ObjectiveTo investigate the application and effect of problem-based learning ( Problem-based learning,PBL ) pedagogy in theoretical teaching of pediatric nursing.MethodsThe two nursing classes in the same grade were selected respectively as a control group of 62 people and research group of 62 people.Traditional pedagogy was taken in control group to give lessons for theory pediatric nursing,while PBL pedagogy was taken in research group to give lessons ftr theory pediatric nursing.At the end of the semester,students' scores in theory exam,practice exam and case care were analyzed by using t tests.ResultsThe results of the two groups of students were compared,with t value 4.96 of the theoretical results,t value 5.312 of the experimental results,t value 8.06 of the case study,and P ＜0.05 in three items,suggesting statistically significant difference.ConclusionsPBL pedagogy can raise nursing students' learning interest in the theory of pediatric nursing,and improve teaching effect on theory of pediatric nursing.

Objective To investigate the effect of hyperthermia combined with chemotherapy on the changes of immunity balance of Th1/Th2 and cytokines such as interleukin-2 (IL-2),interferon-γ (IFN-γ),IL-4,IL-10,and vascular endothelial growth factor (VEGF) in non-small cell lung cancer (NSCLC) patients.Methods Fifty patients with advanced NSCLC were randomly divided into thermochemotherapy group (n =25,received whole body hyperthermia combined TP chemotherapy),and chemotherapy group (n =25,received TP chemotherapy alone).The immunity balance of Th1/Th2,IL-2,IFN-γ,IL-4,IL-10,and VEGF levels before therapy and 1 week after therapy were observed.At the same time,the above observation indices were also detected as normal control in 20 healthy volunteers.Results Compared to the chemotherapy group,the proportion of Th1 cells from thermochemotherapy group was significantly increased after therapy (P =0.002) and that of Th2 cells was dropped markedly (P =0.006).There was no difference in the levels of IL-2,IFN~,IL4,IL-10,and VEGF between thermochemotherapy and chemotherapy groups before therapy (P ＞ 0.05).However,the expressions of ILA,IL-10,and VEGF in the NSCLC group were significantly higher than normal group (P ＜ 0.01).The IL-2 and IFN-γ levels were significantly decreased in NSCLC group (P ＜0.01).After therapy 1 week,the concentration of IL-2 and IFN-γfrom thermochemotherapy and chemotherapy groups were higher than before,with statistical significance (P ＜0.01).In addition,IL-4 and IL-10 levels of two groups were significantly decreased in comparison with before therapy (P ＜ 0.01),and it showed a striking difference in cytokines (IL-2,IFN-γ,IL-4,and IL-10) between two groups (P ＜ 0.05).However,the concentration of VEGF in chemotherapy group patients before and after chemotherapy had no significant difference (P ＞ 0.05).The concentration of VEGF from thermochemotherapy group patients after therapy were lower than chemotherapy group patients,the difference had statistical significance (P ＜ 0.01).Conclusions Thermochemotherapy can significantly prompt the Th2 to Th1 shift in both cells and cytokines of advanced NSCLC compared to chemotherapy alone.Moreover,thermochemotherapy also reduces the expression of VEGF,which may prevent the formation of new blood vessels in tumor tissue.

Objective:To explore the molecular mechanism of Gold Theragran in the protection of renal function,and the inhibition of renal local rennin angiotensin(RA)system in the early diabetic nephropathy(DN),and to provide the theoretical and experimental data for clinical application to prevent the DN.Methods:The experimental rats consisted of 64 healthy SD rats,average 180-220g,female and male both a half,rat model of DN was established by improved method of YANG Junwei,s method of intraperitoneal injection of STZ after nephrectomy.8 Rats was taken as normal group,the rest of them were randomly divided into 5 groups:high,middle and low dose groups of Gold Theragran,Valsartan group,model group.In each group,blood glucose,blood lipids,urine microalbumin per 24 hours,KW/BW,renal angiotensin converting enzyme,plasma and renal angiotensin Ⅱ,and its type 1 receptor were observsd and measured.Results:①Rat's general status in each treatment group was improved compared with the DN model group.②The levels of serum glucose decreased in all treatment groups,especially the high-dose group and mid-dose group were significant(P0.05).③The levels of abnormally high lever of TG,TC decreased in treatment groups,especially the high-dose group and mid-dose group were significant(P0.05).The high-dose group had further function to regulate the lipids in the 28th day than in the 7th day(P0.05).Conclusion:Firstly,Gold Theragran had a therapeutic effect of protecting the renal function on the experimental DN rats;Secondly,Gold Theragran can inhibit the abnormal state of activated rennin angiotensin system in diabetic nephropathy,delay the development of diabetic nephropathy;Thirdly,with the treatment course prolonged,the effective of Gold Theragran might be more obvious,and different dose groups show dose-effect-dependent relationship.

