1.Outlook of dengue in Malaysia: a century later.

Dengue continues to be a major health threat to Malaysia a century after its first reported outbreak in 1902. Examination of the available outbreak data suggested that a major DF/DHF outbreak occurred in Malaysia in a cyclical pattern of approximately every 8 years. All four dengue virus serotypes are found co-circulating in Malaysia, but after the first and only major outbreak involving DEN-4 in 1960's, only DEN-1, DEN-2 and DEN-3 were associated with DF/DHF outbreaks. It is argued that perhaps the spread of the later dengue virus serotypes followed the pattern of spread of the mosquito vector Aedes aegypti, whereas the former was associated with Aedes albopictus, the outdoor and rural area dwelling mosquito. Estimating from the trend and pattern of dengue and the associated dengue virus serotypes, unless there is a major breakthrough in dengue vaccine development, it is likely that dengue outbreaks will continue to occur in Malaysia throughout the 21st century.

2.Antihyperglycemic and Glucose Tolerance Activity of Ficus deltoidea Ethanolic Extract in Diabetic Rats

Ficus deltoidea or Mas cotek is one of the common medicinal plants used in Malaysia has been claimed to have
antidiabetic activity. However, scientific evidence to confirm its efficacy is still lacking. Thus, the present study was undertaken to evaluate the potential of ethanolic extract of Ficus deltoidea to reduce hyperglycaemia in streptozotocininduced
diabetic rats at different prandial state. The results showed that, ethanolic extract of Ficus deltoidea significantly reduced fasting and postprandial hyperglycemia particularly after 4 and 6 hours of extract administration. Likewise, glucose tolerance activity was significantly improved in the presence of Ficus deltoidea ethanolic extract at a low dose, 100 mg/kg. It is suggested that ethanolic extract of Ficus deltoidea at particular doses, possess fasting and postprandial
antihyperglycemic activity as well as glucose tolerance activity in streptozotocin-induced diabetic rats.
3.Corneal Thickness and Curvature of One Sample of Young Myopic Population in Malaysia

The purpose of this study is to determine the standard value of corneal thickness (CCT) and its relationship with the degree of myopia amongst one sample of
myopic students in Malaysia. Eighty-four myopic subjects (168 eyes) were examined in this study. Ocular examination included subjective refraction, keratometry (Shin-Nippon, Japan) and optical pachometry (Sonogage INC).
Corneal thickness was measured at five different locations i.e. central, superior, inferior, nasal and temporal of the cornea. However, only association of CCT
with myopia is reported in this article. Mean age of subjects was 21.42 ± 1.47 years old (20.34 ± 1.15; 22.46 ± 0.88). Mean refractive error in spherical
equivalent for the whole sample was –3.50 ± 2.10 DS. Mean corneal curvature for the all subjects was 7.74 ± 0.52 mm and mean value of CCT was 594.62 ± 66.51 mm. Significant differences (p < 0.05) were found between mean CCT of
low myopes (-4.00DS and below) and higher ones (-4.25DS and above), but not with mean of keratometer readings. Poor correlations were obtained between refractive error and CCT (r = 0.125, p > 0.05) and between refractive
error and corneal curvature (r = 0.24, p > 0.05). It was concluded from this study that high myopic subjects has significantly thinner CCT than lower ones.
Measurement of CCT should be considered when undertaking procedures such as LASIK and orthokeratology.
4.Isolated Optic Neuritis Secondary to Presumed Tuberculosis In An Immunocompetent Child

Isolated optic neuritis as a presenting sign of tuberculosis in children is uncommon. We report a case of an immunocompetent child who displayed features of bilateral optic neuritis secondary to presumed tuberculosis. It is essential to highlight this alarming presentation in a child, as the presence of tuberculosis has re-emerged as a serious public health problem especially in developing countries.
5.Pleomorphic Adenoma Originating from Submandibular Salivary Gland in an 8-year-old Girl: A Case Report

Pleomorphic adenoma (PA) typically presents as a benign slow growing, painless
neoplasm of the parotid gland. PA arising from the submandibular gland in the paediatric
age group is rare. Surgical excision is the treatment of choice. A case of PA in an 8-yearold
Malay girl is presented. Clinically she presented with a painless right submandibular
mass measuring 2x2cm. A diagnosis of Pleomorphic adenoma was made on Fine Needle
Aspiration Cytology (FNAC). She underwent submandibulectomy uneventfully.
Postoperatively after two years follow up, she is asymptomatic.
6.Sodium and Potassium Urinary Excretion Among Malaysian Adults Claiming to Practice Salt-control: Findings from the Malaysian Community Salt Survey (MyCoSS)

