1.Relationship between the family composition and medical expenses of diseases of the circulatory system. An analysis on medical expenses for the elderly people in national health insurance.

To examine relationship between the family composition and medical expenses for circuration diseases in the elderly people who were 65 or more, we analysed the 1982 and 1983 data of medical expenses of national health insurance in the rural area of Fukuoka Prefecture.
The elderly people were classified into five groups according to whether they were living with their children and whether their spouse was alive.
The results were as follows:
1) In the elderly people who were living with their children, the medical expenses were low among those living with their spouses and high among those living without.
2) In the elderly people who were not living with their children, the medicalexpenses were low among the elderly men with their spouses and high among the eldrly women without.
3) In the elderly people who were living alone, the elderly men and women had low medical expenses in hospital services and ambulatory services.
4) The medical expenses for circulation diseases were more influenced by the presence of spouses than living with their children.
2.Immediate Effects of Scapula Muscle Training in Healthy Individuals

The purpose of this study was to elucidate the effects of “shoulder girdle training” that is used to correct scapular muscle imbalance. We investigated 100 shoulders in 50 healthy volunteers free of any complaints related to the shoulder joint. In the posture for performing the infraspinatus test, male volunteers held a 5-kg load and female volunteers held a 3-kg load for 3 s thrice, and the muscular activity of the trapezius muscle was simultaneously measured. Next, shoulder girdle training was performed for 5 s twice, followed by muscle activity measurement in the same manner described above. Muscle activity levels before and after this training and also between the dominant and non-dominant sides were compared. The shoulder girdle training significantly reduced the level of muscle activity of the upper fibers of the trapezius muscle and significantly increased the level of muscle activity of the middle and lower fibers of the trapezius muscle (both p<0.05). Moreover, the levels of muscle activity of the upper, middle, and lower fibers of the trapezius muscle were significantly higher on the dominant side than on the non-dominant side (p<0.05).
3.Impact of Shoulder Shrugging Motion on Muscles Surrounding the Shoulder Joint during Forward Elevation of the Upper Limb

Rotator cuff repair provides favorable outcomes for range of motion of the shoulder joint and repair of the cuff. However, we often have patients for whom shoulder elevation is difficult due to failure to improve shoulder shrugging motion. This study investigated how the shoulder shrugging motion affects activity patterns of the muscles surrounding the shoulder joint. Subjects were 50 shoulders in 50 young healthy volunteers free from shoulder joint complaints (27 men, 23 women; mean age, 26.3±4.3 years). The subjects were placed in the lateral position with the dominant arm facing upwards and suspended with a sling. They performed two types of movements: forward elevation of the arm with no particular instructions (group N) and forward elevation combined with shoulder shrugging (group S). Simultaneously, the activity of the anterior, middle, and posterior fibers of the deltoid muscle and of the upper, middle, and lower fibers of the trapezius muscle of the dominant upper limb were measured by surface electromyography and compared between the two groups. In the trapezius muscle, upper fiber activity was more enhanced in group S than in group N, whereas lower fiber activity was more strongly suppressed in group S. However, the anterior and middle fiber activity of the deltoid muscle was more strongly suppressed in group S than in group N. Thus, it was observed that an elevation pattern with shoulder shrugging reduces the strength of forward flexion.
4.Utility of High-density Barium to Improbve the Quality of X-ray Images

Aim:The usefulness of high-density barium, which was made to 150w/v% (MIX150), was examined to obtain better radiographs of the intestines. Methods: “MIX150” was compared with existing pharmaceutical “BAM100” to evaluate x-ray images (barium adhesion, dryness evaluation). MIX150 was used in 232 patients (mean age 56 years), while BAM 100 in 210 patients (mean age 55 years). Results: In terms of adhesion, MIX150 was excellent in the the cecum significantly and in dryness evaluation, in the entire region of the colon. Conclusion: High-density barium (150w/v%) was useful for enhancing the quality of radiographic images.

5.High Density Barium Dosage and its Effect on Excretion. A Survey.

