1.Microsurgical treatment for corneoscleral melting in severe ocular burns

Objective To evaluate the strategy using cyanoacrylate tissue adhesive (CTA) in amniotic membrane transplantaton(AMT) to manage corneoscleral melting in severe ocular burns. Methods Twelve eyes from eleven patients with chemical or thermal burns of grade Ⅳ were recruited for this study.After medical treatment failed to arrest corneal or sclera melting,CTA were applied directly on the ulcerating corneal or sclera surface to augmented AMT. Results In all cases,all sclera ulcerations (five of eleven) were prevented or healed,epithelialization of burned cornea (eight of eleven) was complete,corneal ulceration in the rest three eyes was successfully arrested. Conclusion Tissue adhesive aided amniotic membrane transplantation is a promising altermnative in managing severe corneoscleral melting in chemical or thermal burns to maintain the integrity of the ocular wall.
2.Intraocular pressure and anterior chamber angle changes after phacoemulsification in different types of acute angle-closure glaucoma

Objective To evaluate the relationship between intraocular pressure (IOP) and anterior chamber angle in patients with different types of acute angle-closure glaucoma after phacoemulsification and intraocular lens implantation. Design Retrospective case series. Participants 173 cases of acute angle-closure glaucoma coexisting cataract (173 eyes) in Mingren Eye Hospital from January, 2005 to June, 2007. Methods All patients with acute angle-closure glaucoma underwent phacoemulsification and intraocular lens implantation, and were examined with high-frequency ultrasound biomicroscopy (UBM). Anterior chamber depth (ACD), intraocular pressure (IOP) and anterior chamber angle (ACA) were measured. Follow-up was 1-48 weeks after surgery. Main Outcome Measures Intraocular pressure, phacoemulsification complications, re-open rate of anterior chamber angle. Results At 8th week after surgery, the rate of IOP control in pupillary block group, non-pupillary blocking group, and multimechanism group was 88.9%, 52.0%, 83.4%, respectively and there was a statistically significant between these groups (?2=7.13, P=0.022). At 48th week, the rate of IOP control was 54.2%, 33.3%, 35.8%, respectively(?2=12.56, P=0.003). Fifty patients were follow up with UBM, and 66.7% in pupillary block group, 33.3% in non-pupillary blocking group and 33.3% in multimechanism group in 48 weeks postoperatively showed evidence of the widened anterior chamber angle. No specific postoperative complications were found in this study compared with those with phacoemulsification without glaucoma. Conclusions The use of phacoemulsification and intraocular lens implantation for acute angle-closure glaucoma coexsiting cataract can get best IOP-controlled rates in pupillary block group. It is related to the degree of widened anterior chamber angle.

Objective: To study the effect of a potent angiogenic in hi bitor TNP-470 on the growth of sarcoma. Methods:1?10 6 sarcom a S-180 cells 0.1 ml were inoculated into submandibular region in each of 40 K M mice. The mice were divided into control and treatment groups with 10 in each group. Treatment was started 8 hours after inoculation. TNP-40 at 10 mg/kg, 30 mg/kg and 100 mg/kg was given subcutaneously every other day in the 3 treatment groups,total 6 times. On the 12th day, the mice were sacrificed, tumor and mice were weighted. Apoptosis of tumor cells was observed by TUNEL method and transmi ssion microscope. Express of VEGF, bFGF were detected by immunohistochemical sta ining. Results:Sarcoma was developed in all of the mice. The sa rcoma cells invaded deep into adjacent organs and tissues such as muscle, subman dibular gland, parotid and facial nerve. 10 mg/kg, 30 mg/kg and 100 mg/kg TNP- 470 inhibited the growth of the tumor by 27.62%,63.81% and 85.71% respective ly, increased the apoptosis cell number by 54.46%,156.69% and 432.48% respect ively (P
4.Effect of surface treatment using Er:YAG laser with different parameters on the bond strength of fiber posts

BACKGROUND:Recent years, fiber posts and resin cores have been widely used in repairing the endodonticaly treated teeth with satisfactory effect. Erbium:yttrium aluminum garnet (Er:YAG) laser is a new type of water power laser system, which can be used for surface treatment of fiber posts. But studies on the effect of Er:YAG laser surface treatment on the bond strength of fiber posts are rarely reported. OBJECTIVE: To evaluate the effect of surface treatment utilizing Er:YAG laser irradiation with different parameters on the bond strength of fiber posts to root canal dentin. METHODS: Fifty human maxilary central incisors that had similar dimensions were used. After endodontic treatment, removal of the crown and canal preparation, ParaPost FIBER LUX glass fiber posts were cemented into the root canals. According to the method of surface treatment, 50 teeth were randomly divided into: no surface treatment as control group; four groups undergoing surface preparation with Er:YAG laser with four different power settings (150, 250, 350 and 450 mJ at 10 Hz for 60 s at 100-μs pulse duration), named 1.5, 2.5, 3.5, and 4.5 W Er:YAG laser irradiation groups, respectively. RESULTS AND CONCLUSION: The mean bond strength values reduced from the cervical to the apical root canal, and the bond strength of the dental cervix was significantly different from that of middle and apical thirds (P < 0.05), but there was no difference between the middle and apical thirds (P> 0.05). Regardless of the different part of the root slices, the bond strength was highest in the 4.5 W Er:YAG laser irradiation group, showing significant difference from other groups (P < 0.05). These findings indicate that 4.5 W Er:YAG laser irradiation significantly increases the bond strength of the fiber posts to root canal dentin.
5.Using markov model to cost-effectiveness analysis of traditional Chinese medicine interference.

