1.Reconstruction of the posterior cruciate ligament with ligament advanced reinforcement system artificial ligament transplantation under knee arthroscopy

AIM: To observe the functional recovery of knee joint in patients with posterior cruciate ligament (PCL) under knee arthroscopy with ligament advanced reinforcement system (LARS) artificial ligament transplantation. METHODS: From January 2005 to May 2006, six patients with PCL injury treated by reconstruction of PCL with LARS artificial ligament transplantation were selected from the Department of Orthopedics, Huadu Hospital Affiliated to Southern Medical University. All subjects agreed with the operation before the surgery and all reconstruction surgeries were of second period. ① The tibial tunnel was established firstly. ② Then was the femoral tunnel. ③ Under knee arthroscopy, the "Y"-shape double cords of LARS artificial ligament were cut through ante-out and meta-in bone tunnels of femur, and fixed at 90o and 30o flexion respectively. Patients were required to motor 3 days postoperative, and patients actively exercised their quadriceps muscle of thigh at one week later. Regular follow-ups were conducted postoperatively, and functional evaluation of the knee joints was performed by according to Lysholm standards, including 8 aspects: Limping, supporting, interlocking, unstable, painful, swell, upstairs and downstairs and squatting with a total score of 100 points. The higher the score was, the better the functional recovery of knee joint. RESULTS: All of the 6 patients were available at follow-up, and no one withdrew from the study. The follow-up was conducted at the 7th, 7th, 10th, 10.4th, 12.8th, and 14th months with an average duration of 10.2 months. After operation, unstable symptoms of knee joint in all subjects disappeared with negative sag sign and posterior drawer. 2.THE EFFECT OF APOLIPOPROTEIN B POLYMORPHISMS ON THE LIPID REGULATING THERAPY FOR HYPERLIPIDEMIA

To study the efficacy of small dosage of simvastatin on the treatment of hyperlipidemia, and the effect of polymorphisms in ApoB gene expression. Sixty one patients with hyperlipidemia were selected and simvastatin 5mg/d was used. TC, TG, HDL, LDL, ApoAI and ApoB and the function of liver and kidney were measured in every patient prior to the treatment and 4, 8, 12 weeks after treatment. RFLPs was applied for determination of polymorphisms on Xbal locus of the ApoB gene. In the total of 61 patients, X + X - genotype was found in 5 patients (8.20%), and X - X - genotype in 56 patients (91.80%), but no X + X + genotype was found. The relative frequency of X + allele was 0.041, and X - allele was 0.959, suggesting X - allele was still a dominant one in the patients with hyperlipidemia. The results showed that small dosage of simvastatin could effectively reduce blood levels of TC, TG, LDL C and ApoB. 3.EXPERIMENTAL STUDY OF BONE REPAIR INDUCED BY CRYOPRESERVED ALLOGRAFT PERIOSTEUM AND FETAL BONE COMPOSITION IN BONE DEFECT

Objective To investigate the repairing effect of transplantation of allogeneic fetal bone in combination with a covering cryopreserved periosteal allograft to bone defect. Methods Twenty Long-eared white male rabbits were chosen as experimental model of bilateral 12 mm combined bony and periosteal radial defect. Cryopreserved allograft periosteum with allogeneic fetal bone were implanted in the left defect as experimental side and fetal bone was simply transplanted in the right defect as control side. Bone repair process in the two groups were compared by macroscopy, microscopy, roentgenograms and the contents of calcium and phosphate in the defect area at 2、4、8 and 12 weeks after transplantation. Results There was significant statistic difference in the contents of calcium and phosphate between the experimental and control sides at 4、8 and 12 weeks after transplantation (P<0.05). With time passing by, the contents of calcium and phosphate have the increasing trends. In the experimental group, lamella bone was seen and medullary canal recanalized at 8 weeks postoperatively. The histological section showed the bone lacuna and lamella bone were formed. Conclusion It suggests that allogeneic fetal bone in combination with a covering cryopreserved periosteal allograft can promote bone repair, and allogeneic fetal bone is excellent bone substitute.
4.Individual and combined effects of ginkgo biloba extract and tanshinone Ⅱ A against cerebral ischemia and reperfusion injury in gerbils

Objective To investigate effects of ginkgo biloba extract(GBE) and tanshinoneⅡ A preventative administration against cerebral ischemia and reperfusion injury in gerbils.Methods Cerebral iscthemia and reperfu- sion injury were made by 10 minutes'occlusion of bilateral carotid arteries followed by 24 hours' reperfusion in ger- bils.GBE,tanshinone Ⅱ A,nimodipine or GBE plus tanshinone Ⅱ A were administrated intragastrically 3 days prior to and at the day of ischemia and reperfusion.Effects of experimental agents on mortality,stroke index,brain body index,water content in brain hemisphere,level of SOD activity and MDA in cerebral tissue were measured,and pathological changes of cortex and hippoearnpal CA1 sector were observed.Results GBE 48 mg/kg and tanshinone Ⅱ A 25mg/kg preventive administration could significantly reduce the stroke index,brain body index,and water in brain hemisphere,together with a reduced mortality of cerebral ischemia and reperfusion injuried gerbils.The two a- gents could also significantly improve the activity of SOD,reduce the level of MDA in cerebral tissue and the injury in cortex and hippocampal CA1 sector.Moreover,combined treatment of these two agents demonstrated more signifi- cant effects.Conclusion GBE,tanshinone Ⅱ A and combined therapy of these two agents may protect cerebral func- tion from ischemia and reperfusion injury through reducing the cerebral edema and attenuating the injury of oxygen free radicals.

