1.Variation of Pathogenic Bacteria Causing Urinary Tract Infection in Children and Drug Resistance Analysis in Recent 8 Years

OBJECTIVE To investigate the distribution,epidemic trend and transition of drugs′ sensitivity about the pathogenic bacteria of children′s urinary tract infection(UTI) in the past 8 years in the local area.METHODS The medical records of 391 UTI cases which were hospitalized in our hospital and confirmed as UTI through urine culture from 2000 to 2007,were reviewed and the clinical features were analyzed.RESULTS A total of 402 pathogenic bacteria were determined.Gram-negative bacilli were predominant,accounted for 72.9%,and Escherichia were the most commonly found(34.4%).Gram-positive cocci accounted for 27.1%,including 12.9% of Staphylococcus and 10.4% of Enterococcus.The proportion of Escherichia,Klebsiella and Enterococcus was increased year by year.Gram-negative bacilli had a high resistance rate to ampicillin and 1st to 4th generation cefalosporins and a low resistance rate to piperacillin/tazobactam.Gram-positive cocci had a high resistance rate to ampicillin and erythromycin and a low resistance rate to nitrofurantoin and vancomycin.In the past 8 years,the resistance rate to most of the antibacterial agents has been increased,especially ampicillin and cephalosporins.Only piperacillin/tazobactam and vancomycin maintained high antibacterial activities.CONCLUSIONS Gram-negative bacilli are the major pathogens in pediatric UTI,and Gram-positive cocci appear to be increasing.Ampicillin should not to be chosen commonly.The observation of variation of pathogenic bacteria and drug resistance is of great importance.
2.The biological effects of HPT protein on the drug resistance of gastric cancer cell

Objective To prepare an antiserum of human putative magnesium transporter protein (HPT protein) to study its biological effects on the drug resistance of gastric cancer cell by observing its expression level. Methods The cDNA encoding HPT protein was cloned and sequenced. Prokaryotic expression vector was constructed and HPT protein fused with histidine tag was expressed. Mice were immunized with the polyacrylamide gel particles containing the fusion protein, and the antisera of them were detected for antibody activity by Western blot. Using the antiserum, HPT protein expression level was assessed in different gastric cancer cell line. Results The sequence of cloned cDNA fragment was consistent with that of HPT. After BL21/pRSETB bacterium was induced with IPTG, a new protein band with a relative molecular weight of 55kD was showed on SDS PAGE profile, and further proved to be fusion protein with histidine tag by Western blot. The antiserum of immunized mice only recognized a unique protein band with a relative molecular weight of 50kD. HPT protein expression level in SGC7901/ADR was higher than in SGC7901. Conclusion The antiserum of HPT protein was successfully prepared, and HPT was found to probably take part in the gastric cancer cell MDR
4.The optimal dose reduction level in chest CT with CT volume scan mode using iterative reconstruction in piglet model

