Chinese Journal of Minimally Invasive Surgery 2001;0(04):-
Objective To assess the feasibility of combined laparoscopic multi-organ surgery in clinical appliation. Methods 48 combined laparoscopic multi-organ surgeries performed from August 2000 to November 2001 were analyzed. Results All laparoscopic procedures were successfully completed. No one needed to be converted to laparotomy and had postoperative complications.Postoperative stay was similar to that of patients under laparoscopic mono-organ surgery in the same time. Conclusions Combined laparoscopic multi-organ surgery can be performed conveniently. It offers significant benefits to patients such as decreased postoperative pain, reduced trauma to the abdominal wall and cost-saving.

Objective To compare the therapeutic results of laparoscopic and open treatment for choledocholithiasis. Methods A prospective comparison was conducted between 29 cases of laparoscopic choledocholithotomy and 35 cases of open choledocholithotomy in terms of stone removal, operative time, hospitalization day, hospitalization cost and postoperative complications. Results Stone removal was achieved in 28/29 patients by a laparoscopic approach and in 32/35 patients by a open approach( ? 2=0 71, P =0 399);average operative time was (178 3?32 4) min in laparoscopic approach and (145 8?27 1)min in open approach( t =4 27, P =0 01);the patients in laparoscopic procedures had flatus in (20 8?2 9) hour and those in open procedures did in (58 2?4 9) hours( t =37 8, P 0 05);no major complication was found in laparoscopic approach. Conclusions Laparoscopic choledocholithotomy is safe and effective and it is superier to open treatment.

Objective To investigate the feasibility and techniques of laparoscopic biliary re-operations. Methods Laparoscopic surgery was performed in 13 patients with bile duct stones who already had undergone biliary tract operations. Results Laparoscopic choledochotomy was successfully completed in 12 cases, while a conversion to open surgery was required in 1 case. The success rate of laparoscopic surgery was 92.3% (12/13). The operation time was (101~300) min, with a mean of 155 min. No intra- or post- operative complications occurred. Conclusions Laparoscopic biliary re-operations are feasible and experiences are needed to ensure the success of surgery.
5.Analysis on second operation after laser in situ keratomileusis
International Eye Science 2015;(7):1293-1295
AlM:To observe clinical effects between the laser in situ keratomileusis ( LASlK ) surgery and the laser subepithelial keratomileusis ( LASEK ) surgery as the second operation after an unsuccessful LASlK surgery.METHODS:Forty-nine patients (98 eyes) with refractive regression after LASlK operation received the second surgery. All patients were divided into two groups:group A and B. Group A (48 eyes of 24 patients) received LASlK surgery and group B ( 50 eyes of 25 patients ) received LASEK surgery. lnspect the main parameters included visual acuity, refraction, corneal curvature, and the total value of high-aberration after 1wk, 1mo and 1a, t-test of groups was used as statistical analysis method.RESULTS: There was statistically significant (P<0. 05) between the two groups in visual acuity after 1wk. There was no statistically significant between the two groups in visual acuity after 1mo, and there was also no statistically significant between the two groups in visual acuity, average spherical equivalent refractive degree, average corneal curvature, and the total value of high-aberration after one year (P>0. 05). There developed one case of epithelium in growth after LASlK surgery, and one case of haze Ⅱ after LASEK surgery.CONCLUSlON: Both LASlK and LASEK surgery can obtain satisfactory therapeutic results after an unsuccessful LASlK surgery.

Objective To explore the clinical application of laparoscopic splenectomy in treatment of spleen disease at our hospital. Methods We reviewed laparoscopic splenectomy carried out at our hospital since 1995,patients were grouped by date.63 laparoscopic splenectomies were divided into six groups.Operation time, intraoperative blood loss, postoperative hospital stay, time to feeding, days of drainage, amount of drainage, postoperative complications and indications for surgery were compared. Results Patient's age averaged at 44.19 years, body mass index averaged at 23.75,3 patients were converted to open surgery.Mean operating time, blood loss, postoperative hospital stay, time to feeding, converting rate in the 53 cases which had the surgery after 2003 were much better than the 10 cases before 2003.Surgical indications for laparoscopic splenectomy were limited to hematopoietic disease related splemegaly before 2003,the indication range significantly expanded after 2003,during which laparoscopic splenectomy were mainly applied to treat spleen tumors. Conclusions There is a marked learning curve in laparoscopic splenectomy after 10 surgeries before 2003 we have achieved the level. With the suitable approach, apparatus and skillful technique, laparoscopic splenectomy is safe and feasible to treat tumors of the spleen.

Objective To establish the rep-PCR DNA fingerprinting technique for typing K. pneumoniae strains and apply it in the epidemiological investigation. Methods The 39 strains of K. pneumoniae were typed by plasmid profile analysis, then their chromosomal DNA were extracted and purified by NaI-lyses-glass-powder absorption method for rep-PCR. Results Different strains of K.pneumoniae showed different rep-PCR fingerprint patterns. Phylogenetic analysis showed that they could be classified into six types, mainly type 1, type 2, and type 3. Otherwise, plasmid profile analysis suggested that there were four types; mainly type 1 which contained only one plasmid. Conclusions (1) Rep-PCR DNA fingerprinting technique developed in this study is enough for epidemiological studies with its high typeability, strong discrimination, simplicity and rapidness. (2) There were three predominant K. pneumoniae transmitted between patients in Taihe Hospital during the last two years, and there might be serious cross-infection among different types.

Objective To study the possible relationships between expression of matrix metalloproteinase(MMP)2,9 and the pathogenesis of preeclampsia in which trophoblast invasion is impaired. Methods MMP-2,9 expression were detected by immunohistochemistry streptavidin-biotin complex (SABC)method in 20 normal term placentae and 20 preeclampsia placentae, respectively. In addition, mRNAs for MMP-2,9 were analyzed by real time PCR in both groups. Results The intensities of both MMP-2 and MMP-9 immunostaining in preeclampsia placentae were significantly declined compared to those of normal term placentae(P

Objective To test the hypothesis that homocysteine can decrease MMP-2 and MMP-9 expression in cultured trophoblasts of early pregnancy and that homocysteine can prevent trophoblasts invasion in the early stage of preeclampsia. Methods Cytotrophoblasts from early pregnancy were isolated and cultured.Trophoblasts were treated with or without Hcy(1 mmol/L)for 48 hour, and real time RT-PCR and gelatin zymography were used to quantify the mRNA and protease activity of MMP-2,-9.Results Treatment with Hcy(1 mmol/L)induced a decrease in MMP-2 mRNA by 21% and MMP-9 mRNA by 11%.At protein level MMP-2 expression decreased 14% and MMP-9 expression decreased 52% compared with control. Conclusions Homocysteine can decrease MMP-2,-9 expression in trophoblasts of early pregnancy and influence its invasion process.
