1.The expression of vascular endothelial growth factor and clinical significance in breast cancer

Objective To study the expression of vascular endothelial growth factor(VEGF) in breast cancer and its clinical significance.Methods The expression of the protein was determined by immunohistochemistry in 60 cases of breast cancer and 20 cases of benign breast diseases.The relationship of VEGF expression with clinicopathological characteristics was statistically analyzed.Results Of all the 60 cases of breast cancers,49 cases showed positive VEGF expression,while only 4 cases expressed VEGF in the group of breast adenofibroma.There was significant difference between breast cancer and breast adenofibroma( P < 0.05 ).The overexpression of VEGF was related with the tumor size ( P < 0.05 ),histological grade ( P < 0.05 ),lymph node state ( P < 0.05 ) and TNM stage ( P < 0.05 ),but was irrelevant to age,ER and PR( P > 0.05).Conclusion VEGF might have important significance in the development,metastasis and prognosis of human breast cancer.It may be taken as an independent marker to predict the prognosis of breast cancer.
2.Intra-articular injection of autologous adipose-derived stem cells for knee osteoarthritis: a randomized controlled trial

Objective To evaluate the clinical effects of intra-articular injection of autologous adipose derived stem cells (ADSCs) or ADSCs combined with hyaluronate acid (HA) for knee osteoarthritis.Methods From May 2013 to May 2015,a total of 108 patients with knee osteoarthritis (Kellgren-Lawrence grades:1-3) were recruited in the present study.The patients were randomly divided into three groups:ADSCs,HA and ADSCs+HA.All patients (36 cases in each group) were injected with the drug in the joint cavity once a week for three weeks.The methods used for evaluating the clinical manifestations and joint damage on MRI included visual analogue scale (VAS),Western Ontario and McMaster University (WOMAC) osteoarthritis index and whole-organ magnetic resonance imaging score (WORMS).Evaluations were conducted before injection and at 3,6,12,24 and 36 months after injection.Results All patients were followed up for 36 months without any dissociation.No adverse reaction was observed during the treatment and at the follow-up duration.The VAS score and WOMAC total score of the ADSCs group and the ADSCs+HA group were better than those in the HA group at each time point after injection (P<0.05).The average VAS of the ADSCs group decreased from 4.14±1.42 at pre-injection to 2.39±1.74 at 36 months after injection.The WOMAC total score decreased from 42.86± 31.24 to 27.17±27.99.The average VAS in the ADSCs+HA group decreased from 4.25±1.13 to 2.31±1.74,and the WOMAC total score decreased from 34.92±22.62 to 21.33±21.38.However,the average VAS and WOMAC total score of the HA group at 36 months after injection were higher than those before the injection.In terms of the VAS at 3 months after injection,the ADSCs+HA group scored better than that of the ADSCs group (P<0.05).There was no significant difference in WOMAC scores between the ADSCs and ADSCs+HA groups at each time point after injection (P>0.05).The WORMS cartilage injury score improved in 10 patients with ADSCs after injection (P<0.05).The subchondral bone wear score improved as well (P<0.05).The difference of WORMS cartilage injury scores before and after injection was correlated with the difference of the WOMAC total score (r=0.790,P=0.007) and that of VAS score difference (r=0.800,P=0.005).Conclusion Autologous ADSCs and ADSCs combined with HA intra-articular injections can effectively relieve pain and improve function of patients with knee osteoarthritis for 36 months.In addition,ADSCs combined with HA injections can relieve pain more effectively within a short duration.Cartilage repair is associated with joint function improvement.

