1.Clinical application of anti-proteinase 3 capture ELISA in diagnosis of Wegener's granulomatosis

Objective To investige the sensitivity and specificity of anti-proteinase 3 (PR3) capture ELISA in diagnosis of Wegener's granulomatosis (WG),and the correlation between the capture ELISA and indirect immunofluorescence assay (IIF).Methods Anti-PR3 antibody and anti-neutrophil cytoplasmic antibody (cANCA) in sera from 72 patients with WG,206 healthy blood donors and 24 patients with autoimmune diseases were detected by classic ELISA,capture ELISA and IIF.Results The sensitivities of classic ELISA and capture ELISA for detection of anti-PR3 in WG diagnosis were 73.6% and 87.5% respectively.The specificities of both the ELISAs were identical (100%).Detection of anti-PR3 by ELISA or IIF alone led to the serological hit rate of 87.5% and 84.7% for WG respectively,but the combination of capture ELISA and IIF increase the hit rate up to 91.6%.Conclusions The sensitivity of anti-PR3 capture ELISA as well as its correlation with IIF is prior to classic anti-PR3 ELISA.The combined detection of anti-PR3 capture ELISA and IIF may increase the diagnosis rate of clinically suspected WG.
2.Investigation of Perioperative Prophylactic Utilization of Antibiotics in Cataract Patients and Its Rationality Evaluation

OBJECTIVE:To investigate the perioperative prophylactic utilization of antibiotics in cataract patients in ophthalmology department of our hospital.METHODS:100 cases of cataract surgery in 2009 were randomly collected from our hospital.Category of antibiotics,administration time point,medication time,wound healing and drug costs in hospitalization period were analyzed statistically and the rationality of above indexes was evaluated.RESULTS:100 cases all received prophylactic antimicrobials by local and systematic administration in the perioperative period(100%).Drugs for systematic administration contained Levofloxacin(n=67,67%),Cefuroxime sodium for injection(n=25,25%),Clindamycin phosphate injection(n=2,2%) and Ceftazidime for injection(n=6,6%).63 cases were only given drugs 30 min before operation(63%);12 cases were given medicine 30 min before operation and within 24 h after operation(12%);above situation accounted for 75%.25 cases were given medicine before operation and within 24 h after operation(25%),in which the average medication duration was 2.5 days and the longest was 4 days.CONCLUSION:The irrational use of drugs in our hospital manifests inappropriate selection and too long use of antibiotics after operation.The interference of perioperative prophylactic utilization of antibiotics should be reinforced and it is urgent to intensify consciousness of physicians about rational use of antibiotics in perioperative period.
3.Sorafenib plus Capecitabine for Patients with Advanced Hepatocellular Carcinoma

OBJECTIVE: To evaluate the efficacy and toxicity of sorafenib plus Capecitabine in patients with advanced hepatocellular carcinoma (HCC). METHODS: 20 patients (treatment group) were assigned to take sorafenib 200 mg bid for 3 consecutive weeks plus capecitabine 1 500 mg? m-2?d-1 for l4 days followed by 7 days discontinuation in 3-week treatment cycle. 22 patients in the control group only received Capecitabine 1 500 mg?m-2?d-1 for l4 days followed by 7 days discontinuation in a 3-week treatment cycle. Tumor response was assessed after 2-cycle treatment using modified WHO criteria. RESULTS: In the treatment group and the control group: the median survival times were 10.9 months and 7.2 months, respectively; the median time for tumor progression was 6.8 months and 4.3 months, respectively; the overall response rates were 20.0% and 9.1% respectively; the clinical benefit rates were 70.0% and 40.9%; the ?-foetoprotein (AFP) reduction rates were 65.5% and 39.0%, respectively. The toxicities were not significant between the two groups. CONCLUSION: Sorafenib plus Capecitabine is safe and effective for advanced hepatocellular carcinoma patients.
4.Influence of the Altered-Cast Impression Technique on Distal-Extension Alveolar Ridge Morphology and Restorative Result

