2.Clinical investigation of laparoscopic biliary re-operations

Objective To investigate the feasibility and techniques of laparoscopic biliary re-operations. Methods Laparoscopic surgery was performed in 13 patients with bile duct stones who already had undergone biliary tract operations. Results Laparoscopic choledochotomy was successfully completed in 12 cases, while a conversion to open surgery was required in 1 case. The success rate of laparoscopic surgery was 92.3% (12/13). The operation time was (101~300) min, with a mean of 155 min. No intra- or post- operative complications occurred. Conclusions Laparoscopic biliary re-operations are feasible and experiences are needed to ensure the success of surgery.
3.CT appearance of Menetrier disease (a report of 4 cases)

Objective To define the CT imaging characteristic of menetrier disease. Methods Four patients with proved Menetrier disease were retrospectively studied by means of CT performed with the water as oral contrast, hypotonic agent and dynamic or spiral CT technique. Results All cases showed that the enlarged rugae were seen projecting into the gastric lumen, but the gastric wall was otherwise normal. CT detected all lesions and made a confirmed diagnosis before pathology biopsy. Conclusion Our primary result revealed that the CT appearance of Menetrier disease was quite characteristic, CT is a very valuable tool for the diagnosis of Menetrier disease.
4.Metal-on-metal hip resurfacing in patients with arthritis secondary to congenital dysplastic hip

[Objective]To evaluate the safety and efficacy of total hip resurfacing arthroplasty in treating osteoarthritis secondary to congenital dysplastic hip.[Method]From May 2005 to June 2005,13 hips in 12 cases with osteoarthritis secondary dysplastic hip were adopted in this study.There were 8 females and 4 males,aged from 30 to 59 years,average 47 years.All the 13 hips were subluxation according to Hartofilakidis classification system.Posterolateral approach was used during operation and all the acetabular cups were reconstructed at the true acetabular location.The pronthesis were cementless acetabular cup and cement femoral head.[Result]The average operation time was 110 min,blood loss volume was 387 ml during operation,drainage volume was 200 ml and the extremity was lengthened for about 9 mm.All the incision healed well and there have not any complications such as femoral neck fracture,infection,dislocation and neurovascular injury.Follow-up for 12 to 13 months,all the joints had good or excellent clinical results.The average range of motion of the hip:flexion improved from 106.7? to 120?,abduction improved from 25.8? to 46.4?,external rotation improved from 20.8? to 45?,internal rotation improved from 3.3? to 28.6?.The Harris score increased from 42 preoperatively to 96 postoperatively.Radiographically,the positions of the prostheses were normal,the average abduction angle of the cup was 39?,the average stem-shaft angle wes 141?,the positions of acetabular rotation centre were moved down and medially for average 9 mm and 11.5 mm respectively,the acetabular superolateral bone coverage of the hips was 89.5%,no radiolucent line was observed.[Conclusion]Total hip resurfacing arthroplasty has got good short-term results in the treatment of osteoarthritis secondary to dysplastic hip.Because of the less quantity and short follow-up time,the long-term results and indications for other kinds of dysplastic hip are worthy of further studying.
5.Effects of Glutamine on Hypermetabolism Reaction and Hormone Secretion After Severe Burn Injury