Objective To determine the feasibility and the optimal dose reduction of AIDR 3D on piglet chest CT which can provide image quality (IQ) comparable to filtered back projection (FBP).Methods Twenty-nine normal pigs with the weight of 3-12kg underwent 640-slice MDCT chest CT (Aquilion one,Toshiba) for 5 times with 80 kvp and various mAs.SureExposure3D technique were used and the index of noise were set to SD 10.0,12.5,15.0,17.5,20.0(Group A,B,C,D,E) to reduce dose successively.Group A were reconstructed with FBP,Group B,C,D,E were reconstructed using AIDR 3D (strong level).Quantitative image noise and signal to noise ratio(SNR) were measured in each group.Two radiologists graded subject image quality on both lung images (artifacts,central airway,lung tissue) and mediastinal images (mediastinal structure) and overall image quality using a 5-point scale in a blinded manner.Analysis of variance (ANOVA) and LSD test were used for comparisons of objective evaluation indices (CT value,noise,SNR)and radiation dose(CTDIvo1,DLP,ED) among the five groups.The Friedman test and Wilcoxon test were used for comparisons of demographic data and for detection of differences in subjective evaluation of IQ among groups.A receiver-operating characteristic (ROC) analysis was performed to establish a radiation reduction threshold up to which comparable IQ(score≥4) was maintained.Results Compared with Group A[noise(15.25 ± 3.14)HU,SNR 3.23 ± 1.07],Group B,C had significant lower noise and higher SNR [noise (12.11 ± 2.75),(13.18 ± 3.16)HU; SNR 4.13 ± 1.38,3.80 ± 1.20;F =7.38,3.11,P <0.05].Group D,E showed no significantly different noise and SNR with Group A[noise (15.14±4.51),(15.79 ±4.17)HU;SNR 3.40± 1.56,3.45 ± 1.70;P>0.05].Group B,C had significant better subject image quality compared with Group A (P <0.01).Group D had no significantly different subject image quality compared with Group A (P > 0.05).Group E had 5 pigs with overall image scores of < 3.The ROC curve of IQ established SD 17.5 (Group D) as optimal cut-off point (AUC 0.75,95% CI 0.58-0.92).Group D provided equivalent subjective image score and objective IQ measurements compared with FBP images in Group A.The ED of group D was 59% lower than that of group A[CTDIvol (1.14±0.27) mGy vs (0.47 ±0.16)mGy,F=183.83,P<0.01].Conclusion Using AIDR 3D technique,80 kvp with SureExposure3D (SD 17.5) can provide comparable IQ compared with routine dose with FBP reconstruction,and reduce 59% dose in piglet model.
5.Detection and genotype determination of beta-human papilloma virus in actinic keratosis lesions

Objective To detect beta-human papilloma virus (HPV) and determine its genotype in actinic keratosis (AK) lesions.Methods Tissue specimens were collected from the lesions of 39 patients with AK and normal skin of 40 healthy controls.A nested PCR was performed to detect alpha-HPV and beta-HPV DNA in these specimens.The genotype of beta-HPV was determined in beta-HPV DNA-positive specimens by a common PCR using specific primers targeting 12 HPV genotypes,including HPV 5,8,15,17,19,20,21,23,36,38,49 and 80.Results The detection rate of beta-HPV DNA was 84.6% (30/39) in the patients with AK,and 30.0% (12/40) in the healthy controls (x2 =6.76,P < 0.05),while no significant difference was observed in the detection rate of alpha-HPV DNA between the two groups (12.8% vs.7.5%,x2 =0.91,P > 0.05).HPV 38 was the predominant genotype of beta-HPV in these patients with a detection rate of 36% (12/33),followed by HPV36.The prevalence of all the 12 genotypes of HPV was consistently low in the healthy controls.Mixed HPV infections were observed in 10 AK lesions,but in none of the healthy controls.No statistical difference was noted in the positivity rate of beta-HPV among patients at different ages,of different genders,with different occupations or clinical courses (x2 =0.53,0.94,0.81,0.73,respectively,all P > 0.05).Conclusions Compared with healthy controls,the patients with AK showed a higher beta-HPV infection rate,with HPV38 as the predominant genotype.
6.Application study on catheterization to pseudoaneurysm of membranous ventricular septal defect with patent ductus arteriosus occlusion devices in children

Objective To study on transcatheter closure of membranous ventricular septal defect (VSD) with pseudoaneurysm by patent ductus arteriosus(PDA) occlusion devices in children and summarize the skill and clinical experience. Methods The study included 20 membranous VSD cases in children.According to the finding of the left ventricular angiography, various kinds of the PDA occlusion devices was implanted. The mean diameter of the waist of the occluder was ( 10.4 ± 2.6) mm. Examination by transthoracic echocardiography (TTE) immediately and left ventricular angiography after the occluder was implanted 15minutes later to evaluate the efficacy. Results In the 20 patients, one of the Ⅳ type VSD patient was quitted because of the significant residual shunts(≥2 mm). Slightly residual shunts ( < 2 mm) was found in one Ⅲ type VSD patient with multi-outlet. And disappeared in 1 month after the procedure, which VSD patient was confirmed by TTE. Thirteen cases were normal by EKG examination (or the same before procedure).Incomplete right bundle branch block was found in 4 cases. First degree atrioventricular block was found in 1 case and paroxysmal junctional tachycardia was found in 1 case. All of them were recovered in 1 week.Conclusions Transcatheter interventional therapy with PDA occlusion devices for membranous VSD with pseudoaneurysm is safe and effective. The key of the procedure is to select suitable occluder and suitable position to plant them according to the membranous morphologic characteristics,size and position of the pseudoaneurysm. It is a facultative method for transcatheter therapy this kind of congenital heart disease.
7.Astragaloside combine with Cetuximab inhibits proliferation and regulates autophagy of human colon cancer cell RKO

