2.Laparoscopic pancreaticoduodenectomy procedure: Pushing the limits of laparoscopy

Whipple operation, or open pancreaticoduodenectomy, is perhaps one of the most technically challenging operations in the abdomen. The overall perioperative mortality rate is about 2% to 5% and with operative morbidity of about 25% to 45%. In the past, minimally invasive techniques were only used for laparoscopy, staging of pancreatic cancer, and palliative procedures for unresectable pancreatic cancer. Recent advances in operative techniques and instrumentation have empowered laparoscopic surgeons to perform virtually all procedures in the pancreas, including the Whipple procedure. Presented is the first case of laparoscopic pancreaticoduodenectomy.

Executive Summary
The Coronavirus disease 2019 (COVID-19) pandemic has triggered a global crisis and has affected
millions of people worldwide. With the evolution of the different variants of concern, the incidence of COVID-
19 in the pediatric population has risen. The Surveillance and Analysis of COVID-19 in Children Nationwide
(SALVACION) Registry, developed by the Pediatric Infectious Disease Society of the Philippines (PIDSP) and the
Philippine Pediatric Society (PPS), has reported 3,221 cases as of March 31, 2022, with 90.4% requiring
hospitalization and 36.2% with moderate to critical disease severity. Given the magnitude of the impact of
COVID-19, with most of the clinical recommendations available designed towards adult patients, there was an
urgent need for clinicians, public health officials and the government to also prioritize evidence-based clinical
practice guidelines for the pediatric population. Hence, the development of the Philippine Pediatric COVID-19
Living Clinical Practice Guidelines was conceptualized. This independent project, funded and supported by the
PPS and PIDSP, aimed to formulate up-to-date, evidence-based recommendations on the treatment,
diagnosis, infection prevention and control of COVID-19 in children.
Following the standard CPG development process outlined in the DOH Manual for CPG Development
and the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) methodology, 15
evidence summaries and 24 recommendations were generated by 12 consensus panelists representing their
specific health organizations and institutions.
4.Depressive Symptoms in Newly Diagnosed Lung Carcinoma: Prevalence and Associated Risk Factors

BACKGROUND: Depression is a recognized complication of lung cancer underreported in developing countries such as Malaysia. Treating and identifying depression in cancer patients increases survival and quality of life. Our objectives are to study prevalence of depressive symptoms in newly diagnosed lung carcinoma, and examine the relationship of depressive symptoms with other influencing risk factors. METHODS: A 2-year, cross sectional study February 2015–February 2017, was conducted at Hospital Tengku Ampuan Afzan, and Penang General Hospital. One hundred and three patients with newly diagnosed, biopsy confirmed primary lung carcinoma were recruited. Self-rated patient's identification sheet, validated Center for Epidemiologic Studies Depression (CES-D), and Dukes University Religion Index score from three different main languages were used. RESULTS: Prevalence of current depressive symptoms (CES-D total score ≥16) is 37.9%. The result suggests prevalence of those at high risk of moderate to major depression, may need treatment. Multivariate analysis reveals those with good Eastern Cooperation Oncology Group factor (η2=0.24, p<0.001) married (η2=0.14, p<0.001) with intrinsic religiosity (IR) (η2=0.07, p<0.02) are more resistant to depression. CONCLUSION: One in three of lung carcinoma patients, are at increased risk for depression. Clinicians should be aware that risk is highest in those with poor performance status, single, and with poor IR. We suggest routine screening of depression symptoms as it is feasible, to be performed during a regular clinic visit with immediate referral to psychiatrist when indicated.

By 2050, it is predicted that six million hip fractures will
occur each year of which the majority will happen in Asia.
Malaysia is not spared from this predicted rise and its rate of
increase will be one of the highest in this region. Much of
this is driven by our unprecedented growth in the number of
older people. Characteristics of individuals with hip fractures
in Malaysia mirror what has been reported in other countries.
They will be older multimorbid people who were already at
risk of falls and fractures. Outcomes were poor with at least
a quarter do not survive beyond 12 months and in those that
do survive have limitation in their mobility and activities of
daily living. Reviewing how these fractures are managed and
incorporating new models of care, such as orthogeriatric
care, could address these poor outcomes. Experts have
warned of the devastating impact of hip fracture in Malaysia
and that prompt action is urgently required. Despite that,
there remains no national agenda to highlight the need to
improve musculoskeletal health in the country

During the Coronavirus Disease 2019 (COVID-19) pandemic, practices of gastrointestinal procedures within the digestive tract require special precautions due to the risk of contraction of severe acute respiratoy syndrome coronavirus-2 (SARS-CoV-2) infection. Many procedures in the gastrointestinal motility laboratory may be considered moderate to high-risk for viral transmission. Healthcare staff working in gastrointestinal motility laboratories are frequently exposed to splashes, air droplets, mucus, or saliva during the procedures. Moreover, some are aerosol-generating and thus have a high risk of viral transmission. There are multiple guidelines on the practices of gastrointestinal endoscopy during this pandemic. However, such guidelines are still lacking and urgently needed for the practice of gastrointestinal motility laboratories. Hence, the Asian Neurogastroenterology and Motility Association had organized a group of gastrointestinal motility experts and infectious disease specialists to produce a position statement paper based-on current available evidence and consensus opinion with aims to provide a clear guidance on the practices of gastrointestinal motility laboratories during the COVID-19 pandemic. This guideline covers a wide range of topics on gastrointestinal motility activities from scheduling a motility test, the precautions at different steps of the procedure to disinfection for the safety and well-being of the patients and the healthcare workers. These practices may vary in different countries depending on the stages of the pandemic, local or institutional policy, and the availability of healthcare resources. This guideline is useful when the transmission rate of SARS-CoV-2 is high. It may change rapidly depending on the situation of the epidemic and when new evidence becomes available.
