1.The clinical efficacy of olecranon osteotomy approach and bilateral approach through triceps brachii for the treatment of intercondylar humeral fractures

ObjectiveSystematically evaluate the clinical efficacy and complications of olecranon osteotomy and bilateral approach through triceps brachii for the treatment of intercondylar humeral fractures. MethodsTo search the domestic and foreign literatures published from January 1980 to March 2008 about the treatment of intercondylar humeral fractures, which use olecranon osteotomy approach and bilateral ap-proach through triceps brachii. Systematically evaluate these literatures using the methods of systematic re-view provided by the Cochrane collaboration. ResultsAfter the screening and evaluation to the literatures, 33 published literatures that met the inclusive criteria of this study were collected. There were a total of 876 cases of patients whose follow-up data was integrity (olecranon osteotomy approach group for 509 cases;bilat-eral approach through triceps brachii group for 367 cases) and these cases had provided the postoperative scores of elbow joint function. The excellent and good rates of postoperative score of elbow joint function for the type C2 and C3 fracture patients in the olecranon osteotomy approach group were 82.32% and 79.38% respectively; while those in the bilateral approach through triceps brachii group were 70.59% and 64.71% respectively. The former was significantly higher than the latter. The excellent and good rate of the type C1 fracture patients between two approaches groups showed no statistical significant. Fourteen literatures provid-ed the data about postoperative complications, the incidence of complications of patients that received ole-cranon osteotomy approach was 4.72%(95%CI 4.70%-4.74%), and the results also showed that compared with bilateral approach through triceps brachii (8.17%, 95%CI 8.13%-8.21%). ConclusionTo treat inter-condylar humeral fractures, postoperative efficacy of olecranon osteotomy approach is better than that of the bilateral approach through triceps brachii. Using olecranon osteotomy approach can get adequate exposure, easy open reduction and internal fixation, quick recover and less complication.
2.Epidemiologic and injury characteristics of 1 328 road traffic injuries in Shenzhen

Objective To analyze epidemiologie and injury characteristics of traffic injuries in Shenzhen through emergency department. Methods A questionnaire was pre-designed to collect data of patients admitted to six general emergency departments of six district in Shenzhen from January 2007 to December 2007 for clinical analysis. Results There were 1 328 patients with road traffic injuries, at age range of 2-84 years (average 31 years). According to occupation, most patients were local workers and migrant workers (57.8%). The accidental vehicles were mostly cars (48.5%). Most accidents (78.6%) occurred in urban streets. As for time distribution of traffic injuries, most injuries happened on Friday and Saturday (accounting for 17.8% and 18.7% respectively), especially on Friday (r = 0.166 0, P <0.01). There was central tendency around 15:35 p.m. in one day (r = 0.285 6, P < 0.01). The age of male was (32.36±8.70) years and that of female (29.96±4.32) years, with statis-tical differences (P < 0.05). Of patients followed up, 84.4% patients could work on their original jobs, 8.5% needed ehange of their jobs and 7.1% lost the ability of work. Conclusions It is neeessary to strengthen road safety education among local workers and migrant workers to improve their awareness of road safety. Traffic administrators and road safety marks should be added in the chief period and districts where road traffic injuries happen most frequently. Controlling the number of small cars may be effective to reduce traffic accidents.
3.Morphology character and reduction methods of sagittally unstable intertrochanteric fractures

Objective:To investigate the morphology character of sagittally unstable intertrochanteric fractures and reduction methods.Methods: A retrospective study was used to analyze the clinical data of sagittally unstable intertrochanteric fractures cases,which were treated with proximal femoral nail anti-rotation (PFNA) fixation from March 2009 to June 2016.In the study,36 cases were followed up completely,in which 17 cases accepted open reduction,and the other 19 cases accepted minimally invasive reduction.The operation time,amount of bleeding,the fluoroscopy times,postoperative radiographic measurements,such as tip-apex distance (TAD) and sliding distance of the spiral screw,and hip Harris scores were analyzed.The morphology character of the fractures was documented and investigated.Results: The average follow-up time was 15 months.The amount of bleeding of the open reduction group was (170.5±19.7) mL,and the amount of bleeding of the minimally invasive group was (54.7±12.5) mL.The amount of bleeding of the minimally invasive group was significantly less than that of the open reduction group (P<0.001).Between the two groups,there were no significant differences in other evaluation parameter,including operation time (P=0.054),the fluoroscopy times (P=0.053),fracture healing time (P=0.305),postoperative radiographic measurements,such as TAD (P=0.317) and sliding distance of the spiral screw (P=0.206),and hip Harris scores (P=0.459).In regard to morphology character of the fractures,the proximal anterior unstable fractures with separation displacement were more common than the proximal posterior unstable fractures with impaction.The characteristic feature of the proximal anterior unstable fractures was the proximal anterior and medial long oblique fracture,and a V shape cortical defect in the distal fracture fragment on the externally rotation X-ray.Conclusion: Sagittally unstable intertrochanteric fractures can be divided into the proximal anterior unstable fractures with separation displacement,and the proximal posterior unstable fractures with impaction.The two types have their own morphology character individually.The reduction should be performed by minimally invasive techniques.

