1.Compare the curative effect of Non-Hodgkin lymphoma by four types of therapy

Objective To evaluate the curative effect of nasal non-Hodgkin lymphoma(N-NHL) by four types of therapy such as pure chemotherapy, pure radiotherapy, chemotherapy combined with radiotherapy, and APBSCT combined with TBI. Methods One hundred and thirty five patients with nasal NHL were treated between 1980 and 2000. All patients received radiotherapy alone or chemotherapy alone or radiotherapy combined with radiotherapy or TBI combined with APBSCT. The median radiation dose to the nasal cavity was 56.0 Gy with a range of 35.2 to 75.5 Gy. Six patients received TBI combined APBSCT. The TBI dose was 8 Gy. Two patients received 30 Gy in nasal of the six patients. The chemotherapy regimen consisted of 2-6 cycles of COP, COPP, COMP, CHOP, COBDP. Results The local control rate and 5-year survival rate of the four groups of pure chemotherapy, pure radiotherapy, combined chemotherapy and radiotherapy and APBSCT combined with TBI were 12 %, 69 %, 76 %, 83 % and 9 %, 52 %, 63 %, 83 %. For the four groups, the best is APBSCT, then combined chemotherapy and radiotherapy group ,then pure radiotherapy, the last is pure chemotherapy. 2.Treatment and prognosis of 135 nasal non-Hodgkin′s lymphoma patients

Objective To evaluate the effects of nasal non-Hodgkin′s lymphoma(N-NHL) treated with chemotherapy alone, radiotherapy alone, chemotherapy plus radiotherapy and autologous peripheral blood stem cell transplantation(APBSCT) combined with total body irradiation(TBI);and to analyze the impact factors of prognosis. Methods 135 patients were treated between 1980 and 2000. All were confirmed by histopathology as N-NHL, including 122 T cell in origin, 12 B cell and 1 NK cell in origin. The main radiotherapy portal was set in front of the nose with a spade-like protrusion, supplement with a portal next to the ear on one side or both sides. Combined portal in facial cervical area was first used when there was invasion of the oropharynx. The median dose to the nasal cavity was 56.0Gy with a range of 35.2 to 75.5Gy, with added 30Gy to the primary lesion in two patients. Six patients received TBI combined with APBSCT, with 8Gy in the TBI group. Chemotherapy, given before or during after radiotherapy or alone, consisted of 2-6 cycles of COP, COPP, COMP, CHOP or COBDP. Prognostic factors were analyzed with Cox model. Results The local control rate was 12%,69%,76% and 83% in chemotherapy alone, radiotherapy alone, chemotherapy plus radiotherapy and APBSCT combined with TBI, respectively(P=0.057).The 5-year survival rate was 9%,52%,63% and 83%,respectively(P=0.032). Multi-factor analysis showed that tumor extension and treatment methods were the most important prognostic factors besides Ann-Arbor stage, but gender, pathology, age and symptoms had little effect on prognosis .Conclusions Chemotherapy plus radiotherapy group achieves a better survival rate than radiotherapy alone. It is helpful to evaluate prognosis to make more detail subareas on basis of local extensions in Ann Arbor staging system.For some N-NHL patients with good financial condition, APBSCT combined with TBI is a good choice.

Objective To explore the diagnosis and treatment rule of eczema by Zhuang Guokang. Methods Data of 58 eczema patients treated by Zhuang Guokang were collected as research objects to set up the database. Support vector machine (SVM) method was adopted to take data mining research of the key points of syndrome differentiation and the rule of medication. Results The nourishing Yin method was extracted as the category property to get 3 classification models of the syndrome differentiation key points, the related treatment methods and medication rule and 63 relevant quantitative data. The accuracy rate of model classification of the main differentiation points of nourishing Yin method corresponding to Yin reached 77.586%, and Kappa value was 0.536. Among these points, the weight of chapping(1.558), papules(1.456), thready pulse(1.036)and desquamation(0.858) got higher significance. The accuracy rate of model classification of medication feature reached 86.2069%, and Kappa value was 0.719. Among these medicines, Pseudostellaria heterophylla(0.776), Semen plantaginis (0.550), Asparagus root(0.525) and Solomonseal (0.388) got higher significance. Conclusion The research revealed the syndrome differentiation key points and the medication rule of Zhuang Guokang’snourishing Yin methodin the treatment of eczema, and explored the path of experience transforming to knowledge of the famous old TCM doctors.
4.Baicalein induced Nrf2 activation and its protection against hepatotoxicity

