Objective To observe the curative effect and safety of recombinant tissue plasminogen acti-vator (rt-PA) in acute ischemic stroke (AIS) patients in more than 15 points of NIHSS scores and less than 4.5 h in onset time. Methods One hundred and twenty-seven cases with AIS in more than 15 points of NIHSS scores and less than 4.5 h in onset time were included into the study. According to the patients′ option to accept the intravenous thrombolytic therapy , 68 patients received intravenous thrombolysis with alteplase as the treat-ment group, 59 patients did not receive intravenous thrombolytic therapy as the control group. The incidence of intracerebral hemorrhage and mortality were observed in the two groups after thrombolytic therapy. Their nerve function defect was scored according to the NIHSS scores before and after treatment at day 7 and day 30. Their living ability was assessed by mRS in modified Rankin chart after the treatment at the day 90. Results The NIHSS scores were significant lower in the 2 groups after therapy (P < 0.05). The NIHSS scores at each time point and mRS scores in the treatment group were lower than the control group (P < 0.05). No significant differ-ence was found in the incidence of intracerebral hemorrhage and mortality between the 2 groups , although these were higher in the treatment group than in control group (1.5% vs 0%, 7.5% vs 5.1%, P > 0.05). Conclusion Intravenous rt-PA thrombolytic therapy is effective and safe for AIS whose NIHSS scores are more than 15 points and onset time are less than 4.5 h.

Purpose　The source of earthworm toxin, its ingredients and the methods to take it off were studied.Methods　The irritable secretor was dialyzed, and then examined by UV-Vis spectrum instrument.The proteins was analyzed with electrophoresis. The toxicity was tested in vivo.Results　The proteins of irritable secretor 0.242mg/kg iv cause a mouse to death.Many bands were separated from the proteins with SDS-PAGE， among which two kinds were the principal. Even with different stimulus the secretor of earthworm was identical in ingredients.Conclusion　The results demonstrated that the earthworm proteins of irritable secretion had toxicity and the toxin could be removed from earthworm by adding NaCl or 40V electricity.

Objective　To investigate the safety and efficacy of intravenous thrombolysis with recombinant tissue plasminogen activator （rt-PA） in patients with acute ischemic stroke complicated with ischemic white matter lesions.MethodsOne hundred and ninety-nine patients with acute ischemic stroke complicated with ischemic white matter lesions were studied retrospectively.Ninety-nine patients received thrombolytic therapy （thrombolytic group） and 100 patients received routine antiplatelet aggregation therapy （control group）.The NIHSS scores of patients in each group at admission，24 hours and 7 days after treatment were accessed.Modified Rankin score 3 months after treatment，adverse events （symptomatic intracranial hemorrhage，asymptomatic intracranial hemorrhage）and stroke-related deaths within 3 months were recorded.Results　Compared with the control group，the levels of NIHSS in the thrombolytic group were significantly lower at 24 and 7 days（P<0.05）.The long- term prognosis of thrombolytic group was better than that of control group at 3 months （P<0.05）.Severn cases had of intracranial hemorrhage transformation after 24 h of thrombolysis (7.1%),in contrast,no patient had that in non-thrombolytic group （P<0.05）.However,there was no significant difference in the incidence of symptomatic intracranial hemorrhage and mortality.Conclusion　Merger of ischemic cerebral white matter lesions of AIS patients with venous thrombolysis treatment prognosis was obviously better.Thrombolysis group had increased bleeding risk,which will not lead to the occurrence of adverse reactions such as symptomatic intracranial bleeding,also does not increase mortality,in patients with venous thrombolysis treatment for merger of ischemic cerebral white matter lesions AIS patients have high security.

AIM: To evaluate the safety and tolerance of ibandronate in Chinese healthy volunteers. METHODS: The trial protocol was designed according to the Good Clinical Practice(GCP). After physical examination and laboratory tests were performed, 36 healthy volunteers were divided randomly into 6 dose groups, including 1 mg , 2 mg , 3 mg , 4 mg , 5 mg and 6 mg , with 6 subjects in each group(3 male and 3 female). Clinical symptoms, vital signs, routine blood tests, routine urine tests, hepatic function, renal function, blood electrolytes, electrocardiogram, and electroencephalogram were observed or examined before and after a single intravenous infusion of ibandronate. RESULTS: After single intravenous infusion doses of 1- 6 mg , the vital signs, clinical symptoms and laboratory tests were all in the normal range, but there were some slight ADRs concerned with the drug, such as hypophosphataemia, increased body temperature, perspiring,pain of bone or muscle and hypocalcaemia. But the ADRs were found vanishing in one or two weeks. CONCLUSION: Single intravenous infusion (up to 6 mg ) of domestic ibandronate in 36 chinese healthy volunteers is safe and tolerable.

