1.Study on mitochondrial DNA gene tRNA~(Leu(UUR))A3243G mutation in patients with below-40-year-old-onset diabetes mellitus in China

Objective To investigate the clinical characteristics and the prevalence of mitochondrial gene A3243G mutation in patients with below-40-year-old-onset type 2 diabetes mellitus in China. Methods Body mass index (BMI), waist hip ratio (WHR), metabolism control, insulin secretion and HOMA-IR were determined in 207 patients with below-40-year-old-onset diabetes mellitus and 50 subjects with normal OGTT as control. There were 117 cases with diabetic family history. The mitochondrial gene A3243G mutation was assayed by PCR-RFLP. Results TherewasnosignificantdifferenceofBMI〔(25.1?3.1) vs (24.1?3.4)kg/m 2〕 between two diabetic subgroups. The fasting blood glucose (FBG) and HbA 1c were both higher in two diabetic subgroups. Inthosepatientstheinsulin resistance was obviously (HOMA-IR 3.41~9.29). The mitochondrial gene A3243G mutation was found in one case and 5 members of her family (three cases had diabetes mellitus and two cases had normal glucose tolerance). Conclusion The prevalence of the mitochondrial gene A3243G mutatuion is 0.48% in early onset diabetic patients in China and 0.85% in positive diabetic family history patients.
2.Pathological mechanics and treatment of plantar fasciitis:future prospects of platelet-rich plasma therapy

BACKGROUND:The treatments of plantar fasci tis as an important cause of calcaneodynia are different in medical institutions, as wel as the therapeutic results.
OBJECTIVE:To summarize various treatments of plantar fascia, and based on the pathogenesis and pathological mechanics of plantar fasci tis, to investigate the effect of various treatment methods and indications.
METHODS:A computer-based online search of PubMed database and Wanfang database was performed for articled related to plantar fasci tis published between January 1993 and January 2014. The keywords were
“plantar fasci tis, mechanism, treatment”in English and Chinese, respectively, by screening the titles and abstracts. Final y, 34 articles were included in result analysis.
RESULTS AND CONCLUSION:The treatment of plantar fasci tis needs to fol ow the principle from noninvasive to invasive treatment. Stretching the plantar fascia and heel cord, using prefab-ricated orthotics, and wearing night splints are crucial for the treatment of plantar fasci tis. Local hormone injection can be used as first-line therapy for patients with acute plantar fasci tis if they can tolerate the adverse reactions due to hormone therapy. Platelet-rich plasma therapy has a promising prospect, but there is lack of experimental evidences. Extracorporeal shock wave therapy can be a choice for recurrent chronic intractable plantar fasci tis. Surgical intervention remains the last line of therapy, for which, rigorous screening is important, but it is not always effective at reducing pain.
3.Expression of miR-140-3p in synovial fluid of knee osteoarthritis patients reflects the progression of osteoarthritis

BACKGROUND:Osteoarthritis is an age-related joint disease characterized by articular cartilage degeneration.
OBJECTIVE:To assess the expression of miR-140-3p in synovial fluid of knee osteoarthritis, and identify whether miR-140-3p levels in synovial fluid are correlated with disease severity.
METHODS:We col ected 10 healthy volunteers, 10 patients with gouty arthritis, 10 patients with rheumatoid arthritis, and 45 patients with knee osteoarthritis (15 cases in early stage, 15 cases in middle stage and 15 cases in the late stage). Real-time fluorescence quantitative PCR was used to detect miR-140-3p expression in synovial fluid in each group.
RESULTS AND CONCLUSION:(1) Expression of miR-140-3p was significantly down-regulated in knee osteoarthritis group. Significant differences in miR-140-3p expression were detected between non-knee osteoarthritis group and knee osteoarthritis group (P<0.05). No significant difference in miR-140-3p expression was determined among healthy control group, gouty arthritis group and rheumatoid arthritis group (P>0.05). (2) The miR-140-3p expression in the healthy control group was 11.4 times that in the knee osteoarthritis group. The expression of miR-140-3p decreased with the increase of the severity of osteoarthritis. (3) Spearman rank correlation analysis suggested that the expression of miR-140-3p was negatively correlated with osteoarthritis severity. (4) Results suggest that the expression of miR-140-3p in synovial fluid can reflect the progression of knee osteoarthritis to a certain extent.
4.Causes of intrapartum rupture of the scarred uterus and the nursing strategies

