3.Cross-cultural adaptation and validation of the Filipino translation of the knee injury and osteoarthritis outcome score in Filipinos with knee osteoarthritis at a tertiary hospital

Objectives:
1) To translate and cross-culturally adapt the Knee Injury and Osteoarthritis Outcome Score (KOOS) into Filipino; 2) To validate the Filipino translation of KOOS using the Filipino Short-Form 36 Health Survey (SF-36) among patients with knee osteoarthritis (kOA) at a tertiary hospital.
Methodology:
A Filipino version of the KOOS was translated and cross-culturally adapted from the original English version and validated following standard guidelines. Adult Fiipino patients with knee osteoarthritis at the University of the Phiippines-Philippine General Hospital were asked to complete identical questionnaires containing the Filipino KOOS and Filipino SF-36, with re-test on the same patients after a median of 14 days. Reliability was assessed using Cronbach’s alpha and intraclass correlation coefficients (ICC); dimensionality using convergent and divergent construct validity.
Results:
The Filipino translation of the KOOS was administered to 30 patients with knee OA (kOA). Cronbach's α across the Filipino KOOS domains ranged from 0.71 to 0.89 suggesting good internal consistency. The reproducibility of measurements of all KOOS subscales by ICC ranged from 0.97 to 1.0. For convergent construct validity, there was moderate correlation between KOOS ADL (0.38, p =0.03) and knee-related QoL (0.42, p=0.02) by SF-36 Physical Functioning (PF). A strong correlation (0.51, p=0.003) was observed between KOOS sports and recreation domain with SF-36 PF. For divergent construct validity, there was weak correlation between KOOS pain (0.015, p=0.93) and symptoms (0.15, p=0.42) with SF 36 Social Functioning.
Conclusion
The Filipino version of the KOOS is a valid and reliable instrument to measure the different aspects of disability affecting quality of life of Filipino patients with kOA.

5.Association of Meniscus Protrusion with Radiographic Findings and Clinical Assessment in Both Knee Osteoarthritis.

OBJECTIVE: To evaluate the association of ultrasonographic (US) findings of medial meniscus protrusion (MMP) with clinical and plain radiographic assessment in knee osteoarthritis (OA). METHOD: One hundred and twenty six knees of 63 patients were studied. The degree of protrusion for each knee of patients was measured during standing by US. Radiographs were examined in order to determine whether the participants had radiographic osteoarthritis, graded using the Kellgren-Lawrence (K-L) grade. Clinical assessment was performed by recording a visual analogue scale (VAS). Correlation was obtained between the difference of VAS and MMP in the same patient. Assessment of pain, stiffness, and disability were performed by comparison of K-WOMAC index and MMP in a patient. RESULTS: Mean protrusion (mean+/-S.D.) for knees with each K-L I, II, III, and IV grade were 0.27+/-0.52 cm, 0.38+/-0.60 cm, 0.55+/-0.76 cm, and 0.75+/-0.08 cm, respectively. The difference was significant (p<0.05). Significant correlation was observed between MMP and VAS in K-L grades II (p=0.002, r=0.500) and III (p=0.002, r=0.684), also between DeltaMMP and DeltaVAS (p=0.000, r=0.558). With the K-WOMAC index, MMP were correlated with pain and stiffness (p<0.01), but not with disability score. CONCLUSION: The degree of MMP measured by US is associated with K-L grade in knee OA patients. The degree of MMP can be a reliable indicator, like K-L grade, for radiological severity of knee OA. MMP has been correlated with VAS and with subscore of pain and stiffness, but not disability in the K-WOMAC index.

9.Cross Cultural Adaptation and Translating the IKDC Subjective Form into the Korean Language.

PURPOSE: To develop and translate into Korean the International Knee Documentation Committee Subjective Knee Evaluation Form. MATERIALS AND METHODS: There were 6 steps for the cross-cultural adaptation to standardize the language and cultural differences as follows translation for stage I, synthesis for stage II, back translation for stage III, a review by an expert committee for stage IV, pre-testing for stage V, and submission of the documentation to the AAOS Committee for appraisal was for the final stage VI. A total of 120 patients from 4 hospitals underwent the pre-testing which is the stage V on this process. RESULTS: 30 patients who fully underwent the preoperative and postoperative testing at 3 month were evaluated. The average age of participants was 43.1. There were 12 cases of meniscus tears, 8 cases of primary degenerative arthritis, 4 cases of patellofemoral pressure syndromes and 8 cases of other knee disorders. In the results, we found that there was statistically the strong correlation as follow the test and re-test reliability (p=0.6373), the internal consistency for each evaluation item (p=0.9135), the validity was 0.8882 and the responsiveness (p=0.6605). CONCLUSION: The Korean version of the IKDC subjective knee evaluation form was excellent as our results showed on its reliability, validity and responsiveness through the rigid steps of cross-cultural adaptation. It was submitted to the AAOS Committee for Appraisal. We strongly suggest that it should be considered as part of an effective evaluation for a variety of knee diseases.