OBJECTIVETo analyze the correlation between olfactory bulb (OB) volume with depth of olfactory sulcus (OS) and olfactory function in patients with idiopathic Parkinson's disease (IPD).

METHODSOne hundred patients with IPD between January 2011 to December 2013 in Tianjin Huanhu Hospotal were compared with one hundred controls in terms of olfactory function T&T testing, OB volume and depth of OS assessed with Magnetic resonance imaging (MRI). One hundred IPD patients were investigated and estimated H-Y degrees, UPDRS, MMSE, MoCA. SPSS 13.0 software was used to analyze the data.

RESULTST&T olfactory testing revealed that IPD patients (3.0 ± 0.3) had higher scores than controls (1.3 ± 0.2, t = 2.537, P < 0.01). Both men and women with IPD were affected by the same extent of olfactory loss (t = 0.893, P > 0.05). Both men and women as controls were affected by the same extent of olfactory loss(t = 1.184, P > 0.05). OB volume of left side in IPD patients was (34.25 ± 5.14) mm(3), right side was (35.79 ± 5.28)mm(3), average OB volume was (35.28 ± 5.21) mm(3); OB volume of left side in controls was (47.38 ± 6.47) mm(3), right side was (47.75 ± 6.51) mm(3), average OB volume was (47.53 ± 6.49) mm(3); OB volume were lower in IPD patients as compared to controls (t value were 2.876, 2.747, 2.798, all P < 0.01). OS depth study revealed no statistical difference between IPD patients and controls (t value were 0.914, 0.987, 0.951, all P > 0.05). Olfactory discriminate threshold was negatively correlated with average OB volume in IPD patients and controls (r value were -0.537,-0.526, both P < 0.05); was no correlated with average depth of OS (r value were -0.142, -0.157, both P > 0.05). There was relation between the average OB volume and UPDRS III, UPDRS, the degree of H-Y in IPD patients (r value were 0.312, -0.419, -0.358, all P < 0.05). However the average OB volume was not related to the course of disease (r = -0.089, P > 0.05).

CONCLUSIONSThe OB volume is lower in IPD patients as compared to controls, the depth of OS have no significant changes in IPD patients; The OB volume is correlated with olfactory function, the depth of OS is not correlated with olfactory function; Olfactory function lower degree is accordance with serious degree in IPD patients, is not accordance with the course of disease.

Objective: To evaluate the occupational health risk of decorative coating manufacturing enterprises and to explore the applicability of occupational hazard risk index model in the health risk assessment, so as to provide basis for the health management of enterprises. Methods: A decorative coating manufacturing enterprise in Hebei Province was chosen as research object, following the types of occupational hazards and contact patterns, the occupational hazard risk index model was used to evaluate occupational health risk factors of occupational hazards in the key positions of the decorative coating manufacturing enterprise, and measured with workplace test results and occupational health examination. Results: The positions of oily painters, water-borne painters, filling workers and packers who contacted noise were moderate harm. And positions of color workers who contacted chromic acid salts, oily painters who contacted butyl acetate were mild harm. Other positions were harmless. The abnormal rate of contacting noise in physical examination results was 6.25%, and the abnormality was not checked by other risk factors. Conclusion The occupational hazard risk index model can be used in the occupational health risk assessment of decorative coating manufacturing enterprises, and noise was the key harzard among occupational harzards in this enterprise.

Objective To study the application of walking combined with Buerger exercise in the treatment of grade 0 diabetic foot.Methods Eighty patients with grade 0 diabetic foot were randomly divided into observation group and control group.The control group was trained by walking exercise .The observation group was trained by walking combined with Buerger exercise through 12 months of training.The changes of self-sensory symptoms,ABI and DAWV were observed.Results The self-sensory symptoms of the two groups were significantly decreased (observation group: markedly effective 15 cases,effective 19 cases;control group: markedly effective 5 cases,effective 23 cases).The effective rate of the observation group was higher than that of the control group (85% vs.70%,χ2 =7.831,P <0.05).The ABI ratio of the two groups were higher than those before treatment ,and the ABI ratio of the observation group was higher than that of the control group [(0.889 ±0.113) vs.(0.842 ±0.124),t =1.772 P <0.05].The ratio of DAWV in the observation group was higher than that in the control group [(6.772 ±0.435)cm/s vs.(6.543 ±0.552)cm/s,t =2.061,P <0.05].Conclusion Walking combined with Buerger exercise can improve the blood flow velocity and blood flow of the collateral circulation of the lower limbs of patients with grade 0 diabetic foot,and improve the self-sensory symptoms of peripheral neuropathy .