Before the introduction of high density barium for contrast studies of the upper gastrointestinal tract, we conducted a survey concerning the adverse reactions to a swallow of barium and barium concentration.
The incidence of side effects rose as the density of barium increased but the effects were transient. There were no cases requiring medical treatment.
Furthermore, the constipation group and the normal group were examined separately.
The ratio of adverse reactions was high in the constipation group even when the barium density was low. In this group stool hardening and delayed excertion were also noticed.
The constancy of barium stool excretion was basically normal, and the barium density had little effect.
The effects of a laxative on the excretion consistency were investigated. The administration of a laxative did not always have a positive affect on excretion. The timing of the administration of the laxative and the amount of water intake should be examined in the future.
We also investigated how the patients feel when they are swallowing barium. We found that whether feel uncomfortable or not depended on the properties of barium rather than its density.
From these results it appears that appropriate guidance is necessary about the use of high density barium, in order to supress the occurrence of side effects, especially in the constipation group.
6.Study of Risk Factors for Lifestyle-related Health Problems in Men in Their 40's and 50's in Kochi Prefecture

In Kochi prefecture, the mortality rate of males in their 40s and 50s exceeds the mean death rate of Japanese men by more than 10%. The causes of death in general in Kochi are cancer (27.1%), cardiac disease (16.8%), cerebrovascular disease (11.9%) and pneumonia (11.2%) in that order. We analysed the data of a total of 1,826 males in the 40-50 age band who had panticipated in the one-day health screening course provided by our hospital. Our findings showed that many of these middle-aged males, were heavy drinkers and obese people with BMI of 25 or more. The amount of alcohol consumption in Kochi is the second largest in Japan, which may impact on the incidence of alcohol-related diseases and death. As for blood pressure, the ratio of those people in their 50s and 60s with hypertension in Kochi was higher than the mean of this country. To prevent the development of the metabolic syndrome, we must keep on making efforts and encourage them to adopt a healthy life style.
7.Surveillance of Usage of Drugs Corresponding to Doping, and Construction of Management System for Anti-doping

Objective: Doping is strongly prohibited in sports. Sports pharmacist was born in 2010 in Japan, and the anti-doping activity is expected. On the other hand, doping by arising from a lack of knowledge about prohibited substances in athletes, so-called “unwilling doping” is developing into a social issue. In this study, we investigated the percentage of prohibited substances in all drugs and prescriptions in a general hospital, to collect information to prevent an unwilling doping.
Methods: We constructed system to extract the drugs corresponding to prohibited substances in the prescription order entry system in Otaru Municipal Hospital, and we analyzed 3,306 prescriptions of 10 to 59 years old patients, from July to September 2010.
Results: Thirteen point five percent of our hospital drugs met definition of the prohibited substance. The number of prescriptions including prohibited substance(s) was 350 (10.6%), and its category was different from each age-group and clinical department.
Consideration: Because prohibited substances are included in approximately 10% of prescriptions, athletes are exposed to danger of becoming an unwilling doping. Pharmacist should be well informed about prohibited substances to prevent athletes from unwilling doping. And they should provide information promptly and adequately for athletes.

The main purpose of basic clinical training for housestaff is to acquire the ability to be a primary physician who can properly manage acute medical problems, develop intimate bonds with patients, and provide them with continuous care. We emphasize the importance of training in the office, clinic, or patients' homes. Although residents have so far spent most of their clinical rotations in an inpatient setting, a training program that devotes substantial time to ambulatory care is indispensable to improve basic clinical training in primary care medicine.

Objective: Combination therapy with glecaprevir and pibrentasvir (G/P) has been shown to provide a sustained virologic response (SVR) rate of >97% in patients with chronic hepatitis C virus (HCV) infection in the first published real-world Japanese data. However, a recently published study showed that the treatment was often discontinued in patients ≥75 years old, resulting in low SVR in intention-to-treat (ITT) analysis. Thus, our aim was to evaluate real-world data for G/P therapy in patients ≥75 years of age, the population density of which is high in “rural” regions.Patients and Methods: We conducted a multicenter study to assess the efficacy and safety of G/P therapy for chronic HCV infection, in the North Kanto area in Japan.Results: Of the 308 patients enrolled, 294 (95.5%) completed the treatment according to the protocol. In ITT and per-protocol analyses, the overall SVR12 rate was 97.1% and 99.7%, respectively. The old-aged patients group consisted of 59 participants, 56 of whom (94.9%) completed the scheduled protocol. Although old-aged patients tended to have non-SVR factors such as liver cirrhosis, history of HCC, and prior DAA therapies, the SVR12 rates in old-aged patients were 98.3% and 100% in the ITT and PP analyses, respectively. Of 308 patients enrolled, adverse events were observed in 74 patients (24.0%), with grade ≥3 events in 8 patients (2.6%). There was no significant difference in any grade and grade ≥3 adverse events between the old-aged group and the rest of the study participants. Only one patient discontinued the treatment because of adverse events.Conclusion: G/P therapy is effective and safe for old-aged patients.
