7.22 ± 0.47) d,(3.10±0.71) vs (5.66±0.80)d](t=7.026,4.721,5.774;P<0.01).However,the time of anal first exhaust,pull the tube,resume normal diet in the experimental group were significantly lower than those in the control group [(2.91 ±0.62) vs (3.93 ±0.79)d,(3.59 ±0.33) vs (4.27 ±0.45),(8.07 ± 0.74) vs (9.62±0.83)d] (t=-5.403,-4.492,-6.415;P<0.01).Conclusions The rapid rehabilitation surgery nursing can shorten the hospital stay of patients with colon cancer surgery,reduce hospital costs,speed up the recovery of gastrointestinal function,so it is worthy of wider promotion and application.
