1.Features of ambulatory blood pressure in 540 patients with chronic kidney disease

Objective To explore the features and influencing factors of ambulatory blood pressure in chronic kidney disease (CKD) patients.Methods A total of 540 CKD patients from May 2010 to May 2012 in our department were enrolled in this study.Ambulatory blood pressure monitoring was carried out.Blood pressure (BP),proteinuria and other clinical parameters were measured regularly.Ultrasonography was used to evaluate cardiac structure and function,carotid intima-media thickness and plaque.Univariate and multivariate analysis were used to examine the association between BP and clinical parameters.Results 63.9% of CKD patients was non-dipper BP pattern,and 36.1% was dipper BP pattern.As compared to dipper BP patients,those with non-dipper BP had higher ratio of nighttime/daytime proteinuria (0.51±0.29 vs 0.42±0.21,P < 0.01),lower estimated glomerular filtration rate (eGFR) [(56.2±48.2) vs (75.5±56.5) ml· min 1 · (1.73 m2)-1,P < 0.01],higher serum cystatin C[(2.8±2.0) mg/L vs (2.1±2.0) mg/L,P < 0.01],higher left ventricular mass index [(53.7±23.2) vs (45.1± 16.3) g/m2,P < 0.01] and severely damaged left ventricular diastolic function and higher carotid intima-media thickness [(0.7±0.3) vs (0.6±0.2) mm,P< 0.01].Nighttime blood pressure was independent predictor for proteinuria,eGFR and left ventricular mass index.Conclusions Nondipper blood pressure pattern is very common in CKD patients.Nighttime pressure is closely associated to renal damage and cardiovascular injuries.
2.A clinicopathologic analysis of 42 cases with sarcomatoid carcinoma of the lung

Objective To study the clinicopathologic characteristics of sarcomatoid carcinoma of the lung.Methods Clinicopathologic characteristics and immunohistochemical results of 42 cases with sarcoma- toid carcinoma of the lung were analyzed.Results Forty-two cases were pathologically proved to be spindle cell carcinoma,1 cases;giant cell carcinoma,1 case;carcinosarcoma,4 cases;pleomorphic carcinoma,36 cases.The tumors were composed of both carcinomatous and sarcomatous elements.Immunohistochemistry showed that CK was positive in 24 of 24 cases,EMA was positive in 18 of 20 cases,Vim was positive in 25 of 25 cases.Eighteen cases were survival One-year survival rate was 61.2 %,with mean survival 13.3 months. Conclusion Diagnosis of sarcomatoid carcinoma of the lung depends on pathologic and immunohistochemical results.
3.Study on 1H-MRS of prefrontal lobe and executive functions in patients with post-concussion syndrome

Objective To identify the metabolic levels in prefrontal lobe in patients with post-concussion syndrome by proton magnetic resonance spectroscopy (1H-MRS), and to explore the relationship between metabolic levels and executive function. Methods The study was conducted in 40 patients with post-concussion syndrome and 20 normal controls. 1H-MRS on prefrontal lobe was performed in patients and controls, the NAA, Cho and Cr were measured and the ratios of NAA/Cr, Cho/Cr and NAA/(Cho + Cr) were determined. They were also evaluated executive functions by verbal fluency test (animal), Wisconsin Card Sorting Test (WSCT) and Tower of Hanoi (TOH). Results Compared with normal controls, the patients with post-concussion syndrome were significantly lower NAA/Cr and NAA/(Cho+Cr) ratios in left prefrontal lobe (P < 0.05). The NAA/Cr ratio in left prefrontal was significantly positive correlated with total scores of verbal fluency (r = 0.66, P < 0.05), categories of WSCT (r = 0.54,P < 0.05) and total score of TOH(r = 0.58, P < 0.05). The NAA/Cr ratio was significantly negative correlated with total errors (r = -0.53, P < 0.05) and persistent errors (r = -0.47, P < 0.05) of WSCT and mean executive time of TOH(r = -0.67, P < 0.05). Conclusions The metabolic levels of NAA in left prefrontal lobe in patients with post-concussion syndrome is significantly decreased , it is one cause of impaired executive functions.
4.Influence of reversed dipper blood pressure pattern on target organ damage in patients with chronic kidney disease

