1.Effect of family constellation therapy on the spouses of female tumor patients

Objective To explore the effect of family constellation therapy on the spouses of female tumor patients . Methods Two hundred and two spouses of 202 female tumor patients were divided to the control group ( n = 43 ) and experiment group according to their wills(n=159):the former were intervened with health education and skills training and the latter trained in a workshop with family constellation by psychology training specialists in addition to health intervention and skill training intervention. After the intervention, the two groups were compared in terms of quality of life, family function and marriage status. Result The total scores of the experiment group on quality of life , social relationship and environment were all higher than those of the control group (all P<0.05) and the experiment group got higher score in trusting their spouses than the control group (P<0.05). Conclusion The family constellation can improve female tumor patients'quality of life and promote the trust in their spouses.
2.Analysis the occurrence and affecting factors of the Cirrhosis and portal hypertension complications after splenectomy

Objective To research the occurrence of liver cirrhosis and portal hypertension splenectomy treatment,and analyze the risk factors for postoperative complications.Methods Retrospectivly analyzed the clinical data of 390 liver cirrhosis cases from March 2009 to May 2013 who were admitted to department of digestion and vascular surgery,First Affiliated Hospital of Xinjiang Medical University,for splenic resection.Child classification,diabetes,amount of fluid drainage,drainage time,blood loss,emergency surgery,combined with other surgery were alalysised by Logistic regression.Results Among three hundred and ninty cases of splenectomy patients,liver cirrhosis complications occurred in 53 cases (53/390,13.58%).In these 53 cases,45 cases combined diabetes,Child grade A 12 cases,B 22 cases,C 19 cases,and theirs 24 hours drainage volume was (422.45 ±297.09) mL,drainage time was (4.96 ± 1.48) d,blood loss was (874.72 ± 106.19) mL,K value was 0.10 ±0.06,R15 value was 0.27 ±0.016,albumin concentration was (30.09 ±6.87) g/L,Among the rest 337 cases,228 cases were combined diabetes,Child grade A 134 cases,B 129 cases,C 74 cases,and theirs 24 hours drainage volume was (234.06 ± 171.15) mL,drainage time was (3.18± 1.72) d,blood loss was (261.90±98.27) mL,K value was 0.14 ± 0.09,R15 value was 0.19 ± 0.015,albumin concentration was (32.51 ±5.58) g/L.Multivariate regression analysis had indicated that Child grade,drainage volume of 24 hours,drainagetime,and K value were independent factors affecting the occurrence of complications.Conclusion The major risk factors of incidence happen on postoperative complications in cirrhosis and portal hypertension splenectomy are high Chill grading,draining a long time,and 24 hours blood loss,and k value.
3.Application of Spiral CT for Pancreatic Adenocarcinoma: Emphasis on the Value of the Tumor-pancreas Contrast

Objective To evaluation the application value of two-phase enhanced spiral CT in pancreatic adenocarcinoma. Methods Two-phase enhanced spiral CT was performed in 27 cases of pancreatic adenocarcinoma. After plain CT examination, performed arterial phase(25s after infusion)and portal venous phase(60~70s after infusion)with a 150ml bolus injection of contrast material at 3ml/s. CT attenuation values of tumor and normal pancreas in all scanning phase were obtained and the tumor-pancreas contrast was calculated. Results Mean tumor enhancement were 48±8 HU(arterial phase)and 72±6 HU(portal venous phase), normal pancreas enhancement were 125±18 HU、98±11 HU, mean tumor-pancreas contrast was significantly differenced on arterial phase (73±21 HU)and portal venous phase (25±12 HU). Conclusion Excellent tumor-pancreas contrast is obtained with arterial phase scans, as such it is of significance reference value for diagnosis pancreatic adenocarcinoma.

5. Research progress on carbazole alkaloids from plants of Clausena Burm.f.

The genus Clausena Burm.f. (family Rutaceae) concerns 25 species all over the world, and 13 species are native to China especially in the southwest and south of China, Yunnan Province has 11 species. The chemical composition from Clausena Burm.f. included carbazole alkaloids, coumarins, sesquiterpenes, benzene derivatives, tetranortriterpenoids, flavonoids and so on. The carbazole alkaloids have attracted much attention in recent years. The recent research progress on carbazole alkaloids from Clausena Burm.f. was reviewed in this paper about the structures, isolation, structure elucidation, and biological activities.

10.Extravascular lung water index monitoring and fluid management in patients with pulmonary contusion

ObjectiveTo guide fluid management and evaluate the clinical index through monitoring extravascular lung water index(EVLWI) of patients with pulmonary contusion.MethodsThirtyone severe chest trauma patients with pulmonary contusion were selected,performed with central venous catheterization or femoral artery catheterization,and connected to pulse index continuous cardiac output (PICCO).EVLWI after pulmonary contusion were measured by using thermal dilution.The intake and output of patients were recorded in detail,fluid intake was controlled,furosemide was prescribed,and the changes and correlations of oxygenation index,EVLWI and liquid balance difference at different time points were evaluated.ResultsEVLWI after pulmonary contusion at 1st to 7th d after hospitalization was respectively (9.25 ±0.71),(8.98 ±0.61),(8.61 ±0.59),(7.75±0.53),(6.64 ±0.49),(6.22±0.36),(5.59 ±0.39) ml/kg.Comparing with 1st d after hospitalization,EVLWI declined from 3rd d (P < 0.05).Oxygenation index at Ist to 7th d after hospitallzation was respectively( 145.76±23.61 ),( 144.19±20.24),( 146.67±19.25 ),(159.33±15.42),(177.38±14.25),(199.33±19.04),(213.71±18.51) mm Hg(1 mm Hg =0.133 kPa).Comparing with 1st d,oxygenation index from 4th to 7th d had significant difference (P < 0.05 ).The fluid volume at 1st d to 7th d were all negative balance,that of 3rd d to 6th d had significant difference comparing with 1st d [(-431.43±121.17),(-601.43±127.09),(-629.52±140.69),(-320.01 ±93.71) ml vs.(-213.81±63.91 ) ml](P < 0.05 ).Oxygenation index and EVLWI had negative correlation(r =-0.824,P<0.01).Liquid balance difference and the changes of oxygenation index and EVLWI had no correlation.ConclusionEVLWI effectively monitoring after pulmonary contusion can not only evaluate the changes of pulmonary vascular permeability of patients with pulmonary contusion,but also have important significance to guide fluid management.
