Objective To assess the diagnostic accuracy of NT-proBNP in hypertension patients by observing the effect of decreased GFR on N-terminal pro-brain natriuretic peptide (NT-proBNP) concentration in patients with different cardiac function. Methods Eighty-nine hypertension patients were divided into two groups based on the results of UCG.Forty-three patients had normal left ventricular function and 46 patients had dysfunction.GFR,NT-proBNP and other biochemical markers of cardiac and renal function were measured. The factors affecting the NT-proBNP concentration under normal left ventricular function were analyzed, and the diagnostic value of NT-proBNP affected by normal or decreased GFR(> 80 ml/min or ≤80 ml/min) under different left ventricular functions were further assessed. The two-sample t test, rank sum test and multiple regression analysis were used to analyze their statistical difference and relationship. Results In patients with normal left ventricular function,GFR (β =-0.361,P < 0.05) and left ventricular end-diastolic diameter (LVEDD,β =0.385,P < 0.05) were significant factors to NT-proBNP level. They were both meaningful compared with LVEF (β =0.189,P >0.05) and septal thickness (β =0.003,P > 0.05).The median concentration of NT-proBNP was 13.18 and 24.14 μg/L in patients with normal left ventricular function and dysfunction, respectively (Z =-3.19,P < 0.01).While in patients with decreased GFR,6 cases with normal left ventricular function and 19 cases with dysfunction had a median concentration of NT-proBNP of 38.45 and 44.20 μg/L,respectively (Z =-0.45,P > 0.05).In patients with normal GFR,37 cases with normal left ventricular function and 27 cases with dysfunction had a median concentration of NT-proBNP of 12.51 and 20.31 μg/L,which was lower than that of patients with decreased GFR (Z =-2.76,both P < 0.05).The NT-proBNP concentration had no significant difference between patients of normal left ventricular function with decreased GFR and patients of dysfunction with normal GFR (38.45 and20.31 μg/L,Z=-2.18,P>0.05).Conclusion In hypertension patients, the effect of decreased GFR on assessing the diagnostic accuracy of NT-proBNP for left ventricular dysfunction should be considered.

Objective To explore whether Cl2MDP-loaded gelatin particles can induce the depletion of macrophage or can depress the immunity of macrophage in SD rat models of immune thrombocytopenic purpura(ITP) to treat the ITP rats. Methods SD rats were intravenously injected with 150 ?L of anti-platelet serum once per 24 hr to induce the ITP model. The platelet count was persistently less than 50?109L-1 in these models. After intravenous injection of a group dose of Cl2 MDP-gelatin particles, the platelet counts of these models were examined at the time of 4 hr,24 hr,48 hr,72 hr and 96 hr,respectively. The pathological change of macrophages in spleen was observed by electron microscopy. Results Cl2MDP-loaded gelatin particles can significantly increase the platelet counts of ITP models from less than 50?109L-1 to mean 180?109L-1 in 24 hr after injection. The pathological changes of macrophages were seen under electron microscope, such as degeneration, necrosis and so on.Furthermore, rats pre-treated with Cl2 MDP-loaded gelatin particles can evade the decrease of platelet counts when they were injected anti-platelet serum. Conclusion Cl2 MDP-loaded gelatin particles can promptly, effectively restoreplatelet counts of ITP models to physiological level with a dose dependent manner. So,the targeting therapy of drug-loaded gelatin particles offers a new approach to treat ITP.

BACKGROUND: Decrease of regional cerebral blood flow (rCBF) perfusion of different degree exists in depressive patients, but the decreased ranges were different, different cerebral cortical and subcortical regions are involved.OBJECTIVE: To observe the types of abnormal rCBF perfusion in patients with depressive disorder.DESIGN: A diagnosis-based case-control analysis.SETTINGS: Department of Nuclear Medicine, Ruijin Hospital Affiliated to the Medical College of Shanghai Jiaotong University; Staff Room of Biomedical Engineering, Medical College of Shanghai Jiaotong University;Putuo District Mental Health of Shanghai City.PARTICIPANTS: The patients with depressive disorder were selected from the Putuo District Mental Health Center from January 1999 to December 2000, and 19 normal subjects matched by age were selected from the medical staff of the hospital as normal controls.METHODS: The patients with depressive disorder and healthy subjects were examined by single photon emission computerized tomography (SPECT), ethyl cysteinate dimmer (99Tcm-ECD) was performed, and the images were analyzed with the statistical parametric mapping (SPM). All the SPECT images were converted into DICOM format or ANALYZE-7 format at first, and then the images were standardized by taking MRI template as the template and smoothed, and the significantly different regions were evaluated by the statistical analysis and inferences with the Gaussian random theory.MAIN OUTCOME MEASURES: The differences of the cerebral blood flow perfusion in specific region and the changes of average rCBF images were mainly observed in the depressive disorder group and normal control group.RESULTS: All the 12 patients with depressive disorder and 19 normal healthy subjects entered the analysis of results. The SPM showed that the blood flow perfusion was decreased in bilateral frontal lobes, left superior temporal gyrus, left Brodmann area 20, left mamillary body of thalamus, right cingulated gyrus and right putamen in the patients with depressive disorder. The blood flow perfusion was increased in the right central frontal gyrus, right superior temporal gyrus, right occipital lobe and left central frontal gyrus.CONCLUSION: The rCBF is obviously changed in the frontal lobe, temporal lobe and limbic system of patients with depressive disorder.

Objective To characterize the vascular cognitive impairment of stroke patients with different injury sites (right or left cerebral hemisphere) and pathological type (hemorrhage or infarct). Methods A total of 119 stroke patients were assessed with regard to their cognitive functions using the LOTCA within one week of admis-sion, and comparison was made among patients in terms of VCI characteristics, injury site, and pathological type. Results In patients with left hemisphere injury, there found no significant difference between those with cerebral in-farct and hemorrhage with regard to the total score of LOTCA, but the score of perception of those with hemorrhage was lower than those with infarct(P<0.05). in patients with right hemisphere injury, both total score of LOTCA and the subscore were not significantly different between those with infarct and hemorrhage(P>0.05). In patients with infarct in left hemisphere, the total score of LOTCA was lower than those with infarct in right hemisphere(P<0.05), but the scores of orientation and thinking operation were lower than those with infarct in right hemisphere(P<0.001 or <0.01). In patients with hemorrhage in the left hemisphere, the total score of LOTCA was not significantly different from those with hemorrhage in right hemisphere, but the scores of orientation and perception were lower than thosewith hemorrhage in right hemisphere(P<0.001 or <0.01). Conclusions More attention with regard to perception training should be paid to those with cerebral hemmorhage than those with cerebral infarct. In patients with cerebral infarct, more attention with regard to orientation and thinking operation training should be paid to those with infarct in left side, while for those with cerebral hemorrhage, more attention with regard to orientation and perception training should be paid to the left hemisphere insult.