Objective To study the protective role of AngiotensinⅡreceptor inhibitor in ventilator-induced lung injury of rats.Method Forty healthy male SD rats were equally divided into four group (A,B,C,D group,n=10).Group A served as control group,group B had low tidal volume (V_T=10 ml/kg) with breathing rate (P)=80/min;group C had high tidal volume (V_T=40 ml/kg) group with breathing rate 80/min;group D had high tidal volume (V_T=40 ml/kg) group with breathing rate 80/min,all rats in group D were pretreated with Losartan.The duration of ventilation in 'all groups was two hours.Rats were sacrificed after experiment finished. The lung lavage liquid and lung tissue were collected and preserved with well established methods.Lung pathological change was observed by microscope;lung cell apoptosis was assessed with TUNEL;the expression of ANGⅡwas assayed with RT-PCR.The measured variables also included total protein,WBC,W/D,MPO. 6.Protective effect of Xingnaojing injection on traumatic brain injury.

OBJECTIVETo investigate the protective effect and mechanism of Xingnaojing(Traditional Chinese Medicine) injection on brain injury in rats.
METHODSSixty-three healthy adult male SD rats were randomly divided into 3 groups (n = 21): sham operation group, model group, xingnaojing group. The model of traumatic brain injury model group and Xingnaojing group used the free fall impact injury method, the sham operation group underwent craniotomy, did not cause brain damage. Xingnaojing group in rats after 10 min by tail vein injection Xingnaojing injection 10 ml/(kg x d), model group and sham operation group were intravenously injected with 0.9% sodium chloride solution, three groups were administered continuously for 7 days. At administration of the seventh days compared the S-100B protein in the serum and neuro specific enolase (NSE) level, the water content of brain tissue, serum superoxide dismutase (SOD), malondialdehyde (MDA) and glutathione peroxidase (GSH-Px) content, and neurological function of rats among groups.
RESULTSCompared with the sham operation group, the nerve defect, brain water content, MDA, S100B protein and NSE levels were obvigusly increased in Xingnaojing group and model group; SOD, GSH-Px content decreased significantly; In Xingnaojing group nerve impairment and brain moisture were significantly lower than those of model group, the serum MDA, S-100B protein and NSE levels were significantly lower than those in the model group, the SOD, GSH-Px activity was significantly higher than that in the model group.
CONCLUSIONXingnaojing injection has protective effects on rat brain injury, and its mechanism may be related to reduce brain edema after traumatic brain injury and inhibit the reaction of oxygen free radical, protect nerve cells.

7.Research progress of optic neuritis epidemiology

Optic neuritis is one of the most important neuro-ophthalmologic diseases which cause vision loss in young adults. There are obvious differences in the prevalence of optic neuritis in different countries and regions. Through the review of the related literature at home and abroad in recent years, this article summarizes the etiology and epidemiological characteristics of optic neuritis. lt is expected to provide reference and comparison for the epidemiological investigation of cross regional, multi-center cooperation.

Allergic rhinitis and rhinosinusitis often complicate with asthma,and their relationship has long been investigated.From the view of epidemiology,all of these three diseases have higher prevalence,complicate with each other,are risk factors and prognostic factors for each other.Besides,they share common in anatomy and pathophysiology.In this paper,the interrelationship among allergic rhinitis,rhinosinusitis and asthma in pathogenesis is discussed.
9.Effect of ischemia/reperfusion on expression of endothelin in the rat ventral prostate and preventive measure

AIM:Effect of ischemia/reperfusion on expression of endothelin-1(ET-1) in the rat prostate and preventive measure were studied. METHODS:The abdominal aorta of rat was clipped briefly and repeatedly so as to treat the prostate with ischemia/reperfusion and expression of ET-1 mRNA in the ventral prostate was determined by RT-PCR.RESULTS:Expression of ET-1 mRNA in the ventral prostate was significantly increased at 1 h and 3 h after 90 min repeated ischemia/reperfusion (P<0.05), and was not significantly changed after previous treatment of Dizocilpine maleate (MK-801) (P>0.05). CONCLUSIONS:Expression of ET-1 in the prostate can be affected by repeated brief ischemia/reperfusion and it may play a role in the development of benign prostatic hyperplasia. Ischemia-reperfusion-induced ET-1 expression in the prostate of rats can be inhibited by prectreatment of MK-801.
10.Construction of double eyelid with incision of palpebral margin

Objective To evaluate results of double eyelid operations with different incisions.Methods 100 patientswere selected randomly from out-patients that accepted double eyelid operation.Half of them were objected to double eyelid operation through a palpebral marginal incision.Incision was designed along the palpebral margin,close to boots of eyelashes.Most of the subcutaneous tissues was cut and removed,and then dermis and tarsal plate were fixed with 5 stitches of suture.About 1-3 mm wide of skin along superior margin was removed reasonably.In another 50 cases.double eyelid operation was performed with conventional incision.Incision was designed 6-8 mm far from the palpebral margin.Most of the subcutaneous tissues was removed.and then dermis and tarsal plate were fixed with 5 stitches of suture.Results Postoperative results were evaluated 5 days and 1 month after operation respectively.Postoperative double eyelid shapes were all satisfled in two groups.Swollen eyedids and scar were more inconspicuous in the cases with the palpebral-marginal incision without impairment on eyelashes.Conclusion Compared with double eyelid operation with conventional incision,blepharoplasty with palpebral margin incision has the same results but with lighter impairment,milder hydropsia and more inconspicuous scar.The key point of this procedure is the fixation of dermis and tarsal plate,keeping a good radian of double eyelid line.
