Objective To observe the effect of hybrid renal replacement therapy (sustained lowefficiency hemodiafiltration) in recipients with delayed graft function (DGF) after kidney transplantation. Methods In 33 kidney transplant patients with DGF,there were 15 cases subject to sustained low-efficiency hemodiafiltration (SLEDF group),18 cases subject to hemodialysis (HD group). The renal function was tested, and the expression levels of complements C3,C4,and CRP,IL-1β,IL-6,TNF-α were detected before and after therapy. Results In SLEDF group, the renal function recovered in (29 ± 13) days after therapy, and in (47 ± 21) days in HD group. After therapy, the BUN and Scr in both two groups were lower than before (P<0.01).After therapy, the levels of IL-1β,IL-6 and TNF-α were also lower than before in two groups, and the levels were lower in SLEDF group than in HD group (P<0.05).The levels of complements C3,C4,and CRP in two groups were decreased after therapy, but there was no statistically significant difference (P>0.05).Conclusion SLEDF can decrease the level of SCr,BUN and some cytokines in recipients effectively, and It's helpful to recovery of the renal function.SLEDF can be used by DGF recipients in transit time.

Objective To observe the clinical effect of the club exercise designed ourselves on the prevention of should-hand syndromeafter stroke. Methods 100 stroke patients in early stage, whose motor function of upper limb were lower than Grade Ⅲ evaluated byBrunnstrom assessment and without shoulder pain and swelling of hand, were randomly divided into control group (n=50) and observationgroup (n=50). All of them received routine rehabilitation therapy, and the observation group received club exercise in addition. The incidenceof shoulder pain, the active range of motion of shoulder, the motor function of upper limb and activities of daily living (ADL) of 2groups were evaluated before and 2 courses (60 d) after treatment. Results There was no statistical difference between 2 groups before thetreatment (P>0.05). After 2-course treatment, the incidence of shoulder pain was significantly lower in the observation group than in the controlgroup (P<0.001), the active range of motion of shoulder, motor function of upper limb and ADL were significantly better in the observationgroup than in the control group (P<0.001). Conclusion The club exercise can prevent the pain of shoulder joint, increase the activerange of motion of shoulder joint and improve the motor function of upper limb and ADL in patients after stroke.

Objective To Btudy effect of standardized acupuncture combined with rehabilitation therapy (function training)on improvement of comprehensive function among patients with stroke in communities,Changning District of Shanghai. Methods One hundred and seventy-six stroke patients were divided into rehabilitation therapy group(88 cases)and control group(88 cages)with block randomization.Patients in rehabilitation therapy group were treated with standardized acupuncture combined with function training in addition to regular medical treatment, and patients in control group regular medical treatment only. All the patients were evaluated with functional comprehensive assessment(FCA)scale by the end of 2-and 5-month of treatment, respectively. Results By the end of 5-month of treatment, scores of FCA in both groups increased significantly (P<0.01),as compared to those before treatment, more in rehabilitation therapy group than that in control group(P<0.05).Conclusions Effect of standardized acupuncture combined with rehabilitation therapy Can obviously improve their comprehensive function among stroke patients in communities.

Objective To evaluate the effects of community-based rehabilitation therapy on activitv of daily living performance of the Chinese stroke patients. Methods In a single blinded, randomized, controlled multicenter trial,737 consecutive stroke patients were stratified by two groups of cerebral infarction and hemorrhage. All the patients were randomly divided into a rehabilitation group and a control group. The patients in the rehabilitation group received atandardized community-based rehabilitation therapy, while those in the control group did not.All the patients were followed up for 5 months for evaluation of their performance with activity of daily living by using Modified Barthel Index before intervention, after follow-up 2 and 5 months, respectively. Results Patients in the rehabilitation group performed better in ADL as reflected by the Modified Barthel Index score than those in the control group after 5 months of intervention(P<0.05).Although both the rehabilitation group and control group improved over time, the rehabilitation group showed a greater improvement with the Modified Barthel Index scores when compared with the control group(P<0.01).After 5 months follow-up,the total gain of Modified Barthel Index scores was 26.28 and 32.89 for those with cerebral infarction and hemorrhage, respectively,in the rehabilitation group. In comparison, the gain of Modified Barthel Index scores was 7.65 and 2 1.70 for those with cerebral infarction and those with hemorrhage, respectively in the control group. This implies a difference in improvement of 1 8.63 in cerebral infarction group and 1 1.19 in hemorrhage group,in favor of the rehabilitation group. Conclusion Standardized community-based rehabilitation therapy may help stroke patients to improve their performance in activity of daily living significantly.

Objective To investigate and analyze the nutritional knowledge needs and cognitive level of the main family caregivers of patients with gastric cancer. Methods A total of 208 patients who underwent radical resection of gastric cancer and received complete chemotherapy at the Department of gastrointestinal surgery from September 2015 to February 2017 were selected. Each patient selected 1 family caregivers.A questionnaire survey was conducted among 208 family caregivers, including general information, nutritional knowledge needs questionnaire, nutrition knowledge, attitude and practice and so on. Results The total score of nutrition knowledge, attitude and practice among the family caregivers of patients with gastric cancer was (41.11 ± 9.33) points. The score of nutrition knowledge was (6.26 ± 1.87) points. The score of nutrition attitude was (15.45±4.75) points. The score of nutrition practice was (17.47± 4.45) points .There was a significant positive correlation between nutrition knowledge and attitude(r=0.88, P<0.05), knowledge and practice (r=0.766, P<0.01), attitude and practice (r=0.186, P<0.01) by using Pearson correlation.Family caregivers had a strong demand for nutrition knowledge. 37.5%(78/208) of family caregivers chose to cooperate with doctors and nurses to carry out nutrition knowledge. 51.4%(107/208) of family caregivers preferred bedside individualized guidance. Conclusions Family caregivers have a strong demand for nutrition knowledge, but the nutrition cognition level is poor. Health care providers should strengthen the education on patients and family caregivers' knowledge of nutrition, improve the nutrition knowledge level of caregivers and care ability.

Objective To observe the influence of nutrition cognition and behavior intervention of family caregivers on nutritional status of gastric cancer patients during chemotherapy. Methods We selected 100 patients who received complete chemotherapy at the Department of gastrointestinal surgery from September 2015 to February 2017.Each patient selected 1 family caregiver. It was divided into the control group and the intervention group by the draw method. The control group carried out routine nursing measures. The intervention group carried out nutritional cognitive behavior intervention. The effects of cognitive behavior intervention were investigated by observing the scores of PG-SGA,albumin, prealbumin in gastric cancer patients and the scores of nutrition knowledge, attitude and practice in family caregivers. Results The scores of nutrition knowledge, attitude and practice of the control group and the intervention group were respectively 44.17 ± 9.35 and 46.50 ± 9.32, and the difference was statistically significant(t=15.02, P<0.05).There was significant difference in PG-SGA score, hemoglobin and BMI in patients with gastric cancer patients (P<0.05). There was no significant difference in the incidence of malnutrition and prealbumin in patients with gastric cancer patients (P>0.05). Conclusion Nutrition cognition and behavior intervention can effectively improve the nutritional cognition of family caregivers, improve the nutritional status of gastric cancer patients during chemotherapy, and promote the rehabilitation of patients with gastric cancer and improve their quality of life.
