2.Study of salt-hot compress on mechanical phlebitis after PICC catheterization based on theory of“Maiweiji”

Objective To explore the clinical effect of salt-hot compress based on the theory of“Maiweiji”on mechanical phlebitis after PICC catheterization. Methods According to random number table, 202 patients after PICC catheterization were divided into the control group including 100 cases and the experimental group including 102 cases. The study were divided into the prevention stage and the treatment stage. In the prevention stage, the control group were given routine nursing while the experimental group were received routine nursing plus salt-hot compress. In the treatment stage, all the patients in mechanical phlebitis after the prevention stage were randomly divided into the group of salt-hot and the group of wet-hot by lottery, meanwhile, the group of salt-hot were given salt-hot compress, the group of wet-hot were received wet-hot compress. Results In the prevention stage, the mechanical phlebitis rate of the experimental group was 8.82%(9/102), and it was 21%(21/100) in the control group, the difference had statistically significant (Z=-2.28, P<0.05). In the treatment stage, the cure rate in 3 days and 7 days of the Group of Wet-hot were 5/15 and 10/15, and were 9/15 and 6/15 in the Group of Salt-hot, the average treatment time were (3.71±1.82) d and (4.31±1.58) d respectively, but the difference had no statistically significant(P > 0.05). Conclusions Salt-hot compress has the positive effect of preventing mechanical phlebitis. Besides, the therapeutic effect on mechanical phlebitis between the group of salt-hot and the group of wet-hot is about the same. Because of the simple operation, bargain price, being not contaminated clothing, salt-hot compress is more advantage.

4.Therapeatic effects of two different dosage of rosuvastatin on endothelial dysfunction in diabetic rats

Objective To evaluate the effects of two different dosage of rosuvastatin on endothelial dysfunction in diabetic rats. Methods The 24 diabetic rats were randomly divided into three groups (n=8,each): diabetic control group, 20 mg rosuvastatin daily (RV 20 mg group) and 10mg rosuvastatin daily for 8 weeks (RV 10 mg group) and normal control group (SD group). The levels of blood glucose, lipid, nitric oxide(NO) and endothelin-1 (ET-1) were measured before and 8 weeks after treatment. Results The levels of blood glucose were higher in all diabetic rats groups than in SD group before experiment (P<0. 01). Compared with diabetic rats control group, blood glucose was slightly lower in RV 10 mg group and RV 20 mg group at 8 weeks (P>0. 05). The plasma NO level was significantly lower in diabetic rats control group than in SD group (P<0. 05).After 8 weeks, plasma NO levels were significantly higher in RV 20 mg and RV 10 mg groups than in diabetic rats control group (P<0. 01 or P<0. 05). The plasma levels of ET-1 was significantly higher in diabetic rats control group than in SD group (P<0. 01). After 8 weeks, plasma ET-1 levels were significantly lower in RV 20 mg and RV 10 mg group than in diabetic rats control group (P<0. 01).Meanwhile, the plasma lipids were lower in RV 20 mg and RV 10 mg group than in diabetic control group (P<0. 05 or P<0. 01). Conclusions Rosuvastatin can adjust blood lipids and significantly improve endothelial function in diabetic rats by increasing plasma NO level and decreasing plasma ET-1 level.
5.Applied value of 320-slice CT perfusion examination in ischemic-type biliary lesions after liver transplantation