Objective To investigate the antitumor mechanism of astragaloside combine with cetuximab on the colon cancer cell line RKO of EGFR over expression through cell proliferation and autophagy. Methods The cell proliferation of colon cancer cell line RKO intervened by astragaloside with or without cetuximab was detected by 4- methyl- teerazolium (MTT). The expressive changes for protein of EGFR 、P62 and LC3 in RKO cells was detected by western bolt. Results MTT showed that 100 ug/mL astragaloside combined with 120 ug/mL cetuximab had significantly inhibiting effect on RKO cells of EGFR expression. Western blot showed that astragaloside can affect the expression of P62 and LC3 to suppress the occurrence ofautophagy. Conclusion In the vitro studies showed that astragalosidecan enhance the antitumor cell proliferation of cetuximab through inhibit autophagy in the treatment of colon cancer.
8.Eccentricity changes affect joint pain and function after total hip arthroplasty

according to the difference of femoral eccentricity on the affected and healthy sides. In the reduction group (30 cases), the difference value was <-5 mm. In the normal group (87 cases), the difference value was -5-5 mm. In the increased group (45 cases), the difference value was > 5 mm. Femoral stem and femoral prosthesis type were compared among different groups. SF-12 scale score, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and Harris score were evaluated and compared before and after replacement. RESULTS AND CONCLUSION:Standard eccentricity of femoral prosthesis mainly accounted for 77%, 63% and 73% in the reduction group, normal group and increased group, respectively (P > 0.05). No significant difference in average eccentricity was detected in each group between the affected and healthy sides (P> 0.05). WOMAC score was significantly higher after replacement than that before replacement in three groups (P < 0.05). No significant difference in WOMAC score was found among three groups before replacement (P > 0.05). No significant differences in pain and stiffness were detected among three groups after replacement (P > 0.05), and significant differences in physical function were found (P < 0.05). The physical function was highest in the normal group. No significant difference in SF-12 scale score was seen in the three groups before and after replacement (P > 0.05). The physical ability score was elevated after replacement in the three groups (P < 0.05). Mental scores were significantly elevated in reduction and normal groups (P < 0.05). The excelent and good rate of Harris score was significantly higher in the increased group (96%) than in the reduction group (70%) and normal group (87%) (P < 0.05). These findings indicate that the increased eccentricity contributed to the pain easement after total hip arthroplasty and promoted functional recovery. The decreased eccentricity could worsen patient’s pain, and was not conducive to the restoration of joint function after replacement.
9.Application of systematic health education in the ultrasound guided interventional therapy for patients with cysts

Objective To investigate the effect of systematic health education on the ultrasound guided interventional therapy for pa -tients with cysts.Methods Totally 181 patients with cysts who received the ultrasound guided interventional therapy in our hospital ,were randomly divided into observation group and control group .The patients in the observation group received systematic health education ,while patients in control group received convention health education .Results The differences of pain score between two groups were statistical significance(P<0.05).By health education,the patients in the observation group had a deeper knowledge of the operation and became more cooperative(P<0.05).Conclusion Systematic health education could alleviate the pain and improve the psychological indisposition reac -tions of patients,and enhance therapeutic compliance of patients with ultrasound guided interventional therapy .In the meantime,systematic health education could improve the quality of work and enhance the diathesis of paramedic .
