1.Clinical observation on the effect of mannitol combined with Xueshuantong on the treatment of early swelling and pain in the patients with tibia and fibula fracture

Objective To summarize and analyze the effect of mannitol combined with Xueshuantong on the treatment of early swelling and pain in patients with tibial and fibula fracture. Methods In Huzhou central hospital from June 2014 to January 2017, 100 cases with early swelling and pain with tibia and fibula fractur were divided into the observation group (50 cases) and the control group (50 cases). The control group were given routine mannitol combined with Xueshuantong treatment and routine nursingcare, at this basis, the observation group were received nursing intervention. The effect of the treatment and nursing in the two groups were compared. Results The total effective rate, nursing service satisfaction and the VAS score after treatment between the two groups were statistically significant (P<0.05). Conclusion The treatment of mannitol combined with Xueshuantong and strengthening clinical nursing intervention is helpful to improve the therapeutic effect and alleviate the pain of the patients with early swelling and pain with tibia and fibula fracture.
2.Clinical significance of plasma N-terminal pro-brain natriuretic peptide during intraoperative period in children with congenital heart disease

Objective To explore the clinical significance of plasma N-terminal pro-brain natriuretic peptide (NT-proBNP) during intraoperative period in children with congenital heart disease (CHD).Methods Ninety-two CHD patients (CHD group) and 92 healthy controls (control group) were enrolled.For CHD patients,the level of plasma NT-proBNP was assayed preoperative and 1st,3rd and 5th day postoperation.The relationship between NT-proBNP and recovery of cardiac function after operation was analyzed.The relationship between complications and days for hospitalization was also determined.Results Before operation,the level of NT-proBNP in CHD group was significantly higher than that in control group (median:231.26 ng/L vs.62.18 ng/L)(P<0.05).After operation,the level of NT-proBNP in CHD group was significantly increased at 1st day (median:357.26 ng/L) (P<0.05),3rd day(median:310.66 ng/L)(P<0.05 ),and decreased at 5th day (median:139.37 ng/L)(P<0.05 ).The difference of LVEF preoperative and postoperative was significantly lower in high level of NT-proBNP patients(46 cases) than that in low level of NT-proBNP patients (46 cases)[ (4.61 ± 1.69)% vs.(6.31 ± 1.27)%](P<0.05).The rate of congestive heart failure and the third atrioventricular block,the days for hospitalization were higher in high level of NT-proBNP patients than those in low level of NT-proBNP patients [ 17.39% (8/46) vs.2.17% (1/46),15.22%(7/46) vs.0,(9.36 ± 1.27) d vs.(7.21 ± 1.39) d](P<0.05).Conclusion Plasma NT-proBNP is a useful marker during intraoperative period in CHD patients.

4.The effect of atorvastatin on heart function and brain natriuretic peptide, C-reactive protein in chronic congestive heart failure

ObjectiveTo observe the effect of atorvastatin on heart function of patients with chronic congestive heart failure(CCHF) and the changes of brain natriuretic peptide(BNP),C-reactive protein (CRP).MethodsSeventy-eight patients with CCHF were divided into control group and atorvastatin group by random number table with 39 cases each.Patients in control group were treated with routine treatment such as diuretic,inotropic agents,beta receptor blockers,and atorvastatin group were treated with atorvastatin calcium 20 mg once a day at the base of routine treatment.After treatment for 6 months,the levels of left ventricular ejection fraction (LVEF),left ventricular end-diastolic dimension (LVEDd),left ventricular end-systolic dimension (LVESd) and CRP,BNP were detected and compared between two groups.ResultsAfter treatment,the level of LVEF was significantly increased and the levels of LVEDd,LVESd were significantly decreased in two groups (P <0.05).Moreover,the level of LVEF in atorvastatin group was significantly higher than that in control group [ (46.75 ± 5.82)% vs.(40.58 ± 4.92 )% ] (t = 4.016,P <0.05),and the levels of LVEDd,LVESd in atorvastatin group were significantly lower than those in control group [ (49.13 ± 4.02) mm vs.(57.62 ± 4.77) mm,(37.76 ± 3.02) mm vs.(41.89 ± 3.53 ) mm] ( t =4.135,4.253,P < 0.05 ).The levels of CRP,BNP after treatment in atorvastatin group [ ( 11.86 ± 4.11 ) mg/L and (247.92 ± 32.78) ng/L] were significantly decreased compared with those before treatment [( 19.62 ±7.93) mg/L and(496.61 ± 78.87) ng/L] (t = 4.668,4.821,P < 0.05 ),and the levels of CRP,BNP were significantly lower than those in control group [ (16.79 ± 6.52) mg/L and(475.41 ± 65.62) ng/L] (t =3.889,4.011,P < 0.05).ConclusionAtorvastatin can significantly improve heart function in patients with CCHF and reduce the plasma levels of CRP and BNP,which is good for CCHF patients to improve conditions.
5.Effect of psychological nursing intervention on continuous ambulatory peritoneal dialysis patients with chronic renal failure

