Objective To explore the mechanism of combined treatment with methotrexate (MTX) and Ga-Al-As laser irradiation for rheumatoid arthritis (RA) and to assess the effectiveness of Ga-Al-As laser therapy for RA. Methods Twenty-two patients with RA were randomly and evenly divided into two groups: the treatment group treated with Ga-Al-As laser irradiation combined with MTX and the control group treated with MTX only. Ten age-matched normal subjects were observed as normal controls. The amount of CD4 + CD25 + regulatory T cells in peripheral blood (PB) of the normal controls and that in the PB and synovial fluid (SF) of the 22 patients before and after therapy were counted by flow cytometry. Meanwhile, the amount of prostaglandin E2 (PGE2) in synovial fluid of the patients was measured before and after treatment by enzyme-linked immunosorbent assay(ELISA). Results After combined treatment the clinical symptoms of the patients were improved significantly, and the amount of PGE2in SF decreased significantly. The count of CD4 + CD25 + regulatory T cells in PB of RA patients was ( 3.84 ±3.20) % , compared to ( 10.05 ± 7.04) % in healthy individuals. The count of CD4 + CD25 + regulatory T cells in SF of RA patients was ( 14.89 ± 12.30) % , much higher than that in PB. The count of CD4 + CD25 + regulatory T cells in SF decreased significantly in treatment group compared to control group (P ＜0.05). Conclusion Ga-Al-As laser irradiation eombined with MTX can effectively improve the clinical symptoms of RA patients. It may be related to the decrease of amount of PGE2 and count of CD4 + CD25 + regulatory T cell in PB and SF.

Objective To observe the effect of ritodrine hydrochloride on peripheral blood and tocolysis rate of patients with threatened premature labor.Methods89 cases of patients with threatened premature labor were selected from October 2015 to September 2016 and randomly divided into two groups, research group with 45 cases treated with ritodrine hydrochloride and control group with 44 cases treated with epsom salt.The peripheral blood, extinction time of uterine contraction, extended days of pregnancy, postpartum hemorrhage, neonatal birth weight and tocolysis rate were compared between two groups.ResultsAfter treatment, CRH, NO, PGE2 and IL-8 of control group were obviously higher than research group, the difference was statistically significant (t=10.826,3.839,7.534,8.075,P<0.05).The extinction time of uterine contraction in control group was obviously longer than research group, while extended days of pregnancy and neonatal birth days in research group were obviously longer than control group, the difference was statistically significant (t=4.566,7.855,3.237,P<0.05).There was no significant difference in postpartum hemorrhage between two groups (t=0.889,P>0.05).The overall efficacy of tocolysis in control group was 70.5%, while that in research group was 86.7%, which was obviously higher than the control group, the difference was statistically significant (χ2=7.801,P<0.05).ConclusionRitodrine hydrochloride can effectively control uterine contraction factor in peripheral blood and extend gestational days of patients with threatened premature labor, so as to improve tocolysis rate and birth quality.

Objective To investigate the effect of Propess on cervical collagen remodeling and cervical ripening in full-term pregnancy.Methods 94 cases of mid-term pregnant women enrolled in obstetrics and gynecology of our department from January 2016 to January 2017 were divided into two groupaccording to the random number table method.47 cases of the control group, taking oxytocin intravenous infusion;observation group of 47 cases, taking propess suppository vaginal medication.Labor time, medication to labor time, cervical Bishop score, mode of delivery, postpartum hemorrhage and neonatal situation of two groups were compared.Results Compared with the control group, the Bishop score of the cervix was increased, the total effective rate of cervical ripening was improved, the labor was shortened, the time of medication was shortened, and the delivery rate of vaginal delivery was improved(P<0.05), while the two groups were bleeding, neonatal Apgar score and the incidence of fetal distress were not statistically significant.Conclusion It can promote cervical collagen remodeling, promote cervical ripening, thereby reducing the rate of cesarean sectionwith taking propess in the vaginal.