Objective To investigate the impact of the modified cast impression technique on distal-extension alve-olar ridge morphology of partially edentulous casts and the restoration outcome of removable partial dentures (PRDs). Meth-ods Fifty-four patients with mandibular free-end dentition defect were selected. Impressions were taken from each patient using both modified cast technique and conventional technique. Coronary cross-sections of the casts were scanned from dis-tal to mesial extension;the images were compared to reveal morphological differences. And the relationship between these differences and location of the cross-sections were analyzed. After the patients were evenly divided into two groups per paired design, the impressions were taken using the respective modified cast and conventional impression techniques to pre-pare RPDs. The subsequent comparisons between the two groups included frequency of adjustments during the adaptive phase, masticatory efficiency of the denture at ending of the adaptive phase and visible movement of the extension base dur-ing mastication. SPSS 16.0 software was used for statistical analysis. Results The number of pixels in the same cross-sec-tion was less in the impression taken with modified cast technique than that with conventional technique. And their differ-ence is of statistical significance (P < 0.01). The difference in the number of pixels (D) was negatively correlated with the number of cutting and grinding, and N (location of the cross-sections). In addition, there were statistically significant differ-ences in frequency of adjustments during the adaptive phase (times:1.12 ± 0.77 vs 3.41 ± 0.82), masticatory efficiency of the denture (0 vs 18.5%) and visible movement of the extension base during mastication (96.3%vs 74.1%) between the modified cast group and conventional group. Conclusion Morphology of distal-extension alveolar ridge of the casts differs signifi-cantly between the impressions obtained with modified cast technique and the impressions obtained with the conventional technique;the difference increases with greater distance to the distal surface of the terminal abutments. The restoration out-come of RPDs is significantly better with the modified cast impression technique than with the conventional impression tech-nique.
5.Induction of specific cytotoxic T lymphocytes using hepatoma antigenic peptide mixed with HSP70 in vitro

Objective:To explore the possibility of inducing cell mediated immune response with HSP70 antigenic peptide complex in vitro.Methods:HSP70 peptide complex was reconstituted in vitro.Granulocyte/macrophage colony stimulating factors and interleukin 4 were used to cultivate DC from peripheral blood of HLA A2 positive healthy donors.HSP70,HSP70 peptide complex or peptide was used to activate the DC individually,which will initiate homogenize T lymphocyte to form cytotoxic T lymphocyte(CTL).The cytotxicity of the CTL was detected by MTT assay.Results:It was found that peptide specific CD8 + CTL responses were readily elicited by HSP70 peptide complex or peptide.The CTL response primed by HSP70 peptide complex was more potene than peptide alone.Conclusion:The results suggest that HSP70 peptide complex is immunogenic and HSP70 can lead to great efficient CTL response,antigenic peptides and HSP70 complex may be used as peptide vaccines for cancer immunotherapy.
6.Normal values of space in front of the femoral neck at different age of normal children and their clinical significance

Objective To measure the width of space in front of the femoral neck at different age of normal children for diagnosis such as hip disease in children as bilateral transient synovitis of the hip. Methods Four hundred and twenty normal children below 14 years old were divided into 14 groups according to their age. Every group contained 30 children. They underwent bilateral ultrasonographic assessment. The width of space in front of the femoral neck was measured and collected. The mean width value in each group was compared mutually by statistical analysis (t test). Results The mean width of space in front of the femoral neck of normal children below 14 years old was 2.0 - 5.9 mm, the range was 1.2 - 7.8 mm. Conclusions The width of space in front of the femoral neck is different at different age of normal children, the mean value below 14 years old
7.Stroma reaction and clinical significance in benign lesion and invasive carcinoma of breast

