1.The Clinical Application of Esophageal Stent of the Curved Type

Objective:Since 1996,the esophageal stent of the curved type has been applied clinically by authors in order to solve the defects which the straight tube like stent could not coincide with the shape of the esophageal of the patient.Methods:The curved type stent made of "Z" type stainless steel was implanted after fixing it direction along the selected esophagus.Results:The application of the esophageal stent of the curved type in 29 cases indicated that the stents were coincidence with the folded esophagus,could decreases the pain and discomforts of the patients,and improved the stability of the stents.Conclusion:The esophageal stent of the curved type applied to the folded esophagus is much more effective clinically than the straight tube like type used before. [
2.THE NUTRITIONAL VALUE AND THE EDIBLE SAFETY OF FASTFOOD EUCHEUMA

Objective:The nutritional value and edible safety were evaluated for fastfood Eucheuma. Methods:The nutritional composition and the contents of heavy metal element and microbiology index were tested, and the test of acute toxicity and mutation were performed to evaluate the nutritional value and edible safety. Results:The main components of fastfood Eucheuma were dietary fibre and mineral components, and the contents of protein and fat were very low. The gross content of dietary fibre was 88.6%. The contents of Zn and Ca were the highest among the mineral components.The index of heavy metal elements and microbes(cfu) were lower than that of the national standard. It was non-toxic and non-mutagenic. Conclusion:The fastfood Eucheuma is a functional food, and also a safe food of new resource.
3.Adsorption effect of dietary fibers from seaweeds on estrogens

Objective Adsorption effect of dietary fibers from seaweeds on extrinsic estrogens was studied.Methods The methods included the tests in vivo and in vitro which simulated environments of stomach and small intestine of rat. Results The adsorption effect of four kinds of dietary fibers on estradiol in simulated environments of small intestine was better than that in simulated environments of stomach. The effect of Laminaria japonica was the best. And the rate of adsorption on estradiol was 108 g?g~ -1.It is 1.5,1.8, 3.1, and 3.0 times that of Sargassum,wheat blan,Gracilaria and Eucheuma,respectively. The rates of adsorption on estradiol of four kinds of dietary fibers from Gracilaria, Eucheuma, Sargassum and laminaria japonica was 11.1%,13.6%,27.5% and 38.7%, respectively. Adsorption rate of Sargassum and laminaria japonica was 1.9 and 1.3 times than that of wheat bran, respectiveiy which was 20.4%. While for Gracilaria and Eucheuma, adsorption rate was lower than that of wheat bran. Conclusion The dietary fibers from four seaweeds showed adsorption effect on extrinsic estrogens. The dietary fibers from brown seaweeds (Sargassum and laminaria japonica) had more adsorption effect than that of red seaweeds (Gracilaria and Eucheuma).
4.Observation of serum oxidative and antioxidative indices in patients with Graves′ disease

The changes of serum malondialdehyde, myeloperoxidase, total antioxidant capacity, superoxide dismutase activity, and glutathione peroxidase were observed in patients with Graves′ disease. The myeloperoxidase level increased in patients with Graves′ disease. The balance between oxidation and antioxidative defense was disrupted in patients with newly-onset Graves′ disease. Oxidative stress seems to be related to hyperthyroidism.

6.Current status of job burnout among anesthetists in Ningxia

Objective To investigate the current status of job burnout among anesthetists in Ningxia and to analyze its related risk factors.Methods A total of 310 anesthetists from 30 public hospitals in Ningxia were conducted to collect data on the job burnout by Maslach Burnout InventoryHuman Services Survey (MBI-HSS).The factors associated with severe job burnout and its three domains (high emotional exhaustion,high depersonalization and low personal accomplishment) were included in the multinomial logistic regression analysis.Results The incidence of job burnout and its three domains (high emotional exhaustion,depersonalization and low personal accomplishment) were 18 (5.81%) cases,134 (43.22%) cases,35 (11.29%) cases and 128 (41.29%) cases,respectively.Multinomial logistic regression analysis results indicated that the risk degree associated with job burnout was master and higher degree (OR=4.695,95%CI 1.556-4.172).The risk facts associated with three components of job burnout were work time per week [40-60 h (OR=4.420,CI 2.504-7.802);≥60 h (OR =7.469,95%CI 1.758-31.733)] and tertiary hospital (OR =1.847,95%oCI 1.112-3.069);master and higher degree (OR=2.306,95%CI 1.032-5.155),working years [6-15 years (OR=0.358,95%CI 0.135-0.949)] and cases of anesthesia per year [≥390 cases (OR =3.352,95%CI 1.301-8.639)];secondary hospital (OR =1.717,95%CI 1.045-2.823).Conclusion This survey indicates that job burnout exists among anesthetists in Ningxia and is mainly displayed in emotional exhaustion and low personal accomplishment domains.Master and higher degree tend to occur serve job burnout.
7.Identification and characterization of a taxol-producing endophytic fungus from Taxus media.