Objective: To construct a prediction model for sleep disorders in shift workers of a chemical fiber enterprise, so as to provide the basis for early identification and prevention of sleep disorders in shift workers. Methods: Shift workers were sampled from a chemical fiber enterprise in Tongxiang City, Zhejiang Province using a cluster sampling method from August 2022 to July 2024. Demographic information, length of service and average weekly working hours were collected through questionnaire surveys. Depressive symptoms, anxiety symptoms and sleep disorders were evaluated using the Pittsburgh Sleep Quality Index, Patient Health Questionnaire and Generalized Anxiety Disorder Questionnaire, respectively. The shift workers were randomly divided into a training set and a validation set at a ratio of 7∶3. Predictive factors were selected using a multivariable logistic regression model based on the training set, and a nomograph model for prediction of sleep disorders in shift workers was established. The predictive values of the model were evaluated using the receiver operating characteristic (ROC) curve and calibration curve based on the training set and validation set. Results: Totally 673 shift workers were included, with a median age of 32 (interquartile range, 12) years. There were 493 males, accounting for 73.25%. There were 471 (69.99%) workers in the training set and 202 (30.01%) workers in the validation set. There were 274 workers with sleep disorders, accounting for 40.71%. The equation for the prediction model was ln[p/(1-p)]=-8.391+1.906×average weekly working hours+1.822×depressive symptoms+1.667×anxiety symptoms. The area under the ROC curve was 0.769 (95%CI: 0.661-0.835) for the training set and 0.655 (95%CI: 0.593-0.737) for the validation set, and Hosmer-Lemeshow test showed a good fitting effect (both P>0.05). Conclusion The nomograph model constructed by average weekly working hours, depressive symptoms and anxiety symptoms can be used to predict the risk of sleep disorders in shift workers of a chemical fiber enterprise.

Objective To explore the effect of medical cooperation health education after conservative surgery placed Mirena treatment with uterine gland myopathy.Methods 30 patients were collected and divided into general group and cooperation group by random number table method,with each group including 15 cases.General group accepted regular health education,while cooperation group received medical cooperation health education.6 months of home visits and telephone follow-up of two groups,statistical comparisons of menstruation,menstrual days,life satisfaction,adverse symptoms and satisfaction of diseases,Mirena,nursing methods between the two groups before and after the treatment.Results Patients' satisfaction and disease knowledge,Mirena knowledge,nursing knowledge the cooperation group scored higher than the conventional group (x2=2.184-6.184,P ＜0.05 or 0.01);the number of higher anxiety level was lower than the conventional group (x2=13.258,P ＜ 0.05);after treatment,dysmenorrhea severity score,menstrual flow,menstrual days,the quality of life the cooperation group were (0.35±0.08) points,(14.20±16.54) ml,(4.21±1.14) d,(9.01±1.36) points while the conventional group were (1.37±0.34) points,(39.64±8.62) ml,(6.74±1.07) d,(7.81±1.30) points,and the difference was statistically significant (t =2.47-11.31,P ＜0.01).the number of adverse symptoms of cooperation group was lower than that of the conventional group (x2=13.594,P ＜ 0.01).Conclusions Compared with single health education,cooperative medical health education has more advantage after conservative surgery placed Mirena treatment uterine gland myopathy which should be promoted in clinic.

Total 406 community residents entered in fitness improvement program with “management by objectives model of health promotion” from 2009 to 2011.The fitness status was monitored by Xuhui Physical Constitution Center before and after entering the program.The results showed that the level of fitness assessment,the composite score,lung capacity and reaction time were improved with the years (P ＜ 0.01) ; the step index and sitting-and-reach test in 2010 was better than those in 2009(P ＜ 0.01) ; the grip strength in 2010 and 2011 was better than that in 2009 (P ＜ 0.01); the test of single leg-standing with closing eyes in 2011 was better than that in 2009(P ＜ 0.05).There was no difference in body mass index before and after program (P ＞ 0.05).The results indicate that physical fitness and overall health can be improved by management by objectives model for health promotion.

The theory of traditional Chinese medicine (TCM) deems that kidney essence is the material basis of formation, development and function of the brain and it maintains human being's psychomotility and behavior. Moreover, kidney essence is the material basis of emotional activity. The emotion theory in TCM deems that kidney stores will and responds to fear. The so called will in this theory refers to memory which includes three basic steps such as memorization, storage and recollection. Meanwhile, Western medicine considers stress as a kind of information that forms memory trace in central nervous system. Combining the TCM theory with the modern psychological theory on stress, and from the points of views of the emotion regulation, the formation, consolidation, extraction and extinction of fear memory, and the plasticity of amygdala-hippocampus-prefrontal cortex nervous pathway, as well as the results of pharmacological studies of prescriptions and herbs for invigorating the kidney, the authors explained in this article the pathological mechanisms of kidney deficiency due to attack of fear, and revealed the material basis, the action modes, and the neurobiological mechanisms of the kidney in controlling and regulating emotional activity.