Objective To study the expression and clinical significance of CD34 and smooth muscle actin (SMA) in stroma of breast tissues and lesions. Methods Seventy cases of breast tissues and lesions, including 20 fibroadenomas, 10 sclerosing adnoses, 30 invasive ductal carcinomas and 10 invasive lobular carcinomas were investigated, and 10 normal breast tissues were served as controls. Immunohistochemical staining was applied to compare the distribution of CD34+ fibrocytes and SMA-reactive myofibroblasts. Results The stroma of normal breast tissue contained CD34+ fibrocytes, whereas SMA-reactive myofibroblasts were absent (100% for both). All benign breast lesions exhibited astromal CD34+ fibrobytes, and fibroadenomas showed SMA-reactive myofibroblasts as well. In invasive ductal and lobular carcinoma the stroma was devoid of CD34+ fibrocytes, but a varying number of stromal SMA-reactive myofibroblasts were detectable (100%). Conclusion In breast carcer, immunohistochemical staining used in detecting expressions of CD34 and SMA is helpful in distinguishing benign lesions from malignancies.
8.Application of activated T lymphocytes induced by hepatocellular carcinoma rejection antigenic peptide-pulsed DCs in vivo and in vitro

AIM To study if the activated T lymphocytes induced by hepatocellular carcinoma (HCC) rejection antigenic peptide SLIVHLNEV (mutant peptide of C met) pulsed dendritic cells (DCs) can provide protection against tumor challenge in nude mice HCC model and in vitro . METHODS SLIVHLNEV was eluted from HCC cell line HHCC by mild acid buffer and was identified by the mass spectrometry technique. SLIVHLNEV pulsed DCs isolated from HLA A2+ ruptured spleen were co cutured with isogeneic T lymphocytes. Cytotoxicity was tested using lactate dehydrogenase (LDH) release assay. The induced cytotoxic T lymphocytes (CTLs) were implanted into nude mice in order to protect them against transplanted HHCC tumor cells. The inhibition effect of CTLs on the growth of implanted tumor in nude mice was also observed. RESULTS The CTLs induced by SLIVHLNEV pulsed DCs had unique ability to kill HHCC tumor cells in vitro. They also protected the nude mice against the further challenge of HHCC tumor cells and inhibited the growth of implanted tumor. CONCLUSION DC based HCC rejection antigenic peptide SLIVHLNEV vaccine can induce powerful immune reaction both in nude mice HCC model and in vitro.
9.Roles of galanin peptide in tumorigenesis and tumor progression

Galanin is a peptide with wide-ranging effects,especially within the central,peripheral nervous system and endocrine system.Many tumors of neuroendocrine origin,and also an increasing number of non-neuroendocrine cancers,have been shown to express galanin and/or its receptors.Expression of galanin peptide has been detected in pheochromocytoma,squamous cell carcinoma,pituitary adenoma,gastrointestinal cancers and so on.Galanin peptide plays an important role in tumori-genesis and progression of many kinds of tumors.Therefore,studying the roles of galanin and its receptors are helpful for diagnosis and treatment of tumors.
10.Primary exploration on immune associated genome of patients with Pi-Qi deficiency syndrome.

OBJECTIVETo investigate the abnormal change of immune function in patients with Pi-Qi deficiency Syndrome, and to explore the genomic mechanism of its genesis by cDNA chip techniques.
METHODSThe cross probe was made by extracting and microamplifying the total RNA and mRNA of peripheral white blood cells (WBC) in healthy subjects and patients with chronic gastritis and ulcerative colitis, which were labeled by Cy3 and Cy5 respectively. Then equal quantity of the two labeled probes were mixed and hybridized with cDNA chip, fluorescent signal of the chips were scanned with scanner. Data obtained were analyzed for comparing the difference of the expressive levels of immune associated genome in peripheral WBC in healthy subjects with those in patients.
RESULTSExpressions of CD9, CD164, PF4 and RARB gene in WBC of patients, both gastritis and colitis, were down-regulated while those of IGKC, DEFA1 and GNLY were up-regulated.
CONCLUSIONThe genesis of Pi-Qi deficiency syndrome has its immune associated genomic basis, and the immune functions are disordered in patients with that syndrome.
