The Effect of Pantoloc IV (Pantoprazole) on Gastroduodenal ulcer Bleeding: * AIM: Effect of Pantoloc IV on Gastroduodenal ulcer Bleeding and side effect of Pantoloc IV. * Conclusion: Pantoloc IV is drug good to stop bleeding cause gastroduodenal ulcer: Stop bleeding in the first day, short of the hospital stay, reduce unit of blood to treatment per case, reduce cases to sclemotherapy, operating. Successful: 88,6%. Most of them have not side effect.

A retrospective study on 146 cases of upper digestive bleeding clinically and endoscopic diagnosed within the first 24 hours during 1995-1999, who received the endoscopic emegency injection for bleeding stop in the gastroduodenal ulceration has shown that the primary rate of bleeding stop in the ulceration focuses was 86.30%. The method showed inexpensive, easy to implement which helped early and precisely diagnosing the lesions with continuous bleeding and risks of relapsed bleeding and remove the patients from the emegency condition to a stable condition. The method can be applied widly in the whole country.

A prospective randomized study was performed to study the hemostatic effect of injection therapy with adrenaline 1/10,000 in the treatment of ulcerated peptic bleeding (PUB). 30 patients with PUB were included. The treatment was performed until bleeding stopped, in case of relapse or in risk of relapse, the injection was repeated. Successful rate was 100%. Stable condition (stop bleeding) after the first injection was observed in 16 patients (53.3%). The number needed the second injection were 14 patients (36.6%).There were 3 patients needed the third injection. Dose of adrenalin for each bleeding site: 8.9 +/- 1.2 ml, number of injections: 4.2+/- 1.0 injection (maneuver) time: 21+/- 6.2 minutes. Adrenalin were effective in stop bleeding, but had to reapeat. Using adrenalin is safety and low cost.

From March 2001 to August 2001, at the Military Hospital N0 108, 185 cases of gastrointestinal hemorrbiage due to gastroduodenal ulcer were managed, among them 55 bleeding cases by HSE injection. The incidence of this hemorrhage was higher in male patients than in femal and a majority of patients was in productive age. HSE injections gave successful results on 100%. The rate of success in the 1st injection attained in 47 cases (81.2%), in 8 cases (18.2%) the bleeding recured or had got the risk of recurrent bleeding and an injection again to cease the hemorrhage was needed. There was 1 patient needed the third injection. The mean dose of efficacy for 1 bleeding ulcer attained 8.82.6ml with 4.01.2 injections. The time of injection was 17.35.1 minute. HSE were effective in stop bleeding, using HSE is simplicity, safety and low cost.

The study involved 30 patients (21 males, 9 females, aged from 18 to 77) who had gastroduodenal ulcer-caused bleeding at the Hospital 103 between October 1999 to July 2000. Common symptoms included dizziness, tar-like stool, fatigue and fast pulse. The patients received Sandostanin 0.1 mg two ampoule mixed in 500 ml isotonic glucose or saline solution intravenous for 4 - 5 hours in consecutive 3 days. Results: Sandostanin (octreotid) is effective for gastroduodenal ulcer-caused bleeding, stops bleeding rapidly even in first day (93.4%), reduce the amount of transfused blood, shorten the stay duration, reduce the relapse, rate of induced sclerotization and operation is lowered. Sandostanin is safe, almost free of adverse effects and should be indicated early.

From January to December 2002 at Cho Ray Hospital Ho Chi Minh City, bleeding peptic ulcer was recognized in 273 patients. Surgical treatment was used in 11.4% (31/273 cases). The indication for surgery was correlated with site, size, general status, lesion of the bleeding peptic ulcer and rebleeding or failure of endoscopic treatment as well as bleeding degree. Surgical procedures including simple suture (n=10), Weinberg's operation (n=3), and subtotal gastrectomy (n=18). Postoperative rebleeding rate was 19.4% (6/31) and was the highest in simple suture procedure. Mortality rate was 25.8% (8/31)

So-called, the double channel pylorus has been, relatively rarely, reported through out the world. All reported cases were almostly those of gastroduodenal fistula as a complication of peptic ulcer disease. The congenital forms of double pylorus were reported even more rarely. We found three cases of double channel pylorus in 25,000 cases endoscopy done at our unit. All those three cases were thought to be acqired form endoscopically, radiographically and histologically. One of those was undergone to operative resection because of uncontrollable bleeding and the others were on medical theraphy with satisfaction.