Objective To study the changes of white matter fiber tracts in mild traumatic brain injury(mTBI)patients by using DTI technique on 3.0T MR system,and evaluate the clinical diagnostic value of DTI.Methods Thirty mTBI and 30 matched health-y controls underwent conventional CT,MRI and DTI examinations.FA values and ADC values in the genu of the corpus callosum, the splenium of the corpus callosum,bilateral internal capsule,the cingulum bundle,the bilateral superior longitudinal fasciculus and the inferior longitudinal fasciculus of mTBI were measured in mTBI patients and controls at acute phase,subacute phase,5 weeks and 3 months post-trauma.The temporal changes of MRI data were observed.Results The FA values in the part of the white mat-ter fiber tracts reduced in the acute and subacute phase in mTBI patients (all P <0.05)in relative to controls (all P <0.05),At 5 weeks and 3 months post-trauma,the FA values also slightly lower in patients than controls.As for the genu of the corpus callosum and the splenium of the corpus callosum,the FA values in mTBI patients increased slightly at the acute stage (all P >0.05),but de-creased at 5 weeks-3 months post-trauma phase,though there was no significantly difference(all P >0.05).ADC values significantly decreased in the acute and subacute phase in mTBI patients,while they recovered thereafter (all P >0.05).Conclusion The sensi-tivity of DTI in detecting the lesions of mild traumatic brain damage is high,and it can display the damage of white matter.DTI could play an important role in the diagnosis of mTBI.

BACKGROUND: There are some reports about the relationship between atherosclerotic cerebral infarction and apolipoprotein E and B, but the results are still controversial. The relationship between apolipoprotein E and B and young adult atherosclerotic cerebral infarction has not been reported yet in China.OBJECTIVE: To investigate the relationship between the polymorphism of apolipoprotein E and B gene and young adults atherosclerotic cerebral infarction (ACI).DESIGN: A controlled case analysis of young adult atherosclerotic cerebral infarction patients.SETTING: Department of Neurology, Shengjing Hospital of Chinese Medical University.PARTICIPANTS: This study was conducted in the Department of Neurology, Shengjing Hospital of Chinese Medical University from January of 1998 to December of 2000. Thirty-six young adult patients with atherosclerotic cerebral infarction, including 30 males and 6 females, with the meaage of (41.6±6.54) years, and 100 healthy young adults, including 66 males and 34 females, with the mea age of (36.16±6.12) years were included in this study.METHODS: 8 Ml venous blood was collected after fasting for 12 hours to assay serum lipid and apolipoprotein. The gene polymorphism of apolipoprotein E and B were detected with PCR method. Enzymic method was used to detect total cholesterol, total triglycerides and high density lipoprotein-cholesterol (HDL-C). Apolipoprotein AI.B was measured with immunoturbidimetry method and lipoprotein (a) with ELISA method.Lipids, lipoprotein and apolipoprotein of six control blood samples couldn't be measured because of hematolysis.MAIN OUTCOME MEASURES: ① The distribution characteristics of genotypic frequency of apolipoprtein E and B in the two groups. ② The relationship between gene polymorphism of aoplipoprotein E and B and the level of blood lipids, lipoprotein and apolipoprotein. ③ The correlation intensity between genotypes of apolipoprotein E and B and onset of young adult atherosclerotic infarction.RESULTS: ① In ACI group, ε3/4 counted for 36.1% and ε2/4 for 27.7%, but was 12% and 7% in control group respectively. The gene frequency of ε4 was 0.320. All these values were higher than that in control group 0.95 (P ＜ 0.05). ② The levels of TG, TC, and LP (a) in ACI group were higher than that in control group. The level of HDL-C was much lower than the control group's (P ＜ 0.05-0.01). ③ ε4 allele caused the increase of the content of TG, TC, and LP (a) so as to induce the relative risk rates of decrease of HDL-C which were 8.23, 4.85, 29.9,4.39 (P ＜ 0.01-0.001) respectively. ④ AI content of the gene frequency of ApoB XbaⅠ X+X- was (1.01±0.30) g/L in ACI group, which was lower than (1.33±0.15) g/L in X-X- subgroup (t=2.55, P ＜ 0.05). The level of ApoA I in X+X group (244.3 mg/L) was remarkably different from that (183.0 mg/L)in control group (t=4.50, P ＜ 0.01). ⑤ Three cases had both ε3 and X+X-in ACI group, 10 had both ε3 and X-X-, 2 had both ε4 and X+X-, and 19 had both ε4 and X-X-. The risk of ACI was 2.85 with the linkage of allele ε4 and allele X-X- in ACI patients (x2=1.52, P ＞ 0.05).CONCLUSION: Allele ε4 is a genetic facilitated factor of young adults ACI. Xba Ⅰ X+X- is another probable genetic symbol. The correlation between atherosclerotic cerebral infarction of young patients during the combination of apolipoprotein E and B should be researched further.
