Journal of Leukemia & Lymphoma 2010;19(5):257-259,261
Remarkable achievements have been made for lymphocyte infusion of lymphoma and leukemia, especially of lymphoproliferative disease within twenty-first century. The donor lymphocyte infusion or mobilized donor lymphocyte infusion (DLI/DSI) offer an opportunity of second remission for relapse patients post-transplantation. Cytokine-induced killer/DC cytokine-induced killer play an anti-tumor activity beyond non-MHC restricted. Cytotoxic lymphocyte infusion activated by synthetic tumor antigen produces targeted effects of anti-tumor. Transgenic CTL of anti-tumor TCR has brought the dawn in lymphoma and leukemia patients with defective lymphocytes. Maternal lymphocyte infusion play an anti-tumor/viral activity by avoiding the immune barrier of HLA mismatch.

BACKGROUND:Previous studies have shown that nerve function may achieve different degrees of recovery in most patients after transvertebral transposition of the spinal cord for repair of spinal cord nerve compression and kyphoscoliosis. However, the effective mechanism of the proposed method to improve postoperative nerve function is stil not clear. OBJECTIVE:To investigate the improvement in neurological outcomes after transvertebral transposition of the spinal cord and posterior correction in patients suffering from neurologic deficit secondary to angular kyphoscoliosis. METHODS:Eighteen patients suffering from neurologic deficit secondary to angular kyphoscoliosis underwent transvertebral transposition of the spinal cord and posterior correction. Preoperatively and 1 week postoperatively, al patients were subjected to somatosensory evoked potential detection. During operation, motor evoked potentials and somatosensory evoked potentials were monitored. Using MRI, the distance from the inner edge of the spinal canal to the outer edge of the convex side of the spinal cord at apical region was measured, and the distance of spinal cord transposition was calculated. RESULTS AND CONCLUSION:(1) There were no significant differences in terms of the latency and amplitude of posterior tibial nerve P40 (preoperatively versus intraoperatively:amplitude:(1.66±0.29)μV vs. (1.68±0.28)μV, P>0.05;latency:(39.25±3.02) ms vs. (38.68±2.67) ms, P>0.05). (2) After surgery, the major curve was improved with (51.1±21.2)%correction rate, and the mean kyphosis was improved with (38.9±18.1)%. The distance between spinal cord and canal on the convex side at the apex was decreased by (2.4±1.7) mm on average. (3) The amplitude and latency of posterior tibial nerve P40 at 1 week postoperatively were (2.21±0.40)μV and (34.98±2.83) ms, respectively (P<0.05). (4) These results suggest that posterior correction and internal fixation can remarkably improve neurophysiological index and nerve function in patients with neurologic deficit secondary to angular kyphoscoliosis.

10.Effects of Small Dose of Epidural Ketamine on Analgesia for Patients after Surgical Intervention for Knee Stiffness
Chinese Journal of Rehabilitation Theory and Practice 2012;18(12):1151-1154
Objective To observe the dose-effect relationship and side effects caused by epidural ketamine for patients after surgical intervention for knee stiffness (SIKS). Methods 80 patients undergoing SIKS at one knee joint under combined spinal and epidural anesthesia were randomly divided into 4 groups. Patients in these groups would receive analgesia respectively provided by femoral nerve block (FNB)(group C, n=20), FNB combined with 0.15 mg/kg epidural ketamine (group K1, n=20), FNB combined with 0.2 mg/kg epidural ketamine(group K2, n=20), and FNB combined with 0.25 mg/kg epidural ketamine (group K3, n=20). When spinal anesthesia was finished, the corresponding dose of ketamine was injected in patients in groups K1, K2, and K3 and 0.9% sodium chloride was injected in group C via an epidural catheter. After the operation, a catheter was left in the femoral nerve sheath and the solution of 60 mg ropivacaine in 30 ml (0.2%) was injected. All patients were performed rehabilitation therapy at postoperative 8, 24, 32, and 48 h and the same ropivacaine solution was injected in the femoral nerve sheath before each rehabilitation therapy started. Visual analogue score (VAS), active range of motion (AROM) of the suffered knee joint, and side effects were observed. Results There were 73 patients enrolled in the final statistic analysis. At postoperative 8 h, the VAS scores were lower in groups K1, K2 and K3 than in group C (P<0.05). At postoperative 24 h, 32 h, and 48 h, the VAS scores were lower in groups K2 and K3 than in groups C and K1 (P<0.05). AROM was larger in groups K2 and K3 than in groups C and K1. patients appeared psychiatric symptoms in group K3. Conclusion Small dose of epidural ketamine is a feasible method for analgesia in early rehabilitation therapy and the dose of 0.2 mg/kg is effective and safe.
