Objective To retrospectively investigate the endoscopic characteristics and pathological features of colorectal polyps in over-aged patients (≥80 years). Methods The 1617 colonoscopies performed between January 2006 and December 2010 were enrolled in our retrospective analysis. The detection rate, size, location, form and pathological feature of polyps in 150 over-aged patients were investigated, and they were compared with those in 832 young patients (＜60 years) and 635 old patients (60-79 years) with colorectal polyps. Results The detection rate of polyps was 62.0% in over-aged group, and was significantly higher than in young group and old group (30.2% and 48.7%, respectively, χ2=56.58 and 8.64,both P＜0.001). The malignant transformation rate of ascending colon polyp was 5.4% in over-aged group, and was significantly higher than in young group (1.2%, χ2=4.90, P＜0.05), but there was no significant difference between over-aged group and old group (3.9%, χ2=0.36, P＞0.05). There were no statistical differences in canceration rate, polyp diameter, morphology and adenomatous polyp rate among the three groups. The malignant transformation rate was significantly increased in adenomatous polyps with diameter over 2 cm, but didn't reach statistical significance. Conclusions The polyp detection rate is noticeably higher in the over-aged than in the old and the middle-aged, and the malignant transformation probability is also increased. The colonoscopy indications in the high risk age groups should appropriately be broadened, they should receive regular intervals follow-up, and undergo polypectomy in time if necessary.

Objective To explore the clinical efficacy of trastuzumab combined with vinorelbine therapy in the treatment of advanced breast cancer with human epithelial growth factor receptor-2 (HER-2) positive.Methods From April 2010 to April 2013,91 advanced breast cancer patients who failed in the paclitaxel therapy received trastuzumab plus vinorelbine (45 cases,vinorelbine group) or capecitabine (46 cases,capecitabine group).The treatment efficacy,toxic effects and 3-year survival rate in two groups were compared.Results The clinical benefit rate in two groups had no significant difference (P ＞ 0.05),but objective response rates in vinorelbine group was higher than that in capecitabine group:44.44％(20/45) vs.23.91％(11/46),and there was significant difference (P =0.039).The toxic effects in two groups had no significant difference (P ＞ 0.05).The 3-year survival rate in two groups had no significant difference (P =0.252).Conclusions In the treatment of HER-2 positive advanced breast cancer,trastuzumab plus vinorelbine or capecitabine shows no significant differences in adverse reaction or in 3-year survival rate.However,trastuzumab plus vinorelbine shows better objective response rate compared with trastuzumab plus capecitabine.

A monoclonal antibody 1C34-5 raised against human peripheral mononuclear cells was produced. This antibody, a monoclonal IgG1. showed positive reaction with about 90% of T and B lymphocytes, monocytes. granulocytes and bone marrow cells, but negative reaction with red blood cells and platelets by indirect immunoflurorescent technique. 1C34-5 also reacted with leucocytic cell lines except a non-T non-B lymphoid line Reh but not with erythroid line K562, HeLa cells and fibroblast cells so far tested.This antibody was also assayed histologically by an indirect immunoperoxidase technique against a variety of human normal tissue frozen sections. It was found that 1C34-5 bound to all lymphoid tissues including lymph nodes, tonsil, thymus. Peyer's patches and leucocytes scattered in other tissues, but not to all non-hematopoietic tissues as well as erythropoietic foci in fetal liver. Thus. 1C34-5 appears to recognize a human leucocyte antigen specifically.

Objective To evaluate the accuracy and utility of percutaneous ultrasonic cholangiography in Bismuth staging of hilar cholangiocarcinoma. Methods Thirty patients who underwent surgery and obtained pathologic diagnosis of hilar cholangiocarcincoma were perspectively ruled in this study. All patients with hilar obstruction underwent baseline ultrasound (BUS), percutaneous ultrasonic cholangiography (PUSC) and percutaneous transhepatic cholangiography(PTC) respectively. Taking operative findings as reference standard,the accuracy of the three imaging modalities in staging of hilar cholangiocarcinoma was compared. Results Among 30 patients, the accuracy of classification among BUS, PUSC and PTC was 23.3 % (7/30), 73.3 % ( 22/30 ), 73.3 % (22/30), respectively. There was statistically significant difference between BUS and PUSC, but the difference between PUSC and PTC was not statistically significant.Conclusions As a new technique for cholangiography, PUSC expands the application of ultrasound in evaluating hilar cholangiocarcinoma and is comparable to PTC in classifying hilar cholangiocarcinoma by Bismuth classification.