Objective To analyze the psychological status of continuous ambulatory peritoneal dialysis (CAPD) patients with chronic renal failure,and to explore the influence of psychological nursing intervention measures on it.Methods Using the retrospective method,50 patients in our hospital were chosen as the control group from January to June 2013,who was given conventional nursing care,Another 50 patients from July to December 2013 were set as the observation group,and was given psychological nursing intervention on the basis of conventional nursing.The scores of Self Rating Anxiety Scale (SAS),Self Rating Depression Scale (SDS) and nursing satisfaction degree were compared between two groups of patients.Resuits After the intervention,SAS score and SDS score in the observation group were significantly lower than those of the control group,the difference had statistical significance.The rates of satisfaction degree with nursing,returning to society after recovery,choosing our hospital once again,recommending others to our hospital in the observation group were higher than those of the control group,the differences were statistically significant.Conclusions The sustained implementation of psychological care in peritoneal dialysis patients with chronic renal failure is of great significance,which can eliminate the anxiety and depression of patients,improve nursing satisfaction,which is worthy of clinical application.

In a variety of pathological conditions,capillaries grow from corneal limbus to the central cornea area,to form corneal neovascularization (CNV).CNV destroys the normal corneal microenvironment, reduces the transparency of cornea, seriously affects visual acuity and leads to corneal transplantation immune rejection.Corneal microenvironment includes inflammatory mediators, cytokines and extracellular matrix (ECM).Studies showed that corneal microenvironment changes have a close relationship with CNV.This article reviewed the relationship of corneal microenvironment with CNV.
7.Clinical characteristics and classification of diabetic patients with ketosis-onset

79 patients with an initial onset of ketosis or ketoacidosis were classified into four groups,based on the presence of islet autoantibodies(A+or A-) and-βcell functional reserve(β+or β-).According to their clinical characteristics,biochemical parameters and therapeutic consequences,the four groups can be considered as type 1 DM,idiopathic type 1 DM,latent autoimmune diabetes in adults(LADA) and type 2 DM respectively.
8.Analysis of surgical effect of the patients with uveitis-complicated cataract

Objective To analyze the surgical results of uveitis-complicated cataract.Methods Cataract phacoemulsification surgery was performed on 31 eyes (29 patients) due to uveitis (iridocyclitis 14 eyes, Fuchs heterochromic cyclitis 7 eyes, intermediate uveitis 7 eyes, Vogt-koyanagi-harada syndrome 2 eyes, and sympathetic ophthalmia 1 eye). Follow-up period was 5~28 months, averaging 15 months. Retrospective analysis was made.Results Postoperative visual acuity≥0.5 was 80%. One eye (Vogt-koyanagi-harada syndrome) showed severe anterior uveitic reaction; One eye (iridocyclitis) had fibrinous membranes on IOL, and white debris on IOL were found in 2 eyes (intermediate uveitis).Conclusion By using less traumatic phacoemulsification technique to ocular tissue, taking meticulous care to avoid stimulating iris, cleaning up cortex thoroughly during surgery and implanting the IOL in the bag, less inflammatory reaction, less complication and better vision acuity postoperatively at relative quiescence period of uveitis can be achieved.