Objective To explore the relationship between reversed dipper blood pressure (BP) pattern with target organ damage in Chinese chronic kidney disease (CKD) patients. Methods A total of 1 116 CKD patients from the Third hospital of Sun Yat?sen university were enrolled from May 2010 to April 2014, all of whom carried ambulatory blood pressure monitoring (ABPM), and clinical data and ultrasonography were collected. Results Total of 23.39% CKD patients were reversed dipper BP pattern, 46.95% patients were non?dipper BP pattern, and dipper BP pattern was only 27.15%. Compared with dipper and non?dipper BP pattern group, patients with reversed dipper BP pattern had higher left ventricular mass index (LVMI), lower estimated glomerular filtration rate (eGFR) and severely damaged left ventricular diastolic function (all P<0.0083). After multiple linear regression analysis, reversed dipper BP pattern was an independent predictor for determining eGFR and LVMI. Age, lower hemoglobin and combination of diabetes were independently associated with reversed dipper BP pattern. Conclusion Reversed dipper BP pattern is common in Chinese CKD patients, which is closely related to renal damage and cardiovascular injuries.
5.Analysis on potential years of life lost of automobile drivers in Jiangsu in 1990 - 1999.

OBJECTIVETo describe the health status of automobile drivers and to define their potential years of life lost (PYLL) resulting from main death causes in Jiangsu in 1990 - 1999.
METHODSThe Reed-Merrell method and Makham-Gompertz equation were used to compile the abridged truncate life tables. PYLL and rate of PYLL was used to analyse death causes in 1990 - 1999.
RESULTS(1) The life expectancy of drivers at age 20 was 55.36, which was 0.70 year less than that of non-drivers. (2) Rate of PYLL was 35.07 per thousand in drivers, and 42.65 per thousand in non-drivers, and the corresponding standard rates of PYLL were 42.45 per thousand and 41.14 per thousand, respectively. (3) Damage and toxicosis ranked number 4 in terms of proportionate mortality ratio, but ranked first with the rate of PYLL.
CONCLUSIONSpecial attention should be paid to the health status of automobile drivers, especially to those who are middle aged. Diseases that made the greatest contributions to drivers' "earlier death" were damage and toxicosis.

6.Proliferation of retinal pigment epithelial cells induced by (R,R)-XY-10 and (S,S)-XY-10 and their action mechanisms

AIM: To investigate the mechanism of proliferation effect induced by (R,R)-XY-10 and (S,S)-XY-10 on retinal pigmented epithelial cells(ARPE-19).METHODS: Human retinal pigmented epithelial cells(ARPE-19) and human umbilical vein endothelial cells (HUVECs) were used to investigate the effect of (R,R)-XY-10 and (S,S)-XY-10 on cell growth,and their mechanisms of proliferative action by using ERK、 AKT、PI3K、Protein kinase C (PKC)and Nitric oxide synthase (NOS) inhibitors.RESULTS: (R,R)-XY-10 and (S,S)-XY-10 dose-dependently increased ARPE-19 cell proliferation,but not on HUVECs. When treated with proliferative inhibitors,H7(5μmol/L)、hypericin(20μmol/L)、PD98059(2μmol/L)、LY294002(50μmol/L)、SH-5 (10μmol/L) and L-NAME (100μmol/L),the proliferative effect was reduced by H7、hypericin、PD98059 and LY294002,but not by SH-5 and L-NAME.CONCLUSION: (R,R)-XY-10 and (S,S)-XY-10 can induce cell proliferation through MAPK and PI3K dependent pathway. 7.Statistical methodologies used in genome-wide association studies

In lieu of large samples of cases and/or controls with hundreds of markers spreading throughout the human genome, researchers started to notice the dramatic increase of genome-wide association study (GWAS) for complex disorders, in the last 5 years. This paper highlights the statistical challenges in such huge-scale genetic studies, and introduces the analytical strategies and steps for handling GWAS data. Such issues as quality control of data, population stratification, methods available to data analysis and results presentation, replication, as well as the limitations of GWAS studies and the challenges presenting for statistics, are addressed.
8.Evaluation of riboflavin photochemical inactivation efficacy of virus in red blood cells by using animal models.