Introduction: Raised blood pressure is associated with diet high in sodium and low in potassium. According to the
Malaysian Community Salt Survey (MyCoSS), Malaysian adults consume a high sodium diet, despite half claiming
that they control their salt intake. The objective of this study was to investigate the association of sodium and potassium with salt-control claims among adults from the MyCoSS study based on 24-h urinary excretion. Methods: Data
of 1,047 respondents aged 18 years and above who completed the knowledge, attitude, and practice (KAP) questionnaire were used in this study. Face-to-face interviews were conducted to acquire information on sociodemographic,
medical reports and KAP on salt intake. Sodium and potassium level were assessed from 24-hr urine specimen, anthropometric measurements were done using WHO criteria, while blood pressure was measured using validated digital blood pressure monitor. Descriptive and analysis of variance (ANOVA) were used to determine the associations
between urinary sodium and potassium with salt-control claims and practices. Results: The consumption of sodium
was higher than recommended, while potassium was lower among respondents who claimed to control their salt
intake. Significant associations were found between sodium intake with marital status, ethnicity, education level and
employment status. Meanwhile, significant associations were found between potassium intake with ethnicity and
employment status. Conclusion: Sodium intake was high, and potassium intake was low among respondents who
claimed they control their salt intake. Further study with detailed intervention on specific target group is required to
improve sodium and potassium intakes among Malaysian adults.
7.Pteridine fluorescence in age-determination of immature Chrysomya megacephala (Fabricius) and Chrysomya rufifacies (Macquart) (Diptera: Calliphoridae)

In the practice of forensic entomology, the chronological age of the maggots
retrieved from the cadaver is used to determine the minimum post-mortem interval (mPMI)
i.e. minimum time of death. The conventional method of aging the maggots is based on
measuring the growth rate of these maggots. Although effective, the constraint associated
with conventional method necessitates the development of new age determination method,
such as pteridine determination. Pteridine, a by-product of protein metabolism in insects is
known to correlate with the age of a variety of dipterans. A number of studies were
conducted on aging the adults of forensically important flies. In this study, pteridine was
extracted from Chrysomya megacephala and Chrysomya rufifacies maggots of known
age using established methods and determined by measuring the fluorescence at excitation
of 330nm and the emissions between 350nm and 600nm. Results exhibited significant
positive linear relationships between the pteridine accumulations and age of the fly
immature. Pteridine determination is a potential new age determination tool that can be
used to determine mPMI.
8.Factors associated with sugar-sweetened beverages consumption among Malaysian adolescents: findings from the Adolescent Nutrition Survey 2017

Introduction: Childhood obesity is a public health problem in Malaysia. Intake of
sugar-sweetened beverages (SSB) is associated with obesity in children. There is a
lack of studies on factors associated with SSB consumption in Malaysia. This study
aimed to determine the sociodemographic factors associated with SSB consumption
among Malaysian adolescents. Methods: Data of 2,021 students, aged 10-17
years on sociodemographic, SSB intake and anthropometrics were drawn from
the school-based Adolescent Nutrition Survey 2017. A multistage stratified cluster
sampling was used to obtain a nationally representative sample of primary and
secondary school students. Body mass index (BMI)-for-age status was determined
based on calculated z-score using the World Health Organization 2007 reference.
SSB consumption was obtained from a food frequency questionnaire. Results: The
prevalence of overweight and obesity among Malaysian adolescents were 16.6% and
14.7%, respectively. Malaysian adolescents consumed 1.4 cups of SSB per day.
Rural children had a significantly higher SSB intake (1.5 cups) than urban (1.3
cups) children, while males (1.5 cups) had a significantly higher intake than females
(1.3 cups). There were no significant differences in SSB consumption between thin,
normal, overweight and obese adolescents. Conclusion: Almost all Malaysian
adolescents consumed SSB during one month prior to the survey and the average
amount consumed was 1.4 cups per day. Rural locality and male were associated
with higher SSB consumption. There were differences in SSB consumption
between Chinese and Malays, between Chinese and Indians, and between Chinese
and Bumiputra Sarawak. There were no differences in consumption between the
different BMI-for-age categories.
9.Calcium and iron intakes of adolescents in Malaysia and their relationships with body mass index (BMI): Findings from the Adolescent Nutrition Survey 2017

Introduction: Adolescent health is a priority considering they represent the future generation. Data from the Adolescent Nutrition Survey 2017 were analysed to determine the prevalence of micronutrient inadequacy, particularly calcium and iron, and the relationship with body mass index (BMI) among Malaysian secondary school students. Methods: This cross-sectional study included students aged 13 to 17 years old studying at public and private schools. Twenty-four hours dietary recall via face-to-face interview was conducted by trained nutritionists to obtain data on dietary intake. For nutritional status, BMI-for-age z-score (BAZ) was analysed using WHO Anthroplus software. Results: From 999 respondents, 449 were boys and 550 were girls. Overall findings indicated that both boys and girls had inadequate intakes of calcium and iron in their daily diet. Mean intakes of calcium (695.7±463.2 mg/day) and iron (23.4±21.0 mg/day) were higher among boys aged 16 to 17 years old. Mean intake of iron were higher among the older age groups. Majority of the respondents (boys: 94%; girls: 97%) did not meet the recommended nutrient intake (RNI) for calcium and more than half (boys: 50%; girl: 80%) did not achieve the RNI for iron. Current findings also found significant positive but weak correlations between calcium (r=0.112, p=0.001) and iron (r=0.084, p=0.008) intakes with BMI-for-age. Conclusion: BMI-for-age was related to calcium and iron intakes among secondary school students in Malaysia. Thus, intervention strategies should focus on early screening and nutrition education on food choices of high calcium and high iron contents, including iron supplementation programmes, if needed.