Objective To imestigate the effect of total polysaccharides(TP) from Sijunzi Decoction(SD) on the immune function of mice intestinal intraepithelial lymphocytes(iIEL).Methods NIH mice were eaqually randomized to 4 groups:control group,model group,TP-SD group,free-protein polysaccharides from SD(FPP-SD) group.Mice iIEL was extracted by discontinuous density method.The percentage of CD3+ and CD69+ in iIEL were detected by flow cytometer.IL-2 and IFN-? expression in iIEL supernatant was detected by ELISA.Results The percentage of CD3+ lymphocyte in iIEL was 88.46 %.The percentage of CD69+ lymphocyte and the levels of IL-2 and IFN-? in iIEL were significantly decreased in the model group compared with those in the normal control group(P

6.Effects of calcitonin gene-related peptide on tibial fracture healing at early stage in rats

Objective To investigate the effect of calcitonin gene-related peptide(CGRP) on early-stage bone fracture healing and on the expression of bone morphogenetic protein-2(BMP-2) in bone tissue of rats.Methods Totally 30 SD rats with tibial fracture were randomly and equally divided into 3 group and received normal saline(control group),CGRP or CGRP receptor antagonist injection intraperitoneally.In 1,2 and 4 weeks later,the fracture sites from the 3 groups were subjected to X-ray scanning,callus measurement and HE staining.The expression of the BMP-2 was detected by immunohistochemical method.Results There was no obvious difference on callus amounts among the 3 groups,but at 2 weeks after treatment,there were significantly more callus in CGRP group with enlargement in the fracture site than in the other 2 groups;The enlargement of CGRP group became minimized with fused cortex and almost normal morphology and size.So were the other 2 groups.Immunohistochemical results showed that exogenous CGRP significantly increased BMP-2 expression,but the expression was decreased significantly after administration of CGRP receptor antagonist.Conclusion Our results suggest that CGRP may participate in the regulation of bone fracture healing,and induce BMP-2 production,implying that BMP-2 may be related functionally with CGRP in the mechanism of bone metabolism.
7.Biomechanical comparison of two screw fixations in treatment of tibial plateau fractures ZHANG

Objective To compare the biomechanical stabilities of 2 fixations for oblique split fractures of tibial plateau. Methods Oblique split fractures of the medial tibial plateau were simulated by osteotomy in 6 pairs of fresh tibia of adult swine. One fracture of each pair was fixed with 3 lag screws parallel to the articular surface whereas the contralateral site was stabilized with 3 lag screws vertical to the fracture line. The maximum anti. Compression load, the maximum power consumption at the maximum load.the load and power consumption at the 2 mm subsidence of articular surface were measured and recorded. Results There were no significant differences between the 2 groups in the maximum anti-compression load. The maximum power consumption at the maximum load, the load and power consumption at the 2 mm subsidence of articular surface. Conclusion Vertical fixation may not provide a biomechanical advanrage over the parallel fixation in stabilizing oblique split fractures of tibia plateau.
8.Treatment of femoral condyle fracture in aged patients

Objective To discuss the selection and application of internal fixators in the treatment of aged patients suffering from femoral condyle fracture. Methods 82 aged patients with femoral condyle fracture were treated from February 1995 to June 2002. According to AO/ASIF classifcation,26 cases were classified as type A,17 cases type B, and 39 cases type C. They were treated with internal fixation by 95?angle plate (19 cases), T type plate(5 cases), AO condyle supporting plate (11 cases), dynamic condylar screw (DCS) (23 cases) and Green Seligsen Henry (GSH) (24 cases). Measures of auto ilium transplant or homologous allograft bone transplant (28 cases) were also taken. Results All the cases were followed up for an average of 14.6 months. Merchan grading method was applied to evaluate their knee joint functions. The total excellent and good rate was 90.2%. Conclusions Aged patients with femoral condyle fracture should be treated specially. The fixation should be chosen according to their fracture type so as to ensure fine clinical results. The goal of the operation is to keep the integrity of articular surface of the femoral condyle and to acquire secure internal fixation.
10.Effect of removing stasis medicinal herbs on proliferation and proteomics of THP-1 cell line induced by endotoxin

Objective:To observe the proteomics of blood-activating and stasis-removing(BASR) herbs in cellular model of endotoxin infection in vitro.Methods:Cellular model of endotoxin was induced by LPS,and a technical system by two-dimensional electrophoresis was widely used,proteins in different expression were analyzed by PDQust 7.1.1.Results:There were 176 protein spots in model group,in which 26 protein spots with different expression were sieved.Compared the protein profiles of TanshinoneⅡA group in model group,proteins were found as two-way-regulated.Among them,50% proteins were observed down-regulated and 38.5% proteins were seen up-regulated in TanshinoneⅡA group.11.5% more up-regulated.Conclusion:Cellular model of endotoxin induce was applied to sieving traditional Chinese medicines in laboratory;The Tanshinone ⅡA can inhibit the proliferation of THP-1 cell line in infection model,and can modify expression of some proteins.