This study was aimed to evaluate the efficacy of riboflavin photochemical inactivation of virus in red blood cells by using animal models. human cytomegalovirus (HCMV) plus red blood cells were used as indicator, 30 BALA/c mice were divided into the experimental group (n = 10), virus control group (n = 10), visible light control group (n = 5) and red blood cell control group (n = 5). Mice in experimental group were inoculated with red blood cells inactive by the riboflavin photochemical, mice in virus control group was injected with red blood cells without riboflavin photochemical inactivation treatment, and mice in light control group was infused with red blood cells irradiated by visible light, and mice in red blood cells control group was injected with normal red blood cells. The virus was isolated in vitro from mice of various groups, the HCMV UL83 gene was detected by PCR, the PP65 antigen was identified by indirect immunofluorescence. The results indicated that the virus isolation, PCR detection and indirect immunofluorescence identification all showed positive in virus control group and visible light control group, while the results of detection in experimental and red blood cell control groups were negative. It is concluded that riboflavin photochemical viral inactivation of red blood cells is effective.

9.Clinicopathological features and prognostic analysis of esophageal sarcomatoid carcinoma.

OBJECTIVETo analyze the clinicopathological characteristics and prognosis of a rare histological type of esophageal cancer-sarcomatoid carcinoma.
METHODSClinicopathological data of 31 patients with esophageal sarcomatoid carcinoma who underwent surgery in the Department of Thoracic Surgery of Zhejiang Cancer Hospital from Jan 2000 to Dec 2009 were collected and analyzed. The survival analysis was performed using Kaplan-Meier method.
RESULTSAll the patients underwent surgery. Of the 31 patients, one received preoperative chemoradiotherapy and postoperative chemotherapy, and 8 received postoperative chemotherapy. All the tumors were located in the middle or lower esophagus. Microscopically, the tumors were composed of both carcinomatous and sarcomatous components, and there was a transition between the two components, but no obvious heterogenous elements such as osteosarcoma, chondrosarcoma or rhabdomyosarcoma were found. In the carcinomatous components, positive expression of CK and EMA was found in all the 31 cases, and positive expression of vimentin in 5 of the 31 cases. In the sarcomatous components, positive expression of CK, EMA and vimentin was found in 29, 28 and 23 cases, respectively. The 1-, 3-, and 5-year survival rates were 80.6%, 55.9% and 33.4%, respectively, and the median survival time was 40 months.
CONCLUSIONSEsophageal sarcomatoid carcinoma is a particular type of esophageal malignancy with unique clinicopathological features. The diversity and complexity of the carcinomatous and sarcomatous components and their potential of transformation and differentiation lead to different prognosis from each other.

10.Analysis on the risk factors of maternal weight for fetal macrosomia

Objective To investigate the relationship between maternal weight gain and the increasing speed of weight in different pregnant terms and macrosomia.In order to reasonably manage pregnancy and decrease the morbidity of maerosomia.Methods 106 newborns whose birth weights were equal to or greater than 4000 g were specified as macrosomia,while 106 newborn with birth weights lying in 2500-3999 g were under the control group.A case-control study was conducted to compare the corresponding factors such as maternal BMI.weight before pregnancy and the change of weight during pregnancy respectively.Results Indicated by both simple and multiple unconditional logistic regression analysis,the cause of fetal macrosomia Was mainly associated with the factors including the maternal weight before pregnancy(OR=2.204,95%CI:1.377-3.529),matemal weight gain in 12-pregnant weeks(kgper week)(OR=1.961,95%CI:1.204-3.194),maternal weight gain in 20-gestation weeks(kg perweek)(OR=1.811,95%CI:1.078-3.041),maternal weight gain in 30-pregnant weeks(kg per week)(OR=1.858,95%CJ:1.095-3.153)and virile newborn(OR=2.630,95%CJ:1.420.4.850.When in 30-pregnant weeks.the pregnant women with 0.5-1.0 kg weight gain per week had 1.13 fold risks comparing to those whose weight gains were lexq than 0.5 kg per week.Conclusion Maternal weight before pregnancy,weight gain during pregnancy and fetal sex appeared a closer relation to macrosomia.It is necessary to monitor the change of maternal weight during different pregnancy periods,especially for the 30th-pregnant weeks.