Objective To investigate the survival prognostic influencing factors of patients with hepatocellular carcinoma (HCC) after radical resection. Methods Clinical data of 1 102 patients with HCC undergoing radical resection in Sun Yat-sen University Cancer Center from January 2006 to June 2012 were analyzed retrospectively. There were 872 males and 230 females with a mean age of (55±15) years old. The informed consents of all patients were obtained and local ethical committee approval had been received. All the patients were followed up after operation. The cumulative survival rate was estimated using Kaplan-Meier method according to the follow-up results. Clinical indexes such as age, gender, history of viral hepatitis B, family history of HCC, serum alpha fetoprotein (AFP), serum alkaline phosphatase (ALP), Child-Pugh liver function grading, tumor diameter, portal vein tumor thrombus, tumor differentiation, vessel invasion, lymph node metastasis, distant metastasis, tumor node metastasis (TNM) staging were enrolled as prognostic impact factors and were analyzed using Log-rank test and Cox proportional hazards model. Results The survival time was 4-114 months with a median of 70 months for the patients with HCC after radical resection. The 1-, 3-, 5-year cumulative survival rates were 88.9%, 73.2%, 52.3%. Tumor diameter>4 cm, positive serum AFP, poor tumor differentiation were the independent risk factors for the prognosis of patients with HCC after radical resection (RR=1.951, 3.498, 3.781;P<0.05). Conclusion Tumor diameter>4 cm, positive serum AFP, poor tumor differentiation are the independent risk factors inlfuencing the prognosis of patients with HCC after radical resection.

Objective: To evaluate the application effect of the platform switching implants in maxillary anterior region, to explore the effect of platform switching technology on the surrounding tissues. Methods: 55 patients with 60 single maxillary anterior implants were divided into two groups: platform-switching implants group (Ankylos), 25 patients with 28 implants; butt-joint implants group (Nobel Replace), 30 patients with 32 implants. The patients received follow-up care more than 1 and 2 year after the final setting of the prosthesis, at which time periapical radiographs were taken. The marginal bone level around the implant and Pink Esthetic Score (PES) were measured for comparison. Results : The average marginal bone changes of platform-switching implants after 1 year and 2 year were (-0.41 ± 0.36) mm and (-0.55 ± 0.33) mm respectively; and the ones of butt-joint implants were (-1.77 ± 0.54) mm and (-1.82 ± 0.61) mm. The average PES of platform- switched implants after 1 year and 2 year were 10.43 ± 1.37 and 10.32 ± 1.21 respectively; the ones of butt-joint implants were 9.21 ± 0.97 and 9.16 ± 0.95. There were significantly differences of marginal bone changes and PES between both groups (P < 0.05). Conclusion Platform switching implant in the maxillary aesthetics area is more effective in preserving the surrounding bone tissue and aesthetic effect.

Objective: To explore the value of contrast-enhanced ultrasound in the biopsy of patients with breast imaging reporting and data system (BI-RADS) score of 4 lesions. Methods: Total of 136 cases of breast BI-RADS scores of 4 lesions admitted to Dongguan People′s Hospital from June 2017 to May 2018 were enrolled and randomly divided into contrast-enhanced ultrasound group and general ultrasound group, 68 cases in both group. The contrast-enhanced ultrasound group received contrast-enhanced ultrasound-guided breast nodule biopsy, and the general ultrasound group received general ultrasound-guided breast nodule biopsy. The success rate of puncture, the number of puncture needles, the satisfaction rate of the materials, the diagnostic sensitivity, the diagnostic specificity, the diagnostic coincidence rate, and the incidence of adverse puncture reactions were compared between two groups. Results: The puncture success rate of both groups were 100%. The number of puncture needles in contrast-enhanced ultrasound group was (2.89±0.56)times, and the general ultrasound group was (2.93±0.62)times, with no statistically significant difference (P>0.05). The satisfaction rate of materials in the contrast-enhanced ultrasound group was 100%(68/68), and the general ultrasound group was 94.12%(64/68), with no statistically significant difference (P>0.05). The sensitivity in the contrast-enhanced ultrasound group and the general ultrasound group were 88.89%(32/36) and 81.82%(27/33), respectively, with no statistically significant difference (P>0.05). The specificity and coincidence rate in contrast-enhanced ultrasound group were 100%(32/32) and 94.12%(64/68), which were higher than 80.65%(25/31) and 81.25%(52/64) of the general ultrasound group, with statistically significant difference (P<0.05). The incidence of adverse reactions in the general ultrasound group and the contrast-enhanced ultrasound group were 10.94%(7/64) and 2.94%(2/68), respectively, with no statistically significant difference (P>0.05). Conclusions The contrast-enhanced ultrasound-guided breast nodule biopsy can achieve high puncture success rate in patients with breast cancer with BI-RADS scores 4 lesions, and help to confirm lesion activity, necrotic liquefaction area, more accurate localization, improved material satisfaction rate and biopsy, diagnostic efficacy, with significant clinical value.
