1.Comparison of clinical effect of montelukast and ketotifen in the treatment of children with cough variant asthma

Objective To compare the clinical effect of montelukast sodium and ketotifen in the treatment of children with cough variant asthma,thus to provide theoretical basis for clinical treatment.Methods 61 children with cough variant asthma were randomly divided into control group and observation group according to the digital table method,31 cases in each group.Patients of two groups were all given antispasmodic and antitussive.The control group was given ketotifen tablets drug therapy,1mg/time,2 times/d,and the observation group was treated with montelukast chewable tablets,5mg/time,1 time /d.After 8 weeks of treatment,the clinical effects were compared between the two groups.Results The total effective rate of the observation group (96.77%)was higher than that of the control group (77.42%),and the difference was statistically significant (χ2 =16.65,P <0.05).The clinical symptoms improved time of the observation group was (5.78 ±1.36)d,which was shorter than (8.45 ±2.24)d of the control group,the difference was statistically significant (t =2.71,P <0.05).The recurrence rate of the observation group (6.45%) was lower than the control group (16.13%),the difference was statistically significant (χ2 =4.67,P <0.05 ). Recurrence time of the observation group was better than that of the control group,the difference was statistically significant (t =7.98,P <0.05 ).Conclusion The curative effect of montelukast special chewing tablets in the treatment of children with bronchial asthma is obviously superior to ketotifen,has less adverse reactions.It was worthy of clinical application.

Objective To study the relationship between the expression of human Stanniocalcin-1(hSTC-1) mRNA in the peripheral blood from patients with colorectal cancer and its malignant behavior. Methods RT-PCR was used to detect hSTC-1 mRNA in the peripheral blood from 57 patients with colorectal cancer. The peripheral blood from 14 patients with gastrointestinal inflammatory diseases, 15 healthy volunteers and 5 pregnant women were served as controls. 3.Clinical application of laparoscopy in nephrectomy in elderly patients

Objective To compare the efficacy of laparoscopy versus traditional surgery for elderly patients undergoing nephrectomy.Methods Totally 134 elderly patients undergoing nephrectomy diagnosed by CT or PET-CT were divided into the laparoscopy group and the traditional surgery group (n=67,each).Clinical symptoms,operation time,bleeding volume,analgesic use,the duration of drainage,exhaust time,length of hospital stay,adverse reactions and death rate were recorded.All patients were followed-up for 2 years.Results There were significant differences in the bleeding volume,operation time,analgesic use,duration of drainage and exhaust time between the laparoscopy group and the traditional surgery group [(141.2 ± 23.3) ml vs.(175.7 ± 31.7) ml,(162.4±13.1) min vs.(177.6±13.1) min,(60.7±15.1) mgvs.(89.2±17.7) mg,(2.4±0.9) d vs.(4.0±1.1) d,t=2.92,5.21,15.94,10.5,respectively,all P<0.05].The incidence of postoperation complications was lower in the laparoscopy group than in the traditional group [4 cases (6.0%) vs.15 cases (22.4%),x2=11.03,P=0.001].Tumor relapse was found in 2 cases in the laparoscopy group and 3 patients in the traditional group.1 patient died in laparoscopy group and 2 patients in traditional group,respectively.There were no significant differences in tumor relapse and death rate between groups (both P> 0.05).Conclusions Laparoscopy is safe and effective for elderly patients undergoing nephrectomy and it can reduce the complications and adverse reactions,which can be the first choice for elderly patients undergoing nephrectomy.
4.Clinical study of laparoscopic hysteromyomectomy

Objective To investigate the clinical value of laparoscopic hysteromyomectomy.Methods The clinical data of 98 cases of laparoscopic hysteromyomectomy(LM)and 76 cases of transabdominal Myomectomy(TAM)were retrospectively analyzed and comparison was made on the operative time,operative blood loss,postoperative blind enema time,body temperature recovery time,the rate of postoperative complicating disease and hospitalization after operation.Result Though the mean opertative time is the same,the mean operative blood loss,the mean body temperature recovery time,postoperative blind enema time,and the mean hospitalization was smaller in LM group than in TAM group(P<0.05).Conclusion Laparoscopic hysteromyomectomy has the advantage of minimal invasion,short in-hospital days,fast recovery and low complication rate,which is an ideal treatment of hysteromyoma.

