Objective To observe the changes of cognition and auditory event-related potential P300 on vascular cognitive impairment-no dementia (VCIND) patients before and after cognitive rehabilitation training, then further to explore the application value of P300 in early diagnosis of vascular cognitive impairment (VCI). MethodsA total of 57 patients with VCIND were randomly assigned to the treatment group ( n =30) and the control group ( n =27),and 30 healthy volunteers without cognitive deficits were recruited as normal group. Each patient was examined with auditory-P300 and scored with mini-mental state examination (MMSE) and Montreal cognitive assessment (MoCA) scale. The measurement indicators were analyzed and evaluated with the factors of education, age and so on. ResultsThe P300 latency in VCIND group was longer, and the amplitude lower than those in normal group before treatment (P<0.05 ),the latency lengthening was more significant than the lowering of the amplitude ( P<0.01 ).The cognitive assessment scale scores of MMSE,MoCA and ADL (Barther index,BI) in VCIND group were lower than those in normal group (P<0.05),while no significant difference was revealed between the VCIND and normal groups with regard to naming and speech( P > 0.05 ).A highly positive correlation between age and P300 latency as well as a significantly negative correlation between years of education and P300 latency were demonstrated( P<0.05 ).After cognitive rehabilitation therapy the P300 latency in VCIND group was shortened, the amplitude increased (P<0.05 ),scores of the items of cognitive scale raised(P<0.05 ) and the change of each indicator was more statistically significant in treatment group(P<0.05). ConclusionP300 could objectively reflect early cognitive dysfunction in VCIND patients.Early cognitive training could effectively promote cognitive function of VCI patients.

Objective To survey the antimicrobial resistance of nosocomial Klebsiella Pneumoniae (Kpn) isolates. Methods All the 68 Kpn strains were isolated and collected from nosocomial infections during April 2002 to March 2003. The susceptivity of bacteria and the production of extended spectrum ?-lactamases (ESBLs) were determined by Kirby-Bauer disc diffusion test and phenotypic confirmatory testing, respectively. Results All 68 Kpn strains were 100% susceptible to Menopenem and Imipenem but resistant to 14 other antimicrobials to different degrees. The first five drugs of high resistant rate were ampicillin (100%), amoxycillin/clavulanic acid (79.4%), sulphamethoxazole/trimothoprim (67.6%), cefotaxime (58.8%), gentamycin (47.0%). There were 28 isolates producing ESBLs (41.2%), which showed much higher resistance to ?-lactam, aminoglycosides, quinolones and sulphamethoxazole/trimothoprim than the non-ESBLs-producing strains. Conclusion Nosocomial Kpn isolates were most multidrug resistant, of which ESBLs-producing strains showed a higher resistance and which may involve other resistant mechanisms.

Objective To observe the influence of nasal washing with Bidouguan Perfusate (BP) on nasal mucosal cytokines of nasal polyp patients after operation. Methods Thirty-nine nasal polyp patients were randomized into BP group ( N=18) and normal saline group ( N=21). All patients received operation for nasal polyp and postoperative comprehensive treatment, and additionally, BP group was given washing with BP and normal saline group was washed with 0.9%normal saline for 3 months. The scores of SNOT-20 and Lund-Kennedy were observed for the evaluation of therapeutic effect. Enzyme-linked immunosorbent assay ( ELISA) was used for detection of interleukin 5 (IL-5), interleukin 8 (IL-8), interleukin 10 (IL-10), interferon gamma (IFN-γ), granulocyte-macrophage colony-stimulating factor (GM-CSF), tumor necrosis factor alpha (TNF-α) in nasal mucosa before operation and 3 months after operation. Results ( 1) SNOT-20 and Lund-Kennedy scores were decreased after operation ( P <0.05 compared with those before operation) , the difference being insignificant between the two group after operation ( P>0.05). ( 2) The expression levels of IL-5, IL-8, IL-10, IFN-γ, TNF-αand GM-CSF in the nasal mucosa were decreased in both groups 3 months after operation (P<0.05), and the decrease of IL-5, IL-8, IL-10 and IFN-γ in BP group was superior to that in the normal saline group ( P<0.05) . The differences of TNF-α and GM-CSF levels were insignificant between the two groups after operation ( P>0.05). Conclusion Nasal washing with BP has similar effect to normal saline on improving the quality of life and nasal mucosal histology 3 months after operation, but BP has better effect on regulating nasal mucosal IL-5, IL-8, IL-10 and IFN-γ.