Objective To study the effect of glutamine on hypermetabolism reaction and hormone secretion after severe burn injury. Methods Eighty-eight Wistar rats were inflicted with 30% total body surface area full thickness burns and randomly divided into two groups: namely burned control (B) and glutamine treatment(GLN) group. There was isonitrogenous and isocaloric intake in both groups, and rats of GLN and B groups were given glutamine and glycine at 1 0g/kg d, respectively. The following indices including the resting energy expenditure (REE), the levels of plasma cortisol, insulin and glucagon were determined. Results After burn injury, the REE, plasma cortisol,insulin and glucagons levels were significant increased in both groups. Compared with B group, the level of REE decreased and the ratio of insulin/glucagons increased in GLN group. infection (BSI) species in in a Malaysian population via Randomly Amplified Polymorphic DNA-Polymerase Chain
Reaction (RAPD-PCR) fingerprinting.
Methodology and results: The genomic DNA of 43 Candida BSI blood culture samples obtained from Universiti Malaya
Medical Centre (UMMC) was isolated, after which species identification was carried out using PCR with ITS-1 and ITS-4
pan-fungal primers in conjunction with CHROMagar™ Candida. The predominant Candida species in the BSI samples is
Candida albicans (14 out of 43 isolates). RAPD-PCR on these 14 C. albicans clinical isolates was performed using PST
as the arbitrary primer. Data analysis using MEGA found an overall non-relatedness of these 14 clinical isolates
[average similarity coefficient (SAB) value 0.733±0.172]. Following in-depth analysis, five of the 14 isolates were
observed to be identical (SAB values of 1.00 each), four isolates had SAB values of 0.80-0.99, indicating that they are
highly similar, but are non-identical, while five isolates are unrelated (SAB lower than 0.80). This suggests that
microevolution might have occurred and that these clinical isolates may possibly belong to different strains.
Conclusion, significance and impact of study: A fair degree of genetic heterogeneity was found among the 14 C.
albicans isolates from UMMC. To our knowledge, this is the first report on the genetic profiles of C. albicans bloodstream
infection isolates from Malaysia, warranting further studies in the possible evolutionary trends within this Candida
species in Malaysia.

8.Progress on treatment and research of quadrilateral plate fractures of acetabular.

Acetabular is an important human joint for weight bearing. Quadrilateral plate is a crucial structure of medial acetabulum with special morphology and important function. Quadrilateral plate fractures are common fracture in acetabulum. Quadrilateral plate fracture is hard to expose and reduction because it is in the medial of acetabulum. At the same time,the bone in the quadrilateral plate is not easy to fixed for thinning bones and adjacent to the articular cavity. The operator should know well about the anatomy and choose the suitable internal fixation. After quadrilateral plate fractures, the femur head maybe displace medially even break into pelvis. That make reduction and treatment always be a challenge. With different kinds of fractures,the efficacy of treatment is not the same. This paper intend to review the relation of anatomic features,approaches, internal fixations, key point of treatment and efficacy.

Objective To investigate the effects of the gene expressions of FAS、FASL、FADD and caspase-8 in esophageal carsinogenesis. Methods Immunohistochemical method was applied to detect the expression of FAS、FASL、FADD and caspase-8 proteins in esophageal epithelium. In situ hybridization method was applied to detect the expression of FADD and caspase-8 mRNA in esophageal epithelium. Results The positive rates of FAS, FADD and caspase-8 proteins and FADD、caspase-8 mRNA were decreased from normal epithelium to dysplasia and carcinoma tissues gradually. Objective To study the expression changes of placental insulin-like growth factor Ⅰ and Ⅱ (IGF-Ⅰ and IGF-Ⅰ) between pregnancies following the in vitro fertilization and embryo transfer (IVF-ET) and those conceived spontaneously.Methods The placenta were collected from 49 cases of pregnancies after IVF-ET(case group) and 49 cases of pregnancies who were normal (control group).The expressions of IGF-Ⅰ and IGF-Ⅱ mRNA in placenta were detected by reverse transcription-polymeras chain reaction (RT-PCR).The levels of IGF-Ⅰ and IGF-Ⅱ protein were determined by immunohistochemistry.Results The mRNA levels of the placental IGF-Ⅰ and IGF Ⅱ were 0.30 ±0.13 and 0.28 ±0.04 in the IVF-ET group,and 0.65 ±0.10 and 0.91 ±0.26 in the control group.The protein levels of the placental IGF-Ⅰ and IGF-Ⅱ were 0.26 ±0.04 and 0.29 ± 0.05 in the IVF-ET group,and 0.32 ± 0.07 and 0.34 ± 0.04 in the control group.The mRNA and protein expressions of IGF-Ⅰ and IGF-Ⅱ in human placenta were significantly decreased in IVF-ET group compared to control group (P < 0.05).The incidence rate of low birth weight infant in IVF-ET group was significantly higher than control(P > 0.05).Conclusions Expressions of placental IGF-Ⅰ and IGF-Ⅱ in the IVF-ET group did not affect fetal growth and development.