Objective Using CT perfusion (CTP) technique,to investigate the graft perfusion changes in patients with hepatic artery stenosis (HAS) with or without ischemic-type biliary lesions (ITBL) after orthotopic liver transplantation (OLT).Methods Thirteen recipients with HAS received CTP scan of the liver,including 8 with ITBL and 5 without ITBL.For all patients,the diagnosis of HAS was made by CTA,and the diagnosis of ITBL by percutaneous transhepatic cholangiography.CT perfusion indices were obtained,including hepatic artery perfusion (HAP),portal vein perfusion (PVP),total liver perfusion (TLP) and hepatic perfusion index (HPI).Results Of the 13 patients with HAS,mean HAP in patients with and without ITBL was 59.8 and 35.1 ml·min-1 ·100 ml-1 (P =0.021,two-tailed paired Student t test) ; mean PVP was 125.4 and 166.2 ml·min-1·100 m1-1 (P =0.016) ; mean TLP was 185.2 and 201.3 ml· min-1 · 100 ml-1 (P =0.306) ; and mean HPI was 33.6 and 18.2 (P =0.005),respectively.Conclusion Using CTP technique,liver perfusion changes were reflected by measuring CTP indices noninvasively.Compared to those without ITBL in this study,HAP and HPI in patients with ITBL were higher and PVP was lower,which may be contributed to biliary inflammation.
6.The Changes of Serum Interleukin-37 Levels in Patients With Acute Coronary Syndrome

Objective: To study the serum interleukin-37 (IL-37) level changes in patients with acute coronary syndrome (ACS) and to explore the relationship between IL-37 and coronary atherosclerotic plaque.
Methods: Our research included 3 groups. ACS group, n=60, SAP (stable angina pectoris) group, n=30 and Control group, the subjects with normal coronary artery, n=15. The peripheral serum levels of IL-37 were examined by ELISA and the differences were compared among different groups.
Results: ① The serum levels of IL-37 at admission were as ACS group < SAP group < Control group, P<0.05.②Intervention could transitionally decrease IL-37 level in SAP group. With 4 weeks treatment, IL-37 levels were signiifcantly increased in both ACS group and SAP group than admission time, while they were still lower than Control group, P<0.05.③The serum level of IL-37 at admission was negatively related to IL-18 (r=-0.79, P<0.05), the ratio of IL-18/IL-37 were as ACS group>SAP group>Control group, P<0.05.④In ACS group, IL-37 level was negatively related to GRACE score (r=-0.71, P<0.05), the ratio of IL-18/IL-37 was positively related to GRACE score (r=0.73, P<0.05).⑤The diagnosis of ACS could be basically excluded if the patients with IL-37>77ug/L.
Conclusion: The serum IL-37 might be involved in the inlfammatory process in ACS patients, it could be expected as an index for ACS monitor and evaluation in clinical practice.
7.Analysis of the distribution characteristics and drug sensitivity of nosocomial infection in patient with hematologic neoplasms

Objective To investigate clinical distribution and drug sensitivity of infectious pathogens in our wards for hematology malignancies. Methods Drug sensitivity tests of bacteria were performed by Kirby-Bauer method, 56 strains of pathogens were isolated from all detected samples. Results The results showed that the composition ratio of Gram-negtive bacteria, Gram-positive bacteria was 69.64%, 30.36%. In decreasing frequency, Escherichia coli (37.50%), Klebsiella pneumoniae (10.71%). All of staphylococcies were resistant to meticillin, and sensitive to vancomycin. Conclusion This study indicates that Gramnegative bacteria remain the predominant pathogens in microorganisms causing bloodstream infections for hematological malignancies at Huashan Hospital. The incidence of Escherichia coli is the highest. All of staphylococcies were resistant to meticillin.
8.Clinical analysis on 48 cases of kidney transplantation from brain and cardiac death donors