Objective To explore the causes of intrapartum rupture of the scarred uterus and sum up the nursing experience. Methods The clinical data of 11 parturients with intrapartum rupture of scarred uterus were retrospectively analyzed to find out the causes of rupture and the nursing strategies were summarized.Result All of them resovered after rescuing and nursing,who were hospitalized for 3-5d and got labored without accidents.The causes included their histories of uterine-incision delivery,living places, education level and individual constitution.Conclusion The nursing measures including intrapartum health care and education, antenatal examination,close observation through the labor process,enhanced techniques and monitoring of high-risk gravida to avoid rupture of uterus,are vital for the decrease of parturient mortality.
5.Operation of upper thoracic spine diseases through anterior transsternal approach

[Objective]To investigate the method of anterior transsternal approach for the patients with upper thoracic spine diseases and the clinic results.[Method]Six cases upper thoracic spinal diseases,1 case of C7/T1 grade V dislocation,1 case of T1、2 TB,2 cases of upper thoracic tumor,1 case of T2 fracture and 1 case of T2、3 disc prolapse,were treated with the anterior transsternal approach operation since Oct.2001.The lesions areas were exposed via partial or complete sternotomy.The relative articles were reviewed.[Result]The average follow-up was 12.4 months(range from 6 to 22 months,except the died one).The case of C7/T1 grade V dislocation died of respiratory tract obstruction and one case of malignant schwannoma recurred 6 months post-operation.Good results were obtained in the other 4 cases.No operative complication happened in all cases.[Conclusion]While the transthoracic lateral approach cannot expose the upper thoracic spine clearly,the anterior cervical approach cannot expose the T2、3 clearly also.The anterior transsternal approach can provide a safe access to the lesions located on the upper thoracic spine above T4.

7.Advances in targeting nano drug therapy and its carrier of colorectal cancer

Colorectal cancer is currently the world's fourth incidence of malignant tumors,the early clinical manifestations are not obvious,so the early diagnosis is difficult to find,most of them are in progress.Treatment of advanced stage with chemotherapy,interventional therapy,radiotherapy and other methods,in which chemotherapy in the killing of tumor cells at the same time,the normal cells of the body also has a killing effect.In recent years,all kinds of new nano targeting delivery system has been developed,which can be targeted to the tumor tissue,so it is more and more important in the treatment of intestinal tumor,especially with metastasis.The author has made an overview of the types of nano drug carrier materials,the research status and its application in the research of colorectal cancer.
8.Variation of iNOS expression in periodontal tissues of rats with periodontitis in orthodontic tooth movement

Objective:To study the changes of iNOS expression in periodontal tissues of rats with periodontitis in orthodontic tooth movement. Methods:48 SD rats at 10 weeks were divided randomly into two groups: periodontitis teeth group and normal teeth group. The maxillary first molar was pulled mesially both in the two groups. 4 rats were executed each time after activation of pull-spring 1, 2, 3, 7,14 and 21d both in two groups. iNOS immunohistochemistry staining were done to compare the expression variations. Results: There were significant differences between the two groups in 2,3, 7, 14 and 21 d. The periodontitis teeth group had stronger positive signals. Conclusion: Periodontitis will increase the expression of iNOS in the rats periodontal tissue during tooth movement, and influence the reconstruction of periodontium.
10. HPLC-DAD Characteristic Chromatogram of Baphicacanthis Cusiae Rhizoma et Radix

OBJECTIVE: To establish the characteristic chromatogram of Baphicacanthis Cusiae Rhizoma et Radix and its decoction pieces by HPLC for the identification of authentic and counterfeit products. METHODS: High performance liquid chromatography (HPLC) was used with Agilent Zorbax C18(4.6 mm×250 mm, 5 μm). The mobile phase was acetonitrile-water with gradient elution. The detector was a secondary tube array (DAD). The column temperature was maintained at 35 ℃, the flow rate was 1.0 mL•min-1, and the injection volume was 10 μL. RESULTS: Fifteen batches of genuine crude drug and twelve batches of genuine decoction pieces were determined. Five common peaks were found, among which three peaks were 2-benzoxazolinone, indigo and indirubin. CONCLUSION: The established characteristic chromatogram of Baphicacanthis Cusiae Rhizoma et Radix can effectively distinguish the authentic from the counterfeit. The methodological demonstration shows that the method is accurate, stable and reproducible.
