1.Cytomegalovirus infection in systemic lupus erythematosus: an analysis of 121 cases

Objective To investigate the clinical manifestations of cytomegalovirus (CMV) infection in patients with systemic lupus erythematosus (SLE). Methods Data of the consecutive cases of SLE complicated with active CMV infection including clinical manifestations, SLEDAI score, dosage of corticosteroid and immunosuppressants used for treatment,radiological and laboratory examinations were collected and analyzed.Results Among 2221 consecutive patients of SLE, 5.4%(121 cases) were diagnosed to be complicated with active CMV infection. Fever was the most common symptom, followed by serious liver function damage,respiratory symptoms,hematological abnormalities, myocarditis, and encephalopathy, accounted for 81%(98cases), 6.6%(8 cases), 4.1%(5 cases), 3.3%(4 cases), 0.8%(1 case), and 0.8%(1 case)respectively; in addition, 22 (18.2%) cases had no symptom. SLEDAI was higher than 15 in 47.1% cases, and 10-14 in 28.1% cases. 81% of patients were treated with corticosteroid, and 55.4% were treated with immunosuppressants. Ganciclovir was given once the diagnosis of active CMV infection was established. In most of the patients, active CMV infection had been controlled within 14-28 days, except 4 died and 6 gave up the therapy. Conclusion SLE with active CMV infection is common,especially in patients who are treated with corticosteroid and/or immunosuppressants. Clinical manifestations of SLE complicated with active CMV infection are generally nonspecific.In patients with unexplained fever,or liver damage,or lung disease,or active SLE patients who have no symptom but are refractory to the treatment, CMV infection should be suspected and the relevant laboratory tests should be ordered for early diagnosis and treatment.

Objective This paper sums up professor Zhang Peiying's clinical experience in diagnosing and treating unstable angina(UA) of coronary heart disease. [Methods]Generalizing the pathogenesis, rules of treatment and principle of diagnosis about UA, through the reference of classical theory of Traditional Chinese medicine and clinical observation. Then analyse professor Zhang Peiying's characteristics about diagnosing and treating UA by two examples of medical record.[Results]Professor Zhang considers deficiency of Qi and stagnation of phlegm is the premise and foundation of UA, holds that stagnation of phlegm, stagnation of blood and spasm of heart vessel are the key causes of the disease and removing blood stasis, relieving spasm of vessel, strengthening the spleen and reducing phlegm is the basic treatment principle. He divides UA into 4 categories: stagnation of heart blood syndrome, stagnation of phlegm and blood syndrome, deficiency of Qi and stagnation of blood syndrome,weakness of Yang with stagnation of phlegm and blood. [Conclusions]Professor Zhang Peiying's scholar thought and clinical experience in diagnosing and treating UA of coronary heart disease is effective. His thought and clinical experience are worth extending and applying.
3.Effects of traditional Chinese medicine bath on mixed hemorrhoids postoperative pain and edema

Objective To investigate the effects of traditional Chinese medicine bath on mixed hemorrhoids postoperative pain and edema .Methods 120 cases of mixed hemorrhoids treated patients were selected and randomly divided into the control group and observation group in accordance with the number table .The control group were treated with potassium permanganate bath Fumigation treatment ,the observation group were used traditional Chinese medicine bath treatment , observed after treatment of pain and edema and compared the clinical efficacy of the two groups after treatment .Results The pain scores of 3 days,5 days and 7 days after treatment in the observation group were significantly lower than that of the control group ( t=2.065,2.425,2.311,all P<0.05);The edema scores of 3 days,5 days and 7 days after treatment in the observation group were significantly lower than those of the control group(t =2.007,2.065,1.993,all P <0.05);And observation group of significant efficiency,total efficiency (63.33%,93.33%) were significantly higher (45.00%,78.33%),there were significant differences between the two groups(χ2 =4.062,3.913,all P<0.05).Conclusion Herbal bath for mixed hemorrhoids postoperative pain and edema prevention and mitigation has obvious effect ,which has some clinical significance .
4.Perioperative nursing of babyport embedded