Objective TodiscusstheevaluationeffectivenessofADCofMR DWIinneoadjuvantchemotherapy (NAC).Methods ThirtyGninepatientswithlocallyadvancedbreastcancerwereenrolledinthisstudy.Allthesepatientswerediagnosedbypuncture biopsyandtreatedwithNAC.DWIwasperformedbeforechemotherapyandafter4cyclesofchemotherapyrespectively.Radicalresectionof breastcancerwasperformedwithinoneweekaftertheendof4cyclesofNAC.Accordingtotheclinicalefficacyorpathologicalresponse,the changesoftumorvolumeandtumorcelldensitybeforeandafterchemotherapyweremeasured.Theresponseoftumorwasdividedas clinicallyeffective,completeremission (CR)+partialremission(PR)andclinicalineffectiveness,stabilizationdisease(SD)+progression disease(PD)ormajorhistologicalresponse (MHR)andnonGmajorhistologicalresponse (NMHR),respectively.Toevaluatethe practicalutilityofneoadjuvantchemotherapy,theADCvaluesweremeasuredinallgroupsandanalyzedstatistically.Results Before NAC,therewasnosignificantdifferenceinADCvaluebetweenCR+PR (0.96±0.22)andSD+PD (0.93±0.14)orMHR (1.05±0.22), NMHR (0.99±0.14).TheratiosofCR+PRand MHR were56.4%and66.7%respectivelyattheendoftreatment,andtheADC valuesinallpatientswerehigherthanthatbeforechemotherapy.However,Therewasnosignificantdifferencebeforeandafterchemotherapy intheSD+PD (1.02±0.19)andNMHR (1.08±0.20)groups (P>0.05),whileCR+PR (1.47±0.16)and MHR (1.62+0.13) groupsweresignificantlydifferentbeforeandafterchemotherapy(P<0.05).Therateoftumorvolumechangewaspositivelycorrelated withΔADC (r=0.539,P<0.05).Conclusion TheADCvalue canbeusedtoevaluatethevolumeandpathologicalgradeof tumorafterNACbasedon MRIplainscananddynamicscan, whichishelpfulfortimelyandeffectivepredictiveevaluationof chemotherapyeffect.ADCvaluecanbeusedasearlyevaluationofNACforbreastcancerandprognosticindicators.
4.The risk factors of low anterior resection syndrome and its influence on psychological status and quality of life in rectal cancer patients

Objective:To investigate the effects of low anterior resection syndrome (LARS) on psychological and physical function and quality of life in patients with rectal cancer.Methods:From May 2014 to May 2019, 200 patients were included. LARS scale score was adopted, and the clinical and pathological data were collected. Univariate analysis and multivariate Logistic regression analysis were performed. the European Organization for Research and Treatment of Cancer Quality of Life core questionnaire and psychological distress management screening tool survey were conducted to evaluate the quality of life and psychological state. The incidence of postoperative sexual dysfunction in male patients was analyzed.Results:The incidence of LARS was 43.0%. Multivariate analysis showed that body mass index ≥24 kg/m 2, anastomotic leakage, anastomotic distance ≤5 cm from anal margin, and preoperative radiotherapy were independent risk factors for LARS ( OR=2.123, 15.109, 7.302, 12.682, all P<0.05).The overall health level and the scores of physical function and emotional function in the functional dimension of patients in the severe LARS group were significantly lower than those in the no/mild LARS group ( t=5.788, 8.831, 8.745, all P<0.05). The scores of fatigue and diarrhea were significantly higher than those in the no/mild LARS group ( t=26.280, 49.476, all P<0.05). The psychological distress thermometer score and the scores of communication , emotional and physical problems in the severe LARS group were significantly higher than those in the no/mild LARS group ( t=4.246, 6.563, 5.913, 4.408, all P<0.05). Conclusion:LARS is a common complication after Dixon procedure for rectal cancer. Body mass index ≥24 kg/m 2, anastomotic leakage, anastomotic distance from anal margin ≤5 cm, and preoperative radiotherapy are independent risk factors for LARS.
5.Predictive value of anti-RA33,IL-6 and hs-CRP levels for treatment response in rheumatoid arthritis based on decision curve analysis

Objective To analyze the predictive value of anti-rheumatoid arthritis-33(anti-RA33),interleu-kin-6(IL-6)and high-sensitivity C-reactive protein(hs-CRP)levels for treatment response in patients with rheumatoid arthritis(RA)based on the decision curve.Methods A total of 102 RA patients admitted to the hospital from January 2021 to August 2023 were selected.The clinical data of the patients were collected and their serum anti-RA33,IL-6 and hs-CRP levels were detected.After half a year of treatment with methotrex-ate and etanercept,the patients were divided into good response group and non-response group according to the treatment response.Pearson correlation analysis was used to analyze the correlation between the levels of anti-RA33,IL-6,hs-CRP and disease activity score 28(DAS28)in RA patients.Multivariate Logistic regres-sion analysis was used to analyze the influencing factors of non-response in RA patients.Receiver operating characteristic(ROC)curve was drawn to analyze the efficacy of anti-RA33,IL-6 and hs-CRP in non-response to RA.The decision curve was used to analyze the net benefit of anti-RA33,IL-6 and hs-CRP alone or in com-bination to predict the non-response of RA patients to treatment.Results After half a year of treatment,80 patients had good or moderate response(good response group)and 22 patients had no response(non-response group).The course of disease in the good response group was shorter than that in the non-response group,and DAS28 score was lower than that in the non-response group(P<0.05).The levels of serum anti-RA33,IL-6 and hs-CRP in the good response group were lower than those in the non-response group(P<0.05).Pearson correlation analysis showed that the levels of serum anti-RA33,IL-6 and hs-CRP were negatively correlated with DAS28 score(P<0.05).Multivariate Logistic regression analysis showed that DAS28 score,anti-RA33,IL-6 and hs-CRP levels were the influencing factors of non-response to treatment in RA patients(P<0.05).ROC curves showed that the area under the curve of serum anti RA33,IL-6,hs-CRP single and combined for predicting patients without reaction were 0.729,0.814,0.831,0.948,and the combined prediction value was higher.The decision curve analysis showed that within the scope of the most reasonable threshold.The overall net benefit of serum anti-RA33,IL-6 and hs-CRP combined predicted treatment responsiveness in RA patients was higher than the net benefit predicted alone.Conclusion The levels of anti-RA33,IL-6 and hs-CRP are closely related to the treatment response of RA patients.The combination of the three has high clinical value and net benefit in predicting no response to treatment.
6.Analysis of the factors influencing the onset of acute PACG after COVID-19 outbreak

