1.Detection of pathogens and treatment of children with severe pneumonia

Objective To discuss pathogens findings in children with severe pneumonia.Methods Bacteria was detected by using sputum culture and blood culture in sterile culture media.Viruses and atypical pathogenic antibodies were detected by using indirect immunofluorescence.Influenza A (H1N1) virus RNA were tested using RT-PCR.According to the results of bacterial culture and drug sensitive test,we can guide the use of antibiotics,and individualize treatment was carried out,including anti-inflammatory,organ function support.Results Bacteria was found in 69 children by using sputum culture.Gram negative bacteria accounted for 57.47%.Gram positive bacteria accounted for 42.53%.Escherichia coli(14.94%),Haemophilus influenzae (20.96%) and klebsiella pneumoniae(13.79%) were the main strains of Gram negative bacteria,Staphylococcus aureus (21.84%)and Streptococcus pneumonia(16.1%)were the main strains of Gram positive bacteria.Bacteria was found in 7 (8.00%) children by using blood culture.Virus were identified in 11 out of 123 patients,including 2 cases of respiratory syncytial virus antibody positive,2 cases of adenovirus antibodies positive,4 cases of influenza B virus antibody positive,2 cases of parainfluenza virus antibody positive and influenza A(H1N1) virus from only one case,Mycoplasma pneumonia agents were identified in 8 patients.Eighty-nine children (72.36%) complicated with sepsis,85 children (69.11%) with respiratory failure,48 children (39.02%) with gastrointestinal dysfunction,32 children (26.02%) with heart failure,18 children(14.63%) with septic shock,13 cases (10.57%) with toxic encephalopathy,5 children (4.07%) with disseminated intravascular coagulation.Among them,17 children (13.82%) complicated with multiple organ dysfunction syndrome.In the 123 children with severe pneumonia,46 cases (37.4%) were cured,73 cases (59.35%) improved,and 4 cases died (3.25%) with critical multiple organ dysfunction syndrome.Conclusion The detection rate of pathogen is high in this study.We should pay more attention to individualize therapy for complication,so that the cure rate could be increased.
2.Effect of caveolin-1 expression on high-salt diet-induced endothelial dys-function in type 1 diabetic rats

[ ABSTRACT] AIM: To investigate the underlying mechanisms responsible for endothelial dysfunction of type 1 diabetes mellitus (DM) rats fed with high-salt diet.METHODS:Type 1 DM was induced by intraperitoneal injection of streptozotocin (70 mg/kg).Normal and diabetic rats were fed high-salt food (HS, 8% NaCl) and standard food for 6 weeks, respectively.Isometric tension of the mesenteric arteries were measured .The expression of Akt , endothelial nitric oxide synthase (eNOS) and caveolin-1 (Cav-1) was examined by Western blot .RESULTS:The rats in DM+HS group exhibited more pronounced impairment of vasorelaxation to acetylcholine and insulin compared with either DM group or HS group (P<0.01).Akt and eNOS phosphorylation levels, and nitric oxide (NO) concentration in DM +HS group were significantly lower than those in DM group (P<0.01).The level of Cav-1 in DM+HS group was significantly higher than that in DM group and HS group .CONCLUSION:Impaired endothelial Akt activation , increased Cav-1 expression and re-sultant decreased eNOS activation contribute to aggravate high-salt diet-induced endothelial dysfunction and hypertension in DM rats.

4.Tissue-engineered tooth regeneration:hotspots and clinical application

BACKGROUND:Although several prosthetic methods, such as artificial denture and dental implants, are clinical therapies to tooth loss, they are thought to have safety and usage time issues. With the development of biological and biomaterial sciences, recently, tooth tissue engineering has attracted more and more attention.
OBJECTIVE:To reflect advances and problems of tissue engineering technologies for promotion of tooth regeneration.
METHODS:Using the keywords of“tissue engineering, tooth regeneration”in English and Chinese, PubMed and CNKI databases from 2007 to 2013 were retrieved. A total of 65 literatures addressing tooth regeneration and tissue engineering were col ected, including 25 Chinese articles and 40 English articles. Published early, repetitive, and similar researches were excluded. Final y, 48 articles were included.
RESULTS AND CONCLUSION:The combination of stem cells and suitable scaffolds is widely used in tooth regeneration today, and growth factors or bone marrow which can produce promote tooth regeneration are added as wel , which has achieved partial or whole tooth regeneration. But there are apparent deficiencies in studies which focus on mechanisms behind tooth regeneration.
5.Therapeutic effects of B ultrasound-guided extracorporeal shock wave lithotripsy combined with tamsulosin hydrochloride medication on ureteral calculi