Objective To summarize perioperative nursing of babyport embedded.Methods The preoperative nursing included adequate preoperative evaluation,paying more attention to psychological nursing care for children and their families,introducing babyport characteristics and daily care,eliminating ideological concerns of children and their families; assisting relevant preoperative preparation.After surgery,children were under close observation of vital signs,the surface skin condition of the infusion port implanted,pain management,catering management,psychological care,standardization of nursing,treatment of common problems,enhanced education for discharge.Tracking files were established for each children with babyport implanted,regular follow-up tracing was given.Results Among 59 cases,58 cases were without complications,babyport of one case was removed due to infection.After followed-up for 3~18 months after discharge,58 cases of babyport embedded patients were without complications,patients and families expressed satisfaction.Conclusions Standardized perioperative nursing of babyport embedded surgery and standardized service of tracking and return service can ensure patients' safety and improve the patients'quality of life,increase service quality and patients' satisfaction.
5.Clinical effects of Er:YAG fractional laser for facial rejuvenation

Objective To evaluate the clinical effects and safety of Er:YAG fractional laser for facial rejuvenation.Methods Altogether 559 patients aged from 32 to 58 years old with obvious pigmentary lesions and wrinkles in the face were given 2 courses of restoration treatment with the Pixel Er:YAG fractional laser.The patients paid a return visit a month later after the last treatment.With the results of clinical observation and photograph-based comparison,the therapeutic effects were graded by disappearance of pigmentary lesions and elimination of facial wrinkles.Complications such as hyperpigmentation,depigmentation and hyperplastic scar were observed.Results Among the 559 patients, there were 103 cases (18.4%) of slight effect, 378 cases (67.6%) of moderate effect, and 78 cases (14%) of excellent effect,with the total effective rate of 100% and a satisfactory rate of 81.6%.The was no complication such as hyperpigmentation,depigmentation and hyperplastic scar occurred.Effective maintaining rate within 6 months was 67%.Conclusion Er:YAG fractional laser is safe and effective for facial rejuvenation and worthy of promotion.
6.Efficacy and safety of nonablative radiofrequency in facial rejuvenation

Objective To evaluate the efficacy and safety of nonablative radiofrequency in facial rejuvenation.Methods A total of 198 patients with facial skin relaxing or wrinkles in the 153rd Central Hospital of PLA from July 2010 to March 2015 were treated with nonablative radiofrequency,of which 42 cases were male,156 cases were female,the ages were from 30 to 60 years old,Fitzpatrick skin type was Ⅲ~Ⅳ.The nonablative radiofrequency were performed in all the patients.Output:Monopolar RF 90~110W,Bipolar RF 40~45W,Periorbital RF 40~50W.The total course of treatment had 6 times with a fixed 10-day interval after each therapy.Results In 198 facial skin aging patients,the total effective rate was more than 90%,the half-year effective maintenance rate was more than 70%.Complications included slight oedema and mild erythema that were minor and transitory.Side effects such as post-inflammatory hyperpigmentation,epidermal burns and scar formation were not observed.Conclusion The nonablative radiofrequency is safe and effect on facial rejuvenation with less complications.
7.Effects of tacrolimus on cell proliferation of and secretion of stem cell factors by cultured human keratinocytes

Objective To study the effects oftacrolimus on the cell proliferation of and secretion of stem cell factors (SCF) by cultured human keratinocytes. Methods Human keratinocyte cell line HaCaT was cultured and treated with various concentrations (0, 10, 102, 103, 104 nmol/L) of tacrolimus. After 48 hours of treatment, cell proliferation was measured with MTT assay, the levels of stem cell factor in the culture supernatant of HaCaT cells were detected with ELISA. Results Inhibited proliferation was observed in HaCaT cells treated with tacrolimus of 103-104 nmol/L. The secretion of SCF by HaCaT cells was enhanced in the presence of 10-102 nmol/L of tacrolimus, was inhibited in the presence of tacrolimus of 104 nmol/L, and remained unchanged with 103 nmol/L. Conclusion Certain concentrations of tacrolimus could enhance the expression Of SCF in HaCaT cells, which may be associated with the efficacy of tacrolimus in the treatment of vitiligo.
8.Study of intracardiac electrophysiologic effects of nardostachyos chinensis batal

