2.Meta-analysis of therapeutic effects of computer-assisted navigation versus conventional total knee arthroplasty
Chinese Journal of Tissue Engineering Research 2014;(40):6542-6547
BACKGROUND:It is stil controversial whether computer-assisted navigation elevated the accuracy and clinical outcomes of total knee arthroplasty.
OBJECTIVE:To analyze the literatures in English and Chinese on computer-assisted navigation total knee arthroplasty, and to evaluate the clinical outcomes between computer-assisted navigation and conventional total knee arthroplasty.
METHODWe retrieved PubMed, Ovid, Elsevier, China National Knowledge Infrastructure and Digital Library for literatures concerning randomized control ed trial of clinical outcomes of computer-assisted navigation and total knee arthroplasty published from January 1st 2005 to December 31st 2013. Meta analysis was performed with RevMan 5.0 software from the Cochrane col aboration. Clinical outcomes of computer-assisted navigation total knee arthroplasty and conventional technique were compared. The heterogeneity of data was checked.
RESULTS AND CONCLUSION:A total of 16 randomized control ed papers were included in this analysis. Computer-assisted navigation group consisted of 1 322 knees, and the conventional group consisted of 1 299 knees. Compared with the conventional group, patients in the computer-assisted navigation group had a significantly lower risk of implant malalignment at more than 3° as wel as more than 2°, malalignment for both coronal femoral component and coronal tibial component of>3°, and both sagittal femoral component alignment and tibial slope at>3° malalignment. The meta-analysis did not find a significant difference in the sagittal tibial component alignment, the rates of complications, axial (rotational) alignment of the femoral component and the tibial component between both groups. These data indicated that computer-assisted navigation group had significant improvement in alignment of the limb and the component position, but long-term clinical outcomes using computer-assisted surgery in total knee arthroplasty require further investigation.
3.Efficacy and safety of tranexamic acid application in total hip arthroplasty:a meta-analysis
Chinese Journal of Tissue Engineering Research 2014;(35):5699-5706
BACKGROUND:Whether tranexamic acid can effectively and safely reduce blood loss and al ogeneic transfusion rate in total hip arthroplasty remains controversial at present. OBJECTIVE:To assess the efficacy and safety of tranexamic acid administration in total hip arthroplasty using meta-analysis. METHODS:Clinical randomized control ed trials concerning tranexamic acid application in total hip arthroplasty published from January 1969 to December 2013 were retrieved from the PubMed, Ovid, Elsevier, Cochrane library, China National Knowledge Infrastructure and Wanfang database. Intraoperative blood loss, postoperative blood loss, total blood loss, al ogeneic transfusion rate and incidence of venous thromboembolism were compared using meta-analysis between the tranexamic acid and control groups. Detection and literature references were used as supplementary data. RevMan 5.0 software provided by Cochrane col aboration was used for meta-analyses. Conclusion was obtained according to analysis. The analysis checked the heterogeneity of data. RESULTS AND CONCLUSION:After comprehensive and rigorous screening, 19 high-quality (Jadad score not less than 3) randomized control ed studies were included. Meta-analysis results demonstrated that compared with the control group, tranexamic acid decreased the total blood loss [weighted mean difference (WMD)=-341.04, 95%confidence interval (CI) (-508.10,-173.97), P<0.001], intraoperative blood loss [WMD=-63.26, 95%CI (-111.33,-15.18), P=0.01] and postoperative blood loss [WMD=-229.53, 95%CI (-338.11,-120.94), P<0.000 1], and diminished al ogeneic transfusion rate [relative risk=0.45, 95%CI (0.35, 0.57), P<0.000 1] in patients undergoing total hip arthroplasty. No significant difference in incidence of venous thromboembolism was detectable. These data suggested that tranexamic acid could reduce blood loss and transfusion rate in patients undergoing total hip arthroplasty without increasing the risk of incidence of venous thromboembolism.

Chinese Journal of Laboratory Medicine 2015;38(11):721-723
As the reference method, microscopic examination is still the most basic method of red cell morphology examination. There are several ways to improve the laboratory personnel's skill and ability on abnormal erythrocyte morphology diagnosis, including deep understanding of pathological mechanisms of abnormal red cell morphology, raising the ability of recognizing abnormal red blood cell that are of great clinical value, following and implementing rules to ensure the accuracy and repeatability of the examinations, standardizing reports of abnormal erythrocyte morphology.
5.Comparative analysis on ALP and PFNA in treatment of intertrochanteric fractures
China Medical Equipment 2015;(1):107-109,110
Objective: To compare and observe the clinical efficacy of ALP and PFNA in treatment of intertrochanteric fractures, to provide clinical evidence for such patients. Methods:The clinical data of 122 cases patients with intertrochanteric fractures admitted to our department from January, 2012 to January, 2013 was analyzed retrospectively. 72 patients treated with PFNA were regarded as the study group and 50 patients with ALP as controls group. Two groups were compared with surgical incision, blood loss, operative time, healing time, hip functional recovery and postoperative complications. Results: The incision length, blood loss, operative time, postoperative weight-bearing activity time, fracture healing time, Harris score and complications of study group were significantly better than the control group (P<0.05), with statistical significance. Conclusion: The long-term efficacy of PFNA and ALP has no difference, but PFNA has smaller wound, less bleeding, sooner fracture healing time and weight-bearing activities and better early results.

OBJECTIVETo compare the differences of clinical effects in female patients with obesity and hyperlipidemia of yang deficiency of spleen and kidney syndrome between warm acupuncture combined with auricular acupuncture and simple warm acupuncture.
METHODSOne hundred and thirty patients were randomly divided into an observation group and a control group, 65 cases in each one. In the control group, acupuncture was used at Pishu (BL 20), Zhongwan(CV 12), Shenshu(BL 23), Zhongji(CV 3), Guanyuan(CV 4), Mingmen(GV 4), Taibai(SP 3), Fenglong(ST 40), etc.; warm acupuncture was applied at Pishu(BL 20), Zhongwan(CV 12), Shenshu(BL 23) and Zhongji(CV 3); the treatment was required once every two days and for 3 months continuously. In the observation group, based on the treatment in the control group, thumbtack intradermal needles were embedded at auricular points, including pi(CO13), shen(CO10), pangguang(CO9), sanjiao(CO17), neifenmi(CO18), neishengzhiqi(TF2), etc. Obesity indices [body mass(W), obesity degree(A), body mass index(BMI), body fat percentage(F%)] blood lipid indices [serum total cholesterol(TC), triglyceride(TG), low density lipoprotein(LDL-C), high density lipoprotein(HDL-C)] and clinical efficacy were observed before and after treatment in the two groups. Results After treatment in the two groups, the obesity indices of W, A, BMI, F% and the blood lipid levels of TC, TG, LDL-C were obviously decreased compared with those before treatment (all P<0. 01) and the HDL-C levels were apparently increased than those before treatment (both P<0. 01). After treatment, the improvement of TC and HDL-C in the observation group was superior to that in the control group (both P<0. 01). The difference was not statistically significant in the aspect of improving every obesity index between two groups (all P>. 05). The comprehensive total effective rate of the observation group was 95.4% (62/65), which was better than 84. 6% (55/65) of the control group (P<0. 001).
CONCLUSIONWarm acupuncture combined with auricular acupuncture and simple warm acupuncture can both benignly adjust abnormal lipid metabolism of obesity patients with hyperlipidemia, and warm acupuncture combined with auricular acupuncture are superior to simple warm acupuncture treatment on antiobesity action and improving the TC and HDL-C levels.
