Surgery is an important way to treat patients with primary hyperparathyroidism.Recently,conventional bilateral neck exploration is substituted with minimal invasive surgeries gradually due to the development of the technology of pre/intro-operative localization.Video-assisted surgery is one of the minimal invasive surgeries.It includes video-assisted parathyroidectomy and total endoscopic parathyroidectomy.This article summarize the literatures of treatment of primary hyperpatathyroidism with video-assisted surgery in this article.

Objective:To investigate the relationship between flurbiprofen axetil injection and atrial fibrillation in the elderly pa-tients after cardiac surgery and underlying mechanism. Methods: A retrospective research method was applied, and the medical re-cords of 62 elderly patients with cardiac surgery were chosen and divided into the experimental group and the control group according to the different medication. After the heart surgery, fentanyl was used in the control group, and flurbiprofen was applied in the experimen-tal group. Before and after the treatment, the serum C-reactive protein levels, the incidence of postoperative atrial fibrillation, postop-erative atrial fibrillation duration, length of stay, incidence of cardiovascular disease in 6 months and adverse reactions were observed and compared between the two groups. Results:After the treatment, the serum CRP levels in the experimental group were significantly lower than those before the treatment and those in the control group after the treatment (P<0. 05 or P<0. 01). The incidence of post-operative atrial fibrillation in the experimental group was significantly lower than that in the control group (P<0. 05). The duration of postoperative atrial fibrillation in the experimental group was significantly shorter than that in the control group (P<0. 05). The differ-ences in the postoperative hospital stay, the incidence of cardiovascular disease in six months and the incidence of adverse reactions were not statistically significant between the two groups (P>0. 05). Conclusion: Flurbiprofen axetil injection used after cardiac sur-gery can reduce the incidence of atrial fibrillation, which shows important clinical significance.

Objective To evaluate the diagnosis and treatment of left renal vein entrapment syndrome. Methods The clinical data of 10 cases of left renal vein entrapment syndrome were retrospectively analyzed.Their mean age was 24 years (range,14-37 years).Of them 7 cases presented with non-glomerular origin hematuria;2,with orthostatic proteinuria;1 with hematuria accompanied by proteinuria.Color Doppler sonography showed that the dilated segment of the left vein was three-fold than the strictured segment in diameter,and magnetic resonance angiography (MRA) showed that the left renal vein was compressed in 9 cases. Results Stent placement across the left renal vein was performed in 7 cases.Superior mesenteric arteriectomy and replantation were performed in 3 cases.The operations were all successful.Follow-up of 1-5 years showed that the clinical symptoms were improved and urine tests were normal. Conclusions Non-glomerular hematuria or orthostatic proteinuria is frequently found in left renal vein entrapment syndrome.Left renal vein entrapment syndrome can be diagnosed by Doppler sonography,MRA and cystoscopy in the presence of hematuria or proteinuria.Generally,conservative treatment and careful follow-up are conducted in these patients.Surgical and radiologic interventional procedures are effective alternatives for those with glomerular dysfunction and complications.

Objectives:To clarify whether the bone mineral density (BMD) differed from normal and whether the mandibular BMD changed with age in young adults with moderate periodontitis. Methods:30 patients (20-35 years old) with moderate adult periodontitis and 30 individuals (20-35 years old) with normal periodontal condition as control group were included in present study. BMD of the mandible was measured using panoramic mandibular index(PMI) from panoramic radiographic film. Results:The sPMI and iPMI value were 0.275 0?0.034 and 0.527 3?0.096 (normal group), 0.223 3?0.024 and 0.367 3?0.069 (periodontitis patients) respectively.The PMI value of periodontitis patients was significantly decreased compared to that of normal group, and showed a significant correlation with age.Conclusions:Moderate periodontitis in young adults seem to be a local disorder associated with relatively low PMI in the jaws. Age-related decrease in mean PMI with increasing age in both normal and periodontitis patients is founded.Dental panoramic radiograph may serve as a simple tool in mandibular BMD defection.

Objective To evaluate the practical value of ERCP for etiologic diagnosis and treatment of postcholecystectomy syndrome(PCS).Methods A retrospective review of 116 cases of postcholecystectomy syndrome received ERCP in the recent four years in our hospital was undertaken.Of the116 cases,80 cases(68.97 %) had the indications for therapeutic ERCP and were treated by endoscopy.Results The bile duct was visualized in all of the 116 cases(100 %),and the pancreatic duct was visualized in 97 cases(83.62 %).The etiology of PCS was choledocholithiasis in 56 cases(48.28 %),diverticulum adjacent to duodenal papilla or papilla with in the diverticulm in 19 cases(16.37 %),residual of long cystic duct with stones in 15 cases(12.93 %),benign stenosis of terminal commom bile duct in 13 cases(11.20 %),sphincter of oddi dysfuction in 4 cases(3.45 %),tumour of doudenal papilla in 3 cases(2.59 %),stenosis of injured bile duct in 2 cases(1.73 %),sclerosing cholangitis in one case(0.86 %),cholangiocarcinoma in one case(0.86 %),remnant gallbladder with stones in one cases(0.86 %),and chronic pancreatic ductal lithiasis in one case(0.86 %).Fifty-seven cases were treated by EST stone extraction,20 cases by ERBD,23 cases by ENBD,1 case by EMBE,and 1 case by EPS extraction.In the 80 cases treated with endoscopy,75 cases were followed up for 3~12 months,with relief of symptons in 74 cases(98.67 %).Conclusions ERCP is the gold standard for etiologic diagnosis of PCS.PCS is an indication for therapy by ERCP.Therapeutic ERCP is the preferred treatment of PCS cases with an indication for endoscopic treatment.