Objective To investigate the changes of serum homocysteine ( Hcy),C-reactive protein ( hsCRP) levels in acute cerebral infarction patients and its significance.Methods 64 patients with acute cerebral infarction were selected as research group,and 60 healthy volunteers were selected as control group.Detected the acute cerebral infarction patients after 1d,the first 7d,14d Hcy,hs-CRP,total cholesterol,triglycerides,high density lipoprotein,low density lipoprotein levels and the control group the level of these indicators.Results Of total cholesterol,triglycerides,low density lipoprotein of the study group was higher than those of the control group of high-density lipoprotein(t =3.073,2.874,2.015,all P ＜0.05);after admission ld,treatment of the first 7d,14d showed no significant difference between the two groups ( t =2.223,2.268,all P ＜ 0.05 ).Conclusion Hcy,hs-CRP and the occurrence of acute cerebralinfarction was closely related to the development,and testing to determine the severity of the disease for the guide had important clinical significance.

Objective To evaluate feasibility, effectiveness , safety, and complications of superselective bronchial artery embolization with coaxial microcatheter system for hemoptysis. Methods 32 times of superselective bronchial artery embolization were performed for 29 cases with hemoptysis resulted from a variety of causes, age grade from 35 to 71(mean 54) year old. The causes of hemoptysis included bronchiectasis (9 cases), tuberculosis (12 cases), and pulmonary carcinoma (8 cases). All studies were performed with standard percutaneous transfemoral catheterization. Results The success rate of superselective catheterization was 96 9% (32/33). Hemoptysis control was achieved in 96 9% (31/32), with complete control rate of 65.6% (21/32) and partial control rate of 31.3% (10 /32),inefficacy case:only one.One patient with bronchiectasis, 6 patients with pulmonary carcinoma and 3 patients with chronic fibrosis tuberculosis were included in the partial control group. Rebleeding occurred in 7 patients (4 patients with pulmonary carcinoma and 3 patients with chronic fibrosis tuberculosis) during 2 months to 32 months followed-up period. No severe complications occurred in the procedure. Conclusions Superselective bronchial artery embolization for hemoptysis with coaxial microcatheter is a safe, effective way to control hemoptysis, has excellent long term efficacy, and without severe complications.

The effect of Taurine ( Tau)on 45Ca influx of isolated left ventricular muscles in rats has been investigated. The 45Ca influx of the preparations untreated with Tau, are affected by the concentrations of calcium in KH solution. when the concentration of calcium is 0.62; 1.25; and 1.87mmol/L respectively, the 45Ca influx is 1.02? 0.25; 1.37?0.14 and 1.45?0.14 ?mol/g wet tissue, respectively. After adding Tau, the situations above were changed significantly. It was shown that is 1.11 ? 0.11, 1.45?0.12, and 1.48 ? 0.09 ?mol/g wet tissue in low Ca2?; 1.19?0.07, 1.14 ? 0.23, and 0.97?0.24 ?mol/g wet tissue in normal Ca2?; and 1.12?0.05,0.58?0.18 and 0.53?0.10 ?mol/g wet tissue in high Ca2? treated with Tau 10, 20, and 40 mmol/L, respectively. The results show that the varied ,Ca2? concentrations in KH solution can affect 45Ca influx of cardiac muscles, but Taurine has a bisphasic modulation o,f calcium influx in the same tissue. The effects of modulating calcium may play an important role for anti-arrhythmic mechanism of Taurine.

ObjectiveTo investigate the effects of ARB or/and β-blocker in treatment of dilated cardiomyopathy(DCM).Methods95 cases with DCM were randomly divided into 3 groups.ARB group(A,32 cases) was treated with Irbesartan,150 mg/d without adverse drug reaction and then to 300 mg/d weekly; β-blocker group( B,30cases) was treated with Bisoprolol,1.25 mg/d without adverse drug reaction and then to maximal dose weekly; combination group( AB,33 cases) was treated with Irbesartan and Bisoprolol to maximal dose which patients were able to tolerate.The 3 groups were observed for a period of 12 months.ResultsComparing with pretreatment,the results of 6min walk,LVEDd,LVESd,LVEF were improved significantly( all P ＜0.05).There were no significant differences between A group and B group(P ＞0.05),but the differences between combination group and A group or B group were significant( P ＜0.05 ).ConclusionEffect of ARB combined with β-blocker was better than that of ARB or β-blocker alone in treatment of DCM and well tolerated.