Objective To investigate the anti-arrhythmias mechanism of nardostachyos chinensis batal by intracardiac electrophysiologic study. Methods Intracardiac electrophysiologic study were performed in 40 supraventricular tachycardia(SVT)patients before and after oral nardostachys chinensis batal administraton,and the results were compared with 35 SVT patients or normal persons without nardostachys chineusis batal intake. Results The atrial effective,relative,functional refractory periods(AERP,ARRP,AFRP)and ventricular ERP(VERP)were significantly longer in nardostachyos chinensis batal administration group than control(AFRP(358 ± 76)ms vs.(256 ±69)ms;ARRP(365 ±74)ms vs.(287 ±55)ms;VERP(264 ± 36)ms vs.(213 ± 27)ms respectively);but the sinus and atrioventricular node function had no changes. Conclusions The anti-arrhythmia mechanism of nardostachyos chinensis batal were related to the prolonged cardiac refractory periods and the reduced cardiac excitability.
9.The relationship between the serum carbohydrate antigen 125 level and the prognosis of the threatened abortion

Objective To explore the value of serum CA125 in prediction of the first-trimester threatened abortion.Methods The serum CA125 level of 78 patients with threatened abortionand( miscarriage success group and failure group) and 40 normal early pregnant women before treatment and after 1 ~ 4 weeks were tested by chemiluminescent immunoassay(CLIA) methods and the results were compared.Results The serum CA125 level of the failure group was significantly higher than that of the miscarriage success group[ (28.52 ± 19.12) x 103 U/L] and normal early pregnant women group [ (20.45 ± 9.55) × 103 U/L] ( t =- 1.28,- 1.24,all P < 0.05 ),and it increased by degree and by time,and the sensitivity and specificity of detection was 93.1% and 87.85% respectively.Conclusion Monitoring of serum CA125 had highly clinical value in predicting the prognosis of threatened abortion.
10.Clinical characteristics and related factors of cognitive function rehabilitative function in post-stroke patients with aphasia

Objective To study the clinical characteristics and potential influencing factors of rehabilitation of cognition in post-stroke patients with aphasia.methods Forty-one patients with aphasia after ischemic stroke,admitted to Nanfang hospital and Nanfang Yanling Hospital from June 2015 to November 2016 were included.Their primary clinical information and NIHSS scores were registered.In acute phase,Aphasia Battery of Chinese and Boston Diagnostic Aphasia Examination was used to evaluate language ability and classify the aphasia type.Non-language-based Cognitive assessment (NLCA) was used to assess patients' nonlinguistic cognition.Besides,the assessment of post-stroke depression and dependence degree of daily living activities were performed.3 months later,patients were evaluated again.A total of 30 aphasia patients completed the final follow-up.Result s(1) Compared with the baseline in patients with aphasia after stoke,all domains of language ability were significantly improved after 3 months(P<0.05).(2)Every nonlinguistic cognitive score was significantly increased within 3 months(total NLCA score(65.83±13.02 vs 48.00±25.11),memory(17.23±2.49 vs 13.30±5.42),visual spatial ability(10.67±2.43 vs 8.07±3.75),logical reasoning ability(6.53±1.48 vs 4.97±2.43),attention(25.57±5.79 vs 17.43±12.33),executive function(5.77±3.47 vs 4.20±4.23),all P<0.01).(3)The NLCA total score (62.40±14.23 vs 72.70±6.34)and scores in visual spatial (9.95±2.67 vs 12.10±0.74)and abstract reasoning(6.05±1.54 vs 7.50±0.71) of persisting aphasia patients were significantly lower than that of aphasia recovered group(all P<0.05).(4) Multivariate regression showed that initial NLCA score had a negative predictive effect on cognitive function improvement of aphasia patients 3 months after stroke(β=-0.603,P<0.01).Conclusion The rehabilitation of cognitive function in patients with aphasia after stroke is similar to that of language function.Patients with persisting aphasia may have worse performance on nonlinguistic cognition and ability of daily living,even with more serious depressive emotion.Evaluation of nonlinguistic cognition in post-stroke aphasia patients in acute phase is necessary and the score seems to be an important predicting factor of 3-month cognition.