To enrich the resource pool of endophytic fungi from plants which produce taxol, a taxol-producing endophytic fungus TMS-26 was isolated from the stem of Taxus Media. The result of high performance liquid chromatography (HPLC) showed that TMS-26 extract exhibited similar chromatographic peaks and retention time (4.545 min) with authentic taxol. Then mass spectrometry (MS) analysis further confirmed that TMS-26 extracts contained the same mass peaks with authentic taxol ((M+Na)+=876). These indicated that the isolated endophytic fungus TMS-26 can produce taxol. According to the morphological characteristics, the molecular analysis of 18S rDNA and internal transcribed spacer nuclear rDNA gene sequence, the fungus was identified as Aspergillus fumigatus TMS-26.
8.Robotic lung resection for malignant and benign lesions: experience with 1 000 patients

Objective To analyze the perioperative outcome of consecutive 1 000 patients undergoing robotic lung resection and summarize surgical experience. Methods We retrospectively reviewed the clinical data of 1 000 patients undergoing robotic lung resection between May 2009 and June 2018 in Shanghai Lung Tumor Clinical Medical Center. Robotic lobectomy was compared with traditional VATS over the same period using a propensity-matched analysis. There were 327 males and 673 females at average age of 56.21±11.33 years. Lobectomy was performed in 866 patients (11 bilobectomy included), sublobar resection was performed in 129 patients, sleeve lobectomy was performed in the remaining 5 patients. Pathology was as follows: adenocarcinoma in 875 patients, squamous carcinoma in 52 patients, benign tumors in 73 patients. 90.5% of the primary lung cancer were in stage Ⅰ. Results The mean operative time was 90.31±19.70 min; 95.70% of patients’ estimated blood loss was less than 100 ml. Conversion rate to thoracotomy was 0.90% (9 patients) . The average lymph node station and count harvested was 5.59±1.36 and 9.60±3.21 respectively. The mean volume of chest tube drainage on the first postoperative day was 229.19±131.67 ml. Median chest tube time was 3.85±1.43 d. There was 1 in-hospital death due to pulmonary embolism. A total of 189 patients had postoperative complications (18.90%) whose majority was postoperative air leak more than 5 days. The mean overall hospital costs was 92 710.53±12 367.23 Yuan. Compared with VATS, RATS was associated with significant reduction in intraoperative blood loss, time to chest tube removal and postoperative hospital stay. The operative time, conversion rate, lymph nodes removed, morbidity and mortality were similar between the two groups. Conclusion Robotic-assisted lung resection is safe and effective with low conversion rate and less complications, and it can overcome many disadvantages of traditional VATS.
9.Perioperative outcomes of robot-assisted versus video-assisted right upper lobectomy in non-small cell lung cancer: A retrospective cohort study

Objective To summarize the perioperative outcome of patients undergoing robot-assisted thoracic surgery (RATS) or four-port single-direction video-assisted thoracic surgery (VATS) right upper lobectomy (RUL), and to discuss the safety and the essentials of the surgery. Methods The clinical data of 579 patients with non-small cell lung cancer (NSCLC) undergoing minimally invasive RUL in Dr. Luo Qingquan’s team of our center from 2015 to 2018 were retrospectively analyzed. There were 246 males and 333 females aged 33-78 years. The 579 patients were divided into a RATS group (n=283) and a VATS group (n=296) according to surgical methods. Baseline characteristics and perioperative outcomes including dissected lymph nodes, postoperative duration of drainage, postoperative hospital stay, postoperative complications and surgery cost were compared between the two groups. Results There was no significant difference in baseline data between the two groups (P>0.05), and no postoperative 30 d mortality or intraoperative blood transfusion was observed. Compared with VATS, RATS had shorter operation time (90.22±12.16 min vs. 92.68±12.26 min, P=0.016), postoperative hospital stay (4.67±1.43 d vs. 5.31±1.59 d, P<0.001) and time of drainage (3.55±1.38 d vs. 4.16±1.58 d, P<0.001). No significant difference was observed between the two groups in the lymph nodes dissection, blood loss volume, conversion rate or complications. The cost of RATS was much higher than that of VATS (93 275.46±13 276.69 yuan vs. 67 082.58±12 978.17 yuan, P<0.001). Conclusion The safety and effectiveness of robot-assisted and video-assisted RUL are satisfactory, and they have similar perioperative outcomes. However, RATS costs relatively shorter operation time and postoperative hospital stay.
10.Right upper lobe resection assisted by Toumai® endoscopic surgery robot in two patients

Objective To analyze the efficacy and safety of Toumai® endoscopic surgery robot in right upper lobe resection. Methods The clinical data of 2 patients with non-small cell lung cancer who received right upper lobe resection with Toumai® endoscopic surgery robot in Shanghai Chest Hospital, Shanghai Jiao Tong University in November 2021 were retrospectively analyzed. Both patients were male, aged 66 years and 44 years, respectively. Results The operation was successful in both patients with no conversion to thoracotomy, surgical complication or death. The operation time was 65 min and 48 min, and the amount of intraoperative bleeding was 80 mL and 50 mL, respectively. The postoperative hospital stay was 3 days. There was no blood transfusion during the perioperative period. Conclusion The application of Toumai® endoscopic surgery robot in lobectomy is preliminarily proved to be safe and effective. Compared with Da Vinci robotic surgery system, it has similar clear 3D vision and flexible and stable operation, which can become one of the important choices for the new generation of minimally invasive chest surgery.