There are many kinds of research methods in proteomics of breast cancer,one of them is mass spectrometry and have made a great progress.With more progress in proteomics studies,diagnosis and prognosis of the specific markers for breast cancer continue to be found,there would be great benefits for patients with breast cancer in diagnosis and prognosis.

Objective To establish a standardized middle cerebral artery occlusion (MCAO) model with suture method in rabbits and to investigate the value of the assessment for cerebral ischemia with amplitudeintegrated electroencephalogram (aEEG).Methods A total of 34 male New Zealand white rabbits were randomly assigned to either an MCAO group (n =29) or a sham operation goup (n =5).A model of MCAO was induced with intraluminal suture method and the cerebral function monitor was performed.According to 2,3,5-triphenyltetrazolium chloride staining,the MCAO group was further divided into cortex + basal ganglia infarction,basal ganglia infarction,subarachnoid hemorrhage,and non-lesion subgroups.The differences among the physiological indicators,weight,thread end diameter,and insertion length were compared before and after modeling in all subgroups.Results The success rate of MCAO modeling with suture method in rabbits was 62.07％ (18/29),in which 37.93％ (11/29) involved in the cortex and basal ganglia,24.38％ (7/29) only involved in the basal ganglia,17.24％ (5/29) complicated by subarachnoid hemorrhage,and 20.69％ (6/29) had no infarction.There were no significant differences in the body temperature,heart rate,mean arterial pressure and arterial blood pH,oxygen partial pressure,and CO2 partial pressure among all the subgroups before and after modeling.The weight in the non-lesion subgroup was 2.36 ± 0.10 kg,it was significantly lower than 2.55 ± 0.09 kg in the cortex + basal ganglia infarction subgroup (P =0.001) and 2.50 ± 0.12 kg in the basal ganglia infarction subgroup (P =0.017).The length of suture placement in the cortex+basal ganglia infarction subgroup was 5.59 ± 0.24 cm,and it was significantly less than 6.00 ± 0.50 cm in the subarachnoid hemorrhage subgroup (P =0.036).However,it was significantly longer than 5.20 ± 0.50 cm in the non-lesion subgroup (P =0.033).After modeling there were significant differences in aEEG among all subgroups (F =14.059,P =0.000).Compared to before modeling,aEEG decreased 50.02％ (t =9.573,P ＜ 0.001) and 14.20％ respectively after modeling in the cortex + basal ganglia infarction subgroup and the ganglia infarction subgroup (t =2.908,P =0.027).Conclusions A standardized MCAO model in rabbits may be successfully established with suture method.The significantly decreased aEEG indicates that the MCAO model is successful and the lesions involve in the cortex.

Objective To develop a medical service information management system of division aid station(DAS) so as to meet the demand of medical informationization and improve the commanding capability of DAS.Methods Based on a thor-ough research on the general requirement in DAS echelon especially in its command team,Delphi 7 and modularization technology were used to design and exploit information management system.Results The system could satisfy the needs of medical service information management of DAS.Conclusion The system can be provided a powerful means for manage-ment of medical information in DAS echelon and is an indispensable part of informatization of DAS.

Objective To investigate the influence of NO mixture(L-arginine,apocynin and sodium nitroferricyanide) on wound healing of diabetes mellitus(DM) mice.Methods Fifty adult male Kunming mice were induced by streptozotocin to establish type I DM model.They were then randomly divided into control group,L-arginine group(150 g/L),apocynin group(1?10-4 mol/L),sodium nitroferricyanide group(0.1 mmol/L) and NO mixture group(above components were mixed with equal ratio).Full-thickness skin wound was made and injected with corresponding drugs of 0.15 ml once every 2 days.The wounds were digitally photographed to calculate the percentage of wound closure using computer image analysis software in 1,3,5,7 and 10 d post-injury.The density of fibroblasts,the content of collagenous fibers and the neovascularization in the wound samples were measured with the aid of HE staining.Results From the third day post-injury,the wound healing rate of NO mixture group was significantly higher than that of all the other groups(P

Objective Getting the robust exogenous gene expression vector under the control of porcine insulin promoter, and to lay the foundation for pancreaticβ-cells specific transgene expressing pigs.Method Using porcine insu-lin promoter ( PIP, 1500 bp of the 5′UTR from the porcine INS gene including the first exon and the first intron) to con-struct expression vector, the HindIII restriction site which connected the sequences of PIP and EGFP was designed before ATG, named PIP-HindIII-EGFP.Considering that the different location of restriction site may affect the expression efficien-cy of the transgene, we optimized the expression vector.Firstly the HindIII restriction site was deleted to realize the seam-less connection of PIP and EGFP,the vector was named PIP-EGFP.Also we mutated the 3′intron splicing acceptor site( SA) of the first intron into HindIII restriction site, named as PIP-SA( M)-EGFP.Three different EGFP expression vectors were respectively transfected MIN-6 mouse pancreatic β-cells, pig ear fibroblasts and kidney cells.The transfected cells were cultured for 48 h and harvested for RT-PCR, flow cytometry and Western blot analysis, to analyze and compare the expres-sion efficiency of vectors.Results After transfection,green fluorescence was observed only in MIN-6 mouse pancreaticβ-cells.RT-PCR analysis and product sequencing showed that the three expression vectors did have different stability with in-tron splicing.The PIP-HindIII-EGFP construct and PIP-EGFP vector produced two kinds of mRNA with the first intron spliced and no spliced, indicating the instability of intron splicing.Mutation of the PIP splice site would cause the first in-tron not spliced, while flow cytometry and Western blot displayed that the mutation induced a most efficient expression of the downstream gene.Conclusions A robust and specific β-cells expression vector has been successfully generated by mutating the intron splicing acceptor site of the porcine insulin promoter.It provides the foundation for preparation of pigs with pancreaticβ-cells specifically expressing the transgene.