Mild cognitive impairment (MCI) is the transition state between normal aging and AD. There are better detection and treatment methods to screen a population of patients with MCI. Through intervention, the probability of MCI conversion to AD can be significantly reduced. This paper first introduces the concept of cost-effectiveness analysis, reconsiders the concept of cost for the particularity of MCI, and uses QALY to evaluate the health effects of the quality of life. Then measure the health quality of life of elderly MCI population, and use Markov model to study the cost of intervention with traditional Chinese medicine-effectiveness analysis. Finally, according to the QALY measure and CEA results, we draw the conclusion that it's helpful to get early intervention in MCI.

7.The 463rd case: rhabdomyolysis, acute kidney failure and acute hepatic failure

We represented a 22-year-old male patient who developed rhabdomyolysis,acute kidney failure and acute hepatic failure and was finally diagnosed as multiple acyl-CoA dehydrogenase deficiency.The patient appeared temporary stable status after high dose vitamine-B2 supplement whereas deterioration was still fatal with pulmonary infection,acute respiratory failure and acute heart failure.

Acupuncture is effective in the treatment of functional dyspepsia, and its mechanism may be related to abnormal gastric motility, central nervous system dysfunction, brain intestinal peptide secretion disorder, plasma molecular metabolite content changes and abnormal gene expression. This article summarized the mechanism of acupuncture treatment for functional dyspepsia in recent years from the aspects of electrophysiology, imaging, molecular biology, metabolism and genomics, and provided basis for clinical treatment of functional dyspepsia.
9.Identification of a novel mutation in solute carrier family 29, member 3 in a Chinese patient with H syndrome.

BACKGROUNDH syndrome (OMIM 612391) is a recently described autosomal recessive genodermatosis characterized by indurated hyperpigmented and hypertrichotic skin, as well as other systemic manifestations. Most of the cases occurred in the Middle East areas or nearby countries such as Spain or India. The syndrome is caused by mutations in solute carrier family 29, member 3 (SLC29A3), the gene encoding equilibrative nucleoside transporter 3. The aim of this study was to identify pathogenic SLC29A3 mutations in a Chinese patient clinically diagnosed with H syndrome.
METHODSPeripheral blood samples were collected from the patient and his parents. Genomic DNA was isolated by the standard method. All six SLC29A3 exons and their flanking intronic sequences were polymerase chain reaction (PCR)-amplified and the PCR products were subjected to direct sequencing.
RESULTSThe patient, an 18-year-old man born to a nonconsanguineous Chinese couple, had more extensive cutaneous lesions, involving both buttocks and knee. In his genomic DNA, we identified a novel homozygous insertion-deletion, c. 1269_1270delinsA, in SLC29A3. Both of his parents were carriers of the mutation.
CONCLUSIONSWe have identified a pathogenic mutation in a Chinese patient with H syndrome.

10.Effects of Bailing capsules for renal transplant recipients: a retrospective clinical study.

BACKGROUNDThe administration of immunosuppressive agents is always an important factor affecting the long-term survival of organ transplantation recipients. The best therapeutic regimen which either decreases the side effects of immune inhibitors or enhances the immunosuppressive efficacy is the goal of transplantation surgeons continue to search. This study investigated the effects of Bailing (Cordyceps sinensis) capsules on renal function and other systems of the body after renal transplantation.
METHODSClinical data of 80 renal transplant recipients who were administered Bailing capsules and 100 renal transplant recipients in the control group were retrospectively analyzed to compare the incidences of graft rejection and infection after transplantation. The results of routine blood and urine tests, liver and kidney functions, uric acid (UA), 24-hour urine protein (24 h-Upro), as well as 1- and 5-year patient renal allograft survival rates were compared between the two groups.
RESULTSThe follow-up was 3 - 5 years. The two groups were not shown to have statistically significant differences in age, gender, cold ischemia time, donor-recipient human leukocyte antigen typing, panel reactive antibodies, lymphocytotoxicity tests, and the application of immunosuppressive agents at the baseline. The two groups were also not significantly different in the incidence of acute injection after transplantation, recovery of renal function, and blood glucose level. The Bailing group was significantly lower than the control in the incidence of infection, serum aspartate aminotransferase/alanine aminotransferase, total bilirubin, UA, and 24-hour Upro, but significantly higher than the control group in peripheral red blood cell count and white blood cell count (P < 0.05). One-year and 5-year patient survival rates were 98.7% and 98.0%, respectively in the Bailing group, 95.0% and 93.0%, respectively, in the control group. One-year and 5-year renal allograft survival rates were 97.5% and 95.0%, respectively, in the Bailing group, and 92.5% and 84.0%, respectively, in the control group. The comparison of patient and renal allograft survival rates between the two groups using Kaplan-Meier survival curves and log-rank test showed that only the differences in renal allograft survival rates were statistically significant (Log-rank: 5 years: patient survival P = 0.420; renal allograft survival P = 0.049).
CONCLUSIONBailing capsules were effective in preventing allograft rejection, protecting liver and kidney functions, stimulating hematopoiesis, and reducing the incidence of infection and thus are ideal immunoregulators.