Objective To observe the short?term clinical outcomes of kidney transplantation from brain and cardiac death donors (DBCD) and assess its feasibility to expand organ donor pool. Methods A retrospective analysis was performed on 48 cases of kidney transplantation from DBCD. The transplant recipients had finished 12?month follow?up in the First People's Hospital of Foshan from September 2011 to February 2015, with their renal function, rejection reaction and complications at 1 week, 1 month, 3 months, 6 months and 12 months after renal transplantation being collected. Survival rates of transplant recipients and transplant kidneys, incidence of delayed graft function (DGF) and its influence for recipients and graft survival were analyzed by statistics. Results In the 48 cases, the survival rates of recipients at 1, 3, 6 and 12 months after transplantation were 100.0%, 100.0%, 97.9%, 95.8%, and the survival rates of transplanted kidneys were 95.8%, 95.8%, 93.8%, 91.7%, respectively. DGF occurred in 8 of 48 (17.0%), but the occurrence of DGF did not adversely influence patient's survival (P=0.524) or graft survival (P=0.362). Conclusions The short?term clinical outcomes of kidney transplantation from DBCD are ideal. As the legislation of donation after brain death (DBD) has not been ratified in China, the kidney transplantation from DBCD could be an
important way to solve the shortage of organs, and increase the number of kidneys available for transplantation.
9.Effects of pre-transplant dialysis modality on early outcome of kidney transplantation from donation after cardiac death

Objective To compare the influence of hemodialysis (HD) and peritoneal dialysis (PD) on early outcome of patients underwent kidney transplantation from donation after cardiac death (DCD).Methods Patients admitted in the First People's Hospital of Foshan with DCD kidney transplant from January 1st,2011 to June 30th,2016 were analyzed retrospectively.Recipients were grouped into HD group (n=61) and PD group (n=28) according to their pre-transplant dialysis modality.Their short-term outcomes after DCD kidney transplant were compared,including recovery of renal function,short-term complications and laboratory data.Results Patients had longer dialysis duration and lower hemoglobin,serum albumin and phosphorus in PD group than those in HD group (all P < 0.05),but no significant difference shown in age,gender,body mass index,primary disease,blood pressure,and hepatitis B infection (all P > 0.05).HD patients with 6.00(4.00,11.00) d recovery time of renal function,18.00(17.00,21.50) d hospital time,had 24.59% the delayed graft function (DGF),3.28% acute rejection and 16.39% infection during hospitalization.While for PD patients the recovery time of renal function was 4.00(3.75,7.00) d;hospital time was 19.00(15.00,21.75) d;the incidence rate of DGF was 14.29%;acute rejection was 3.57%;and infection during hospitalization reached 17.86%.Above indexes were not significantly different between HD and PD groups (all P >0.05).Repeated measure ments showed that,compared with those before transplant surgery,after 1 month,3 months and 6 months HD and PD groups had decreased creatinine and phosphorus,and increased hemoglobinserum albumin and calcium;Serum albumin and calcium were different between the two groups (P < 0.001,P=0.040),whereas creatinine,hemoglobin and phosphorus did not show difference (all P < 0.05).After transplantation the trends of creatinine,hemoglobin,calcium and phosphorus were not different between the two groups (P values were 0.295,0.310,0.501 and 0.063,respectively).Conclusions No significant difference of the recovery regarding renal function,anemia,nutrition status and mineral metabolites was found between pre-transplant HD and PD modality in patients who underwent DCD kidney transplantations.
10. Prognostic risk factors of patients with refractory acute left heart failure treated by continuous renal replacement therapy

Objective:
To find out the prognostic influencing factors of patients undergoing continuous renal replacement therapy (CRRT) for refractory acute left heart failure.
Methods:
Through the medical system and hemodialysis system in Foshan First People's Hospital, all patients who received CRRT for refractory acute left ventricular heart failure from January 1, 2012 to January 1, 2019 were searched. All patients were divided into two groups by the final outcome: survival group and death group. Age, sex, initial mean arterial pressure (MAP), primary heart disease, use of vasoactive drugs, urine output before treatment, hemoglobin, serum creatinine, serum albumin, C-reactive protein(CRP), brain natriuretic peptide (BNP), cardiac ejection fraction (EF) and CRRT treatment time were analyzed to find out the prognostic influencing factors.
Results:
A total of 130 cases were collected, including 96 cases in the survival group and 34 cases in the death group, with a total mortality rate of 26.15%. Compared to that in the death group, there were higher proportion of males (71.88% vs 50.00%,