BACKGROUND: With social development, the average life span is prolonged. The issue of aging has increasingly attracted people's attention. It is necessary to promote the health status and the guality of life of the elderly..OBJECTIVE: To understand the status and distribution features of several common diseases which influence the quality of life of elderly.DESIGN: Random cluster sampling methods and a cross-section survey.SETTING: Beijing Institute of Geriatrics of Beijing Hospital.PARTICIPANTS:Totally 1 558 old people aged ≥60 years living in 10 communities affiliated to Jianguomen district office of the Eastern District of Beijing City,and 8 villages affiliated to Xiangyang and Henan Villagers'Commissions of Shunyi County of Beijing from May 1997 and October 1997 were recruited.In fact,1 434 persons responded,with a response rate being 92％,319 males and 498 females came from the city;242 males and 375 females came from the villages.METHODS:Questionnaires were filled up,and physical examinations were conducted in all the elderly. MAIN OUTCOME MEASURES: The 6 common diseases were observed among the elderly: Hyperplasia of prostate, abnormal audition, cartaract ,osteoarthritis, fracture and constipation.RESULTS:Totally 1434 elderly entered the stage of result analysis.①The incidence of hyperplasia of prostate in the males from the urban areas was lower than that in the rural areas (61.4％,65.7％, P ＜ 0.01 ). ② The incidence of abnormal audition among the elderly from the urban areas was lower than that in the rural areas (53.9％,64.7％,P ＜ 0.01).③ The incidence of cataract of the two eyes in the elderly from the urban areas was higher than that in the rural areas(46.4％,44.4％, P ＜ 0.01). ④ The incidence of osteoarthropathy in the elderly from the urban areas was higher than that in the rural areas (24.4％,14.9, P ＜ 0.01). ⑤ The incidence of fracture in the elderly from the urban areas was higher than that in the rural areas (14.2％,9.1％, P ＜ 0.05). ⑥ The incidence of constipation of the elderly in urban areas was lower than that in the rural areas(18.2％,23.0％,P ＜ 0.05).CONCLUSION:There is a difference between the elderly from the urban areas and the rural areas in hyperplasis of prostate,abnormal audition, cataract, osteoarthropathy, fracture and constipation, and this is related with the economy, nutrition, medical conditions and cultural traits.Therefore, attention should be attached to nutrition and health knowledge of the elderly and prevention and treatment of common diseases among the elderly people.

Objective To investigate the application of resistive exercise combined with Buerger exercise in lower extremity vascular disease of type 2 diabetes mellitus .Methods According to the order of admission ,120 type 2 diabetes mellitus patients with lower extremity vascular disease were randomly divided into resistance exercise group ,Buerger exercise group and combined exercise group ,with 40 cases in each group .The resistance exercise group was given resistance exercise ,the Buerger exercise group was given Buerger exercise ,and the combined exercise group was given resistance exercise combined with Buerger movement .The changes of self -inductance ,ABI and DAWV of the three groups were observed .Results The clinical symptoms of the three groups were significantly reduced ( the resistance exercise group:markedly effective in 10 cases,effective in 14 cases;the Burger exercise group:markedly effective in 15 cases,effective in 17 cases;the combined exercise group: markedly effective in 22 cases,effective in 13 cases),the effective rate of the combined exercise group was higher than that of the resistance exercise group and Buerger exercise group ( the resistance exercise group:60.0％;the Burger exercise group:80.0％;the combined exercise group:87.5％,χ2 =11.333,P <0.05).The ABI of the three groups were higher than those before treatment,and the ABI ratio of the combined exercise group [(0.933 ±0.113)]was higher than that of the resistance exercise group[(0.866 ±0.112)] and the Buerger exercise group[(0.888 ±0.111)](t=1.796,2.663,all P<0.05 ) .The DAWV of the three groups increased compared with those before treatment , and the DAWV of the combined exercise group[(7.023 ±0.535)cm/s]was higher than that of the resistance exercise group [(6.633 ± 0.552)cm/s] and the Buerger exercise group [(6.772 ±0.435) cm/s] (t =2.302,3.209,all P <0.05). Conclusion Resistant exercise combined with Buerger exercise is helpful to improve the blood flow velocity and blood flow in the lower extremity vessel of patients with diabetic lower limb vascular disease , and can improve the self-sensitization symptoms of peripheral neuropathy in patients .
