1.Clinical observation of alprostadil, benazepril combined with Danqi Decoction and herbal enema in treatment of diabetic renal proteinuria

Objective To observe the clinical efficacy of alprostadil, benazepril combined with Danqi Decoction and herbal enema in treatment of diabetic renal proteinuria.Methods Sixty cases of diabetic renal proteinuria patients were randomly divided into the control group and the experimental group.Each group was 30 cases.The control group was given alprostadil(0.2μg/mL,2 times per day) and benazepril(10 mg,one time per day), while the experimental group was given Danqi Decoction combined with herbal enema on the basis of the control group(one time per day).The urea nitrogen, serum creatinine, quantitative of proteinuria (in 24 hours) and the excretion of urinary micro-albumin of both groups were all tested after three weeks of continuous treatment.ResuIts After three weeks of treatment, the laboratory values of urea nitrogen, serum creatinine, quantitative of proteinuria in 24 hours of both groups were all improved compared with those before treatment(P<0.05).Furthermore, the quantitative of proteinuria(in 24 hours) of the experimental group was significantly less than that of the control group(P<0.05), and the excretion of urinary micro-albumin of the experimental group was significantly less than that of the control group(P<0.05).ConcIusion The therapy of alprostadil, benazepril combined with Danqi Decoction and herbal enema is effective for diabetic renal proteinuria patients, and can significantly reduce proteinuria and renal injury.
2.Expression of Cyclie E in colorectal carcinoma and its significance

Objective: To investigate the expression of Cyclin E in colorectal carcinoma and its significance.Methods: The Cyclin E expression and the cell proliferation(PI,SPF)in 30 cases of colorectal carcinomas (including tumors and normal tissues distant to the tumors)were respectively assayed with Flow Cytometry (FCM), and the relationships between the Cyclin E expressive rate and clinical pathologic features were compared. Correlaction between the Cyclin E expressive rate and the cell proliferation(PI, SPF) was analyzed too.Results: The Cyclin E positive rate and the cell proliferation in colorectal carcinoma were significantly higher than those of the distal normal tissues. As for the Cyclin E positive rate and the cell proliferation,no significant differences were found among the subgroups divided according to malignance, lymphatic metastasis and the site of tumor. There was a significant correlation between the Cyclin E positive rate and the cell proliferation.Conclusion: The Cyclin E overexpression plays an important role in the onset of colorectal carcinoma.The Cyclin E expressive rate is not consistent with the general clinical pathologic features,but with the cell proliferation. The Cyclin E expressive rate may be one of the potential prognostic indicators in coloretal carcinoma.
3.Early microsurgical removal of 31 patients with low-grade (Spetzler-Martin Ⅰ-Ⅱ) bleeding arteriovenous malformations

Objective To explore the safety and effectiveness of early microsurgical removal of low-grade (Spetzler-Martin Ⅰ-Ⅱ) bleeding arteriovenous malformations. Methods The clinical data of 31 patients with low-grade AVM by microsurgical treatment in our hospital from October 2009 to December 2015 were retrospectively reviewed in the acute stage of bleeding (within the first week after bleed). All patients showed a cerebral AVM on DSA or CTA at admission, 18 of whom underwent intraoperative ultrasound. Neurological outcomes were assessed with Glasgow outcome scale (GOS) after operation. According to the GOS scores, the patients were divided into good prognosis group (scores of 4-5) and poor prognosis group (scores of <4); the differences of preoperative clinical data between the two groups were compared. After operation, cerebral DSA or CTA was performed. According to the postoperative brain AVM with or without residual, the patients were divided into residual group and no-residue group; the differences of the clinical data and use of ultrasound in operation were analyzed statistically between the two groups. Results A favorable functional outcome was observed in 27 patients (87.1%); as compared with that in the poor prognosis group, the proportion of Glasgow coma scale (GCS) scores>8 and young patients in the good prognosis group was significantly higher (P<0.05). In 31 patients, there were 5 with residual brain AVM (total residual rate 16.1%); the proportion of patients with Spetzler-Martin grading (SMG) I in the residual group was significantly lower than that in the no-residue group (P<0.05). There was no statistically significant difference in the proportion of intraoperative ultrasound use between the two groups (P>0.05). Conclusions Early microsurgery for grade Ⅰ-Ⅱ bleeding AVMs is a safe and definitive treatment with intraoperative ultrasound, achieving both immediate cerebral decompression and protection against rebreeding, reducing hospital stays and allowing a more rapid rehabilitative course whenever necessary. Patients who are younger or with GCS scores>8 may have a better prognosis and high complete resection is achieved in those with SMG Ⅰ.
4.Statin Therapy Regulates Serum Inflammatory Factors in the Treatment of Abdominal Aortic Aneurysms

Objective To assess the effect of statin therapy on serum inflammatory cytokines in patients with abdominal aortic aneurysm (AAA).Methods The clinical data of 126 AAA patients who were hospitalized in the Department of Vascular Surgery,Peking Union Medical College Hospital,from September 2014 to September 2017 were retrospectively analyzed.Patients were divided into treatment group and control group according to whether statins were used or not.The levels of serum lipids and serum inflammatory factors were compared between these two groups.Results There was no significant difference in gender,age,body height,body weight,smoking ratio,and AAA diameter between these two groups (all P > 0.05).While the levels of total cholesterol (TC),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C) and high density lipoprotein cholesterol (HDL-C) were not significantly different before treatment (all P > 0.05),the treatment group had significantly lower TC (t =2.868,P =0.009),TG (t =3.472,P =0.006),and LDL-C (t =3.924,P =0.005) and significantly higher HDL-C level (t =3.322,P =0.007) after treatment.In addition,the concentrations of interleukin (IL)-1β,IL-6,high-sensitivity C-reactive protein (hs-CRP),and tumor necrosis factor-α (TNF-α) were not significantly different between these two groups before treatment (all P >0.05);after treatment,the serum levels of IL-1β and IL-6 in the treatment group were (224.32 ± 78.54) and (116.49 ± 19.64) ng/L,respectively,which were lower than those in the control group [(254.68 ±96.77)ng/L (t=1.765,P=0.058) and (126.71 ±23.59) ng/L (t=1.692,P=0.063)],although the differences were not statistically significant.The serum levels of hs-CRP and TNF-α in the treatment group were (6.46 ± 1.24) mg/L and (0.77 ± 0.21) μg/L,respectively,which were significantly lower than those in the control group [(10.93 ± 4.18) mg/L (t =2.007,P =0.012) and (1.28 ± 0.49) μg/L (t =2.144,P =0.016)].Conclusion Statin treatment reduces the levels of hs-CRP and TNF-α in AAA patients.