Aim To study the activation of Nrf2 in-duced by baicalein ( BAI ) , and its protection against carbon tetrachloride ( CCl4 ) , ethanol and acetamino-phen ( APAP )-induced hepatotoxicity. Methods A reporter gene assay was conducted in human normal liver L-02 cells to detect the activation of transcription factor Nrf2 induced by baicalein. APAP ( 10 mmol · L-1 ) , CCl4 (10 mmol·L-1 ) and Ethanol (100 mmol · L-1 ) were used to induce hepatotoxicity in L-02 cells. After the pre-incubation with Baicalein (0, 1, 10, 25, 50, 100 μmol·L-1 ) for 15 min, cells were administrated with or without those above hepatotoxins. 48 h later, cell viability was detected by 3-(4, 5-dim-ethylthiazol-2-yl ) 2 , 5-diphenyltetrazolium bromide (MTT) method. Results Baicalein (25, 50 μmol· L-1 ) induced the activation of Nrf2 ( P <0. 01 , P <0. 05) in the reporter gene assay. As compared with control, three hepatotoxins ( APAP, CCl4 , Ethanol ) all decreased cell viability ( P<0. 01 ) , and baicalein significantly reversed such decreases in a concentra-tion-dependent manner ( P<0. 01 ) . Conclusion Ba-icalein can induce the activation of transcription factor Nrf2 , which is probably one of the mechanisms con-tributing to the protection of baicalein against hepato-toxins (APAP, CCl4, Ethanol)-induced hepatotoxici-ty.
7.Value of ~(18)FDG PET-CT after radiotherapy in nasopharyngeai carcinoma

Objective To evaluate the role of 18F fluorodeoxyglucose positron emission tomography computed tomography (PET-CT) in nasopharyngeal carcinoma (NPC) after radiotherapy. Methods A total of 27 NPC patients received 18FDG PET-CT 8-32 weeks after radiotherapy. All the patients were followed up for about 12 months after the examination. Metastasis and residual were evaluated by PET-CT. The correlation between SUV and prognosis was analyzed. Results Of these 27 patients, metastasis was found in 2 patients by PET-CT. Local persistence was diagnosed as for SUV≥2. 5 by PET-CT in 20 patients, among whom 18 were confirmed by biopsy and then received brachytherapy or conformal radiotherapy. One year local control and survival rates were 70% and 81%. Based on SUV, the patients were divided into group one for SUV between 2. 5 and 5(9 patients) or group two for SUV≥5 (11 patients). In group one and group two, the one year local control rate, survival rate and metastasis rate were 67% , 55% (P=0.670) , 64% ,89%(P=0.319), and 22% , 82% (P =0. 022) , respectively. Conclusions PET-CT is valuable for the identification of residual nasopharyngeal carcinoma. SUV of residual tumor is related to metastasis.
8.An analysis of correlation between post-stroke depression and relevant factors

Objective To investigate correlation between post-stroke depression (PSD) and multiple factors during onset of acute cerebral infarction. Methods Depression was measured with Hamilton Depression Rating Scales (HAMD) in 58 patients with acute cerebral infarction, and their neurological function were evaluated by neurological function defect (NFD) score. Their immunoglobulin G (IgG) index was calculated and level of nitric oxide (NO) in cerebrospinal fluid (CSF) was measured. Lesion and nature of cerebral infarction in 58 patients with acute stroke were located by CT. All the data were statistically analyzed with student-t test and ? 2 test, as well as linear regression model. Results Seventeen of 58 patients of stroke appeared PSD with an occurrence rate of 29.3%. 9.Analysis of plasma glucose change in 61 cases of severe acute respiratory syndrome

Objective To analyse the plasma glucose change in the patients with severe acute respiratory syndrome (SARS) and to discuss its possible mechanism. Methods The clinical data of 61 cases with SARS in Tianjin were retrospectively analysed. Results Plasma glucose level increased in 26 cases with SARS, among them, 2 cases suffered from diabetes before SARS onset, 24 cases were diagnosed as the secondary diabetes whose blood glucose level recovered normal after SARS were cured, except 4 cases who were dead. All of the 61 cases were treated with glucocorticoids. If the patients were grouped by the dose of methylprednisolone they received, 9 out of 14 cases with large dose, 14 out of 26 cases with middle dose and 1 out of 21 cases with small dose were led to secondary diabetes. Objective To investigate the feasibility and safety of laparoscopic-assisted gastrectomy for distant gastric cancer.Methods All 18 patients with distant gastric cancer receiving laparoscopic-assisted gastrectomy were analyzed.Results Laparoscopic-assisted distal gastrectomy was performed successfully in all patients.The mean operation time was(291.33?19.61) min.The mean blood loss was(151.32?71.78) ml.The mean numbers of harvested lymph node were 14.57?3.11.The mean time of gastrointestinal function recovery was(3.46?0.93) d,the mean out of bed activity time was(1.75?0.45) d.All patients were followed up for 1-24 months,mean 11 months.No local recurrence,trocar implant or distant metastasis happened.Conclusion Laparoscopic-assisted gastrectomy is a feasible and safe surgical procedure combined with minimal trauma and fast recovery.