Objective:To analyze the proportion of patients with acute primary angle-closure glaucoma (PACG) after the outbreak of coronavirus disease 2019 (COVID-19) in patients undergoing glaucoma surgery and the factors influencing morbidity.Methods:A cross-sectional study was performed.The cluster sampling method was used to collect 141 glaucoma patients hospitalized for glaucoma surgery after the outbreak of COVID-19 in Xuzhou First People's Hospital from December 16, 2022, to January 16, 2023, and 231 glaucoma patients hospitalized for surgery in the same 1-month period from 2019 to 2020, 2020 to 2021, 2021 to 2022 through the hospital information system.Ninety-two eyes of 92 patients with acute PACG after the outbreak were selected as a study group, and 21 eyes of 21 patients with acute PACG hospitalized during the same 1-month period from 2021 to 2022 were selected as a control group.The proportion of patients with different types of glaucoma during the observation period was analyzed.The proportion of patients with acute PACG and the clinical characteristics of acute PACG were analyzed, including age, sex, visual acuity, intraocular pressure (IOP), and anterior chamber angle status.Epidemiological data such as the use of anti-cold medications, and changes in living habits and moods (including daily water intake and anxiety) of patients after COVID-19 infection were obtained by telephone follow-up.Anxiety levels of patients in the study group were assessed using the Hospital Depression and Anxiety Scale (HADS).The triggering factors of acute PACG attack after the COVID-19 outbreak were analyzed.This study adhered to the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of Xuzhou First People's Hospital (No.xyyll[2023]114).Written informed consent was obtained from each subject.Results:The proportion of acute PACG patients in Xuzhou First People's Hospital within one month after the outbreak in 2022 to 2023 increased significantly compared with the same period in 2021 to 2022, 2020 to 2021 and 2019 to 2020, and the differences were statistically significant ( χ2=31.066, 33.331, 20.804; all at P<0.001).There was no statistical significance in the distribution of the number of eyes with different grades of visual acuity, IOP, the number of eyes with IOP ≥30 mmHg (1 mmHg=0.133 kPa), and the distribution of the number of eyes with different anterior chamber angles between the two groups (all at P>0.05).Patients in the control group had no history of COVID-19 infection or use of cold medicines at disease onset.During disease onset, all 92 (100%) patients in the study group had positive COVID-19 nucleic acid test results, of which 57 (61.96%) patients had a history of oral anti-cold medication use.During the same period, 49 (100%) hospitalized patients with other types of glaucoma had positive COVID-19 nucleic acid test results, of which 20 (40.82%) patients had a history of oral cold medication.There was a statistically significant difference in the proportion of patients taking oral cold drugs between acute PACG and other types of glaucoma in the study group ( χ2=5.764, P=0.016).During the outbreak of COVID-19, the study group reported that the daily water intake had increased to varying degrees than before.In the study group, 76 patients had anxiety, accounting for 82.6%.Multiple linear regression analysis showed that IOP=19.052+ 0.009×daily water intake+ 0.858×HADS score ( R2=0.780), and the standardized coefficients of daily water intake and HADS score were 0.542 and 0.452, respectively.Daily water intake had a greater effect on IOP than HADS score. Conclusions:The proportion of acute PACG patients among hospitalized surgical glaucoma patients increased significantly after the COVID-19 outbreak, which is related to risk factors such as oral anti-cold medications containing vasoconstrictors or antihistamines, increased daily water intake, anxiety and other lifestyle, and mood changes.