Objective:To investigate the therapeutic effects of B ultrasound-guided extracorporeal shock wave lithotripsy combined with tamsulosin hydrochloride medication on ureteral calculi.Methods:The clinical data of 130 patients with ureteral calculi who received treatment in Ninghai First Hospital from March 2019 to June 2020 were retrospectively analyzed. These patients were divided into an observation group and a control group ( n = 65/group) according to the different treatment methods. Patients in the control group received B ultrasound-guided extracorporeal shock wave lithotripsy, and those in the observation group received B ultrasound-guided extracorporeal shock wave lithotripsy combined with tamsulosin hydrochloride medication. Total response rate, stone-free rate after the first treatment, time taken to get rid of stone, treatment times, and Visual Analogue Scale (VAS) score 14 days after the first treatment were compared between the two groups. Renal function indexes (serum creatinine, blood urea nitrogen), mean arterial pressure, VAS score, blood loss, and immunoglobulin G, immunoglobulin M, malondialdehyde, superoxide dismutase levels were determined in the two groups. Results:The total response rate in the control group was significantly lower than that in the observation group [89.23% (58/65) vs. 98.46% (64/65), χ2 = 4.80, P < 0.05]. After 14 days of treatment, the VAS score in the observation group was significantly lower than that in the control group [(3.97 ± 0.36) points vs. (5.59 ± 0.87) points, t = 13.87, P < 0.05). After treatment, the stone-free rate after the first treatment in the control group was significantly lower than that in the observation group [61.54% (40/65) vs. 78.46% (51/65), χ2 = 4.43, P < 0.05). The time taken to get rid of stone and treatment times in the control group were (18.98 ± 3.52) days and (2.53 ± 0.50) times, respectively, which were significantly higher than (12.27 ± 2.77) days and (1.64 ± 0.55) times in the observation group ( t = 12.08, 9.66, both P < 0.05). Urine Kim-1 in the observation group was significantly higher than that in the control group [(89.46 ± 42.46) mmol/L vs. (72.75 ± 17.65) mmol/L, t = 2.93, P < 0.05]. Serum creatinine and blood urea nitrogen levels in the observation group were (101.75 ± 24.53) μmol/L and (348.76 ± 29.84) μmol/L, respectively, which were significantly lower than (139.53 ± 30.56) μmol/L and (397.65 ± 35.64) μmol/L in the control group ( t = 5.82, 8.48, both P < 0.05). After 20-minutes of anesthesia induction, the mean arterial pressure in the observation group was significantly higher than that in the control group [(83.45 ± 12.65) mmHg (1 mmHg=0.133 kPa) vs . (61.68 ± 9.75) mmHg, t = -10.99, P < 0.05]. Intraoperative blood loss in the observation group was significantly lower than that in the control group [(112.65 ± 30.74) mL vs. (170.68 ± 35.67) mL, t = 9.94, P < 0.05]. Serum immunoglobulin G and malondialdehyde levels in the observation group were (8.56 ± 1.74) g/L and (7.74 ± 0.74) mol/L, respectively, which were significantly higher than (7.75 ± 1.68) g/L and (5.21 ± 0.65) mol/L in the control group ( t = 2.70, 20.71, both P < 0.05). Serum immunoglobulin M and superoxide dismutase levels in the observation group were (1.23 ± 0.32) g/L and (71.75 ± 8.57) U/L, which were significantly lower than (1.55 ± 0.45) g/L and (90.64 ± 9.73) U/mL in the control group ( t = -4.67, -11.75, both P < 0.05). Conclusion:B ultrasound-guided extracorporeal shock wave lithotripsy combined with tamsulosin hydrochloride medication is more effective on ureteral calculi than B ultrasound-guided extracorporeal shock wave lithotripsy alone. The combined therapy can effectively reduce pain, increases the treatment efficacy, and is worthy of reference and promotion in clinical practice.

To validate the prognostic value of NCCN-International Prognostic Index (NCCN-IPI) for patients with peripheral T-cell lymphoma (PTCL) treated with CHOP-based chemotherapy.

8. Analysis on suicide mortality and self-inflicted injury/suicide hospital cases in China from 2006 to 2016

Objective:
To describe the epidemiological characteristics of suicide death and self-harm/suicide in China from 2006 to 2016.
Methods:
Data in this study are from the datasets of National Death Cause Surveillance from 2006 to 2016 and the National Injury Surveillance Dataset 2016. The age-standardized death rate based on the population census in 2000 was used to describe the trend of suicide over years. This study also described the epidemiological characteristics of suicide death and the demographic characteristics, suicide methods and severity of patients who sought medical help after committing suicide or self-inflicted injury in 2016.
Results:
Age-standardized suicide mortality had been declining from 9.23/100 000 in 2006 to 5.33/100 000 in 2016 by 42.25%. Suicide mortality increased with age. In 2016, the suicide mortality in China was 7.05/100 000, and among people over 85 years old was 49.49/100 000. In 2016, the dominant two suicide methods were poisoning (39.12%) and sharp injury (25.62%). The first leading method of suicide in rural population, urban population and young people aged 10-to 17-year-old was poisoning (65.13%), sharp injury (29.90%) and fall (25.47%), respectively. As age increased, the severity of suicide/self-inflicted injury increased. The proportions of serious cases among young people aged 10-to 17-year-old and older people aged 60 years old and above who sought medical help after committing suicide or self-inflicted injury were 4.88% and 26.86%, respectively.
Conclusion
China′s suicide mortality has been declining and special attention should be given to the suicide of old people.

10. Trends of injury mortality among children in different stages of the China Children’s Development Outlines

Objective:
To describe the trends and potential reasons responsible for injury mortality among children under 18 years old in different stages of the China Children’s Development Outlines (CCDO) for children from 1990 to 2017, in China.
Methods:
Data derived from the Global Burden of Disease 2017 (GBD2017) were used to analyze the change of injury mortality, among children under 18 years old, by sex and provinces.
Results:
Since 1990, the Chinese government had formulated and implemented three CCDOs on Children. Each CCDO proposed corresponding main targets and strategic measures based on the development of children under current situation, in each area, accordingly. The first two CCDOs failed to set clear targets for child injury prevention and control, but the third one did propose a quantifiable target. The injury mortality rate of children under 18 years old showed a declining trend in all periods of the three CCDOs, by 26.07
