The purpose of the oral mass schreening conducted at our hospital was early detection and prompt treatment of oral cancer and precancerous symptoms.
This report will show the epidemiological status of oral mucosal abnormality and precancerous lesions in a selected population of 3, 574 Japanese (1, 066 women, 2, 508 men, ranging in age from 22 to 76) during August 1990 to August 1991.
1. Out of 36 cases in which biopsies were performed, 17 cases were found to have leukoplakia (0.5%),
4 wiht lichen planus (0.1%), and 15 cases with other diseases (0.5%).
2. In 17 Ieukoplakia patients, differences were not statistically significant between men and women (P<0.005) and between age groups (p<0.005). The most common site of oral mucosal diseases was gingiva (47%), follwed by cheek (23%), and tongue (17%).
3. Smokers accounted for 94% of the patients with leukoplakia, and 31% of the individuals who were found heathy. This means a statistically significant difference betwwen smokers and nonsmokers (p<0.005).
4. The percentage of denture using examinees were 35% for patients with leukoplakia and 31% for healthy individuals. There was no significant diffemence between them (p <0.005).

A 22-year-old male was admitted to our hospital with suspected bone tumor of the right mandible. The patient's facial disharmony indicated class III malocclusion and mandibular prognathism. Biopsy was obtained from the bony lesion, and histological fingings revealed typical characteristics of ameloblastoma. During marsupialization, presurgical orthodontic treatment was given.
A surgical procedure was planned by using his X-rays and presurgical model cast. The right mandible was resected without concominant removal of the neurovascular bundles. Subsequently, the left-side mandible was set back by sagittal split ramus osteotomy and the right-side mandible was simultaneously reconstructed by free illiac bone grafting.
After surgery, the facial proportion and profile were improved. The missing teeth and associated structures were replaced by partial denture. The patient restored normal occlusal function and mastication.

We encountered a patient with Paget's disease. The patient was a 35-year-old-female. She visited our clinic, complaing about masticatory dysfunction. Examination revealed cranial bone outgrowth, alopecia, amenorrhea, mobile and malposed teeth, and alveolar expansion of the maxillae and mandibule. Radiographic observation of the skull and facial bones revealed a typical ground grass appearance, periapical cementum hyperplasia of the teeth. Histologic findings showed a mosaic architecture characteristic of Paget's disease.

Indications are that Japan is growing old at a faster pace than in any other nation. In 1993, people aged 65 years and over accounted for 13.5% of the nation's population, and in Nagano Prefecture the rate stood at 17.9%, far higher than the national average. In our district, Minamisaku, it was 22.0%, greater than the prefectural average. As aging goes on at a rapid clip, the number of the bed-ridden is on the upswing.
The Saku Central Hospital has instituted a system of home-visit health care since 1988 with the Department of Internal Medicine playing a pivotal role. The Department of Dentistry and Oral Surgery has also commenced a home-visit dental care program since 1990
The patients to whom home-visit care is delivered have a variety of basic diseases. The attention has focused on their treatment. In many cases, their indifference to oral hygiene was responsible for the ailments. Since the mouth is an inlet for bacteria and other pathogenic microorganisms, it is important to keep the mouth clean so as to prevent the deterioration of basic diseases. That biting may be done with dentures and other prosthetic devices suggests a feasibility of improving quality of life and working for a better health? Because some reports argue that chewing stimulates the brain, thus helpful in preventing senile psychosis, and because eating is one of the pleasures for the bedridden, the role played by us, dentists, is large. Here we report the present status of home-visit dental care delivered by our department from 1990 to 1995

For the purpose of elucidating the conditions of diabetic patients in rural areas, we have been undertaking some statistical studies in regard to various pathological aspects of diabetic patients being treated at our hospital on the outskirts of Nagoya City, as well as those discovered by diabetes mass examinations. This it to report on the total serum protein values and serum protein fractions (by electrophoresis), measured for the study of protein metabolism.
Group I. Diabetic patients treated at our hospital. No remark-able change was observed in their total serum protein value. In their serum protein fractions, a decrease in albumin value and A/G ratio and a slight increase of α 1-, α 2-, β-and β-globulin values. On the other hand diabetic patients with renal complications showed a decrease in total serum protein value, albumin value and A/G ratio, a slight increase in α 1-, α 2-, and γ-globulin values, and a slight decrease in γ-globulin value.
Group II. Insidious asymptomatic cases discovered by mass examinations. No remarkable change was observed in total serum protein value. A for serum protein fractions, they showed a decrease in albumin value and A/G ratio, and an increase in all glabulin fractions. Furthermore. increase of α 1 fraction was more conspicuous in women, that of α 2 in women and in both sexes with renal complications, that of β-and γ-globulin in women, farmers and those with renal complications.
These findings oriented us farther to the analysis of relation-ship between diabetes mellitus and various environmental factors.
