目的探讨以主动运动为主的社区康复训练对脑卒中恢复后期患者日常生活活动能力的影响。方法将85例社区脑卒中恢复后期患者分为对照组42例和康复组43例。出院后，康复组继续在社区接受以主动运动为主的康复训练，对照组不再进行任何康复训练。结果经过90 d左右，康复组Barthel指数明显提高（P<0.01）。结论以主动运动为主的社区康复训练可提高偏瘫患者的日常生活活动能力。

Objective To explore the effective communication between therapists and inpatients during rehabilitation treatment. Methods The communication needs were evaluated among 150 inpatients, including stroke and spinal cord injury, following physical dysfunction in 7 aspects to determine the patient's psychological and demand conditions. Then, targeted and effective communication plans were developed and adjusted according to the individual changes of the patients at all stages of the pathogenesis and the patients' feedback information. Finally, the patients' satisfaction surveys were issued when the patients discharged. Results After effective communication was used, 98.5% patients felt satisfied with shortened hospital stay and satisfactory limb function. Compared with the previous patients, effective communication can significantly improve the outcomes and reduce the incidence of doctor-patient disputes. Conclusion Effective communication is helpful for the patients to understand the disease, which can enhance the rehabilitation of compliance and improve rehabilitation outcome.

Objective To explore the effect of rehabilitation guidance combined with family training on motor function and activities of daily living of subacute stroke.Methods 89 patients with subacute stroke were divided randomly into two groups,control group(43 cases)and rehabilitation group(46),receiving routine neurological medication.In addition,rehabilitation group received rehabilitation guidance and training in the meantime.Results After 60 days of rehabilitation training,the scores of Fugl-Meyer Motor Function Assessment(FMA)and Activities of Daily Living improved significantly(P<0.01;P<0.001).Conclusion Rehabilitation guidance combined with family training is effective to improve motor function and activities of daily living of subacute stroke.

Microwave ablation(MA)has been one of the most quickly developed minimally invasive techniques for trearing hepatocellular carcinoma(HCC)in the last decade.Under accurate imaging guidance,MA shares the common advantages of thermal ablation such as flexible treatment approaches,good tolerability and the ability to consistently create reproducible and predictable necrotic areas.It also exhibits several particular advantages compared with other thermal ablation methods,including higher thermal efficiency,higher capability of coagulating blood vessels,faster ablation time,and an improved convention profile.Over the years,MA equipments,especially the antenna,have been refined for safety and to increase the coagulation zone.With the development of internally-cooled antenna and using the frequency of 915 MHz nficrowave,the technique is becoming safer and the coagulation zone increased significantly.Initially,MA was limited to the treatment of small HCC,but with the improvement of antenna and treatment strategV,large HCC(＞5 cm)can also be ablated effectively.For the"curative"treatment of HCC.the following eriteria need to be met:single HCC with the lesion of 6 am or smaller;three or fewer HCC lesions with a maximum diameter of 4 cm,and absence of portal vein thrombosis or extrahepatic metastases.A five-year cumulative survival rate has exceeded 50%,which is comparable with resection.Moreover,the local immune function is enhanced after MA.A change in immunocyte infiltration takes place in number,configuration and location after patients with HCC are treated with MA.Further research for MA may be focused on the improvement of the maneuverability of the equipment,increase of the accuracy of antenna puncture and consummate the clinical therapeutic standardization.

Purpose To investigate the expression of Lgr5 in cervical squamous cell carcinoma (CSCC) and its significance.Methods Lgr5 protein expression was evaluated by immunohistochemistry of EnVision two-step in 83 paraffin-embedded CSCC specimens,56 normal cervical tissues specimens and 32 cervical intraepithelial neoplasia (CIN) specimens.The expression of Lgr5 mRNA was evaluated by RT-PCR in 13 pairs of surgically removed CSCC and adjacent normal cervical tissues.The correlation between Lgr5 expression and clinicopathological features were statistically analyzed.Results The positive proportion of Lgr5 protein in CSCC tissues was significantly higher than those in adjacent normal cervix (P ＜ 0.01) and CIN (P ＜ 0.05).The expression of Lgr5 in CSCC tissues was associated with grade of tumors (P ＜ 0.01) and depth of invasion (P ＜ 0.05),but not associated with age,tumor size,clinical stage and lymph node metastasis (P ＞ 0.05).Kaplan-Meier analysis showed that the 5-year survival rate of patients with Lgr5 overexpression was significantly lower than that of patients with lower expression (P ＜ 0.01).Conclusion The expression of Lgr5 in CSCC tissues may associated with grade of tumors and depth of invasion and may be involved in initiation and development of CSCC,and it may be a new assistant marker for prognosis of CSCC.

The rabbits were randomly divided into 4 equal groups of 12 animals in each group: Ⅰ control group in which normal saline 0.2-0.3 ml?kg-1 was injected through epidural catheter; Ⅱ epidural group in which 1% lidocaine 2-3 mg?kg-1 was injected epidurally; Ⅲ propofol group in which propofol was infused iv at 20 mg ? kg-1 ?h-1 for 30 min; Ⅳ combined epidural-propofol group in which epidural 1% lidocaine and propofol iv were both administered. Blood samples were taken before and 4 h, 8 h after ligation for determination of serum concentrations of myocardial troponin I (cTn I ) and MDA and serum SOD activity.The myocardial tissue was taken from the infarct area for determination of the apoptotic myocyte count (TUNEL) and p53 and Bcl-2 protein expressions (LSAB) . Results In control group serum cTn I and MDA increased and SOD decreased significantly after ligation of LAD. Thoracic epidural anesthesia, propofol iv infusion and especially the combination of them suppressed the increase in serum cTn I and MDA and decrease in serum SOD induced by ligation of LAD.In epidural-propofol group and propofol group the apoptotic myocytes count and p53 protein expression significantly decreased, the Bcl-2 protein expression increased as compared with those in control group. Conclusion Thoracic epidural anesthesia combined with propofol can significantly protect myocardium from injury induced by myocardial infarction. This action is mediated by the depression of myocyte apoptosis and the generation of oxygen-derived free radicals.

Objective:To detect the effects of porcelain-fused-to-metal(PFM)crown on the periodontal tissue of the restored teeth,and the relationship between the levels of intedeukin(IL)-1β,aspartate transaminase(AST)and alkaline phosphatase(ALP)in gingival crevicular fluid(GCF)and the health condition of periodontal tissue.Methods:Among 28 patients,34 maxillary incisors with PFM crowns were selected for the study.There were 19 Ni-Cr PFM crowns and 15 Au-Pt PFM crowns.The contralateral homonymy natural teeth were healthy and treated as control teeth.The patients were followed up 6 to 8 months after restoring crowns.The values of IL-1β,AST and ALP in GCF were analysed.Results:The concentrations of IL1β and ALP were significantly higher in GCF of Ni-Cr PFM compared with those of control,whereas no significant difference in Au-Pt PFM group compared with that of control.The level of GCF-AST was higher in Ni-Cr PFM and Au-Pt PFM group than that of healthy control.Conclusion:Ni-Cr PFM crowns have negative influence on periodontal tissue,but Au-Pt PFM have little influence on periodontal tissue of the restored teeth.The activity of IL-1β,AST and ALP of GCF are sensitive to reflect the effect of PFM crowns of different materials on periodontal tissue.