Objective: To survey the content of serum total cholesterol, LDL and HDL in pa- tients with gastric cancer and to investigate the correlation between serum total cholesterol, LDL, HDL and the differentiation of gastric cancer. Methods: 143 patients with gastric cancer were selected and assigned into 3 groups according to pathology grouping after operation: 1) the well-differentiated group, consisting of 16 patients with papillary adenocarcinoma, and 43 patients with tubi-form well-differentiated adenocarcinoma; 2) the moderately differentiated group, including 39 patients with tubiform moderately differentiated adenocarcinoma, 3) the poorly differentiated group, including 20 patients with poorly differentiated adenocarcinoma, and 14 patients with mucinous adenocarcinoma, and 11 patients with signetring cell carcinoma. In addition, 30 patients with gastric ulcer and 30 health adults were selected as control. Veinous blood of every member was collected, and then the content of serum total cholesterol was surveyed by enzymic method, and the contents of serum LDL and HDL were measured by precipitation method. Results: The level of serum total cholesterol, LDL and HDL before operation in patients with gastric cancer decreased significantly than those with gastric ulcer and normal people (P0.05). There was statistically significance among different differentiation in patients with gastric cancer. The differences of levels of serum total cholesterol, LDL and HDL were statistically significant in gastric cancer of different differentiated groups. 7.Intervention of Zhuanggu granule on IL-1β and TNF-α level of the patients with osteoarthritis

Objective To investigate the influence of Zhuanggu granule on the concentration of IL-1β and TN-F-α in knee cavity of patients with knee degenerated osteoarthritis. Methods A total of eighty patients with knee degenerated osteoarthritis were recruited into a Zhuanggu granule group (30 cases), a Sulphuric acid Glucosamine group (15 cases) and a Sodium Hyaluronate group (15 cases) according to Doll grouping method. After all groups were treated for 4 weeks, the changes of concentration of IL-1β and TNF-α was detected before and after the therapy Results After the treatment, the concentration of IL-1β and TNF-α in Zhuangu granule group was significantly lower than the other two groups (Sodium Hyaluronate and sulphuric Glucosamine group). Conclusion Zhuangu Granule could influence the concentration of IL-1β and TNF-α in patients of knee degenerated osteoarthritis.

The relationship between tumour necrosis factor-alpha (TNF-alpha) gene polymorphism and inhibitory effects of triptolide on TNF-alpha production from peripheral blood mononuclear cells (PBMC) of healthy humans was investigated. Genomic DNA from 41 healthy people was typed for TNF-alpha--308 polymorphism by allele-specific polymorphism chain reaction (AS-PCR). The TNF-alpha concentration in the supernatant was measured by ELISA. The results showed that the production of TNF-alpha from TNF-alpha--308 non-G/G genotype PBMC was higher than that from TNF-alpha--308 G/G genotype PBMC after stimulated by LPS. Triptolide could lower the production of TNF-alpha from G/ G genotype PBMC, but had no effect on the level of TNF-alpha from non-G/G genotype PBMC. It was concluded that TNF-alpha gene polymorphism was related to the TNF-alpha production from triptolide-inhibited PBMC culture in healthy humans.
9.Stapled haemorrhoidectomy in the choice of anastomosis site in patients with severe circumferential prolapsed haemorrhoids

Objective To explore the correlation of the distance between anastomosis and dentate line in patients with severe circumferential prolapsed haemorrhoids treated by stapled haemorrhoidectomy with the patients' postoperative clinical manufestival score, and assess its value in the choice of anastomosis site in stapled haemorrhoidectomy. Methods One hundred and six patients with severe circumferential prolapsed haemorrhoids was treated by stapled haemorrhoidectomy. The distance between anastomosis and dentate line was documented during the operation, effect of the treatment and complications were also documented postoperatively. All above-mentioned data were analysed statisticaly by one-way ANOVA and ridit test.Results Four groups were established in 106 patients according to the distance between anastomosis and dentate line. Patients with distance less than 1.0cm were defined as group A, between 1.0 cm and 1.5 cm as group B, between 1.5 cm and 2.0 cm as group C, more than 2.0 cm as group D. Concerning the postoperative incontinence score, satisfaction index and complications such as haemorrhage,ederma of anal everage,residal skin-tags, there was no significant difference between all groups. But there was significant difference between four groups in score of pain. Conclusions Patients with severe circumferential prolapsed haemorrhoids treated by Stapled haemorrhoidectomy tend to have good clinical outcome. The appropriate distance between anastomosis and dentate line should be chosed by the status of prolapsed haemorrhoids.

Objective:To investigate the role of service-learning model in medical ethics education. Meth-ods:A qualitative and quantitative research was conducted within 831 undergraduates and 302 graduates in Shan-tou University Medical College. Results: Service-learning model was well accepted by most medical students. Higher medical ethics awareness and more positive tendency of behavior choices were found on the group who par-ticipated in serving-learning model activities, which also revealed a positive correlation relationship to participated frequency. Conclusions:Serving-learning model plays an important role in medical ethics education, which is an effective way to deeply practice Opinions on Deepening the Medical and Educational Reform toward Personnel Training in Clinical Medicine.
