2.Meta-analysis of the effect of home-based cardiac rehabilitation training on ejection fraction, blood lipid and exercise endurance in patients with coronary heart disease

Objective To explore the effect of home-based cardiac rehabilitation training on ejection fraction, blood lipid and exercise endurance in patients with coronary heart disease. Methods Randomized controlled trials (RCTs) on the effect of home-based cardiac rehabilitation training on ejection fraction, blood lipid and exercise endurance in patients with coronary heart disease were searched via computer in PubMed, Web of Science, Embase, Cochrane Library database, Wanfang database, CNKI database, VIP database and CBMdisc. After literature screening, data extraction and quality evaluation, Revman5.3 software was used for meta-analysis. Results A total of 11 articles were included. Meta analysis showed that, compared with the conventional treatment group, home-based cardiac rehabilitation training could effectively improve patients' ejection fraction[MD=1.81,95%CI(0.68,2.95),P=0.002],exercise endurance[MD=21.97,95% CI(1.80, 42.14), P=0.03], and reduce cholesterol level[MD=-0.15,95%CI(-0.23,-0.07), P< 0.01]. However, there was no significant improvement on high-density lipoprotein, low-density lipoprotein, triglyceride (P>0.05). Sensitivity analysis showed that the overall results of the study were relatively stable, and individual study had no significant influence on the overall results. Conclusions Home-based cardiac rehabilitation training can significantly improve patients' ejection fraction and exercise endurance and reduce cholesterol level, but in terms of improving patients' high density lipoprotein, low density lipoprotein and triglyceride, more large sample, multi-center long-term studies are needed to verify.
3.Reliability and validity of Chinese version of the Spirituality and Spiritual Care Rating Scale

Objective:To translate the Spirituality and Spiritual Care Rating Scale (SSCRS) into Chinese and test its reliability and validity.Methods:After obtaining authorization from the original author, we carried out translation, back translation and cross-cultural adaptation based on the Brislin principle, and formed the Chinese version of the SSCRS. From April to June 2019, we conducted the questionnaire survey among 189 nurses at a Class Ⅲ Grade A hospital in Shiyan by convenience sampling to evaluate the reliability and validity of the Chinese version of SSCRS.Results:The Chinese version of SSCRS had 17 items. Exploratory factor analysis extracted a total of 4 common factors, with a cumulative contribution rate of 62.83%. The Cronbach's α coefficient of the entire scale was 0.840, the split-half reliability was 0.842, and the test-retest reliability was 0.860. The scale-level content validity index of all items was 0.91, and the item-level content validity index of each item ranged from 0.81 to 1.00.Conclusions:The Chinese version of SSCRS has a good reliability and validity, which can be used as an effective tool to evaluate nurses' cognition of spirituality and spiritual care in China.
4.Effect evaluation on training mode based on core competence of nurses in clinical practice of nursing

Objective:To explore the influence of training mode based on core competence of nurses on comprehensive performance, critical thinking ability and clinical communication ability of nursing students.Methods:A total of 102 Batch 2015 nursing students from the four-year program of Hubei University of Medicine who were interned in a "three A" general hospital in Shiyan City from May 2018 to January 2019 were selected as subjects, and were randomized into the control group ( n=51) and the intervention group ( n=51). The control group received traditional training mode and the intervention group adopted the training mode based on core competence of nurses. The two groups of nursing students were graded on the critical thinking scale and clinical communication ability scale before practice and at 2 months, 4 months, 8 months after intervention respectively. The comprehensive performance of two groups was assessed before and after 8 months of intervention. All data were analyzed by SPSS 19.0. Results:Comparison of effect, interaction effect, time effect of critical thinking scale and clinical communication ability scale between the two groups showed significant differences ( P<0.05). The comprehensive performance differences between the two groups were statistically significant after the intervention ( P<0.05). Conclusion:The practice training mode based on core competence of nurses can effectively improve the comprehensive performance, critical thinking ability and clinical communication ability of nursing students.
5. Effect of home-based cardiac tele-rehabilitation on quality of life, disease burden status,cardiac function and exercise endurance of patients after percutaneous coronary intervention

Objective:
To explore the effect of home-based cardiac tele-rehabilitation on quality of life, disease burden status,cardiac function and exercise endurance of patients after percutaneous coronary intervention (PCI).
Methods:
A total of 106 patients after PCI from January to December in 2018 in Heart Disease Center of Shiyan People′s Hospital, Hubei Province were randomly divided into observation group and control group according to the order of discharge, there were 53 cases in each group. The control group received routine rehabilitation care, and the observation group received home-based cardiac tele-rehabilitation on the basis of routine nursing. 6 minutes walking test(6MWT), anerobic threshold, oxygen peak and left ventricular ejection fraction(LVEF) in two groups were measured at 3 months after the intervention, the SF-12 scale and Family Burden Scale of Disease (FBS) scale were also scored.
Results:
After 3 months of intervention, the 6MWT, anerobic threshold, oxygen peak, physiological total score and psychological total score of SF-12 scale, and FBS score was (596.26±79.35) m, (14.65±1.82) ml·kg-1·min-1, (25.86±3.46) ml·kg-1·min-1, (47.46±9.86) points, (50.68±9.82) points, (18.94±6.61) points in the observation group, and (532.65±76.57) m, (12.95±1.87) ml·kg-1·min-1, (21.95±3.28 ) ml·kg-1·min-1, (43.28±8.21) points, (48.26±9.69) points, ( 21.75±7.18) points in the control group. The difference between the two groups was statistically significant (t value was -0.317-6.034, all P<0.05).
Conclusions
Home-based cardiac tele-rehabilitation can improve the quality of life, exercise endurance and can help to reduce the disease burden status on patients after PCI.
